I have to start this review with the good- my mother is blessed to be in a hall with great nurses. We are incredibly lucky because that's not the case with every section. I'll say that the management group leaves much to be desired. In our year here there have been 3 directors, none of which have been able to answer the most basic of care questions. There's zero communication from management. While I prefer to communicate with nurses as they're the ones who interact with residents everyday (good luck catching managers out of their offices/meetings), there have been situations that required the help of managers. For instance, my mother does not see facility doctors due to insurance, however, a facility doctor prescribed her a medication for a condition she doesn't have. Mind boggling! I am still waiting for an answer as to how that happened. It's been 7 months now. With that being said, I do realize the difficulties facing facilities like this. I know there are horror stories out there at other facilities and I'm thankful to have the good staff members close by. read more

Local hospitals have established connections with Hollymead, allowing patients to thrive during their recovery. The team here is solid, and there are no major complaints unlike in the past. The Good. The staff, especially Kelly Seiler, their Case Worker and their Daytime Med Nurse... Rebecca , and Afternoon Nurse ... Abby (Abigail ) and Afternoon Aide ... Latasha are exceptional. The head of physical therapy also gets 5 stars. Their expertise and sensitivity needed to be acknowledged. Rachael Beargeon , Assistant Director of Nursing was also a reason I say their staff deserves recognition. The challenges Communication. The physicians have to step up their game. When you feel like you're leading the physician to an appropriate care plan... we are all in trouble. The doctors need to communicate. Medication management needs serious attention. Skilled nursing facilities are facing a staffing crisis due to a combination of high demand, difficulties in attracting and retaining high end employees and challenges related to new federal regulations. SO BE THE BEST PATIENT ADVOCATE FOR YOUR LOVED ONE OR FAMILY MEMBER. And do so with understanding towards the hard working staff. read more

I've been a patient of Hollymead for a little over a month after TWO YEARS of continuous hospitalization, including Several weeks in Residential care & physical therapy centers. It's been extremely hard & I came within seconds of dying on 7 different occasions during this time, Although definitely a painful, scary & difficult journey, the excellent care from the wonderful people I've been privíleged to meet have made this sudden & unexpected detour in my life one of my richest blessings. So many stand out in my mind, friendships were formed that will last the rest of my life. However, one beautiful young lady here in Hollymead stands high above the rest. Her name is Keke. She's quíte an incredible nurse's Aide who goes over & above in performing her duties AND making every patient feel loved & special. She makes me feel like I'm her only patient...she checks on me frequently, knows what I need & handles it before I can even request it. She hàs a smile & personality as big as Texas.. She makes her demanding job, like she enjoys every part of her day,but as hard as she works, she has to cómpletely & totally exhausted, yet she shows no signs of it & that beautiful smile never fades,. Keke ís amazing! Sick & hurting people can be quite difficult at times, but her sweet, calm, & caring manner can generally calm them down within minutes. I honestly had to take my time with this review. Because as I write this my mother is being admitted back to the hospital, due to neglect. Possibly kidney failure & an infection that the wound nurse barely took care of & she has an GJ tube. I want to first start off by saying, that if you want your loved ones alive, THIS IS NOT THE PLACE TO PUT THEM! This facility is a nice facade! The first week my mother came here she had to be admitted back to the hospital for kidney failure in which she's never had! Plus more. She ended up coming back & completely not by choice, we had to kill time. I had to sleep with her everyday for 2 MONTHS to make sure received the proper care that I HAD TO GIVE HER ! Don't waste your time using the call bell because you'd be crying for help. I had 2 people help me & understood the mental hurt I was going through because of this place & it was an Occupational Tp & a LPN who barley worked there and stated that's the reason she doesn't like this place. If you have a conscience please don't ever work here or any nursing home! The actual staff was disgraceful & distasteful& extremely disrespectful. If I didn't brush my mothers teeth, wash her face or even give her a bath, she'd a never had one of if I wasn't there! They barely took vitals and barely wanted to do their jobs. I heard one of their workers literally say " oh they're moving her back, I'm glad she not my patient" as if she did anything anyway but give attitude & her name is Dream. I honestly had to compose myself in there. One of the Rn's had the audacity to say "I'd a went home if it was me, ya'll actually sit here with ya'll mothers" like who even says that ?! My sister is a nurse herself so we were able to catch the symptoms & notice the changes. I'm honestly hurting deeply because of this place. I'd post the wounds of my mother but I won't put her out there like that. I already have a review on google. I literally witness 2 patients code in there and had to have immediate emergency care. Did I mention how Horrible this place is ! I'm going to report them to state & the board, I don't care how much or how long it takes & I will be leaving a review to every site I can ! My mom broke her knee cap and spent two weeks here. The staff is amazing, incredibly attentive, great attitudes, and helpful. The PT staff, namely BJ and Lyndsey were patient and kind with my mom and got her back on her feet in a matter of days. We don't have a lot of experience with skilled nursing facilities but we would highly recommend Hollymead. read more

I just wanted to thank staff that isnt normally recognized ,My mom has been there a long time and despite the care or lack of from nursing others in the facility have stepped up to care for my mom. Carla has been a godsend , she regularly has checked in on my mom and that inst even her job, but she cares. She has alot of responsibilities and she is always working hard, but yet she still takes the time to care for others. Cesar is another person that will stop and attend to patients when the cnas are just sitting at the nurse station eating and complaining. These 2 individuals are a blessing to Hollymead and need to be treated as such. The patients love them and for them to be not be recognized is a disgrace. Best Regards KM read more

I would give zero stars if I could. If I wasn't a Registered Nurse myself, I strongly believe my mother would have died in this facility. IF YOU CARE ARE ABOUT YOUR LOVE ONE, DO NOT SEND THEM HERE. The nursing care is way below substandard, it's practically non-existent. When my mom was admitted to the facility, no one check on her for 3 hours. She was incontinent and was left soiled for hours. Staff would walk pass her door ignoring the call bell until mom would call my sister and I. We practically had to stay with her 24/7 to ensure she got any care at all which was very difficult for me to do as a working mom. My mother was a stroke patient who need suction set up in her room. The portable suction machine was broken so my mom was constantly choking on her saliva and nothing was done. My mom had a feed tube that wasn't care for properly. Her feeds were often off and no one would know why when we questioned it. Her feeding tube insertion site required wound care that was neglect. My mother's skin was excoriated because her dressing was overly saturated with drainage every time we came to visit. She also developed a pressure wound on the back of her head during her stay. We had to make multiple request for someone to look at it. I was told she seen by a wound care nurse but not was done. She was still laying on the back of her had with no padding or wound care dressing applied. My mother ended up in the hospital twice during her stay at Hollymead. First time I noticed my mom was having a change in mental status, so I mentioned it to her nurse. The nurse came to look at her and asked a her a few questions. Then she left and I guess that was the end of her assessment because nothing else was done. The doctor was never notified. So I went out to speak to the nurse again. I asked the nurse to do a set of vitals and I requested that my mom be sent to a hospital. Her nurse told me that the transfer may not granted because it was a family request. This made me obviously upset so another nurse stepped and put in the request for me. She was admitted to the hospital with aspiration pneumonia, hyponatremia, and acute kidney failure. This was Sunday April 7th, 2024. I went back to Hollymead on Monday 4/8 to ask why they didn't notice any issues and why weren't labs done to monitor my mother. I was told labs were done on Friday April 5th, 2024, so I asked for the results. It showed that my mother's WBC was elevated (indicative of infection) and her BUN and Creatinine was severely elevated (indicative of kidney failure) both issues she didn't not have before she call to Hollymead. I asked why nothing was done and I was told by the nurse manager that the nurse caring for her never check for the lab results so it was never reported to the doctor. By time my mom got to the hospital on Sunday, her kidney levels were even worst. My mom was in the hospital from 4/7-4/15. No one gets sent to the hospital from this place unless it a true emergency with EMS running through the halls or you're savvy enough to request help for your family member yourself. This is only one of her major incidents at Hollymead but it should be enough to show how neglected she was. I will be reporting this facility to the state and medicare services. ITS THAT SERIOUS. I have a friend with Parkinson's who has been in Hollymeade for almost two years. I have read the negative reviews and from what I have seen there, I have to agree with every single one of them. They couldn't even provide a Thanksgiving meal for the patients. Hollymeade is a shameful excuse for a rehab facility with haphazard management at best. read more

My 91 year old mother was placed in this facility. Treatment was horrific. There was absolutely no communication between staff, management and relatives. Because of a stroke, my mom is partially paralyzed, blind in one eye and cannot speak. She was sent there for rehab. The occupational and physical therapists were good and saw her immediately. The speech therapist didn't get to her for an entire week, which was detrimental. I put a ring camera in her room and was horrified by the neglect J wouldn't wish this place on my worst enemy. read more

If I could give them zero stars, I would. My 89yo dad was here for rehab in January/February 2020. He had Parkinson's and a brain bleed from a fall, and either I or an independent paid caregiver was with him at all times, 24/7. I was with him 4-5 hours every morning, so I was there a lot. We had been told the nursing was bad but the PT was good, so we took a chance. the PT was good...the nursing was horrific and dangerous. It took three days to get his medication straight when he transferred in...the receiving nurse forgot to add one and then they forgot to order 2 other meds. I am the one who caught it. They consistently ran out of clean bedding...I finally had to raise a ruckus at the front desk to get some...but the other patients were SOL. I nearly always just went into the linen closet myself to get clean bedding if it was there. They would also skip shower days for him. If he was in PT when they came to do him, they'd just skip him. Horrible. They didn't always administer all of his meds...they left his inhaler in his room unsupervised twice...and my caregiver reported that one time a nurse tried to force Dad to take all his pills at once with his pudding rather than one at a time, even though he was on puree food/swallowing restrictions. He choked them back up. So dangerous!!!!! The director of nursing always talked a good game, but nothing ever improved. They consistently tried to move him without a gait belt and both times when he had one of his seizures while an aide was in the room, the aides had no idea what was going on or what to do. Again, dangerous! This place was a horror show. His 1st Dr., (Dr. Higginbotham) only came late at night and never called me even though my phone number was everywhere in the room with signs asking him to call me 24/7 and instructions to my caregivers to tell him. He never made any chart notations after the initial one (my Dr. brother was HORRIFIED) and waited days to order a urine analysis when Dad had a UTI. He was impossible to get hold of. I finally got them to switch me to Dr. Munis who was MUCH better and more available. They consistently brought him the wrong diet and had no good option for a ground diet. They would grind the meat, but give whole veggies and forbidden foods, like beans. We had to BEG for extra gravy to help him swallow when he wasn't on puree. My caregivers and I were there to cut it all up and keep him away from beans, peas, corn, etc. but on his own, he could not have managed. The staff had terrible attitudes and would stand around the hallways complaining about mgmt, their hrs, and how many patients they had. They'd keep chatting even with room lights lit. Even emergency bathroom pull cords. My dad had a seizure in the bathroom and I pulled the emergency cord and it took 10 minutes of me literally screaming for help to get anyone. I'd stand at the nursing station for 30-45 minutes after my private caregiver relieved me at 1pm and I'd listen and watch and I always felt terrible for the patients without 24/hr advocates. They would consistently ignore them and complain about them. The owners/bigwigs toured while a couple of times and my dad had a seizure during one tour. there must have been ten people in the room helping us, including the director of nursing, but any other time he seized, I could barely get a nurse to show up. His new chair was in the hallway for a few minutes one day while we were in the room and the housekeeping mgr just took off with it without even knocking and asking. she dumped our $300 roho cushion and his shirt onto the floor outside our door. I had to chase down his new chair they had just ordered for him to take home. She was anxious to clear the hallway because the bigwigs were coming. Those people are the only ones the mgmt there cares about. The aides consistently took 30 min to and hour to answer call lights and more than once my paid caregivers reported rough handling of my dad by the aides. Two of my caregivers, separately and unprovoked by me, told me this was the worst place they had every worked in, by far. My caregivers and I kept a spiral notebook record 24/7 from 1/1/20 to 2/14/20 of all of this. It reads like a horror show. DO NOT SEND ANYONE YOU LOVE HERE. read more

Don't let anyone you love stay here. My family member came here for rehab after a brief hospital stay. She needed help using the restroom and they left her there multiple times without someone coming to her aid for 20+ min. No menus for food, it took several days of asking until someone from the kitchen came to ask her what she liked before she got food she would eat. The nurses and techs were constantly standing around chatting at the nurses station and if you needed help, you'd need to wait til they were done discussing their personal matter. She received wrong medication while there. Just overall a horrible experience. When I went to get her upon leaving, I waited almost 2 hours for them to have all the items gathered up and the papers to sign. They were informed the day before that I would be coming and then what time early the day of. They had 6 hours to have all this done and it wasn't. read more

These people just don't care enough. Patients are treated like annoying cash machines. I think the workers try to do their job, but they are Grossly understaffed & too busy doing mountains of paperwork instead of caring for the humans in the rooms. read more

Came here for rehab on my knees and strength! I've had fantastic care from Nurses, all the aids, and Ms. Lee that manages all the all special activities for everyone that is permanent or short term disability! Shout out to the perfect Therapy gang they have on staff, fantastic care and know what needs to be done to get the person up and going! Rooms are perfect with TV, microwave, refrigerator, plenty of storage, big bathroom and wonderful view to go outdoors! I highly recommend Hollymead in Flower Mound, TX. Thank You! My mom was there and passed away on Nov.12,2020. They just started letting me and my dad visit once a week as essential care givers. My dad went to see her on Nov. 12 and found her dead in her room all alone. She was on hospice for 2 months. But the hospice nurse only checked on her once a week. She had a bed sore that covered her entire buttock area and bone infection from it. In August she was taken to the ER with multiple holes in her bowel and had emergency surgery to remove most of her bowel and a colonoscopy bag put in. Me and my dad were not even allowed to see her. She also had sepsis. 5 days after surgery she was sent back to Hollymead. 2 weeks later she was put on hospice. I filed a complaint with the state and after there investigation, they found nothing wrong. When essential caregivers were allowed to visit, Holly mead would only allow visitation one day a week, Monday thru Friday for 2 hours between the hours of 11-3:30. Since I work full-time, I could only see her 1 hour per week. This only lasted 3 weeks before she passed away. They lie to you and falsify medical records so you can't prove anything. Before the covid lockdown my dad would visit her everyday for 8 hours a day. He never missed a day. She was there for 4 years. Because he was there everyday, he took care of her. But when the lockdown happened everything went downhill. My mom did not understand why her husband would not come visit. She had dementia and did not understand the lockdown. The loneliness and neglect killed her. This place should be shut down. I sent a letter to Cantex and filled a complaint with the state. But nothing has been done. read more

I've been to visit with my grandmother 4 times since arriving in Texas from California. I've always been great professionally, staff is very responsive and respectful. Facility is extremely well maintained and clean. It offers a library and small store area. Most importantly has been the care so far. No complaints from what I've seen these past few days. read more

I had the unfortunate experience of my Mom being transferred from Texas Presbyterian Hospital to Hollymead in April 2017, the Saturday before Easter. I flew in from Atlanta, Ga. to be with her. She was there 2 weeks then had to return to the hospital because she still had pneumonia. While I was there my Mom was trying to recover from all the medication given to her in the hospital. She could not feed herself and was not very coherent. One day a caregiver brought in a tray of food and left, I told her that she could not feed herself and her response was she had other trays to deliver but would come back later. If you needed a nurse the wait was usually quite long. They had a difficult time getting her medications correct, the food was not tasteful and the care and service received was poor. My mother was to go out to OT/PT daily, some days they dressed her and other days they sent her to therapy with her hospital gown on. I visited her on several occasions and on one particular morning I came to see her and she had been put to bed in her clothing. There was no communication between staff, she had a doctors appointment one day however no one let the OT/PT staff know and they came while she was gone to her appointment therefore she missed therapy that day. While there she got a bedsore because they did not get her up and move her around or on her side to prevent this. She left that facility and is now being taken care of in a group home. Upon her admittance we were told she had a stage 4 bedsore which is horrific. Total neglect of an elderly person. I wish we had never taken my Mom to this place. read more

Hollymead is a good facility.my friend enjoyed her stayed in this facily.very clean and neat.My friend was welcome by a unit manager max,he did a great job by explaining everything in a nice way,at the same time assisting my friend with his needs.such a wonderful nurse,respectful,polite and very considerate of other people,flexible,attention to detail and demonstrate a lot of solving skills. I will recommend hollymead to anyone looking for a good facility for his/her love ones.Great staffs from therapist,administration to nurses.Rooms very clean,hallways and doorways litted and clear of obstacles. read more

No stars! 12 days and my husband went backwards. Rose, Betty, nobody ever called. I called numerous times and they were always in a meeting never returned calls. Best clothes have not been returned. Therapy often canceled or said to be delayed. I called said I was removing him, they called and said I couldn't. Next day I did- against medical advice (ama). Dr comes in once a week don't know if he ever saw dr. Nurse practitioner comes once a week, not sure if she ever saw him. Nurses tried to do a good job. Husband 6'3" so they had problem moving him. How long can you sit in a wheelchair? 3 hrs.? Still waiting to hear from Sarah the director! Was in to gain strength for walking and rising from chair, after mild heart attack. My mother checked in from the hospital on Thursday afternoon. She did not get any therapy until MONDAY AFTERNOON. She was officially "checked in" on Monday. They are woefully understaffed. We didn't even know there was a cafeteria until Monday - 5 days after we arrived. They left my mom on the portapotty for 40 minutes. Dinner came around 7:15pm. Unacceptable. We checked her out on Tuesday. THE WORST! read more

I was sent to HollyMead on May 9 2018 as transition from Presbyterian Flower Mound as needed rehab for open wound and physical therapy to gain strength before going home. 1. Two nurses tried to forcefully flush my IV line after resistance felt. I made them stop, afraid they were going to cause harm. I sat in a chair rest of the night because it scared me so. 2. The doctor prescribed medicines were not given as prescribed and I kept getting behind in dosages and they were not promptly removed from vein once given in timely manner sometimes hours later mainly at night. Even after bringing up, lack of motivation to properly give when needed continued. 3. Woke up one morning only to discover small red marks on arm and ants all over one of my bags sitting on counter. They were everywhere, even in the bathroom. Found a spider behind my headboard. 4. Shortly after arriving, a nurse was about to inject something on my arm and I asked what are you doing. She said I am going to give you a TB test. I said no you are not. Then she said you will have to get a chest x-ray. I explained I had surgery at the hospital and had chest x-rays there and I have no TB. Note: I was there for gallbladder and bowel surgery. She was about to do this WITHOUT MY CONSENT. 5. Level of laziness of wanting to do proper housekeeping in room. I tried my best to at least straighten out by bed sheets and had family bring me sanitizer wipes to wipe down areas of room I was touching. At one point, I was asked by staff member if they could use some of my wipes to clean the room across the hall, which I gave, because of the ants in my room they were cleaning up that room to move me into. I was not going to sleep another night in that room with ants. 6. I asked for compression socks because of swelling in my legs, and they only put them on me the 1st day and never seemed concerned in making sure I had them everyday for the 5 days there. 7. I went to this rehab facility because I had an open wound and needed it changed daily, was on an IV antibiotic, and needed physical therapy to help get around on my own. However, I could not wait at times for someone to arrive because if I needed to get to the bathroom, it was go now. So did my best to do it unassisted. Also, when I finally said I needed a shower, there were no supplies available to cover the open wound area; all they had was plastic bags. REALLY? I had a friend go to a medical supply store and bring back a few things to cover the area so I could at lease rinse off. 8. Because I had gallbladder removal surgery also, I needed healthy choices of food. Most of the time, French fries, hot dog buns, rolls, ground meat that looked like dog food, cold biscuits, gravy and even brought a peach cobbler that was nothing more than goop or a sweet roll that should have had apple filling, only a small corner had filling rest was the crust. I did get chicken and sometime managed to get carrots. Because I had to eat in my room, when what did come was cold. I was not supposed to eat fatty or red meat items. 9. Some employees were doing their best with what they had to work with, but others needed additional training, and those who risked me the most with the lack of giving the antibiotics and not properly knowing how to follow protocol when it came time to flushing my iv line, needed deeper lever of discipline. 10. Last, during this 5 day period, this facility did not provide me with an environment to help me recover and regain my strength. They did not practice their "Committed to Excellence" philosophy that I found online after my doctor got me out of there last week. 11. When I had my appointment with my surgeon last Monday, I immediately showed him pictures of what I had on the food and ants and explained my medicine situation and he immediately got me out of there and was able to go home with home health care. 12. I hope that the parent company of HollyMeade, Cantex Continuing Care Network makes immediate corrective actions. I have mailed them a letter stating such as I believe we who live in this community deserves more than this. I have also sent a letter to Presbyterian Hospital Flower Mound, to make them aware of my experience with HollyMead and how it risked my healing. There was stress, little rest, and most importantly to me, lack of providing my antibiotics in a timely manner putting me at risk as I was taking this because I developed an infection in the hospital. 13. Just because something on the surface looks really nice, be aware before placing your loved one or yourself there. I hope that something good comes out of my experience because it hurt me in my healing. At this time, they don't even deserve a 1 star. read more

BE WARE IF YOUR LOOKING FOR A SAFE PLACE FOR YOUR LOVE ONE RUN FROM HERE: If I could give this place a BIG ZERO!! I would in a heart beat ... My father was sent to this facility for rehab and experience his worst days ever... nurses disconnected his button keeping him from calling them.. he never had therapy as he should.... they fed him the same food every single day... Was found on the floor 2'xs.. I walked in the room and came to see a pieces of papers on the floor social worker Rose Soto noticed the mess and even questioned but nothing was done..numerous times dad had asked me to take him home "me thinking no I wanted whats best for him and that's to walk and get better" Received the call of dad crying devastated begging me to take him home walked in found him on the wheel chair thinking he was going to therapy and found out that they sat him on the chair and left him their the entire day.. notice bad swelling to his legs and hand Immediately notified Rose Soto letting her now that I was taking him she notified the notified the nurse in charge never saw her never got paperwork nor medications list ..and brought him back to the hospital where there once again was admitted..even wonder if the nurses there were REAL RN's. What should have been 45 days ended up doing only 5 days with no hesitation took him out and brought to the hospital and once again was admitted..NEVER AGAIN.!!!! THIS IS A BAD PLACE.. !!!! read more

It is possible to give negative stars to this place? My father went to this place for rehab since my father has been diagnosed with CHF for many years. My father unfortunately caught the flu and was admitted at Flomo Presbyterian for 2 weeks. He was then discharged and was advised to be admitted at a rehab facility (A first time experience for all of us). For the first 3 days, my father was doing well health wise, although the health care services provided by the facility was abysmal at most. It takes more than an hour or not at all for ANY nursing staff to answer any calls from any of their patients to say the least. we also cannot help but notice how acrid the smell of the place was and we as his family did our best to make his room at least comfortable to stay in. The absence and lack of care was compensated by my family by having someone mostly my mother stay with my father from sun up to sundown (which defeats the purpose of him staying at a rehab facility but we made the decision due to out observation of how the facility was being run even just from the first day). three days after his stay, we always ask the nurse on the medications they give my father because we have dealt with his health for more than 5 years. After the 3rd day, my mother asked one LVN why my father was not given his dose of diuretics ESPECIALLY since he has heart failure and has never been off of diuretics for years. She was merely answered by telling her that it was not on the orders of the physician. My mother then asked the nurse to please call the on call physician of the facility to make sure and she was easily brushed off and was told "we ONLY call the physician if it is a case of emergency". this went on for 5 days until my fathers health started to deteriorate and he was no longer able to even do physical therapy due to fluid overload. every day we asked them to call the physician and EVERY DAY we got brushed off. We come from a family of physicians and nurses, and I myself am a physician. It does not take to have our background to know how WRONG this was. Complaints were emailed and filed to the facility and the only thing they have done was move my dad from one wing to another. We finally had to make the decision of taking my father back to the hospital since he has gained more than 25 lbs in less than 2 weeks. Right up to the day that we transferred him to the hospital, the management was RIDICULOUS. My father almost fainted just by going to the bathroom and he used the emergency cord to call for assistance. It took them more than 1 hour for an ambulance to transfer him because it is NOT AN EMERGENCY (yet again). The hospital was just across the street just to let people know. as an UPDATE, my father is now in Hospice/palliative care due to the incident of fluid overload. His kidneys got overworked and his heart could no longer keep up. PLEASE DO NOT TAKE YOUR LOVED ONES TO THIS FACILITY!!!! read more

Very poor service and approach to care. Would not recommend to anyone for Elder Care under any circumstances. Showed very nicely when we were evaluating options, but delivered sub-standard care when it should have counted. Did not handle even moderate medical issues, such as daily meds or given orally, once they became necessary. Insisted I provide an outside-contract 'Sitter' when my father did not conform to their schedule and would try to get out of bed when incapable of doing so. Worst choice we made in Elder Care, now happy to be in a group home. read more

My mother entered Hollymead to rehab from a broken hip. She was 85. She had some dementia. In hind sight I wish now I had hired a sitter to stay with her in the day because I believe she was unable to let the staff know when she needed assistance. I found her wet, in the bed in same clothing for more than 2 days, she would cry help me and her pain was not managed. She did develope a pressure wound which was extremely painful. My mother did pass away while at Hollymead and maybe this review will help someone else. Hollymead was clean, the staff, nurses and aides were always helpful and when i brought issues to their attention they were addressed. But I don't believe they were staffed to assist someone such as my mother and this is why I would recommend a sitter if a family member can not be available read more

My mother was in Hollymead for two years. First in rehab for some broken ribs but stayed for extended care. The facility is very accessible and family friendly. There is a pretty outdoor courtyard with rocking chairs and ceiling fans. We shared a lot of strawberry ice cream out there and our dogs were allowed to visit her anywhere in the facility. The staff was helpful and professional. There was some turnover with the CNAs so I feel that area needs a little attention. They are so important for senior care and she didn't like it when there were new ones. The only area that I feel should be addressed is housekeeping in the rooms. Many occasions, there were random cups and silverware that didn't get picked up and they could have vacuumed more. But that is minor compared to the care she received. In a little over two years she never had bedsores or other injuries and she was pretty clumsy. She passed away this week. We laughed and cried with the staff who clearly loved her spirit. Yes, I would certainly recommend touring Hollymead to those looking for a place for their loved ones. read more

I had the unfortunate experience of my Mother being there for rehabilitation. What glitters is not always gold. She has been there a week.....one night she was upset by a bug being in her bed......Another night I am with her and she says she is in pain. My mother is not a complainer.......if she says something, the pain must be bad. We call the nurse at 8:50pm.......no response. We are reasonable folks.......we waited, and waited, and waited.......finally at 9:55pm i located the nurse on duty and ask for the pain med for my Mom. He says she will have to wait until 10:30 pm, because its shift change. Why couldn't he have just done his job ? Seems like he just wanted to palm off my Mom to the next shift.I waited with my Mom and the nurse for the overnight shift did bring her med at 10:30 pm. But by that time Mom's pain had escalated that she asked for 2 pills. I agree with Larry.....very poor service and approach to care. Scratch this place off your list. read more

My Husband was admitted to Holly mead after having five cardiac bypass grafts. They almost killed him. I would recommend not sending your love ones to this facility. From day one they told us they could not get a bigger or longer bed for 4 days, He is 6'1" his feet hung off the bed. He was left up in a chair for 3 night and 4 days. He developed congestive heart failure. His legs were so swollen they looked like they were going to split open. The nursing home doctor was not up on the best practices with treatments. IM Lasix ordered once daily?? The air concentrated was filthy with dust and dirt on the air intake, when they were asked to change the filter or the machine we were ignored. I cleaned and took pictures of the filth on the air concentrator . I had taken both complains the director. His medical condition worsen. Without being transferred and inadequate treatment of his congestive heart failure, he developed acute renal failure with inadequate treatment of both. He then developed respiratory distress. I told the day nurse, she did nothing. I had brought my own oxygen monitor by that time. I turned his oxygen up when I did not get a response to my request from the day nurse. The night nurse arrive and I still was voicing my concerns. She did call the incompetent nursing home doctor. She did bring in a continuous flow oxygen tank and placed my husband on 100% non-rebreathing face mask his saturation were good for 10 minutes (90-92%). When I looked at his saturation again, I saw he was down to 78% and that the oxygen tank was empty!! Of course I threw a fit to find out she had taken this tank of the crash cart and it only lasted 10 minutes. It was the only continuous oxygen tank that had oxygen in it. This was the second time his oxygen had ran empty. I called the nursing director, never returned my call. She did text me after the ambulance arrived to transfer him to the hospital. I placed him back on the intermittent oxygen until they could get an ambulance to get him out of the incompetent and neglectful place. His incision also opened while he was there. The wound care nurse looked at it and put a dressing on it. She never looked at the wound on his leg that looked redden and swollen. He developed a wound infection. People did not wash their hand before and after care. There was no foam available for had hygiene either. I had brought my own. I have reported this facility to the Texas Board of Health Department and I am awaiting their findings. My husband spent the next two weeks in the hospital and had two wound surgeries to correct the incompetent care he received at Holly Mead. Again do not take you family to this incompetent, neglectful uncaring facility!! read more
