This hospice company used to be owned by Regency Southern Care hospice. Each branch was allowed to retain automity and customize the branch to pt and employee needs within medicare guidelines-Since Gentiva Hospice has taken it over. There are strict policies that center around corporate demands for cost efficiency more than pt needs. Medicare is supposed to pay for pts to live out the end of thier life in a way that they deserve. But instead gentiva will take as much as they can and give the pt only what is absolutely necessary. The focus for the staff is now quantity of care much more than quality of care. A tight budget is maintained likely due lucrative yearly bonuses for management. Gentiva is a corporation that cares for thier bottom line more than the quality end of life care pts and families deserve. There is good reason why the branch lost half of their staff in just a few months. It is because of greed. I am writing this to make people aware before choosing this hospice as either a pt or for employment. I believe darkness should be exposed to light and honesty is always the best policy. Generosity and good pt care bring a good reputation which makes a business successful not strict policies and tight budgets, and lucrative bonuses. I would give a review of 1 if rating just the corporate policy makers and branch enforcers. I would review 5+ if only reviewing the wonderful field staff who are trying their best to make it work. read more

Steer clear! If there was an option to leave no stars I would. My father had negligent care resulting in what I believe contributed to the amputation of his left leg. They never seemed to know what was going on, however the sales pitch on the hold button is incredible! I would find my father diagonal on his bed waiting for help. I would press the nurse call and ask for nail clippers and be handed a greasy pair... My father's rented wheelchair from Apria was lost there. When I called Julie in Social services about it I got nothing but the run around. It went to the Administrator who called Apria 6 weeks later to come pick up my dad's wheelchair.... I am still being billed for the 6 weeks it took her to call and I'd like to know, who's wheelchair is she sending back??? This place was completely unprofessional from the top down! read more

My 78 year old MIL was being discharged from Palomar Hospital about a week ago, and Kaiser gave us 3 choices for a rehab facility. Kindred Village Square happened to be just a couple of miles from our house, so it was the obvious choice by location alone. We were very wary about how MIL would react to coming here as Palomar Hospital is pretty much a hotel. And I had read some of the other reviews here. Kindred is a bit run down on the inside and in need of some decor renovations for sure. But we were delighted when after 24 hours she said she loved it. We were most impressed with the administrative staff and caseworkers. We mentioned on the first day that MIL had some cracked ribs and they ordered her a special low air loss mattress within 10 minutes - it showed up just a few hours later. They were very communicative about what was happening and I felt like they all truly had my MIL's best interests at heart, in particular approving her to stay for a few extra days after her last chemo treatment so that she could be under observation for side effects in a safe place with skilled care. MIL said she really liked the food - she said it really felt like home cooking. She also liked all of the folks who gave her physical therapy each day. All of the nurses and CNAs we encountered seemed very pleasant and helpful. A few things that we were not fans of: first, patients only get full showers two times a week. Yuck. People feel better when they are clean - and if they are able to shower daily, they should get a shower daily. Second, the housekeepers took my MIL's laundry on Saturday and it took until Wednesday for all of her pajamas and undergarments to finally show up. It gave MIL a little stress to be afraid to run out of clothes and we had to bring more for her in case the laundry never showed up again. The negatives were minor points - for a 10 day stay, we were pleased with the outcome for MIL and she was too. read more

If I could give this place a NEGATIVE STAR, then I assure you I would. I do not even know where to start. My father fell ill, and our backs were against the wall and we had no choice but to move him here. I read the only 3 reviews on here and they seemed okay...however I would be doing all of you a disservice if I did not write one and my experience with this place. The right hand and the left do not communicate in this facility. The so called "mobile doctor" is never there, he is out playing golf most of the days. His own staff are always awaiting when he will get in that morning or evening. The nursing staff are few and far in between. There are more patients than the nursing staff can accommodate. I had to threaten lawsuit in order for them to finally start spoon feeding and hydrating my father. I would come there one hour after feeding time and of course 4 days in a row, the food and drinks would just sit there untouched and cold by the time I myself would spoon feed him. How can they practice such negligence and call this place a Transitional Care and Rehabilitation facility, there is no care and no transition into getting better when it seems the staff themselves can care less about the patient. The social worker Julie and her assistant Leslie are even worse. We left 4-5 messages for both while transitioning my father into a board and care facility. No call back, the excuse we keep getting is "they are in meetings" or "out of the office" Really, for one whole week there is no one who can work with family members for proper discharge orders? No one calls back, no one cares, it is one "their" schedule and not the sick dying patients or their loved ones. To make this worse, our insurance did not cover it. The entire fee of 8,000 dollars was out of pocket, and this is the treatment?? This is a dog and pony show...please if you are thinking of this facility be aware. I am being honest and would never write such a review if they were organized and just paid some attention to the weak and helpless. If we could do it all over, I would have never put him here. read more

"F". This place fails on so many levels. Every time you call the nurse, it takes a minimum of 20 minutes before someone responds. My mother wanted her medicine because she was having a lot of pain, 30 minutes later the CNA arrives, turns off the light and says she'll tell the nurse. 20 minutes later, still no nurse or pain meds. So she pushes the button again and the CNA comes back, turns off the light and says she's go check on it (again). Net net, it took over an hour and a half to get the meds. Be prepared to wait a loooooooooong time for any service. The worst is when she needed assistance to go to the toilet. Imagine holding it for 45 minutes and with the pain. My mom was in here for rehab and yet no one EVER took the time to actually DO any rehab, except for 5 minutes one time in FIVE days. They are content having you in the bed flat for as long as possible. The nurses and most of the CNAs lack compassion and care. The food is little and below par. My mom told them many times she doesn't like to eat a few things, they said they'd make a note of it, but day after day, kept bringing the same food. Nothing kindred about this place. Will call the Ombudsman to complain! read more

I would agree with most of the negative reviews. Clearly, Ana G was looking at this as 'oh I am doing a great job and, she could have been but, she is a rare bird at VS. I would give several of the CNAs 5 stars BUT, and it's a BIG butt, administratively, they are upside down. Food: cardboard. Dietitian asked for my wife's preferences but, never saw a change in her meals. Some meds were changed by Naomi. Never got to the pill pushers. Ah, and then there was the aforementioned Julie, in Social Services. Was happy to tell me that Kaiser would provide the wheelchair and walker at discharge. Oh, and don't give the WC away as Kaiser will not give you another one before 5 years. She checked with Kaiser and assured me they would be there in 4 hours. Same thing with her underling. "Oh, do you want to pick a discharge time, for Friday? " Sure, 10 will be fine. I went home to finish up the remodel of our condo to accommodate my wife, thinking all was flowing smoothly: UNTIL, I got an hysterical call from my wife that the Julie plan was all BS. No coverage for a WC and walker. Really? And you could not have verified that before you sent me in another direction? 4:00 PM Thursday Fortunately, I already knew of Eric's Medical supply in Linda Vista. Called them at 4:30 and had the WC and walker in my wife's room at 6:30. Now, for the 10 AM discharged appointment: What a joke. Even Pat the case worker greeted me, at the front door and knew my wife was being discharged. But, there we sat until Noon. No one bothered to tell discharge nurse Janet that I was there (although she had a clear view of me sitting there). Numerous people came thru the room with their goodbyes. Still no one told Janet. FINALLY, I had to scowl at Pat to get her attention. Of course, Janet was on her lunch break. I give one of the physical therapist (Amaratha and Salud) high marks. The rest were way too busy with personal stuff or eating. PT director Johnny was not so personable but, in to discipline. A good thing. The one standout CNA is Natalie. She gets 5 stars. As does Danielle. Wound care was pretty spotty. I would NEVER go back. read more

This place is great as far as nursing facilites go. The grounds are beautiful and well-maintained. I volunteered here as an interpreter as well as at other homes, and this was my favorite mainly due to the staff. They truly practice compassionate care, everyone from the nurses and custodial services to management. I also spoke to family members who appreciated the convenient location. The residents appreciated the delicious food. Keep up the good work, Kindred! read more

One day I walked into this facility and heard music playing in the dining area. There was a jazz band playing...the residents, patients, and staff members were enjoying themselves as the jazz band played their favorite songs. I stepped in for a moment and enjoyed watching them smile, clap their hands and move whatever they could, it was that moment that I realized this facility is like no other I have ever been in. Trust me...I've spent my share time in nursing facilities. The staff members here are of a different breed. Caring for your loved one takes a special kind of individual, and I must say if I had to choose a place for my loved one...it would be the place where the staff smiled, the facility is clean, and a fresh sense of love is spread throughout the building...that place would be Kindred Transitional Care and Rehabilitation at Village Square! read more

I was recently discharged from Kindred, San Marcos, CA after having spent 58 days there recovering from hip surgery. I was very satisfied/impressed with the quality of service from the entire staff. Everyone was professional, courteous, and focused on my needs and requirements. Upon admission, I was assigned a 'Case Manager' who monitored my entire program from interaction with my MDs to dietary needs to issues with my insurance provider. She is the person whom I contacted whenever I had a question or issue. Special conditions related to my injured left hip were factored into an aggressive program of physical/occupational therapy that allowed me to progress quickly to the point of being self-sufficient after being bed-ridden for nearly 45 consecutive days. I moved my 88 yr old aunt from another Skilled Nursing Facility to Village Square Healthcare. The staff is wonderful and attentive here, and they treat my aunt with dignity. Her health has significantly improved since being admitted here, and I now have hope she may be able to transition to an assisted living facility. If she can't go to assisted living, this facility has a long term care wing she can move into. I truly believe she would have died in the other facility had she remained there (from starvation). It's not the Ritz Carlton, but it's clean, has nice enough facilities, and the staff is great (especially Maryann Trinidad, admissions counselor). read more

Our family has been fortunate to have had care at this facility on more then one occassion over the years and from the excellent nurses led by Nancy delaurentis who herself is very involved and approachable to the great social worker julie and the case manager Beulah we have found a commitment to excellence that is consistent. This is a review because I don't want anyone else to suffer like my poor mom did. I wish I could give it zero stars. It breaks my heart every time I pass by that place. She suffered so much there. I truly believe her death could have been prevented. She tried so hard to be nice to them because she thought it would get her better care. I tried so hard to get her out of there, but didn't have the funds. I kept asking them to call her doctor. In the end they ignored her health until her room mate begged them to call 911. She died a few days later. Please keep your loved ones away from here. read more

I am a friend of a back injury patient at this facility. it was chosen for proximity to family and friends. There does not appear to be any management leadership. it is in a state of continual chaos. Prolonged waits are common. It is unusual to see the assigned RN. A CNA or lesser skilled person finally appears, does the minimum needed, and does not return to see that the request has been properly met. This place appears to be severely understaffed and poorly managed. Would not suggest it for anyone. read more

I can only tell you from experience that one part of their care if definitely lacking. My parent was in this facility and they didn't come to my parents room when called several times. They ended up having to clean the bed twice in the same day because my parent could not wait any longer. This was unacceptable for a facility that prides itself on its website like it does( or any rehabilitation center for that matter). read more

This is an update to a previous review. It does not take anything extra to show compassion or to render basic care. If you truly care for your loved one, DO NOT put them in this facility. The staff is more concerned with making money than making sure the patient is taken care of. The staff to patient ratio is nowhere near manageable (one evening it was 3 staff members for the whole floor). Which means it takes a very long time for a nurse or CNA to respond. On a few occasions it took close to an hour. My grandmother ended up back at the hospital on Easter for severe fluid build up in her lungs. It has come to our attention that a nurse had been called because it looked as if she had passed out. When the nurse responded, there was no attempt to determine if she was asleep or in need of help. She got to the door and stated "oh she's just asleep", the nurse was begged to check. When she did she realized she was not sleeping. A few hours after that she was taken to the ER. My grandmother had been complaining to us, and to the nurses, about gagging and having a difficult time breathing. The facility's response was to have a speech therapist evaluate her and conduct a swallow study. Not once did they ever pass these complaints on to the doctor, who has stated had they informed him of her complaints he could have prevented so much fluid build up. My grandmother passed away from the complications. Some of her belongings have been lost by the facility, and they were very indifferent about the situation. They can say they are concerned about their patients and give the best care possible, but honestly actions speak louder than words. read more

My mom had a nasty fall and was sent to this place (from the hospital). This was the only place that had an available place for her and so, there was little input I could have. I have read the negative reviews and I am sorry to learn of those. However, my mom has been treated graciously and with respect. She is very fragile and really in the last part of her life. I think the needs/demands of many of the sick/elderly at these nursing facilities are much less than the rest of us might have. Mom doesn't really care that much about t.v. She likes some conversation and a smile, maybe a visit with someone OTHER than her own family. The people working at Kindred TC & R are pleasant and gentle. The food is more than adequate. We all realize this is not a Gordon Ramsay restaurant, but the food is presented nicely, is a good balanced meal and has the nutrition she needs. I would particularly mention "Beula" (no 'h') who is a case manager and she has been a true angel for our family, making necessary calls, organizing Mom's release back to assisted living, etc. I can only say the best about her. Jamie at the reception area is incredibly pleasant and kind to everyone whether on the phone or in person. Maybe some of these other folks, who are really going thru a hardship with their loved ones, might do well, to just take a deep breath and try to give the workers the benefit of the doubt. I am sure that you and your loved ones have not been perfect in your own jobs, so a little slack and encouragement goes a long way. I am quite pleased at all that has been done--and is BEING done--for my dear mom. I support the folks at Kindred TC&H because they have helped my mom heal to as good as she will get... Don't worry about negative reviews on here. I really think people just want their loved ones to be 100% immediately; are scared, worried and anxious; and feel helpless. Put in some "good" and that's what you'll get back. read more

I am horrified... terrible...Lose her clothes constantly...that was the good news..terrible hygiene..they left denture in a dementia patient..and she swallowed the bottoms..we noticed them missing.., and she complained for a full day of a sore throat and couldnt eat or swallow and was swelling up. She was being ignored until my brother got there ,. By 9 pm we brought her to emergency. They thought she had pnemonia and sent her back...even though she couldnt swallow... for the next 2 days I was able to get minimal thick water down...she gagged and was choking..and I called for a nurse..they didnt know what to do, I called 911 and got her back to emergency... They xrayed her throat and saw wires... I said well that could be her denture. Indeed they found her bottom dentures in her throat. My mom is still recovering.. We are about to sue them, for extreme negligence read more

This place is absolutely horrible! Brought my parent here after being discharged from the hospital for rehabiliation after a mild stroke. We arrived at 6pm and were informed we needed to bring my parent up the third floor and down the corridor to the nurses station for check-in. Okay, no one to greet us but we did as instructed. Once we get to the nurses station we were informed by the nurse supervisor that we needed to bring my parent to the room ourselves and to get her into bed. That is not our job but the job of the employees who work there. As we went into the room an aide came in with a dinner tray and proceeded to help get my parent settled, even though it was not our responsiblilty to do that! The bed needed to be reset (the aide did not know how it worked), the food was cold, and my parent's blood sugar level needed to be checked before eating. I had to go back out to the nurses station to the same supervisor and inform them that the blood sugar needed to be tested. After five minutes of the suprvisor looking for someone else to do perform the blood test, she finally went into the room herself and did the blood test to my diabetic parent. One bite of the food and it was deemed cold and uneatable. We asked the other person sharing the room what the facility was like and the response was disheartening: Press the call button at least twenty to thirty minutes before you need something because it takes them that long to get to you, the food is disgusting, and the staff stand outside patients rooms, who are trying to sleep, laughing and talking. A staff member tried to use my parent's wheelchair for another patient even though their name is displayed all over it. And someone broke the wheelchair by improperly using it and it still needs to be fixed!! A boot was needed to keep a broken foot by parent had stable but that was not seen until the last day of my parent's stay, even though the doctor had ordered it at least five days earlier. My parent became extremely constipated with all the medication that wasprescribed for by the doctor in attendence. She was bound up for seven days!! No help, nothing, until my parent was in extreme pain and could not go to physical therapy. They finally gave laxatives but the damage was done. With all of this "delicate" care my parent received they were transported back to the hospital with increased kidney failure. My other parent had spoke with the director of that horrible place that morning and told her what all had happened. The problem was that the Kaiser rep who was working with my other parent had to call the director and tell her to get to the facility and speak with my other parent about concerns and issues. Why wasn't the director at the facility where they earn a paycheck? But the director did meet us at the hospital to make sure my parent was okay. Seems like the director is afraid of a lawsuit for neglect! Do not bring your loved ones to this horrible place. Even though this facility is Kaiser approved it is a poorly run facility. The only bright spots in this abismal place was a couple of nurses and aides who were knowledgeable and the care and concern for their patients showed. Shame on this facility for allowing my parent's health to become worse and have to be sent back to the hospital for lack of care. read more

My father was recently discharged from Kindred Village Square. We were extremely happy with his stay and believe he was treated very well by everyone, including the nurses, therapists, admissions coordinator, and Medicare case manager. The entire staff was competent, friendly, and always had a smile for him and for our family. We felt he was in very safe hands, which made his challenging medical situation as comfortable as possible. We would recommend Kindred without reservation. read more
