When he complained about not being able to sleep, and how much pain he was in, the nursed would ask him the same questions that another nurse had asked him the day before. Then we got a lot of "I'll leave a note for your doctor". It seems that the note never got left because we were told that several times about my Dads pain. He would occasionally go up to 11 hours without it. It was prescribed for every 6. So I asked for a list of his medications and the times that he is given them. I have medical power of attorney, so it seems easy enough. Nope! Not here. First I asked a woman at the front desk area, who refused to give me her name, and she told me I had to find his nurse.... Well, he was nowhere around my Dad's room, so I asked his CNA, Moreno. She told me that I had to ask his nurse. Like Im supposed to wander around the care facitly looking for his nurse. So I hit his call button. 25 minutes later, I timed it, Moren, his CNA came in. "I don't need you, I need his nurse", I said. He finally came in and asked the same questions and said he'd leave the doctor a note. Unbelievable. So I tried to contact 2 people in charge of his care, Debbie and Joyce. No one answered the number I was given by his nurse Jordan, and Debbie hasn't replied to my email. It's been over a week and still no response. I am beyond pissed off about this. I would tell anyone looking for a care facility, full-time or for recovery, to pass on this one. They obviously don't care much about their patients. And, sorry this is so long. If anyone from the facility would have called me back, I would have told them all of this in such less public way. Please! Pick the next one on the list...

I was in this faciity from mid December, 2024 until January 8, 2025. It was horrible, the food was…read moreawful, and the kitchen ignored requests. I stated several times that I am allergic to cucumbers, and yet they were still in every salad (I ate a lot of salad because so much of the food was inedible.) They were short staffed, so I frequently waited up to 20 minutes for a CNA, not good when the meds give you diarrhea. Most of the staff gave it their best, but not all. One night an older woman (I presume a CNA) came in, bent over my bed and shook me angrily. I was stunned, but later figured out that she thought I was the woman crying, "Help me, someone please help me," on and off all night. Still no excuse for what she did, and she should be fired for patient mistreatment. I would NEVER allow a relative of mine to be here; they don't pay enough attention to what patients need, and at least one of the actually abused me. The liability upon the company and their caregivers is a good recipe for a lawsuit or worse waiting to happen. I see reviews from years ago and the same is still happening. Communication between company and patients is ridiculous and not accommodating for the patient, nor for the caregiver. All they care about is a warm body to care for patients with or without experience. That is really bad.

I was in Porthaven short term to recover from an injury. The physical therapists and other…read moreprofessionals in the gym are fabulous. Very competent, and I felt like I was "on a team with them" for my on care, which is what the medical profession expects between patients and providers. These therapists' competence and care are the only reason I gave this facility three stars, instead of only one star. Porthaven is described as a skilled nursing facility. But it not that, as the nursing staff is short-staffed. Skilled nursing facilities are described as being for short term care, where patients are given therapy and expected to go home soon. But it seemed to me like there are people who actually live there.....the rooms of these people had some personal furniture, and personal pictures hung non their walls. One woman told me she had been there for four years. And as a whole I did not feel like there was teamwork between myself and many others there -- as a whole I felt like a lost my identity as a competent adult, and decisions were made FOR me and ABOUT me, and not in a timely manner, without my knowledge or consent. Fortunately, I have a health advocate, and she worked to make sure I was getting what I needed. CNA staff to patient staffing ratios, I was told by one of the CNAs at Porthaven, are determined Oregon State Board of Nursing. She agreed with me that Porthaven is understaffed. I subsequently researched this ratio and discovered that the state regulations for the bare minimum staffing for safety, and that facilities are able to hire more staff. The only reason I agreed to stay at Porthaven initially is because they allowed me to have my own room, without a roommate. Initially, I had a roommate, who left her TV on day and night. This is totally unacceptable for what should be healing environment, and I told Porthaven this, that I refused to admit myself, but then they assigned me to a private room at my request.

My mother was admitted and on her first night, she called for assistance to use the restroom--no one came. She fell, fracturing her sternum and three ribs, and the facility waited eight hours before calling an ambulance. She is now crammed into an 8x6 ft partitioned space with two other patients, separated only by curtains. In the weeks she's been there, I only received a single call from a doctor about her care, this was only after I threatened to report them. I had left multiple messages and even emailed the administrator and head of nursing--no one responded. This facility is dangerously understaffed, unresponsive, and negligent. I wouldn't wish this level of "care" on anyone. Avoid at all costs.

For reasons beyond anyone's control, the relief caretaker wasn't able to sustain her commitment. Circle of Care was determined and aggressive in finding a replacement to avoid any lapse in care. Most importantly, Cynthia, the CEO at Circle of Care, demonstrated kindness, empathy, and dedication. She was always accessible and responsive to questions or concerns. Shannon, as the COO, clearly explained administrative details, which included the all-important question of fees, etc. My mother passed away after five months of in-home care, and while I don't anticipate needing similar service anytime soon, I would definitely recommend Circle of Care to others.

The response to my negative review was attentive so I must apologize by leaving a more detailed…read morereview granting some extra stars: The case management is on point, seeing to my urgent issue, Room #9 is satisfied. I hope to not require many more amazon packages during my stay here--I needed some essentials... that is to say, they do provide essentials but I needed some things to improve the quality of my visit. Being here a few days, I have gotten to know a few staff members, and a few nurses and CNA have stood out among the rest making me feel more welcome. It has been an unsettling process but acclimation is going well-- I will add the therapy staff is excellent which is the only reason I am here. The food manager is caring checking in, seeing to my dietary needs (I prefer intermittent fasting, skipping lunch) and they maintain a consistent schedule. I shall even venture to say they stay away the finished food tray at a reasonable time. I will leave with some minor criticism however: at admission there was a personalized care form that has not yet been reviewed by staff in regards to personal preferences. It is a mere oversite that led to a weekend with an incompatible roommate who raised some pressing health concerns sharing a room with a very sick man who I hope feels better soon!
