I was reminded of Parkwood while talking with my mom's home health nurse. She has a new patient in Parkwood. I was interested in reading reviews to see if anything had changed. Doesn't look like it. Whatever you do, do NOT place your loved one in their memory care. Take a look at it and you'll know why. Four years ago, we were all still dealing with problems resulting from Covid. I tried to explain the issues we had at Parkwood on that. Primarily, the caregivers were very inattentive. My parents were in Parkwood for six weeks rehab. One day I found my mother lying across the bed with her pants down below her hips. An aide had taken her to the bathroom and left her like that when she mentioned that at home her husband normally helped her with that. The aide said ok and left her. I was more than happy to get my parents out when their rehab was over. I don't know if the director is the same person, but the director at that time was totally indifferent and did nothing about the issues I brought to his attention. Only unpleasant memories of Parkwood. I do not recommend it.

    PLEASE do not put your loved one here. The staff (mostly) were not very attentive. AS with every…read moreplace she did have a couple that treated her with kindness and respect.. When mom pushed the button needing help, they turned it off and said they'd be back later. It was often 30 + minutes before they came back. Once they refused to leave the trash can by her.. ( not the cleaning people the one gentleman that I met was very polite and seemed kind). Mom told that person she has been feeling sick, and needed it by the bed.. That nurse was very aggressive and refused to leave it there.. She ended up puking over the side of the bed. One came in and put a paper towel over it, until another one came back in to clean it. She saw people taking her tray, and eating on it when they were walking out. NO RESPECT , talking loudly on the phone or right outside the door all hours, day and night. They tried to keep her from going to Baylor Hospital, after she began having severe pain. ,She had been seen by their "DR" and she was supposed to be fine. She got to Baylor, went into emergency surgery for gall bladder removal. When she went to Baylor, Bedford Wellness sent NOTHING with her. Every time she has been moved to a facility from hospital or visa versa, ALL her things were sent with her. A nurse or someone asked if they needed to take anything with her in transport. That question was answered with :" no she'll be coming back". She DID NOT. Once leaving from Baylor she came to a facility MUCH closer than moved in with me. I called COUNTLESS times to pick up her things. Left messages AND spoke with her "social worker". NO return calls after the initial conversations. All of her clothes had her name or room number written in them. So it would not be difficult to round them up. All the things I had brought her to snack on, anything I could bring to make her stay easier, I brought.STRAIGHT UP STOLEN. There was always a language barrier with almost every staff member there. This facility was recommended by JPS hospital FTW "social worker" saying that was the closest to me..( again apparently a language barrier , or was it?). Being told that was the closest to Granbury THAT was a lie .Bedford is Over an hour away from me.. THEN when she was going to Baylor for pain. Bedford Wellness sent her to Baylor in Grapevine.. EVEN FARTHER!!!. There were several facilities to choose from closer to me, and Baylor FTW would have been closer as well . After getting her itemized statement from her insurance, there were medicines that she was charged for and didn't get daily as it stated in the statement. I was there at least 3 days a week, and with my job it was always just before bedtime, and mostly on weekends.

I write this to help assure the veracity to what I have to say. Forum Parkway is by far the best skilled nursing facility (SNF) I have ever visited. My mother is currently staying here for rehab after having broken her femur. The staff have been pleasant, helpful, and competent. As a nurse, I have unfortunately seen bad snf's and bad hospital experiences. This place is not one of them. Dyllan Gadberry is the ceo of this location and from what I have seen, he runs a well-functioning center. I know good care and I know what lazy care looks like, and all I have seen for my mother at this location under Mr. Gadberry, has been good quality care. I think many of the comments concerning this place are either unfair or simply unfounded. I have only known good things here.

The four higher ratings appear to be from the physical therapy and Rehabilitation side, I do agree that the physical therapy and Rehabilitation at this facility is generally good however the long-term care side is what we are reviewing and this has been our experience . My mother lived at this facility for over two years. While daytime care was generally adequate, nights and weekends were consistently poor and neglectful. For over a year, I raised concerns specifically about evening and weekend care, yet these issues went unaddressed and showed no meaningful improvement. There were multiple instances--especially on weekends--where my mother was left unchanged for extended periods. She often had to call me so I could contact staff to get basic care addressed. My mother frequently complained that her call light would go unanswered, leaving her unassisted in her room despite being bedbound. On several occasions when I came to help her myself, I observed CNAs at the nurse's station playing on their phones instead of responding to residents, or no one at the nurses station at all for extended periods of time. The food quality and variety were also concerning, with the same meals repeated several days in a row. Communication was one of the biggest failures. In two years, I was never able to speak directly with her physician despite multiple requests. I was never even given the doctor's name, and staff could not provide clear answers. This was especially troubling since I needed to be involved in her care. There were repeated hospitalizations that appeared preventable with proper monitoring and timely care. She was sent to the hospital multiple times for infections, including severe UTIs and pneumonia. In one case, an ER physician described her UTI as the worst he had seen in his career. In 2026 alone, she was hospitalized four times in four months. The facility was also reluctant to arrange hospital transport when needed. At times, my mother had to call 911 herself, or I had to intervene and insist on transport. During these critical situations, I was not kept informed of lab or X-ray results. When I raised concerns and asked what steps would be taken to prevent future incidents, I was told that if I was unhappy, I could move her. After that, repeated attempts to contact leadership--including the Director of Nursing and administrator--went unanswered. We ultimately moved her. No one from the facility has followed up to ask why. The building may look nice, but the level of care, communication, and responsiveness was deeply concerning. I cannot recommend this facility.

The building is run down--peeling paint, holes in the walls, missing electrical faceplates, loose outlets, and damaged furniture. It looked neglected and unsafe. The staff was not attentive to our loved one's needs. One night he waited an hour for help to use the restroom. No one at the facility would answer the phone when we tried to call. I only reached someone because I had the Executive Director's cell number. Communication is almost nonexistent. They do not answer calls, rarely respond to emails, and repeatedly failed to provide basic information we requested. Even the external hospice staff caring for him was ignored. We also have serious concerns about the quality of medical care he received. While he was there, he had a ulcer on his foot that the staff repeatedly told us was "getting better." He is now in the hospital with a bone infection (Osteomyelitis) that required surgery. The surgeon told us there was a great chance he could lose his foot, but fortunately they were able to save it. This raises real questions about how closely the medical staff at Westpark were monitoring him--or if they were monitoring him at all. This facility failed our family, and I would never recommend it to anyone.

Once she moved in, they took such wonderful care of her for nearly three years. What meant so much to us was that many of the original staff members from day one were still there when she passed, and they remained tender, compassionate, and attentive to her from start to finish. They came to know our father and our whole family because we were there so often. Through all of my father's medical ups and downs, they cared for him as well. When he stayed there for respite care so he could be close to her while I traveled, they were amazing with him too. In the final days, after our mother was moved to comfort care, our father suffered a stroke the very next day. The staff helped us keep them together at the end, and their compassion during that heartbreaking time meant more than words can express. We truly could not imagine going through it anywhere else. They were amazing through it all. When we lost our parents and eventually moved everything out, it felt like we had lost an entire family there as well. She was kept on a catheter so that they would not have to take her to the restroom. She was given shots in her stomach that they said were to keep her from having a stroke we still don't know what those were. She said the staff was mean to her in particular a Nigerian nurse. At some point, they are going to have to install cameras in all of these places where our elderly are being abused and mistreated enough is enough.
