He was in a very bad place when he entered Green Oak. It is Christian based and has worked wonders for my son. No more beating himself up for things he has done in the past. Yes there is a lock down period of two months - Yes you cannot have a phone while at the ranch. If you leave the premises you can take your phone for the day. You can work outside the facility after two months. There are random drug tests and when returning from a day out a breathalyzer test. As far as the food - no it is not great but edible. I don't know what people expect when you are paying $750 per month. You get a roof over your head - continual help and sober living. Classes for you to get sober & stay sober. Yes you do have some chores. When my son first arrived he would feed the goats, Tortes, peacock, horses and two pigs and one llama. Not hard work. Animals got fed twice a day and the goats 3x a day. There is also gate duty. He has been working the gate, teaching men how to study for their GED and has found purpose in his life again. It is learning that life is tough and there are restrictions in many things we do - but the rewards are priceless. My son may stay longer than the 9 months - and hopes to find out what he would like to do when his time is up to leave Green Oak. One Day At a Time. Thank you for Giving me My Son back.

They wanted to confront you instead listen to your needs, they wanted to be right and make you wrong. The laundry department lost one or two pieces of my own clothing, even having my name on it. The food is not good, the lady at the Nutrition department that takes special orders is very confused and forgetful, I would get food on my plate that I didn't want quite often. The Physical Therapy department told me I had done 14 sessions, instead of 11, I took pics everyday I was there, they started counting the days from when the insurance accepted and sent the referral but I actually started going there only 3 days later. The social workers were horrible, they didn't do anything they were suppose to, they are there to protect us and I was kicked out to the street and no place to go. The Doctor would come once in a while, not enough and the Psychiatric Doctor only wanted to put me on anti depressants. The transportation department, that calls for your rides to take you to your doctors appointments messed up completely with one of my appointments. I told them about an appointment I had the following day, they said they would call the ride, that I didn't have to worry. The next morning by the time expected I went down and there was nobody to pick me up and take me to the doctor. I missed my appointment, which was a follow up from the surgery, to see if everything was OK and I could start bearing weight on the ankle using the boot. I could only set up another appointment two weeks later, two weeks of my life waiting because their mistake, and not even an apology. THey didn't want me to go out to keep me doing my health treatments I was doing before I arrived there. They told me that if I left the facility to go to a doctor appointment I would get a letter called AMA - against medical advice. Well, because of that I had to cancel a lab test that I had set up to take some blood and test my testosterone levels because I had a visit coming up with my Doctor. All right, I cancelled my visit to the Lab because they offered the service there, the next morning their own lab came to draw my blood. I also had to cancel another visit on the following day, which was a Friday. They told me that if I get an AMA they would put me out on the street, rules and regulations. Next Monday morning, trying to avoid any issues for that week, I gave the nurse department, way ahead of time, all the doctors appointment I had for that week, that were 3 or 4, the names of the Doctors, the addresses anda phone numbers of their offices, the time of the appointment and the reason for the appointment. They should call them and verify. Come Thursday, the day for my surgeon visit to see if I could finally be free to walk without the booth I go to the Nurse station to sign out in the sign out folder, they can't find the folder and there was no nurse at the department. I had to leave or I would be late, I was taking public transportation, so it would take me a long time to get here and be back. They said that when you go out you can stay out only for 4 Hours, but it'd impossible for me to get back in 4 hours using public transportation. Anyway, I left, when I came back ate the end of the day, around 6:30 pm and asked for my pain medication they said they wouldn't give it to me and that I would be discharged on that evening. I spoke with the nurse and asked to speak with the manager of the facility, asked if I should call the police, they said no, they would solve the problem, just to wait. I waited lying down in my bed because I was in pain, after a few hours a couple of police officers come to my room saying that I was being discharged and I should collect my belongings and get out because I was trespassing. I had to ask the police officers to get from the facility my discharge paper. I was escorted out of the facility at 9:30pm with no where to go, literally left on the sidewalk with 5 pieces of luggage to carry. I wasn't able to walk, with an ankle wound still open from the surgery, risk of infection, in pain, with an spinal cord stimulator implanted in my back one they before. I went to the 7 Eleven In the corner and asked if I could sleep there an they were OK with it, sleeping on the concrete, no sleeping bag, no blanket, in pain, with an open wound, no where to go. It was one of the worst nights of my life. The next day I had to go to my clinic to take pain injections and I decided to go to my storage which was 2 1/2 hrs away by bus, left things there and sleep on my tent on the street. Next few days my family sent me $500 to sleep in a cheap Motel because they didn't want me sleeping on the street in pain and with an open wound on my ankle. I'll be suing them for sure.

We chose it for this reason, as we know many of the nursing team and she feels comfortable knowing there will be familiar faces. It is also easily accessible for me to go daily to check in and make sure things are going as they should. I want to be clear that while I am giving a poor rating to this place it is primarily due to the administrative staff and "care team"- I was never contacted by the care manager or social worker to talk about my mom's care, EXCEPT for after I reached out multiple times. And even then one person didn't know what another was doing/telling the family member. If I wasn't so frustrated and angry I would have been embarrassed for them. The nurses, CNAs and therapists are all fantastic- caring, patient and doing the best they can considering their limitations. It seems horribly understaffed and the communication is virtually nonexistent. Again- NOT the people actually caring for your loved one. They are great. But any coordinating you need to do or information gathering or support you need... good luck! In the end, it was literally laughable how many balls were dropped. If you find yourself or a loved one in this facility plan to have an advocate who can be actively involved in coordinating. I'm hoping desperately that my mom won't need another stay in skilled nursing. I'm not naive enough to think the grass would be greener in another facility, but I would be very hesitant in sending her back to Vista View. Having said all of this- MANY thanks to Ruth, Brenda, Anna, Norchell and Sam. Your care for my mom has not gone unnoticed amid the sh*#show going on in the rest of this place. (There were other fantastic nurses but I can't remember their names.)

From the moment I began working with their team, the staff at Glenbrook has demonstrated exceptional professionalism, compassion, and a genuine commitment to providing high-quality care. The nurses and caregivers are attentive, patient, and always willing to go the extra mile to ensure each resident's comfort and dignity. The facility itself is clean, well-maintained, and offers a warm, welcoming environment. Communication is another standout strength which provides great peace of mind for both families and care partners. In my role, I've also had the opportunity to collaborate closely with their team during residents' transitions back home with caregiving support. Their coordination during these transitions is outstanding, ensuring that residents and their families feel fully prepared and supported every step of the way. All of the families we've assisted following discharge from Glenbrook Health Center consistently share positive feedback about their experience, which further reflects the quality of care provided. What truly sets Glenbrook apart is their personalized approach to care. It's clear that the team takes the time to understand each resident's unique needs and works diligently to create a respectful and supportive environment. I would confidently recommend Glenbrook Health Center to anyone seeking skilled nursing care for a loved one.

I wouldn't recommend this facility for a patient that requires significant help or has mild cognitive impairment. Post stroke she was wheel chair bound and required assistance for everything. In summary: Dr. B was excellent. Thoughtful, problem solving mindset and treated patient and family with respect, and some humor when appropriate. Rehab Team: Competent and caring. Speech. OT AND PT good therapists. Management: Poor. All patients receive 2 consecutive days with no therapy. That is 2 days isolated in your hospital room with no activities. This is truly detrimental to the healing process. In my experience this exasperated anxiety and feelings of isolation, reversing progress made during the week, disturbing sleep cycles and recovery. It's very important if you are considering this location for a loved one that you reflect on this. Your loved one will be sitting in a hospital bed for two consecutive days with no interaction except a nurse popping in to check on them briefly. Of course family can visit and we did, however it is not a substitute for professional engagement. This workaround that administrators have finagled to ensure patients hit the certain hours required while limiting weekend staffing is absolutely horrible and very transparent in their motives. Nursing Staff: Very inconsistent and overall not good. Didn't appear in alignment with goals of therapy. I didn't feel safe most of the time with the quality of care my mom was receiving. Not all the time, there were a few standouts that were excellent, but overall very lacking. I share this info. so if you're considering this facility you know what to expect. And with hopes that administrators will make changes for future patients. When searching for a rehab facility I admit I was influenced by the the gold star ratings from outside organizations, but after my loved one's experience, I realize this is more a reflection of savvy administrators working to get accolades from governing bodies vs. true gold star care.

The focus for the staff is now quantity of care much more than quality of care. A tight budget is maintained likely due lucrative yearly bonuses for management. Gentiva is a corporation that cares for thier bottom line more than the quality end of life care pts and families deserve. There is good reason why the branch lost half of their staff in just a few months. It is because of greed. I am writing this to make people aware before choosing this hospice as either a pt or for employment. I believe darkness should be exposed to light and honesty is always the best policy. Generosity and good pt care bring a good reputation which makes a business successful not strict policies and tight budgets, and lucrative bonuses.
