I wish I had looked into this practice a bit more before choosing it as my primary care provider. I recently submitted FMLA paperwork with a clearly communicated two-week deadline, but unfortunately it was not completed or sent to my employer within that timeframe. I made several attempts to follow up by phone over the two weeks, leaving voicemails but not receiving a response. When I was finally able to reach someone, I was told I would receive a callback, but I did not hear back. After additional calls and difficulty getting through, I ended up going to the office in person, where I waited and was informed that my paperwork could not be located. I understand that offices can get busy, but the overall experience felt a bit disorganized and communication was challenging. My initial visit also left me hoping for a more coordinated and attentive experience. I'm sharing this feedback in hopes that it helps improve communication and processes for future patients, especially when it comes to time-sensitive matters. read more

My first visit after choosing the doc as my pcp..the absolute worse care or lack of I should say. I went in the staff was very nice. Short wait. Met the doctor explained everything thay is going on and the doctor requested imaging blood so conform which is standard never commented or gave me an idea or what to do said he wouod send 2 prescriptions to my pharmacy..it's been 6 days now. Sadly this office has made numerous mistakes with basic things such as prescription refills..

I was told my co pay would be $15 but showed up and they tried to charge me $30. I spoke to someone who I didn't get the name of on the phone at 10:57 AM, Delilah the front desk worker said that she was the only one taking calls all day, but she wasn't aware of a conversation of co-pay of $15. Delilah was helpful, Sucks that she works for sketchy company. She canceled my appointment so I wouldn't be reflecting to have a charge, but she wasn't able to give me a confirmation of that so we'll see if they try to continue to charge me. There was another girl that was sitting behind the desk. Her name was Liz, and she was extremely rude, trying to escalate the situation by essentially implying that I may have been lying about speaking to someone. Why would I travel 25 minutes, with a one and four-year-old to not be seen? They're claiming that they don't know who I spoke with which I really don't believe I feel like I may have spoken to Liz and she realized she fucked up and didn't want to get caught. Anyways, word of advice. Don't trust this company. They try to bait and switch you. Have you travel 25 minutes to over charge you and then not even be seen. read more

Was skeptical coming here due to the several bad reviews but please listen to them. I uploaded all of my insurance documents previous to my appointment and yet when I got there they told me I had to call my insurance and change my primary care doctor? I don't get why you didn't know any of this prior to my appointment. Calling my insurance would take up to 30 mins trying to figure out who I'm supposed to contact and be transferred 30x. If you require we upload all of the documents why not look at them before hand so my appointment isn't gonna be longer than expected? I have never went to a doctor office and been told to call my insurance to change my primary care doctor. Please avoid the incapable staff. read more

WARNING This place is craaaazy unprofessional starting with the front desk all the way down to the NP. Not a doctor but NP Joel. He's not consistent, doesn't follow through, lacks eye contact when speaking with you, and is forgetful. I asked him to test what my bloody type was and he forgot so I still don't know my blood type but he took my blood... what? You also get no privacy... when picking up medication they announce what you're on at the front desk in a waiting room full of people you don't know. Isn't that HIPPA? I had to ask for a room or private conversation so the strangers in the waiting room didn't know my business. This place honestly should be reported to the BBB. Jacquelyn is the only one in the office with any sense, but she's not always there. Lastly, they NEVER ANSWER the phone... it's like they block you or something because I had no issue scheduling an inital appointment, but for following up purposes or rescheduling, it's awful. DO NOT GO HERE IF YOU CAN PREVENT IT. NP Joel is unresponsive and insecure in his work... that makes me as a patient uncomfortable. Horrible clinic they never answer the phone you leave a message and they never return your call the doctor orders a referral and they never fax it to the doctor you are being referred to. I have been calling for a month to see if they have fax the referral it's very hard to get a hold of some one and when They had answered they say we already fax it but the place I'm being referred to they say they have not received it. and now this hale clinic don't answer the phone. I had never experienced such a nightmare like this before. I do not recommend them to any one. read more

Hale Family Practice I booked an appointment with Mark Joel Culanag on Aug 12, 2025. The appointment was scheduled for Aug 13, 2025 @ 11:15 AM. I confirm all details, send all my medical specifics, and insurance information within an hour of booking the appointment. I go into the appointment, only to find that no link exists for the video conference. I received an email from the provider stating that the appointment is in person at their Tropicana office. I contacted the office to ask about the appointment, no answer. Twenty minutes later, I received an email alerting me to the conference call and that I should join now. I waited on the conference call for twenty minutes, no show, no follow-up. read more

At first, I really liked Dr. Culanog she seemed super nice during the first visit and said we'd run labs to figure out what was going on with my health. But after the basic tests came back normal, she kind of just brushed everything off and didn't want to do any further testing. I even brought old medical records that showed this has been an ongoing issue, and she didn't even look at them. What bothered me most was when she told me that using ChatGPT or Google gives people anxiety and that I should stay away from it. I wasn't Googling random stuff, I was literally just looking at my own medical records to understand what's been going on with my body. I felt like she didn't take me seriously at all. If you're totally healthy and just need a basic check-up, she might be fine. But if you're dealing with anything more complicated or need someone who will actually listen and try to figure things out, I wouldn't recommend her. She seems more reactive than proactive, which isn't what I need in a doctor. read more

As a healthcare professional, I was greatly disappointed in the care (or rather lack of care) my wife and I experienced at this clinic. One of the main reasons I recommended this clinic was because one of their mid-levels, Lorimel C., was a former nursing colleague of mine and only heard accolades come from our other colleagues as well as patients; however, I am incredibly disheartened to see that upgrading her education and credentials significantly downgraded her basic assessment skills and overall humanity. She was not thorough during an initial appointment as a new patients, didn't ask about some basic information about either of our past medical history and just rushed through the appointment. The staff is no better as they are not familiar with office policy, do not communicate appropriately nor assume any accountability for faults and are so incredibly rude over the phone. I shouldn't have to repeatedly follow up with this office regarding completion of my prescriptions or have inconsistent communication from this clinic staff. There are no words to describe the immense disappointment of who this clinic employs, the lack of communication and the sky-high stress this experience has given me and my wife. People like this discredit the trust and validity of primary healthcare and insult those of us that still care about people read more

I haven't been a patient long. What I first liked about them was that Joel Culinag was very thorough. And I felt the front office would answer the phone or call back in a timely fashion. However it seemed to change a lot. I've left numerous voice messages for them to call me back. I've tried to book appointments on their website. It's not very user friendly. read more

My visit went great and smoothly. After sharing my history with severe allergies with doctor Joel Culanag I was immediately prescribed everything I needed to reduce my systems, given a 3 month allergy shot same day and they immediately referred to an allergy specialist. I was in and out of the office within an hour, will definitely be returning for regular check ups! read more

If I can give this place 0 star I would. Completely disgusting experience with 0 staff having their sh*t together. Nurse drew my blood (5 tubes) and forgot to include the 1 thing -HCG level which is to test If I was pregnant. They asked me to take a urine test literally immediately after I peed- are you joking? Was seen by Mark Joel Culanag for a little over a year. Care was fine for several months, but he was never able to give me referrals the two times I needed them, and then just told me to find help via the back of my insurance card. There was also what I felt was an utter lack of research when his knowledge failed on several occasions regarding my medication. I initially thought I had finally found a decent PCP, but unfortunately, that turned out to be incorrect. Overall, I felt there was poor communication, respect, and trust. These things seem pretty integral in a doctor/patient relationship. Just felt like I wasn't treated as an actual person with real needs and problems. Please do better for patients in the future. read more

My 2 adult children and I have all gone here and never experienced kindness. However, good at filling out forms. I don't think any doctors work here. I went to get my thyroid checked. The appointment was fine. The doc is a little cold and didn't say much but the nurse was AWESOME. However, they get a low rating for a few reasons. The front desk person was not friendly. At all. I came in the day after my initial appointment for a blood draw. They wanted to charge an additional co-pay for that. I had my blood taken, and received an appointment for a week later to go over the results, via telehealth. (They wanted another copay for that). 5 minutes before my telehealth appointment, I got a call saying that I had to come back in to redraw my blood because the first sample went bad due to it's getting too hot. Are they not aware of Vegas? I was furious. I've waited a week, and then 5 minutes before the conversation to go over the results you tell me that my sample is bad and I have to do it all over again? No. If you can't figure out how much to charge and you can't keep samples from getting too hot in the summer in Las Vegas then I'm not going to spend my time and money. As I said, the nurse is awesome, the doctor less so, and the front desk is bad. That's a no for me. read more
