It began when mom fell and broke her hip. Mom is 84 years old Dad is 90 years old. He is legally…read moreblind because of Macular Degeneration. Mom broke her hip and had to have surgery. Dad required care 24/7. He could not even dress himself. We reached out to A Place for Mom. We were contacted by Pacifica at Skylyn in Spartanburg, SC. They came to dad's house on to assess him to see if he would qualify for their services. He did qualify for Memory Care. They also had a place in the facility for mom in Assisted Living once she was released from acute care. (That is another story) The two people who came were so nice and really made their facility sound amazing. Having no experience, we trusted that this was all true. We moved dad into the facility. When we arrived, there were several people to welcome us. We were told that we could call anytime, and someone would let us talk to him. We could not reach anyone, and no calls were ever returned. We left our sweet dad in their care, feeling like we did the right thing. This is what I had given them $2,000.00 for a Community fee and had planned to be paying $4,000.00 a month. We got a call on Thursday morning 11/4 letting us know that dad had fallen during the night. They told us that he had bruising on his hands and were concerned that something might be broken. We gave permission for him to be taken to the hospital for x-rays. EMS took him to Mary Black Emergency. They questioned how the fall happened and were suspicious that no one at Pacifica knew when dad fell or how long he had been on the floor. These are notes from dad's Mychart notes by emergency. Patient Brought from Skylyn today after fall during the night. Patient has dementia and report given by EMS that he keeps complaining of hands and lower back pain. Duration of down time is unknown. Patient has abrasion to right shoulder and bilateral knees. The patient is a 90-year-old male with a history of dementia, seizures, hypertension and blindness that presents today from nursing facility for evaluation of a fall. The history regarding the fall is missing quite a few details. Paramedics state that the nursing staff reports that he fell last night however none of the nursing staff that was there on third shift was available to give report and there are very few details regarding whether the patient was found down, how long he was down, what the patient hit or any other details. Paramedics who stated that the patient's family states that his hands are always bruised however the nursing staff at his skilled nursing facility states that this appears to be worse and wants it evaluated. When the patient is questioned about his fall he is unable to provide many details. He states "while I suppose I did fall last night." When asked where he hurts the patient states "I suppose it might hurt my back but I am is not sure." He states that sometimes he just falls and does not know why. We were told that he had Aspiration Pneumonia. I went and asked to speak to the Administrator at Pacifica. I showed him pictures I had taken in the hospital. He was not aware of the incident. Dad only had his bottom set of dentures. I went to his room to look for them. I looked in the closet. There was a strong odor of urine. I grabbed a trash bag and stuffed the clothes in. I dumped the clothes in the washer. We saw dad's t-shirt that had what looked like dirt all over the front. This was after being washed. This was dried blood! How could someone take this shirt off our dad and not say anything? I sent this picture to the Nursing Director and Administrator at Pacifica. Mom was moving into Pacifica the next day. She knew that dad was in the hospital and had pneumonia, but nothing more. I went to meet mom at the facility. I had a conversation with the Nursing Director about her medicines. She told me that she had talked to the staff that found dad and that they were going to have a meeting to discuss procedures for new residents. I told her that the stains on the t-shirt were dried blood. She said that the staff person thought it was poop. As a trained nurse she certainly should have known what it was. The facility is not clean, smells of urine most times, roaches in stairwells, and elevator(s) are not working . Dad has been in the hospital for six days. The doctor told us that he had a heart attack. He is still having trouble breathing and is not coherent. I was contacted by Hospice that same day! They were gracious and kind. We moved our mom in and everyone was welcoming - employees and residents alike. When the residents started saying "you're going to like it here" without solicitation, we were assured we found a great place. Ashley, the marketing director, was a champion to work with. We feel indebted to her for commitment to our family in supporting the move into Palmettos. She would even text us off-hours to see how things were going for our transition. I witnessed their maintenance director, Sonny - also a peach to be with, training new employees which showed me there is a comprehensive training program for new employees. We had the "pleasure" of participating in a fire drill on day two while we were setting up mom's apartment. You will walk away thinking your loved one is going to be well cared for and supported. After the move in, that level of attention disappears. From that point on, you are largely on your own. If your loved one is fully independent, this facility may work. If they decline, do not assume the care will adjust to meet that need. In my experience, it did not. Care here is inconsistent and unpredictable. There are a few staff members who clearly care, but others appear disengaged. The larger issue is management. There does not seem to be a consistent standard being enforced, which leaves families relying on chance instead of a dependable level of care. Basic expectations were not reliably met. Cleanliness, housekeeping, and routine support were inconsistent. Issues that should have been handled quickly were not, and follow through was lacking. This does not feel like a resident first environment. Too many decisions seem driven by what is easiest for staff rather than what is best for the people living there. Communication follows the same pattern, slow, unclear, and requiring repeated follow up. If you place a loved one here, plan to stay heavily involved. You will likely spend more time checking on their care than you expect, simply to ensure basic needs are being met. At this cost, the level of inconsistency is hard to justify. You may get good care, or you may not. That uncertainty alone should make any family stop and think very carefully before making this decision.
