There are strict policies that center around corporate demands for cost efficiency more than pt needs. Medicare is supposed to pay for pts to live out the end of thier life in a way that they deserve. But instead gentiva will take as much as they can and give the pt only what is absolutely necessary. The focus for the staff is now quantity of care much more than quality of care. A tight budget is maintained likely due lucrative yearly bonuses for management. Gentiva is a corporation that cares for thier bottom line more than the quality end of life care pts and families deserve. There is good reason why the branch lost half of their staff in just a few months. It is because of greed. I am writing this to make people aware before choosing this hospice as either a pt or for employment. I believe darkness should be exposed to light and honesty is always the best policy. Generosity and good pt care bring a good reputation which makes a business successful not strict policies and tight budgets, and lucrative bonuses. I would give a review of 1 if rating just the corporate policy makers and branch enforcers. I would review 5+ if only reviewing the wonderful field staff who are trying their best to make it work.

Well I'm only a couple PT visits in and he is very thorough in his explanations of what's going on and super encouraging throughout the whole ordeal. I've learned so much about my dextro (spelling?) scoliosis and my sep shoulder bone etc. He's put his hours, weeks, years in of study and it shows. He treats all his patients with the same enthusiasm and care from what I've witnessed these past couple days. Super grateful for him and the team he guides in getting me in better health. I had no hopes I'd get back to where I was pre-autoimmune disease / pre injuries, I'd just embraced the new normal of having many limitations, but he has enough hope that I'm starting to think it may actually be somewhat possible. Dedicated Time and hard work will tell. I so wanna give 5 stars. Dr Frankie gives 5 star care, but the not being able to reach anyone via the phone was/is frustrating. Oceanside location was the same way. They recommended calling Escondido and thankfully Escondido office almost always picks up and can schedule for any location. Heads up, San Marco Location was hard to find and with no one answering it was a bit stressful first day, went super early since it was my first time at this location and spent all that time calling with no answer driving around Sprouts parking lot and plaza unable to find the office. Escondido location answered first ring and walked me through how to find it. It's upstairs and you access it next to martial arts place where the breeze way/hallway is, there's stairs and an elevator. 5 star care from PT Frankie -1 star for not being able to reach anyone on the phone ever. Tried for months leaving voicemails. Not one phone call back. Secretary stated all locations list this one as the main phone number so it's nonstop calls she said. She said she checks vm and calls back, but over 3 months stretch I left at least 5 trying to make an appointment and never got a call back. Hoping they fix the situation. A lady commented it's too much for the one secretary to do. There should be more ppl answering phones. I hope soon customers will be able to call and reach a person. For now I'll make appts in person, secretary is great at sending reminder texts. Oh also heads up, not too many handicap spots and spots near hallway is only 20 min parking. So plan ahead if you have limited mobility.

I waited almost 4 weeks to get in to be seen to establish care in San Marcos. Have an appointment in 3 days, and got a call that Frankie Alverez the Director cancelled my appt. for a team meeting. Ok...fine, can I get in later in the day. No, I can get an appt. in 23 days or I can drive 40 miles to another location and be seen at 7am tomorrow. Like another reviewer said cannot get an appointment, and staff is terrible. Do not go here....find anywhere else.

Please consider another location to send your love ones…read more Within a 2 week span of my mom's mandatory rehab stay, she went to the ER 3 times! The elevator was broken and phone lines were down for over 3 days. (Not very safe if there was an emergency evacuation.) Additionally, the folks living here all seemed a bit sedated. My mom included. She was always lethargic and just not the same. I do believe the dosage given were inaccurate or she was overly medicated. Lastly, I witnessed a patient fall and one of the other patients yelled for the caretakers to assist. I literally saw 4 caretakers stand there and did nothing. After the 3rd time the patient called out, one of the nurses yelled, "xx that's YOUR patient!" I did seek legal advice but was informed this was common?? (That's for a later review.). But I was advised to go to Yelp and go the State Medical Board... One down.

The nurses cannot be trusted. Many have abuse allegations, and the call lights don't even work properly -- they flicker on, but no one responds. Residents are left waiting for hours for basic care. I've seen amputees left with dirty, unchanged bandages, patients crying out for help with no response, and staff that are either clueless, uncaring, or both. The facility itself is a disaster: TVs don't work, elevators don't work, the air conditioning doesn't work, and the Wi-Fi has been "waiting on a new router" for months. Cleanliness is poor, odors linger, and the environment feels unsafe. They are so desperate to fill beds that they even recruit homeless people, some of whom end up using drugs inside the facility. This is not the kind of place you want anyone vulnerable to be. Transportation is another disgrace. Their so-called bus is unreliable, and the people who stay there seem like they are routinely missing important medical appointments because the facility fails to provide transport. Imagine your loved one missing dialysis? It also became painfully obvious that they wanted to keep my family member only because they wanted his money. Every conversation felt more focused on "We will give you a private room!" FOR WHAT? Nothing works and you'll still receive the same crappy care. Patient well-being is not their priority -- billing is. Even the leadership is a joke. When I met with the administrator and the social services director, both were unprofessional -- they smelled so badly that I couldn't even tell which one it was coming from. If the people in charge can't maintain basic hygiene or professionalism, how can they possibly ensure residents are being cared for? There is no leadership here, just greed and chaos. Staff run amok, residents are neglected, and families are misled. Please, do not subject your loved one to this facility. Report Village Square to the California Department of Public Health and the Long-Term Care Ombudsman. No family should have to go through what we did.

The nurses and caregivers are attentive, patient, and always willing to go the extra mile to ensure each resident's comfort and dignity. The facility itself is clean, well-maintained, and offers a warm, welcoming environment. Communication is another standout strength which provides great peace of mind for both families and care partners. In my role, I've also had the opportunity to collaborate closely with their team during residents' transitions back home with caregiving support. Their coordination during these transitions is outstanding, ensuring that residents and their families feel fully prepared and supported every step of the way. All of the families we've assisted following discharge from Glenbrook Health Center consistently share positive feedback about their experience, which further reflects the quality of care provided. What truly sets Glenbrook apart is their personalized approach to care. It's clear that the team takes the time to understand each resident's unique needs and works diligently to create a respectful and supportive environment. I would confidently recommend Glenbrook Health Center to anyone seeking skilled nursing care for a loved one.

The search for the "right" sober living community can be daunting and difficult but I couldn't be happier with the one I chose because it not only changed my life it saved me! Villa Kali Ma came highly recommended to me by many professionals in the recovery industry, including my own therapist. I was interested in seeking various methods to help me come to terms and peace with my addiction and struggling spirit. Kay White just airs peace, serenity, strength, courage, and embodies everything I wanted to be as a woman. The house is full of positive energy and the morning meditation and yoga sessions taught me how to check in with myself and be true to myself. It was an outlet for the angst and pain. The women I lived with became family for not just 8 months but for life! Everything from the food to the movies we watched at Villa Kali Ma were grounding and healthy. House activities like meetings (Aa or alternatives) hiking, yoga and breath workshops, spiritual event, dinner, movies, concerts kept us busy and active in our recovery. I am so proud of the woman I am today and I owe it to Kay and Villa Kali Ma. If you are lost and struggling with addiction I have no doubt you will find peace and solace within the house.

The therapist are poor at managing a catty and melodramatic environment, if not participating in the drama themselves. The whole experience felt very high school-ish, cliquey and low-vibe. One would expect better leadership from the staff of a self proclaimed "trauma-focused" treatment center. The facility does get a few stars for the fluff and luxury, just do not expect an experience that will lend to true recovery. UPDATE: I find it interesting that this center had received no reviews since Feb 2022, then three reviews happen immediately after my review on 3/21/23. Seems an obvious possibility that the center retaliated against my post by sending notification out to their network, coaching them for 5 star reviews. Yet another example of the inauthenticity of this place.
