I was checked in and had already started filling out paperwork before the front desk staff guy told me they "don't accept my insurance." This makes absolutely no sense, considering my daughter was just seen there a couple days ago and my insurance has not changed at all. Instead of double checking or trying to resolve it, the front desk employee simply turned us away after we had already gone through the check-in process. It was a complete waste of time and incredibly frustrating, especially having to bring my child in just to be turned away like that. This should have been verified before check-in, especially for a family that has been established with this office for years. The way it was handled was rude and lazy. No effort to help, no explanation, nothing. If this is how they treat people who've been coming here for years.

Lakeshore ranges from excellent to bad, depending on your loved one's concern(s). So, this is an…read moreoverall rating. One reviewer described safety with the check-in desk, the bird aviary, the washing machines, and the food being great. This is accurate, but I also experienced this as a facade. It's true, the first floor is modern, and by appearances, it really is impressive and rehabilitating. The rehab equipment is great too. I would also like to add that they have many resident activities: From nail painting to crafts, musicians, BBQs at the park, and many other social events. I've even seen therapy dogs and they actually once had Hawaiian Luau dancers! The excellent part of this review, however, is mainly only if your loved one is temporarily here, receiving brief rehab services, and, on the first floor. Not all that glitters is gold. The other floors are not so modern, impressive, clean or pretty. And they often smell of feces! As with many nursing homes, there is theft. Personal items go missing, belongings get lost or damaged, in part due to constant room/roommate changes, people with dementia wandering around, confused, as well as honest mistakes when moving things to new places, but there are also dishonest staff and dishonest residents, alike. Additionally, most rooms have at least two beds, but in may cases there are three or even four beds to one room. So, privacy on the upper floors is terrible. Staff longevity is poor; turnover is high! This includes management, upper staff, and directors of nursing (DON). Lower level turnover is much worse! Among RNs, Social Workers, CNAs, and many others, it is ridiculous. Turnover is stressful on residents, because they each have particular needs, and once you find someone you really like and get to know them, well, then they quit. What does this say about the facility? In fact, the only staff that seems long-term is house-keeping, many of whom have been there for decades. Minus a few other wonderful members, most of Lakeshore's care-workers don't stick around for long. Another reviewer also wrote about a resident's death by being burned alive--God Bless... Unfortunately, this is true, but it did cause staff changes. I believe it was a freak accident. Apparently, the resident died of smoke inhalation when his cigarette lighter exploded. He was allegedly in his wheelchair, and his clothes caught on fire. He was apparently on the north deck. It was a traumatic event---for his family, staff, friends, and residents alike. It radically changed the facility's smoking policy. A shock for all...very sad indeed. That said, I've also witnessed things such as a resident begging for help to use the washroom, because he couldn't walk, only to get yelled at by a CNA who told him to wait. He then peed his pants in his wheelchair. He looked humiliated. I also heard of another resident who needed his colostomy bag changed in the middle of the night. He called and buzzed for help, but apparently the staff person in charge of this duty fell asleep in front of the TV. The resident's bag burst, and he had to lay in his own...well, you get the picture... Many of the staff care deeply and they have huge, great big hearts! They have even reached into their own wallets for basic things and small comforts. Too many others, however, seem to have grown callous and/or quickly burn out. In other words, they just don't seem to give a damn. Sadly, this is probably one of the better nursing homes in Rogers Park. That said, even still, I would never send my loved one here, save for maybe a guaranteed stint in short-term rehab (e.g., one or two months), because it's not a happy place for long-term care or end of life issues--in my humble opinion. Yes, they have baked chicken, mashed potatoes and green beans with canned peaches, or ham and salad with decaf coffee. What if though, the same thing is offered to you nearly every week? If you have the same menu as a long-term resident (for literally years), well, guess what, this gets boring real quick. Everyday seems the same, day after day, month after month, year after year, leaving for a monotonous, mundane, upsetting existence. It can become very lonely, very fast. So, my advice, before sending your loved one here is to: Do your homework. Ask hard questions. Ask how long staff has been here and about turnover. Ask for a comprehensive tour of the whole building, not just of the first floor with the pretty bird cage. Don't be afraid to offend! If they do become offended, then maybe this is a red-flag. Verify for yourself, will your loved one be comfortable here? In the end, trust your head, your heart, and your gut. ...peace.

From broken blinds to dirty walls, to old wallpaper coming off the walls. When you come in the facility everything on the first floor looks great but my daughter's room on the second floor was terrible !! There was one or two good CNA's, but the rest were not caring, not professional and always had attitudes. Maybe my daughter was on the floor with those who were on state funded programs and those with money or private insurance had the nice rooms. Therapist, administrator' and wound care were great . The food was horrible.

I have been coming to this clinic for a couple of years to avail myself of the sexual health…read moreclinic. The place was never a paragon for efficiency or excelled in any way, but I recognized its worth as a walk-in clinic in an underserved area and I put up with the odd nuisance. My visit a few months ago however was a disaster that forever swore me off this place and I would advise anyone but those folks who have no other options to chance it. Everything on my visit went to ***, starting with a young provider who admitted he was in training for just a couple of months (and still was not supervised or accompanied by his trainer) who had to pop out every few minutes to ask "if we can do that" and who almost swabbed my throat with the swab already used for another test (ewww). Things really went downhill when I went to the lab to get my blood tests; when I got there, I noticed two people already checked in ahead of me who were sitting and waiting. I signed myself in and waited for about 25 minutes myself - after already spending an hour and a half in this place even though I had an appointment I had arrived promptly for. After those 25 minutes had elapsed and the door to the lab remained closed with no one calling patients, I went to a gentleman who appeared to be some sort of concierge and asked him to check with the lab if they were aware that there were patients waiting there for over half an hour (including the people who were already there waiting before me). He obliged and did so, and when he emerged from the lab he said someone was coming. And sure enough, SHE DID. A person emerged from the lab and without drawing in a breath tore into the poor youth who had been waiting there the longest, confronting him why he had the temerity to complain and lie that he was waiting there for half an hour. I drew her attention away from the underserving victim and said it was me. She then fixated me with her gaze and yelled at me that I had not been waiting for half an hour since I had signed at XX:XX. I said yes, at this point it had been about 25 minutes, but people had been waiting before I got there, which made it half an hour FOR THEM. She then yelled me again to "not be telling no lies as that makes it look like they're not doing their jobs." At this point I walked away and asked the concierge for assistance in filing a complaint. He was very obliging and tried to help, but after half an hour spending going from floor to floor and room to room he said no one was availalbe to take my complaint and promised he would have the responsible person call me. At that point, I left because I was not about to have someone who was verbally abusive or her colleagues stick a needle in my arm. To summarize the next couple of weeks, it took a while to get a call from the liason in charge of complaints, who mouthed a lot of platitudes but had little to show for it. In the end, I did get a call from the lab director, but other than "reassuring" me and apologizing to me he could highlight no definitive actions taken against the person he employed or any meaningful corrective action. On top of that, he could not explain the final nail in the coffin - why two out of the four swab tests had not come back or resulted. In summary, I will NOT trust my health to a clinic where a) a swab already used for one test can be used to swab you again, b) unsupervised personnel in training is not accompanied by a - imagine that - a supervisor, c) the lab staff makes people wait inordinately long for no reason at all and then d) yells at and abuses the patients who dare raise questions, only to e) have the liason responsible for complaints not be found for days until he is and then he is ineffective, topping it all with f) the lab director neither being able to explain how 2 out of 4 swab test results never came back and then failed to outline any concrete steps to discipline an abusive employee or to implement a concrete plan to root out these serious and unsafe practices.
