So far from what I hear about this place it's a horrible place to be in the staff is awful and lazy. My friends husband died there for being neglected and left without being cared for. She would come in the morning or anytime of the day and he would be wet from urine his bed would be soiled and soaked. Disgusting . The food always cold . My neighbor is there now and he almost died from negligence. Friends went to see him and noticed he was not himself they advised the head nurse he needed antibiotics or something but she said he was fine. That night around 11 o'clock they called his daughter to say he was transferred to the hospital in icu where he almost died because they should have listened to the friends earlier. He was given antibiotics through IV fluids. Now he's back to normal. I don't trust this place don't put your family members here. I would not give any Star of if I could get away with it. read more

My father was admitted to this nursing facility for physical therapy after a leg amputation -- a situation where my family and I fully expected him to recover and come back home. Instead, he passed away here on October 1, 2025, at only 48 years old. Our family is devastated by the neglect and complete lack of compassion we witnessed from this facility. On the day he died, painters entered his room and asked patients to leave so they could paint, which led to my father falling. Shortly after, my mother--not the nursing staff--was the one who found him deceased. When she ran to alert the nurses, they did not act urgently. Instead, someone asked, "Who is the patient?" -- which is completely unacceptable in a full code situation. During the code, it became clear that the staff was unprepared and unequipped to handle a true emergency. They did not have a stethoscope or an Ambu bag kit available to properly perform CPR -- an absolutely shocking and unacceptable failure in basic medical protocol. When we went into his room afterward, we found my father's medications left sitting on his bedside table, including a pain medication that nurses are required to watch their patients take. This makes it clear that nurse Samantha did not even attempt to wake my father to ensure he took his medication. He may have already been coding at that time -- a heartbreaking example of the neglect he suffered. While we were being told that the staff had "done everything they could" for my dad, a group of CNA students stood nearby just staring at us, watching for our reaction. It was deeply inappropriate and incredibly insensitive -- no grieving family should ever be made to feel like they're being observed in their most vulnerable moment. To make matters worse, no condolences or compassion were offered. The nurse on duty, Samantha, displayed a shocking lack of empathy and professionalism after the incident. The charge nurse was equally unprofessional, offering not a single word to my mother or our family during this horrific moment. There is so much more I have not listed, because it pains me deeply that this traumatizing day even happened to my father -- and at a place that was supposed to care for him. Out of all places, we never imagined this level of neglect could come from a nursing facility. According to the California Department of Public Health, this facility received two citations related to my father's case: (1) medication compliance and (2) failure to report an unusual occurrence -- a serious citation that reflects noncompliance with state health and safety regulations. Unfortunately for this facility, my family and I, who work in the medical field, immediately recognized the events that day as major red flags. We trusted this facility with my father's care, and instead we experienced poor communication, substandard care, and a lack of compassion. Do better. I hope this facility knows that my family and I are not going to just sweep this under the rug. No family should ever have to go through what we did. I would NOT recommend sending your loved one to this facility. read more

I've had to encounter this facility two separate times in my life one in 2015 and again now in 2026 and let me tell you it is a completely different experience. The staff is extremely nice. From the marketing team to the nursing team to the environmental service team, even the food smelled appealing for hospital like food. I sent my mother here for rehabilitation she had spent 29 days in the hospital and lost her ability to walk. I had to practically live there to keep her alive. She was on oxygen 24 hrs a day. I came several times and she had not been on her oxygen till I got there. This happened 3 days in a row on one occurance. I would go to the nurses desk to report it and nothing ever was done. She had a UTI for a straight month because it took the facilities so called doctor (quack) to give the order for antibiotics and leaving her wet for hours. She was finally able to walk with a walker and sustained 2 falls 2 weeks before she was schedueled to come home once when she was dumped on to the shower floor due to being in the wrong shower chair the second in physical therapy where she fell face forward due to the therapist not holding onto her by the gait belt she sustained brusing from head to toe which caused her to not being able to walk again but was sent home anyway. I sent her there to get better and came back worse than when she went. She also got blood clots in her lungs while there thank god she had her cellphone to call me so I could get her the help she needed she had told them she wasnt feeling good and nobody did anything. I am thankful for the PT she received at home to get her mobile again but it was a slow process. I do not reccommend anyone sending their loved one here unless you want them to die! read more

My mother was a resident at this facility for close to 3 months. I visited on a daily basis different hours and I can honestly say this is a place that feels safe and also through seeing my mom she was always happy and grateful to all the nurses. Her favorite times were playing Bingo and the freedom to be able to get fresh air outside in the mornings. This place is very organized they gave my mom daily baths same times and they stayed consistent with meals and with the administration of meds. The concierge staff Rosemary was very attentive in insuring my mother and others were safe and happy. My mother expressed that she was very grateful for the nurses that attended to her needs. She gives the physical therapy staff thanks for all they have done for her and for being so kind and attentive. She explains how energetic staff are very positive atmosphere. Thanks to Steven Aguilar the social worker who made her transitional exit smooth and for always keeping himself open for any questions. I would highly recommend this facility. read more

The worst customer service I've encountered at a nursing home. I have tried to call and call and no one ever answers the phone. Once they finally do answer I am greeted with nothing but attitude and they are not helpful with any of the questions I have. As someone actively looking for a place of care for their loved one, it's discouraging to be met with this sort of service. read more

Highly recommend this well run and caring facility!!! Its Director of Admissions, Ms. Nancy, is very efficient, caring, and responsive, and the staff is also kind, professional, and customer-oriented (i.e. if they don't know the answer to a question, they find out and get back to you with the answer). My dad spent a little over 3 weeks in this facility back in December 2021 and had a good stay. He became well enough to return home on Christmas Eve but got hospitalized again a couple weeks later and was discharged on 1/19/22. He was, unfortunately, admitted to another nursing facility where the staff's attitude and patient care left much to be desired. My mom was frustrated and in tears nearly every day that my dad was at the other facility because her poor experience with the staff there was unlike her previous experience with the kind and responsive staff at Hanford Post Acute. With Ms. Nancy's help, my mom was able to get my dad discharged from the other nursing facility and transferred to Hanford Post Acute yesterday evening, 2/2/22. It was the next best thing to bringing him home, and when we stopped by with a couple items for my dad last night, his nurse (Michael) assisted us very kindly. Just want to thank the warm hearted staff, including Ms. Nancy, at Hanford Post Acute. You are all doing such an important job, and what you do (and how well you do it) makes such a significant impact on families! Last night was the first time in two weeks that my mom was finally able to relax, not worry about my dad, and get a good night's sleep. We checked in with my dad today and he told us how nice his nurse has been to him. He sounds much happier and well taken care of since his transfer to Hanford Post Acute. Brought my father here for rehabilitation after a 3 week stay at the hospital. All I needed was for him to be able to walk using his walker, to the bathroom which is only about 10 feet from his bedroom. They assured me that they would be able to return him to that level. He was in the facility for four weeks and I would visit him every day after work and on weekends. The staff was always pleasant but after the first two weeks I began questioning as to why they weren't taking him to the toilet when he needed to or taking him for walks in the hallways to build up his strength. The nurses kept telling me that he was a fall risk, that they're not allowed to stand him up and that only the physical therapist could do so. I was also questioning the cleanliness of the facility after seeing the biggest cockroach I've ever seen in my life scurrying around the room. During my father's stay here he kept telling me that they weren't doing anything for him. After four weeks he was released and they assured me that he was able to walk 50 feet only to discover after getting him home that he wasn't able to walk even 5 feet. They completely lied to us. I truly believe that they aren't concerned about rehabilitation, they're just after whatever they can milk from the patient. read more

If you have a loved one at Hanford Post Acute, you have to be the advocate for your loved one. They don't listen to their patients concerns and act accordingly. For example, my grandmother was in a room with another lady that screamed all night long. My grandmother could not sleep and would get up and go to the family room. She asked to be moved to another room and no one did anything...until I called. Once I called and complained, they ended up moving my grandmother to a different room. Also, people are stuffed in rooms with 3-4 beds. They don't care about that. All they care about is money! Go somewhere else for your loved one. Patients there are stuffed in a room of 3-4 people. I've visited a few people here and I am very impressed. This place is clean (smells good). The staff is extremely helpful. Even the business office staff know the patients. I had the pleasure of spending a lot of my time at Hanford Post Acute in which I am very pleased with all the nursing staff along with the administration. They are beyond welcoming and they treat you as part of their family in which I love. Putting my loved one in a home would be extremely difficult but I wouldn't choose any other place then here to help get my family member back on their feet and well. I would give zero stars if it were possible. First and foremost, "sub-par" is a generous description of the care given to my family member. Three plus patients to a room, clothing lost in the laundry on multiple occasions and never recovered, sheets, clothing, AND bed were soaked in urine every time a family member visited(frequently). Clothes were changed every two hours, NO EXCEPTION, leaving patients soaked in urine for long periods of time. HERE COMES THE BEST PART- the family member responsible for the remainder of the balance after our stay was a victim of FRAUD. The woman responsible for adding a 1 in front of a $714 check STILL WORKS FOR THE FACILITY! This leaves us in suspicion of all upper management. I wouldn't be surprised to find out that they were all a part of this scam. To top it off, healthcare provided was questionable. If you can help it, DO NOT bring your loved one to this facility. read more

Do not put your loved one in this horribly run place. My father had Alzheimer's when he entered this place and had a bad knee but was otherwise healthy. He only lasted two months before he died of hemorrhaging from a peptic ulcer, probably caused by the pills he was taking. He was on these pills for a couple years before entering Post Acute. Another patient we talked to said my Dad told him he had stomach pain days before he died. But the nurses and doctors at Post Acute did not catch this, or did not do anything about his complaints. They also did not clean my Dad's clothes and bed enough, which smelled of urine every time we visited. This place is awful! read more

Our dad 93 yrs young, need rehabilitation after a stay in the hospital. Going into Hanford Post Acute we found all the staff to be very kind and caring. They were always ready to answer our questions and make changes if needed. Therapy was excellent! My aunt was having some medical issues and was hospitalized and needed to get stronger for a surgery that needed to be done and we had no idea of where to send her for her to regain her strength; we knew where we DID NOT want to send her, so a family friend of ours recommended Hanford Post Acute-this was the best descion we made! My aunt was there for 1 week before she was rushed back to the hospital; the medical staff called us and notified us of what was happening. My aunt was hospitalized for 10 days and was released to go back to Hanford Post. They kept her room/bed for her and had organized her belongings for her for when she came back. She did physical and occupational therapy there and grew close to the therapists. She was getting better! My aunt ended up taking a turn for the worse health wise and my family and I had to make a difficult descion and met with the medical staff to help advise us, they were very open and honest and advise us the best they could. Unfortunately my aunt passed; Hanford Post Acute was very respectful of her and to those around. We visited the facility daily for 3/4 weeks, they became like our second family. The admins (Nancy, Shayna [HPA representative] Aric, and Justin [admin in training]) social worker (Ashley) and even the billing department were great and answered all questions anytime we had one. Kaitlyn at the front desk is so polite and friendly and if she couldn't answer a question she knew who could and would direct you to that person. They have activities for the residents, even a covered patio area, a separate one for smokers too, a family room with books, t.v., and games. The nursing staff was great! To Jackie and all the other nurses who tended to my aunt; thank you for all that you do and for helping care for my aunt. We're really going to miss coming in and visiting. I highly recommend Hanford Post Acute for any rehabilitation or long term care for a loved one in your family! read more

I'm an employee at Hanford Post Acute. Last November i found out my cancer had returned. The news was devastating to my family. I have Stage 4 Metastatic Breast cancer to the bones. The employees at Hanford Post Acute have really rallied around myself and my family. Today they held a fundraiser for me to help with the cost of Chemo and other medication. What an amazing day.
