I have been coming to this office for around 6 years and have never been disappointed in the service or quality of care here. They have amazing customer service and the facility is always clean and up to date. All of the staff is very welcoming and extremely knowledgeable on procedures. When I inquired about Invisalign, the front desk and office manager were quick to provide me with any information I needed and they answered any and all of my questions! Dr. Astrid is beyond. She cares so much about her patients and always has a smile on her face! She will guide you through anything she's doing and is quick to answer any questions that will help soothe your anxiety. It's hard to find dentists you can trust in Miami and this is one which raised red flags very early! Perhaps this could be a place that services individuals who would like to pay extra for the aesthetic and a "pleasant" staff, but don't let them scam you or your insurance. I'm very disappointed with how they try to upsell services , and over charge you and your insurance even if they are in your insurance network. For those of you who don't know, usually in-network providers are only allowed to charge you an amount which aligns with the policy and all in network providers should be charging the same. If they didn't try to charge me 4x more than the allowed amount per procedure for an in-network provider, $600 for 3 fillings post-insurance, and hold my x-rays hostage until my "claim was filed" only to later release them if I said I would pay until the insurance reimbursed me then go say " it's ok I don't have to pay," maybe I wouldn't have gotten suspicious and they could have gotten away with me paying more and kept my business. They in no way had a convincing story or reason for why the quotes were so high after insurance so I had to spend days learning about how insurance works and investigating only to come to find that they also charged me 4x more than the allowed amount for my X-rays. If they are in your in your network and you are thinking about going with them or had a procedure done with them in the past (OR ANY DENTIST YOUR SUSPICIOUS ABOUT) I HIGHLY recommend calling your insurance to learn what the max allowed amount is for a service code and filing a claim if you happened to already over pay. If this office wants to charge their own rates then that's definitely fine, but don't say you are "in-network" to gain new clients but not follow the policy and over charge the co-pay Thankfully I have found a new dentist who is in my network, follows the policy and was appalled by the rates they gave me and told me that it falls under "Florida Recommended Guidelines" but it shouldn't have been the rate for in-network. My insurance advocate said that these scams happen pretty often so checking that the fees align with the in-network policy is an important part of doing your due diligence with new providers. I found the best place for YOU (the terrified patient) and people like myself (oddballs who actually relax during a routine cleaning and may even fall asleep - yes, I know!). First of all, the place is very welcoming with brand new spa-like lobby and facilities and the staff (from front desk to the doctor) is fantastic. Top technology is being used to make patients feel comfortable with all procedures being done to them. I tried this place today for the first time and I already have highly recommended it to family. It pains me to write this review because they are generally nice people but i really sense theyre trying to sell me procedures that aren't really necessary. At the end of the day they get paid on us getting procedures, dentists are the equivalent of car mechanics in that you dont really know much about what is going on, but often times they will tell you you need to top up your windshield wiper fluid when you really just needed an oil change and refilling your 90% full windshield wiper fluid isnt needed. i really feel like this was happening here. Here is the extent of my interactions with them: 1- Dr. Alves messaged me on linkedin to see if i needed a dentist, thats a little weird, but whatever its 2021 people market in different ways now. 2- They pushed hard for me to get a "pericornal excision" removal, after i showed up and got my x-rays, this is when the gum overlays the last of your molars very slightly, and they perform a minor procedure to remove the overlay. I have been to 3 different dentist in the past 4 years, not because of a fault of mine, but because i just have been moving for work. Not one place i have been, mentioned this procedure before right now. My gums with the overlay have been like that the entirety of my life, this is a red flag. They said its because i have a cavity in my back teeth, but i had a cavity in my back teeth before, and it never came up. (I have had ortho work and bonded two teeth along with whitening as well, this surely would have come up with one of my previous dentists). This occurred when i got my x-rays, my initial x-ray and cleaning were broken up into two different days. Whatever, i have a cavity in my back teeth, it could make sense to recommend it now. But i'm still on the fence about it. 3. When i went for the cleaning, i was expecting to sit down, get a normal cleaning, then leave, but no, the hygienist was pushing me to get a deep cleaning, to remove excess plaque built up under my gums. I have never heard of this procedure before, and even though i declined it multiple times. They insisted on it while i was in the chair waiting for my normal cleaning. I was literally in the chair for 10-15 mins while they tried to tell me i needed a special cleaning, that i never heard of, right at that moment. I just wanted a normal clean, i don't want to sit there and be sold something that should have been brought up on my first visit when i got the x-rays. I had a hygienist and the doctor look at my when i got my x-rays, didn't mention it, but now i'm being sold it? When i come back for my cleaning? This was literally the last thing i wanted, i just wanted to be in and out. Most likely the hygentist thought they could upsell me right at that moment. My gums are the same condition they have been all my life - no previous dentist ever mentioned this to me, a google search shows its not really necessary per a few non-dentist sources (dentist sites will always say their procedures are necessary) and comparing my teeth to the ones i see online recommended for this procedure, mine are in much better condition. They then have me sign a consent form that i declined treatment, which truthfully just feels like a round about sales pitch, where they list extreme consequences if i don't agree to the service. All i wanted was my teeth cleaned, and not be pressured into services i have never heard of on the spot. read more

The lady over the phone has an attitude problem, she confronted my right away because I asked the reason of the cancellation, a complete lack of respect, and I thank you this happen before, now I know what are the true values of this place. read more

I loved Forma Dental! By far the best experience I've ever had. From the second you walk in to when you leave, everyone is extremely nice and accommodating. It's also such a beautiful office space that instantly relaxes you. I also want to mention that this isn't a dentist office that just starts doing procedures on you that aren't covered by your insurance and then hands you the huge bill at the end...Maria (the manager) did a great job of explaining to me what was and wasn't covered by my insurance and how much each option Dr. Alves gave me for future procedures would cost. I really appreciated this after past experiences with dentist offices I have been to. Another thing that I was grateful for was how both Raquel (the dental hygienist) and Dr. Alves took the time to explain everything to me. The appointment wasn't rushed at all and you can tell they pride themselves on quality appointments over quantity of appointments. Horrible experience!!.. My Friend and i had an appointment at 10am, I wanted to get a X-ray and a cleaning, they didn't let us inside until 10:30am saying they didn't have any chairs, after that they told me I have to go back outside and wait in the car for another 10 mins, then they called me saying I can now come inside, after waiting for a total of 40mins!... I went inside they put me to sit, then I had to wait another 30 mins, after someone came and did my X-rays, they left me again for another 30 mins! At that point I got frustrated!.. I got up found the lady and told her that's it i have to leave because I had another appointment at 1pm... I only did a X-ray and I was there for over 3 hours!!.. Pooor service!! read more

After my visit today I couldn't wait to get home and write this review. I've been going to the same dentist for the last 12 years but it was time for a change. Going in i knew that I wasn't going to get good news after my ex-rays especially after having a baby and not following up on my usual cleaning. The doctor and staff made me feel so comfortable. Unfortunately I thought my husband and I had found a dentist to stay for a long time but we were wrong. The place is modern/clean and Raquel (hygienist) is really nice and good, however the new office mgr Sissi is not. My husband had a routine cleaning and they never informed him he had to pay, then a few weeks later, surprise! he received a bill for that cleaning.  He went to ask why and new office mgr "Sissi" got somewhat upset (not professional) she had a defensive attitude, and in a serious tone told my husband that the bill has to be paid given that the services were rendered! . In conclusion if you go there be very careful and don't trust what they tell you about what your insurance covers.  What we have learned about FORMA is that they care more about your money than their patients, we will no longer go to that place, we will send the check to pay the bill ASAP and that's it. They have lost 2 clients and of course I won't be recommending them to friends/family. I think going to any doctor is not pleasant but specially going to a dentist and at FORMA they "want you" to feel like in a spa and have a good experience but that's not possible if the office mgr doesn't want to help you or at least try to work with you in a professional manner; but instead make you feel like they don't care if you go somewhere else. The least she could have done is talk it over with the owner (the head dentist there) who is a nice person, and try to think about the ramifications of us paying for their mistakes would be. We had a great dentist before and we switched just because this place was closer (big mistake) we are going back to our former dentist now because we never had any issues with them, they always double checked with the insurance and we still have a great relationship with them.
