Went in to get referral for bloodwork. Terrible allergies. Over a month after doing my bloodwork to see what I'm allergic to I hadn't heard from the doctor. I called and made an appointment to go over my results. The doctor couldn't get me out of there fast enough. Rude dismissive and couldn't care less what I was allergic to. Waited over an hour to see the doctor. She used F words while talking to me. The soul purpose of my visit was to get my prescriptions refilled. I was there on a Monday. By Saturday that week nobody had called my meds in to the pharmacy. Called three times and some answering service in another country answers and tells me the Doctor never entered in any of my meds. I asked for a referral as well. No record of any referral. This place is atrocious. Any time I start to bond with a doctor here they quit. read more

We saw Daisy De Guzman to establish patient care and for an annual check. We did the routine labs and she asked us all the questions then even said you need refills of medication. 3 months after my refills from my previous doctor finished, my pharmacy called and said no refills was ever sent. We were also never scheduled for a follow up to explain lab results nor called to schedule for a follow up to explain the results! AVOID this clinic at all costs! read more

I just loved going there but after you saw the doctor, your referrals lagged for months the constant calls back was just exasperating. Joy really had there best interest at heart but her efforts were invisible they just couldn't see it. The assistant only states I sent on the referral. The Breast care center, felt sorry for me for I had been calling for months back and forth the took me in fear something could be happening not favorable to me. I nor they never received the referral. From Nov 2024 to May 2025. One of my other doctors wrote my referral another concern again due to the wait. There's so much more but I won't. Impossible to get in touch with anyone. No one calls back. For every small thing they want you to go in to pay copayment when they can easily send prescription. Just not a very thorough office. I've since they took over my previous doctor's practice. That dr knew me to the point I recommended him to my wife, parents and friends. I can't do that here. The dr's at Forte don't possess the charisma I believe a dr should have. I'm in the room waiting on the dr and everyone outside the room who works here are laughing and messing around. This is a primary care physician office. The office staff is incompetent & I have had multiple issues over time here. They are severely understaffed & never answer their phones, let alone call you back. They never called my pharmacy back either!! The only reason I stayed was because of Dr. Beth Moreno. She was the best. She also recently left & moved out of state. I am now looking for another Dr. I definitely would not recommend this practice & they have 3 locations. When I made the first appointment, the office had specifically asked what I was coming in for. I expressed it was for a prescription for Prep HIV prevention drug. After arriving, I filled out a lot of paperwork and waited over an hour. The MA told me Dr Gardner refuses to prescribe PreP or any other LGBT type prescriptions. I then said there was no use of me staying to see the Dr. and started to leave, but she suggested I stay and talk to him. After another 40 minutes of waiting, Dr Gardner came in and said he does not write prescriptions for trans people (I'm not trans), but he could refer me to a specialist. I left the room very upset. As I was walking out, an office rep stopped me, inquired on what was wrong and went to ask another provider in the office to see if she would work with me. The other provider agreed to see me at a later appointment and said there was a note to waive my payment for the next new appointment. I showed up for new appointment Saturday 11/5/2022 at 7:30 and the office manager asked me for another copay. I explained the situation and she did not collect additional money from me. They scheduled me with a provider who was leaving the practice. He saw me for about 4 minutes and explained that since he was leaving the practice, he would not be able to write any new prescriptions. He created a lab order to follow up with another provider and the office never followed up with me after. Ended up going to the health department who took care of all my preventative health needs. Forte family medicine was a huge disservice, waste of time and a copay I could have applied to better health care. read more

Phone service is horrible. They offer to return your call without losing your place in line and they never call of you get disconnected and they don't call back. You can leave a message but your call never gets returned that way either. The good thing about this office Dr joe. Went to doctor Didn't see joe because I was a walk in I was unable to breath, sleeping sitting up to help me breath. The doctor I saw told me to take some Tylenol and wanted to send me home. Didn't listen to my lungs nothing. I begged for something. He gave me some time of antibiotic for 5 days. I returned two days Later worse and seen joe and I had pneumonia and two ear infections. He said the antibiotics that were prescribed weren't even correct to give. read more

I started going to Forte Family Practice because insurance covered it and it was close. I really wish I had done some research first, because this place does not at all care about their patients. My doctor pushed a bunch of medications without doing any sort of exam or blood work. When I discussed with my doctor changing my prescriptions, the office called in the wrong prescriptions to my pharmacy. I called weekly for a month without their office ever returning my calls and without the prescriptions getting corrected with the pharmacy. There's literally nothing this office can do to right this but I also doubt that they'd care enough to try. Trust me when I tell you, you can find a MUCH better doctor's office than this one. read more

After more than 15 years I found another doctor. Non MGM employees do not get the same service. On multiple occasions I was told they never received my lab results, I never ever get a call back. They do not seem to care about there patients at all. I physically had to walk in to the office because their hold time was always extended. The recording says don't leave a duplicate message. They offer a call back options that says you won't lose your place in line, however the recording then says you will get a call back in 24 -48 hours which I never did. It's been since 2/22/24 and no one has got back with me. They refuse to request prior authorization for my medication, referred me to Good RX the medication was over $900 versus my $25 copay. My husband and 2 daughter's will also be leaving. It is like the walmart of doctors QUALITY is not a priority! read more

I will say that the doctors that I have seen here have been good, with the exception of Dr Su. He was cold and useless. I requested someone else, got Malyssa Moore as my new provider and she is WONDERFUL. But, the office itself is run horribly. The front staff is rude, and trying to call for any reason is an absolute joke. No matter what time you call, you are 14th in line, and even if it is a call from another provider they are kept in hold for an hour. The appointment wait time is insane, no matter what time you schedule your appointment. I wish that they would run this place better as it is very conveniently located, and I have had mostly good providers in the past here. But, the office staff needs some training, or it's time for new hiring. I fear that any good provider won't last at this location, because of how the office itself is run. read more

I had an ear infection a couple of weeks ago. Dr. Yudez treated me...perfect job. But two weeks later, it started hurting again. I was afraid the infection was back. Dr. Yudez was away, so I went to Spring Valley location as a walk-in appointment. Thank you to the nice office person in the Henderson office for getting me a same day appointment at the Spring Valley location. No ear infection, but the gross waxy build-up Dr. Yudez told me about was the new culprit. I want to give a shout-out to Dr. Kim, a very engaging and young doctor who somehow talked her assistant in to helping me. Elsa, you get a hero's shout-out. Somewhere things change within the last 4 yrs, service is terrible , front desk people are so rude and don't get me started on phone service. I was left on hold for 42 min. (i wanted to see how long they would leave me hanging ) another time I left my number for them to call back (per their request ) well that was a bust . This review is based solely on the office experience. My appointment was rescheduled by the Forte Practice the day it was scheduled for. I arrived the following day for the rescheduled appointment at the dr office request early to fill out paperwork, which I did and then sat for 45 minutes while patients arrived after me and were taken back to be seen before me. I finally just walked out. The office is in a sketchy parking lot and the office staff refers to the patients as hun. It was a weird experience to say the least. read more

**SECOND UPDATE**: Today I filed a formal complaint with the NV state board of Osteopathic Medicine against Dana Forte for denying me my medical records. I have had 2 other Dr offices and my insurance company fax requests for medical records and Dana Forte refuses to respond. ***For anyone looking to hold Dana Forte accountable*** Her license number is: 649 The website to file formal complaints is: nsbom.org Call the NV State Board of Osteopathic Medicine: 702-732-2147 Medical offices legally have 10 days to respond and provide medical records...it's been almost 5 MONTHS since my first request. Give me my medical records, Dana Forte!! ***UPDATE*** Had to go to the office in person to demand my records and was told they are not in the computer system, and they're not at either location. They are in a storage unit. Was told be DIONNE that she would get back to me in 72 hours with my records. That was on a Friday. It is not Wednesday and I have not received a phone call or my records. I've called the past three days and left messages with no return calls. I cannot believe how inept this office is. *INITIAL REVIEW* After two months of leaving messages with the records department and 45 mins on hold today, I was denied access to my medical records and was told via Dana Forte that I have to make "reestablish six months of appointments" to access my medical records. I have not been a patient for 3 years!! The office is trash, Dana Forte is trash, and I have now filed a complaint with the U.S. Department of Health and Human services for violating my HIPPA rights. read more

Now let's get this straight: Been with Dr. Forte for almost 20 years, and I will say this; The Trop & S. Rainbow office has some practitioners that need to go. Now, if you let Dr. Forte know personally, one on one, I PROMISE and GUARANTEE you, she will take care of it and take whatever action is needed. And I speak from experience Dr. Forte herself is and EXCELLENT Dr. and very good at her practice. But, in my opinion, she is just a little overwhelmed with her workload. And working in different offices is not an easy thing to do. My issue was let's say medium to severe, and the actions she took were more than I ever expected. She expects nothing but the best from all her staff and if you have an issue, no matter how small it may seem, let her head it from YOU. We can't blame her for other people's faults if she is not aware of them. I am not making excuses, just stating the facts. So do that next time, and if she doesn't respond to your satisfaction, let her know and then post something her But at least give her the opportunity to correct the problem. Getting someone to respond to refill requests are a nightmare. After my last appt. to renew my scripts, they never called in my mail order scripts. I've called 3 times to get them to call in and also requested a call back. They STILL HAVE NOT CALLED IN MY REFILLS. I am now almost out. What is it with healthcare in this town? My well being is being brushed off, for what reason? It takes 5 minutes to call in a refill and 5 minutes to call your patient that TRUSTS you with their healthcare. My only reason for not leaving is I can't find anyone better. Guess I'll have to drive to the office, as I've done in the past, to just get them to call in my prescriptions. Horrible, horrible. Dr. Del Gardner is the worst doctor I have ever seen. He has absolutely no respect or regard for peoples time. If you have an appointment at 3 pm, he will take until 6 pm to see you, and then you will spend about 30 to 45 minutes with him. But not with him examining or speaking to you about any issues. He waists all the time on an old tablet talking into it, saying the same thing over and over. He spent 2 minutes from the entire time that I was with him speaking to me, and most of it was him being rude. I literally had to tell him to stop. Went for a medication refill of several chronic medications and was told at the front desk ( no privacy) after they collected my copay and let me sit for 30 minutes that the new PA ( insert any name because they never stay) wasn't comfortable filling my prescriptions this time and go to a psychiatrist?! Called and and gave insurance information an scheduled an appointment 2 weeks out. The day I go to their office for my appointment they told me I needed to get Pryor authorization from insurance and needed to list them as my primary Dr. I have never had to do this with Medicare. Since it was a Saturday they could not verify. Wasted 2 weeks. Used different doctor and no issues with medicare. read more
