I could not hug her enough as we were leaving. This is the only program that my daughter has completed. This is the first time we've had real hope in many years. The program is tough and asks a lot of the participants, as it should. My daughter left the program twice, but finally went back and muscled through the hard work. I thought I would never see my beautiful daughter healthy, and sober again. I know that she did the work, but thank you to everyone at Stonegate. Prayers answered.

This cost her much pain and led her to cry and scream, which the staff dealt with in the most humane and compassionate manner. They spoke softly, encouragingly, and continue to let her know that they were doing the best they could to help her without causing her any pain. She is always clean, dressed and clean clothes, and in a good mood when I come to visit. The staff to patient ratio seems extremely high, and the executive director is very approachable. I would refer my other clients here in this part of DFW without thinking twice. What a wonderful secret.

After my mom's spinal surgery, we chose Encompass as her rehabilitation facility. She arrived on a…read moreSaturday, and the weekend crew did not appear knowledgeable about the new patient process. The nurse said the charge nurse would come and check in my mom, but she never came. The temperature in the room was at 81, and the nurse did not know how to cool it down. Once she was settled in, she was at a point where she needed help to use the restroom. There was an instance where they did not attend to her personal hygiene afterwards. When my mom complained of pain in her legs, she felt nobody listened to her. They told her the pain was normal, but my mom insisted that was not normal. They took an ultrasound of her leg and said she was fine. Instead, they kept medicating her. She finally asked to get transferred to the ER and the doctor rudely said that her things get packed, so she could leave. Upon her arrival at the hospital, an ultrasound was taken of her legs, and a blood clot was found. An MRI was also conducted and a hematoma was found on her spine causing most of her pain. Needless to say, my mom will not be returning. Nor will we recommend Encompass to anyone.

They not only have done my rehab but have made suggestions for massage places, dry needling, and encouragement that helps me get through these times. Stifling even if you visit after 5 pm. Administration stays in their office so they don't have to inhale it. If you bring attention to it, they will turn around and say that you're raging or are being aggressive with them because you're wanting and needing and begging for care for your loved one or begging them to make sure your loved one has a safe space that is not littered by wheelchair parts in their room and random people's underwear thrown in her closet Or even worse when they put someone else's underwear on my mother. Anything in this facility that is not bolted down has the ability to grow legs and disappear and I do mean anything. They have known thieves that are residence in the facility yet nothing is done about it. The other residents suffer for this. Please note there are few nurses that are gold to the core!! I've given them reviews and accolades to other admin there and kudos but it stops there. Morale here is down for the entire facility bottom to top, admin to clinical, including the residents. This reflects clearly that there is a DIRECTOR problem, and it is evident that this place is not being directed in a very good direction. I have seen nothing but decline since my mother has been there at this facility under current management. It's just nasty. Residents have lost all morale and just rock back-and-forth in their doorways. There's no activities director so the biggest thing they do is play bingo for a soda. If you do not do anything else ever, please understand that this facility has violent offenders in this facility and there have been physical altercations while my mother has been there. Please , If you were having to make this hard decision for your loved one, I can tell you with a calm, caring voice to please do your research do your visits, do not listen to the sales pitches their marketing throws out, do not trust ANY pictures posted!! You have to dig deep in that place just to find a chair that is safe to sit in. The entire facility is just dilapidated and that is the truest word I can think of to describe this place. My mother went in there, clean and mobile. Now we do not bring family to the facility because we're embarrassed of the stench that meets us at the door, my mother is always in dirty clothes that does not belong to her and smell smells like a sewage plant. We have to pick her up from the facility, take her to our home, bathe her ourselves, clean up the chronic fungus that she has from the facility that will not treat her Medically for it consistently, Bandage everything up, make sure we have new clothes for her before we can even sit down and enjoy a dinner and relax,. This is not a dramatization or an exaggeration. This is what our life is with a loved one at this facility. I have posted what 30 months ago looks like vs 18 months in this facility. We were told that my mother would get all kinds of cognitive therapy and physical therapy, but again, the residents are just there to breathe and the evening shift are all too happy to hurry up and get their medicine and food and make them get in their rooms because they don't want to deal with them. This Facility has more issues than Cosmo magazine. Do not put your loved one here I beg you!! We'll see how long this post stays up over on Google since the executive director likes to block reviews so the fake ones that are posted from the facility's family members and friends with fake accounts make the facility look better . When I attracted the attention of the nurses to the patient, they just looked at her thru the door and WALKED AWAY. They showed total lack of concern. I had to call the rehab hospital TWICE for someone to help her while I stayed with her. Truly terrible and uncaring workers at this place that should not be in the medical field at all.

The four higher ratings appear to be from the physical therapy and Rehabilitation side, I do agree that the physical therapy and Rehabilitation at this facility is generally good however the long-term care side is what we are reviewing and this has been our experience . My mother lived at this facility for over two years. While daytime care was generally adequate, nights and weekends were consistently poor and neglectful. For over a year, I raised concerns specifically about evening and weekend care, yet these issues went unaddressed and showed no meaningful improvement. There were multiple instances--especially on weekends--where my mother was left unchanged for extended periods. She often had to call me so I could contact staff to get basic care addressed. My mother frequently complained that her call light would go unanswered, leaving her unassisted in her room despite being bedbound. On several occasions when I came to help her myself, I observed CNAs at the nurse's station playing on their phones instead of responding to residents, or no one at the nurses station at all for extended periods of time. The food quality and variety were also concerning, with the same meals repeated several days in a row. Communication was one of the biggest failures. In two years, I was never able to speak directly with her physician despite multiple requests. I was never even given the doctor's name, and staff could not provide clear answers. This was especially troubling since I needed to be involved in her care. There were repeated hospitalizations that appeared preventable with proper monitoring and timely care. She was sent to the hospital multiple times for infections, including severe UTIs and pneumonia. In one case, an ER physician described her UTI as the worst he had seen in his career. In 2026 alone, she was hospitalized four times in four months. The facility was also reluctant to arrange hospital transport when needed. At times, my mother had to call 911 herself, or I had to intervene and insist on transport. During these critical situations, I was not kept informed of lab or X-ray results. When I raised concerns and asked what steps would be taken to prevent future incidents, I was told that if I was unhappy, I could move her. After that, repeated attempts to contact leadership--including the Director of Nursing and administrator--went unanswered. We ultimately moved her. No one from the facility has followed up to ask why. The building may look nice, but the level of care, communication, and responsiveness was deeply concerning. I cannot recommend this facility.
