My father was a patient at Harbor Villa for about 60 days, and what our family experienced there was heartbreaking. The facility frequently smelled unpleasant. There was often a resident yelling loudly throughout the day, creating a chaotic and distressing environment. We saw rats in the parking lot and even cockroaches inside my father's room. It was incredibly upsetting to see him living in those conditions. Visitor parking is also an absolute nightmare most days. For families already under immense stress, struggling just to find parking makes visiting even harder. Most concerning, when my father pressed the emergency call button at his bedside, assistance was often extremely delayed. Knowing that your loved one is pressing for help and waiting -- sometimes far too long -- is a feeling I cannot adequately describe. It caused constant anxiety every time we left the facility. Although some staff members were personally kind, there was a serious lack of coordination and awareness about my father's medical condition. When his health declined, staff could not clearly tell me his recent vitals, explain his blood pressure trends, or provide accurate information about his care. I had to speak directly with the paramedics myself and provide his medical history and symptoms because the staff could not. While on feeding tubes, my father's sodium levels dropped significantly because he was not receiving adequate water flushes. We had to rush him to Hoag Irvine, where doctors determined he also had pneumonia and extremely high blood pressure. It was terrifying. Watching your parent suffer from something that should have been closely monitored is something no family should ever experience. Instead of supporting his recovery, the facility then declined to accept him back after we sought emergency treatment -- simply because we advocated for his health. We trusted this facility during one of the most vulnerable times in my father's life, and that trust was deeply broken. Please do your research. Ask detailed questions. Advocate fiercely. And if you have other options, I strongly encourage you to take your loved one elsewhere. No family should have to go through what we did. read more

When I was finding placement for my mother I was a little skeptical about sending her here to Harbor Villa. Honestly, I couldn't have been happier with my experience. I love the fact that they have an open door policy in which I was able to take my concerns to Rob the Admin & Jean the DON. The CNAS & nursing staff were so lovely. Broken hip not fun at 81. Following surgery--yes, I'm Medi-Medi with an additional HMO thanks to my Primary, Martin Zapata, D.O.--I was assigned to Harbor Villa Care for a month. Clean rooms with all the amenities (including wide screen tv), good "instutional" food, wonderful physical therapists (all compassionate and most with a great sense of humor, 5-star medical and support staff and a large and lush patio-gasrden. Tonight, my heart feels heavy as I process the devastating findings from the Department of Health and Human Services about the care my dear mom received at Harbor Villa Care Center. Reading the detailed complaint form feels like reliving a nightmare, knowing that my mom--and others--suffered neglect and abuse in a place meant to protect them. They failed to provide the necessary care and support for their patients. It's weird that I received the details tonight, exactly four months since my mom passed away. The audit uncovered heartbreaking truths: 2 out of 3 patients with conditions like my mom's were neglected and abused. One woman was even tied to her bed. My mom's chart was reviewed, staff were interviewed, and every single detail of my complaint was validated. The nursing staff admitted to it all. Harbor Villa failed my mom in ways that cut so deep: they neglected her worsening condition, made unauthorized medical decisions, ignored her advance directives, failed to communicate with us, delayed calling 911 despite our pleas, and admitted they were understaffed and unprepared. These failures sent my mom to the ICU, and on December 30, 2024, I lost her forever. This isn't just my story--it's a warning for every family trusting a facility to care for their loved ones. I'm fighting for accountability and change, not just for my mom, but for every vulnerable person still in their care. Please, hold your loved ones close and ask questions about their care. No one should endure what my mom did. Sadly Harbor Villa Care Center only got a slap on the wrist. Reading their "action plan" they wrote--in simple terms-- we are not legally liable and will do better next time. Review in 3 months. This is not a safe place for your loved ones. While the facility struggles with understaffing and a lack of communication, there are a few truly incredible staff who deserve to be recognized. What could improve? Everything . Hospitality, customer service, nursing, physical therapists, ALL OF THE ABOVE. The only good thing at this place are the CNAs and it's sad that because they're just starting their nursing careers they're treated poorly. Also need to give credit to Rob for always hearing us out and taking care of our concerns immediately. However it shouldn't take us going to higher ups for them to change things nor did those changes last long. Some days would be good but most days right back to where we started. From day one of my grandma being here we felt uneasy and came to visit her everyday to ensure her care was taken seriously. And everyday seemed to be a fight with the nurses and/or other staff due to poor care. We even requested a meeting with the care team and at one point the social worker walked out. Very professional. My grandmother was being over medicated (confirmed by her PCP upon discharge), nurses were lying about her medications and/or when asked about meds they simply said we don't know, we just pass them out. Scary we're just handing out nursing licenses. Physical therapy was okay but she made no improvements despite them telling her she was doing so good. They gave her false hope. And upon discharge they told us they felt a little uneasy with her being discharged. So why give the okay ?! Imelda (night nurse and supposedly in charge) was rude, one night I had a question and she waved her hand and said go ask over there station B. My grandma's belongings were stolen, other patients would go into her room making her uncomfortable. We were told they do rounds hourly but there was no way if so much more than what I wrote was happening. I wish I could write all of it but point is, I do not recommend. read more

Very open to working with you and your input on your loved one's care. Like all these places they are understaffed from the ideal levels so once they see you're for real and committed to your loved one they work with you and you become part of the team. Only issue was we had to leave at 8 PM daily; but eventually they stretched their rules and let us stay to 10PM. The place's physical plant could use a refresher but that's insignificant because of it's very good, competent, caring staff and their overall program commitment. read more

I have to give Bernardo the evening nurse a 5 Star review because he demonstrated genuine care and empathy towards my sister. He listened to her concerns and addressed them effectively. He provided us with a clear explanation of the Harbor Villa Care Center policies. His communication was clear and concise. My sister missed her shower appointment and he effectively advocated for my sister needs. I highly recommend Harbor Villa Care Center. The staff is professional and I am impressed with the facility. My uncle was in there for TWO days before he passed and within the three days for us to go and collect his belongings, someone on staff stole his blanket and radio. Why?!? We were there the day before he passed and, with our own eyes, saw his blanket and radio, yet they have "no recollection of those items being there". read more

They should call themselves "Harbor Villa NO Care Center" The staff doesn't seem to know anything about the patients. They lost my mom's medication. I only found out because I inquire whether my mom have taken her meds. The nurse in charge had no clue and could not even find the meds and instruction which we had handed in to them at admission time. I even have the signed paper work for it! That was day 2. So, that means they did not give her any meds on day 1! They had inquiry about what kind of food the patient need and whether feeding is needed. Nobody at the facility were aware of what my mom's needs. We ended up having to feed her ourselves. There was no staff available. No matter how long and how many times you ring the staff to call them, nobody show up. Every time we visit, it didn't look like they have changed her diaper and had to ask them to do it. read more

I would give negative stars I'd I could. This place is what caused my mom to go downhill. She developed UTIs repeatedly and they went undetected and untreated until it was too late. They finally transferred her to a hospital. She was almost dead. I'd never seen her look so awful. The UTIs left her at a constant state of altered consciousness. This place is neglectful, unfriendly, dirty, smelly, incompetent, understaffed, etc. They don't have room phones so you have to call the front desk. They rarely answered. They don't follow patient special diets. They don't "rehabilitate" the patient at all. My mom went in there to improve her mobility and she got worse. I brought her home to live with me. She was never the same after leaving that place and passed away 11 months later. I truly blame this place for her demise. It needs to be shut down and investigated. read more

The place can be cleaned better. The staff is not well trained. I think the nurses are over worked to the extend they lie to the family of the patient. I called at night bec my husband was in pain and not able to sleep. Nurse said ok she will take care of it. When I came in the morning nothing was done for my hubby. All they can say was she refused care yet.... They knew that she was unable to make that decision for herself. Terrible care. My father developed a UTI, sepsis and had a fever for 2 days. Was not discovered until he went to the emergency room. The nurses act like they are too good to provide care. My father would have died if he stayed in this facility. Nurses wouldn't even acknowledge family members or my dad. They get annoyed if you ask any questions and will avoid you. He will never return and I hope this place get shuts down. I will be reporting for negligence and malpractice. read more

Do not let your loved ones go here. My sister was taken here yesterday 10/29/21 from Global Medical. She was taken to a room where her bed had no sheets. There was no admission orientation, just nothing. She is a fall risk and has bracelets stating such. She had to walk to push a button to ask for help. Between 30-45 mins later someone showed up and asked what she wanted. She said sheets. 30 mins later one was brought. She witnessed patients leaving based upon bed sores and dirty sheets. This morning witnessed one patient being given their meds twice. She called and our family picked her up and took her out of there. They never gave her her medication to cake and said they did t have any upon leaving. On the phone this afternoon they said they have it but will not give her the prescription because she left. The prescription is hers, not theirs. Her insurance will jot cover more because they already paid for it. They are essentially stealing her medication from her. Please keep you loved family members away from this horrific place. read more

I have an aunt that has been at Harbor Villa Care Center for about 5 years and each time I visit my aunt she appears clean from head to toe. I just visited her today and her skin looks good. The staff knows their patients very well as well as their needs. The front desk staff appears friendly and kind. The Case manager/social worker were also kind and I was updated with my aunt's condition. The place appears clean and has enough lighting. This place appears a lot better than other Care Centers that I have been to. read more

I was in this facility for a month and a half in 2024 00 God, oh shit, oh, help me hurt. I can say the sum of the staff is good. Some of them aren't. The first day I was at the facility I was in a room with a 90-year-old woman who didn't like Black people after she made a few derogatory statements and I had a few statements of my own to make to h... I will say that they did take me out of the room within an hour and place me in another one with a lady. I also fell in the facility after a trip to the bathroom with help from my CNA. It took them about 15 minutes to find some help to pick me up off of the floor then I laid my bed for almost 2 hours before they even called an ambulance to take me to the hospital to get x-rays. I would not recommend this facility to anyone. read more

This place is terrible. Many times you call and no one answers the phone there. There is also no phone to speak to your family member. You have to call Monday through Friday only. Maybe a staff will call on cell phone but only if you get the lucky. Some staff are helpful and caring but several are no and are rude. I don't recommend this place facility at all for your love one. Be cautious and go somewhere else. read more

This place is staffed with completely incompetent people. My mother had a stroke a couple of months ago and, unfortunately, this was the only facility with a bed available. It took them 3 days to get her medication and they didn't even know that she was diabetic upon admission so didn't even check her blood sugar the first day. Me and my siblings visited multiple times a week to check on her and assist with her meals when we could and we always noticed she was VERY THIRSTY. I asked the staff to please help her and encourage her to drink water (she has severe memory loss from her stroke) but I know they didn't. As a result of her stay there, she developed KIDNEY FAILURE, MRSA, and a bed sore...she has never had a bed sore in her 74 years. I personally had to request that a swallow evaluation be completed because I noticed she was having a hard time eating. This wasn't done until ALMOST TWO WEEKS after she was admitted. Even after they changed her diet to "mechanical soft", they kept giving her sandwiches! This is a complete choke risk...and thankfully she didn't choke while in their care! She was rushed to the ER during her doctor's visit when they discovered that her kidney's were functioning at only 15%! She was then admitted to the hospital, at which time, they did not hold her bed. I'm actually glad that she can't go back to that place but they showed no sympathy or compassion whatsoever during the entire experience there. When my brother went to go retrieve her belongings, they were very rude, even huffing, when he asked a simple question about filling out the form. They lost my mother's personal, hand made quilt and blamed US for not making sure her inventory list wasn't updated. PLEASE DO NOT HAVE YOUR LOVED ONES STAY HERE!! It's so sad that so many medically vulnerable people have no options and are subjected to such poor care. I am a medical professional and I'm disgusted that they are still in business. read more

My grandma who I love dearly was here after surgery got treated with so much respect! The nurses were beyond amazing, they answered to all of our needs and concerns. We were worried about leaving her alone because this was her first time in a home like this but the staff made us feel comfortable and treated my grandma like family. They provided activities that my grandma enjoyed and the social services staff scheduled us a meeting with the managers, they answered all our questions, and updated us on her care. I wanted to share our positive experience with everyone at Harbor Villa Care. My mother was placed there after a hospital stay and had never been in a care facility before. She and our family were really worried. The care our mother received was wonderful. She was evaluated and put on physical and occupational therapy the first full day she was there. Alicia, Griselda, Christina were her CNAs and all were very kind to her. Her OTs etc. Marlisa and Tatiana helped her gain strength and strategies to get stronger to return home. Her charge nurses, Mary Grace and Suzette as well as Martin all had everything she needed med and care wise from her hospital stay as well. Her care team of Julie, Yohanna and Hector were extremely involved with her plan of care and making sure she was safe, while being trained to gain strength. The entire care team was available whenever we had a question or request. Harbor Villa staff was very careful regarding COVID which of course made her placement even harder for all. It was even more important during this time, we knew they were professional, organized and very kind as we could not visit in person. Meetings were arranged with web capabilities where my mother was included via Zoom and we were all able to interact with her and see her go through her routine as she was evaluated. Certainly, this is not a brand new facility, but the professional care and kindness provided to our mother was literally life changing. We are extremely grateful that Harbor Villa Care Center was where our mother was placed for her PT, OT and overall transitional care. read more

Bad service! Very messy and dirty patients are treated very poorly patients have to yell In order to get the attention they deserve. As walking down the halls all I hear is "I need help anyone in the hallway" and they just walk past or make faces at each other! These people are in need of help there's absolutely no way they can do things on there own there's a reason why they are in here! These "employees" have to remember that this place is the patients home for a while and imagine if that was you, you would want to feel welcomed and taken care of. As I'm sitting here writing this a employee rolls an old woman to the hallway and leaves her here she has been sitting here for about 20 mins and no one cares to come help her!!
