So my Grammy was here for rehab rehabilitation and they didn't even turn her regularly, they didn't change her frequently enough and she was calling me crying saying she hurt and needed help and so I would drive from Crescent city to Eureka, which is a 2 Hour Dr. finally my mom agreed to take her out of this facility. A nurse made a comment that she hoped my Grammy wasn't gonna be there the next day when she got back like what is that supposed to mean? Talk about unprofessional. To all front line staff at this facility from the cleaning services to all folks who meet all of our desires regardless of the time and your resources. You are the glue who make this facility a success. NOT middle management nor upper level management. It is you, medical, cleaning staff and the daily staff who serve us with dignity and respect regardless of the time and how many clients you are expected to serve. This facility is horrible. The CNA's are horrible, the nurses are horrible the administration is horrible as well as the custodians. This place is disgusting and smells of bodily waste. They starve their patients and lie to family members. They will accept patients just to fill a bed. State comes out but does nothing. This place as well as it's sister company Fortuna Rehab needs to be shut down. If you love your family, please do not send them here. They will leave in a body bag. read more

I have mixed feelings. I am extremely grateful to the many fabulous CNA's who cared for my brother and to the Nurses who got to know him and truly cared and liked him. But many parts were terrible . The food was inedible , even a grilled cheese b/c of grease on the grill. I know it's challenging but if the administration had to eat there then maybe improvements would ensue. The medical coverage was concerning. As there is a lack of MDs, they contract for MD coverage to fly in occasionally and it is uncoordinated and even dangerous. I observed that the doc relied on info from nurses which was not akways accurate and complete and way too fragmented. I worked to get someone to provide appropriate w/c seating, spoke to a revolving therapy department, wrote recommendations (I'm a therapist), pleased , offered to pay. It was deplorable that after 7 months no one addressed this serious issue., even illegal . I was poorly informed of incidents, medical info, even hospitalizations and repeatedly posted my number and they never got it right . The social service person was horrible , gave misinformation, disrespected patients, created havoc and I question her credentials. Very inappropriate and should be replaced. ( in my view and through my experience) I know it's very challenging and in many ways I applaud them all but there were many incidents of concern. And that horrible sounding alert buzzer ( call light) going off all the time right where they place patients , so harmful and unnerving. That's a no brainer and could be remedied easily. And to place a dying man in a room with a sundowner patient is just absurd and no one seems to even really consider how to allow a peaceful death., with dignity and respect . Just go somewhere else. Seriously. Bad behavior from workers who act like adolescent children, they are rude to residents and admin looses the good ones because no one can stand to stay there. The sad part is most of the CNA's gossip in front of residents and talk down to them. read more

I have a loved one in Granada. She has been there for about two months now. We had a lot of guilt putting her there but every time I visit her I realize how good of a decision we made. I know that the previous owners of Granada had some serious issues and lost a huge lawsuit, but I can say that whatever they did to change the place has worked wonderfully. Recently my relative showed signs of CHF, and I am sure that if she was not in Granada, the symptoms would have become life threatening before we would have recognized them and taken action. For the quality of the staff, the food, and the environment I am more than happy with what Granada is providing. If anything changes I will assuredly update my review. read more
