    I would give this facility a minus rating if possible. I have encountered every possible situation…read morehere...from lying staff to refusing to administer required meds, to inadequate therapy.The food is downright comical (as in "what is it?") I have not seen anything fresh in the week I have been here. Germ control is frightening. I pray I don't end up with sepsis. Where is leadership? Post-discharge, when I called to speak to the social worker regarding my dad's home health referrals, the calls were frequently disconnected during the transfer process. Sometimes I had to call 4 times consecutively before the call finally transferred to her (or, more frequently, to her voicemail). When I asked for a direct line for her, I was told there wasn't one. Pruitt's Corporate Hotline, which I called on 12/1/23, did not get a response from the Evans facility either, or at least none that they relayed to me. Finally, after multiple unreturned voicemails and emails to the social worker, I finally alerted the employee who answered the phone at the Evans facility that I'd be filing a complaint with the state licensing board for the delay of care--because they had still not sent over the paperwork for home healthcare referrals, several days after my dad had been discharged. At this point, the social worker, Myesha, finally speaks to me on the phone. She then claims that the orders for home healthcare are "never signed," and implies that it's our family's fault for wanting to use Enhabit for home healthcare services because she'd "never had a problem with an unsigned order before and it's your fault for wanting to use this home healthcare agency." She added, "All I can do is ask the physician to sign it." After I tell her I'll hold for her to check if the physician signed it or not, she then says the physician has signed it and she's sending it over. By the time my dad finally got home healthcare services, about a week post-discharge, the home healthcare nurse commented how it was a shame that my dad had lost so much rehabilitation time as he was now behind in his recovery. She expressed shock that the paperwork hadn't been sent over to the home healthcare service as soon as Pruitt had learned of my dad's expected discharge date. Please don't let your loved ones stay here.

The DON and ADON were both unprofessional with how they communicated to me (a guest), as well as the employee. Not only were the attitudes of the management unprofessional and unpleasant, as was the attire being worn throughout the building. The apparel being worn is indistinguishable unprofessional and unacceptable. If the other workers have to come in with a uniform on, and still looking professional, then that example should be being set by upper man. The lack of care is becoming visible more and more as I visit. This place needs to be investigated by the state. Ever since I started visiting, i've noticed that its nothing but a liability to the city. Patiently waiting on mom to finish her therapy so I can rescue her from this place of hell!

We were pleased with his level of care & the attentiveness of the care staff. I was pleased with their level of communication - they ensured I was in the loop regarding what was going on with my father. He adjusted well and we felt confident his needs were being met. He moved to the Memory Care unit (MC) a few weeks ago and seems to be thriving in this new area. The level of care is different, but still great! I have no doubt my dad is in the best place for his needs. I would definitely recommend the Presbyterian Community. The only less than positive thing I can say is the facility is quite dated and is in need of being updated. But, the facility is safe, clean and appropriate.

    We had a stay at this location. To say the least it was an awful experience. Most of our day and…read morenight we spent scared, as call lights were answered an hour or more after turned on. Staff was rude and not helpful in any way. Staffing at this location was lacking. Was told several times by head nurse there was only 1 CNA on floor to support 30 plus patients. Meals were severed when staff had time to prepare and server and of course never hot and always missing something from the tray. Administrator of this facility was in denial. Had several conversations and nothing changed or acknowledged. This facility should be shut down. Please do not allow your loved ones to stay in this facility.

What makes this so painful is that every other property we contacted had no problem reviewing her application fairly, and several even told us she clearly met their criteria. But at The Oaks, despite having the same qualifications that other communities accepted without issue -- including income, references, rental history, and all requested documents -- they still refused to accept her. We were never given a clear, consistent explanation as to why she was denied, especially when other places had no hesitation approving the very same information. It feels like they apply their criteria differently depending on the person, and that lack of transparency is incredibly frustrating. All we wanted was for my mother to live somewhere she loved and felt safe, and The Oaks was the one place she truly connected with. It's heartbreaking that she wasn't even given a fair chance when other communities had no problem approving her."
