To say that I am deeply disappointed with the care my mother received at the SN Genesis HC - Holly Manor Center (84 Cold Hill Road, Mendham, NJ 07945) is an understatement. What should have been a safe and supportive environment for a two-week rehabilitation stay after hip surgery turned into a constant source of stress and concern for our family, ending with the subsequent decline in health of our loved one and her passing away. The facility has issues with being short-staffed. This created an atmosphere whereby staff seemed overwhelmed and inattentive to the needs of the patients/residents they were responsible for providing quality care. My mother entered this facility with no issues related to incontinence and within a few days the facility was putting her in an adult diaper. Family members witnessed call lights going unanswered for extreme amounts of time. This could lead to an increased risk for patient/resident injuries due to falls as well as incontinence. My mother was transferred to this facility with medical notes from nurses and doctors at the hospital she had her surgery in. As the receiving facility, Holly Manor Center was privileged with the knowledge that my mother was being treated for Congestive Heart Failure as well as finishing up a course of antibiotics used to treat pneumonia. Part of Holly Manor Center's care plan reports included vigilant assessment of her CHF and for resurgence of infection(s) since this was still a critical post-operative period for a geriatric patient. I was assured by the social worker that this facility was able to provide those assessments and cares should the need arise. This, however, was not the case. Communication was another major issue. It was difficult to get updates about my mother's condition, and when I reached the nursing staff and asked questions, responses were dismissive or felt like there was an uncertainty in how to respond to questions. On a few occasions, receptionist could not even find nursing staff to receive my call, and would leave a message, but no nursing staff called back. Nursing staff were often aloof with responses and this left you with the overall impression that they did not care. My mother's health began to decline within a few days of being at this facility. Nursing staff would not take our concerns seriously, despite their own care plan including vigilant assessment of signs and symptoms related to possible infection(s). Less than a week after she was transferred to this facility for physical therapy/rehabilitation, she had declined so badly that we demanded she be transferred back to the hospital. With reluctance, they finally agreed. Once readmitted to the hospital, it was determined her pneumonia had become so severe that now she was septic. She was received back to the care of the hospital with a noted bed sore, and visible decline and health. Within a few days of this, my mother passed away. I would strongly urge others to carefully consider their options before placing a loved one in this facility. What was supposed to be a short-term stay of two weeks, showcased how problematic elderly care is in this country. Poor quality of care, lack of medical assessment, and a reluctance to see the actual signs and symptoms as serious will forever leave our family wondering if this facility's dismissive treatment of my mother hastened her decline subsequently her death. read more

Terrible. Short on staff, didn't receive a shower in 2 weeks. Finally when I was getting ready to go home I received one. Left a catheter in for weeks when it should have been removed in days. Dirt and stale smell. Didn't get the rehab scheduled. Husband had to bring in drinking water. Bed was broken. Refused to change it till I threatened to call the police for senior abuse. The bed folded and kept me in a V SHAPE. I had fractures in my back and to this day I can not walk without a walker. Should have sued for medical malpractice but I left in too much pain and duress. Some of the staff were trying so hard to make up for the deficits and were lovely people. read more

Had a stroke on 02/10/26 was immediately taken to Morristown hospital for treatment, when I was discharged, I was transferred to Holly Manor rehab facility, in Mendham, NJ. when I first got to the facility, I really did not what to expect, but was pleased to find out that the same level of care I received in Morristown, I also got at Holly Manor. They have a very professional staff that takes care of all your wants and needs. They did a thorough evaluation, asking all the necessary questions, and informing me of what to expect in the coming weeks. I was very pleased, with the rehab and the caring/loving staff! read more

Place is terrible care is borderline neglect no physical therapy. Not getting the care I'm paying for.

My 100 year old father spent time at Holly Manor for respite care. He was well taken care of by the friendly capable staff. When I picked him up he said" they take good care of me here" - as we were leaving he told one of the nurses he would miss her! He does like the ladies... read more

My Mother was in Holly Manor from late 2016 until her passing in February of 2019. She went in on hospice care and between the hospice team and Holly Manor caring for her she graduated! She remained at Holly Manor because her condition was too difficult to handle at home. I can only speak of the short term care side which is where she was. The entire staff cared for Mom with such love and compassion. I was there on a daily basis and everyone on staff worked with me whenever something came up with Mom from the social workers, nurses, awesome Medrine, Janel, Lorrie, Kelly and many others, administrators, aides, physical therapy, activities, kitchen staff and maintenance. By always working together with me, it made Moms final days as good as they could possibly be considering she would have rather been at home and I wish she could have been too but it wasn't possible. This place is horrible. It smells really bad. The food is disgusting, obviously poured out of cans. A lot of the staff don't speak English. Patient concerns are ignored. Including pain ! It is dirty. DO NOT bring your loved ones here ! read more

I was transferred here from a hospital. As soon as I arrived a patient was calling 911 to be discharged voluntarily. Because of COVID I was quarantined for 10 days. Then luckily put in a room by myself. The rooms & the BATHROOMS are filthy!! The place is very noisy & sometimes I waited an hour for an aide to help me. At night the aides stand in the hall doing nothing & talking very loudly. My laundry was lost a few times & this place is terrible. The food is awful & I rarely received what I ordered.!Fresh fruit was almost non existent. Instant coffee was disgusting & a few times my food wasn't delivered to me. I kept a journal every day & this old place is falling apart. I wouldn't send my worst enemy to this shit hole. The first time my husband came to see me he couldn't believe the filth!!!!! But on certain days when the staff knows who is getting visitors, they put on a good show. Only 2 people were very sincere in caring for me. My father is literally struggling to stay alive right now after a chain of events started during his 'rehabilitation' stay at Holly Manor. Four and a half months ago my father went to the hospital after slipping to the floor from his bed. He sustained no injuries from this slip yet his leg ulcers were treated there. After six days he was sent to Holly Manor to build up his strength after what atrophy six days in bed caused. He left Holly Manor twenty days later weaker and unable to stand from a seat without great assistance or walk as well as he had before going into the hospital. As a result, he slipped again and this time injured his knee. At the time he was presented with only two options again for rehab and Holly Manor was the better of the two. Dad was discharged from Holly Manor totally bedridden, sporting a big bed sore on his buttocks and a catheter that his family only discovered trying to get my father into the van unsuccessfully. Not even four days home my father got sepsis from a urinary tract infection and was rushed back to the hospital, almost dying the following night. After going to a new rehab facility, the bed sore was being treated but continued to spread to where he was returned home bedridden once again (Aetna cut coverage as the new place's rehab therapists were making strides to get my father standing again) and less than a month later had to have the bed sores surgically removed leaving (and I'm not kidding) a large soup bowl wide and deep open wound because the bed sores had tunneled. To have this surgery my father also needed a colostomy so as not to infect that open woound that now requires a wound vac. During the time he was bedridden at home, he developed arthritis in his hip and knees that made any movement of both excruciating. My father is now dying from fluid in his lungs due to being bedridden and inactive. NONE of this had to have happened if Holly Manor's physical therapy was effective. Instead, my father split his time between his wheelchair and his bed. Contact with the Holly Manor team only came one or two days before his discharge so for my father and his family, it was useless. This kind of meeting should have taken place -- as it has at Care One in Parsippany (where Dad was previously before having to be rushed back to the hospital three days ago) -- shortly after the patient was evaluated so the therapists know more about their patient and a goal could be set. My father needed to be able to get himself to his knees after falling to the floor so he could pull himself up and when going to bed, pull himself further up the bed. THAT would have stopped the cycle of hospital/rehab/home months ago to where right now my father would be upstairs reading the newspaper in the dining room and drinking his coffee. Instead, he is in the hospital hooked to a breathing machine with himself using only one and one-third his lungs to breathe. The staff may be pleasant but if you care for the health of the patient, please look at other places. And for Heaven's sake, do not pick Aetna HMO to cover the other 20% of what Medicare doesn't cover. The insurer sets what therapy and how long the rehab stay. (We just received a phone call from the hospital as I was writing this that my father's oxygen assimilation has dropped below acceptible levels and it will not be long now. We're going to be losing our father because.) read more

My mom was a resident in Holy Manor for almost 3 years after losing the ability to take care of herself and I can tell I have had a piece of mind knowing that mom has great help and care in this Home. I read other reviews about this place. Yes, it is dated, yes, sometimes it is smelly, but what is the most important, visual appearance or people who dedicate themself to help those in need? All the stuff is amazing here: the Helpers, Nurses, The Doctor, Social Workers, Therapists, Administrators, even Receptionists! They all come to work here with one goal, to HELP patients and residents, to make their lives as close to life at home as possible. I can say they all put their hearts into their work. And they give a lot of Love to patients and especially to the residents of the Home. The Doctor and the nurses have always listened to my concerns, always were available for me and my mom if it was needed and always they all have been trying to help. They are amazing people. So, what do you think is more important for your loved one who will be living in a nursing home, 1)super clean place or 2)super caring personnel working there? I choose the second one. read more

I wish my experience with Holly Manor Rehabilitation Center was positive like this last reviewer, but it has not been. My mother was admitted to Holly Manor Rehab Center 10 days ago. They leave her in bed all day most days. When they tried to move her from her wheelchair to her bed last night, the aid dropped her and broke her leg. She now requires surgery to repair the fracture and she is worse off than she was when she arrived. More egregious is the fact that my mother told the aid more than once that it requires two (2) people to move her from the wheelchair to her bed, not one. The aid said "Don't worry, it will be fine". She then proceeded to drop her on the floor and fracture her leg. Even worse: when Holly Manor called me last night to tell me something had happened, they said she was "Placed" on the floor when in fact she had been dropped by the aid. They told me she was perfectly fine and not to worry. Had my mother not requested an x-ray, they would have left her in bed with a broken leg indefinitely because according to them, she was "fine". Do not go here for rehab, they are terrible with patient care. I have removed her from this facility and she will not be returning. Of course now she has the broken leg to deal with too. I wanted to write a review about my experience at Holly Manor. I was a rehab patient there for 3 weeks, after a knee replacement and had a very good experience. My son also came for a few weeks for rehab. They did a great job. The rehab therapists Carmel, Debby, Debbie and Fred were all fantastic. My wife was a resident for 12 years. The rehab was great. The aides all took great care of her. They all really liked her and took really good care of her. The aides Rita, Maddie, Audrey and Marjorie, Nancy Faye and Marie were especially good! All the nurses were great. Tasha the nurse was outstanding! I have recommended this place to many! read more

When our younger sister fell and broke her hip about 10 weeks ago it was tough to find a sub-acute care facility in Morris County willing to take her and get her back to walking. She was under the age of 55, deaf and Downs syndrome, so special needs for sure. Holly Manor clearly stepped up to the challenge. The staff at Holly Manor were so welcoming, and when we asked to tour the facility beforehand on short notice they could not of been nicer. We met with admissions, nurses and the physical/occupational therapy staff and they answered all our questions on how our sister would be cared for and how they would handle communication since our sister is deaf. Our sister was well cared for at Holly Manor not just by one or two people but the entire staff of aides, nurses, physical/occupational/speech therapists, food service, housekeeping, social work and recreational staff. A true team effort there by all staff to rehabilitating their patients and your loved ones. The end result for our sister was a full recovery--walking and moving again like new without the assistance of a cane or walker, and well on her way to returning to her normal life. NEVER take anyone that you love to this place. It's dirty, it smells, the equipment is OLD, the rooms are all OLD and in bad shape. The food was pathetic, I wouldn't tell my mom that I wouldn't eat it myself. It was NOTHING like the place that she had just come from and I thought that place was a bit dated. Now to beat that, on the second to last day she was in the place she FELL and hit her head again due to the fact that the staff was working with someone else on another emergency(?) I have been there late in the evening and have seen staff laying out on the sofas taking naps, doing their hair and nails and just congregating in the kitchens talking to each other. Not what I consider patient care. While some of the people that looked after my mother were good and caring some of the rest were not and we were concerned. On one occasion I asked a nurse who came to the room to help her go to the bathroom. She said it wasn't her job and she would get an aid to help. That never happened. I asked another nurse if that was policy and I was told that anyone should be able to help your go to the toilet. When we were ready to check out NONE of the prescriptions were available and no one knew if there were any. We had to make a separate trip back to get them later after we raised hell. And last but not least they were supposed to get paperwork to the visiting nurses to go to do a home evaluation on the Monday after she was discharged. My call to visiting nurses was upsetting because I was told that they never received the paperwork, and it was MONDAY MORNING. a number of calls to the ADMINISTRATOR of Holly Manor FINALLY got the paperwork executed but it was too late for the day. Do not send your loved ones here. I was previously a patient, and they almost killed me. I had asked several times to go to the hospital (and my own doctor even called and requested to have me transferred) and they refused to send me. I called my own physician, and made an appointment. I ended up saving my own life, because by the time that I got to my doctor, I was in septic shock and was rushed to the emergency room. By that time, my kidneys were failing. I was also over medicated while at this facility, and was often left lying on my bed for over half an hour without clothing after being assisted in the shower while the aide assisting me went to their morning meeting. Unable to get out of bed at that time to get my clothing, I had to endure the family members of my room mate walking in to see me without clothing on several times. This is just an example of the less than substandard care which I received while at this facility. read more
