When I finally reached her case worker I was told to call her social security worker, Carol. Carol told me to meet with her physical therapy team and nurses to understand her care and discharge plan. After the PT team did not show for the appointment, I rescheduled. They met and said that Carol was managing the discharge plan. The. I was told to speak with a nurse. I was constantly sent from one person to another. They seemed very disorganized and I couldn't accomplish anything over the phone and when I was onsite, it still took forever to find someone that knew what was going on. As for care, she contracted pneumonia while there. They tried to discharge her with it, which her insurance objected to. They told me that she would be going home in 48hrs. Then she died. While the EMT's were trying to revive my mother, a nurse from the facility called and asked me what I wanted to do with her body. They didn't bother getting this info upfront and I was forced to send her to their default mortuary. When I called to ask to collect her belongings they told me they had to find them. They may have been thrown out. She was 72. Walked 8 miles every day. This is not a place to send loved ones.

Every day there is a schedule of activities, from balloon badminton to trivia to bingo. They have tea parties, live music, happy hours. Today when I visited mom activity coordinator Mariah was doing her nails. That is above and beyond in my book. They decorate for every holiday. The lobby, dining room and individual rooms are inviting and bright. The energy is great. Bottom line: I haven't seen my mom this happy in years. He's solely dependent on assistance at all times! **He was a patient for 11 days and was bathed ONCE!! (Shower days were Tuesday & Friday's) and was given one on **They FALSIFIED paperwork stating that someone bathed him in actuality his Physical Therapist was the one who gave him a bath. **No one came to look at his bandages to see if they were soiled, leaking or falling off for 11 days! **He's steady pressing the button for assistance to use the bathroom and no one comes!. *He's asking for help and he gets audacious response of, "that's not my job!" (By the CNA) *He's pulling the cord for assistance from an empty bed *He's wearing depends (that the family has brought him) because he doesn't trust the CNA's will come in a timely manner to assist him to the bathroom. *He'd asked the CNA to bring him a bucket with warm water & soap to wash up and the CNA's response is, "It's too late, you'll disturb your neighbors!" *** The physician assistant, NICK COLLINS IS RUDE, he absolutely has NO BEDSIDE MANNERS!! He's talks at you and not to you, he talks down to you, makes empty threats, he assumes EVERYONE is on Medical, the tone he speaks to you is very undermining. Needless to say, I'm VERY displeased Very about the series of events that has happened since my Uncle has been a patient here. My uncle broke his femur bone, so needless to say he's very dependent on staffing until he's completely rehabilitated. He's been a patient for 16 days (as of today)

My loved one has been a resident at Fremont Hills Memory Care for several years. With every visit I am impressed with the level of care she is receiving. My loved one always seems content. I know she is being fed, always dressed in clean clothing and receives the best attention. She has received nothing by outstanding care from Marta Galdamez(Memory Care) and Dorinda. I cannot express the peace in mind in knowing that my loved one is cared for so well, especially since I am hundreds of miles away. Marta anticipates my loved ones every need and makes sure that it is taken care of in the most exceptional manner. Fremont Hills always keeps me informed and up to date with what is going on with my loved one, especially since I live so far. I never need worry about my loved one because I know Marta and the team at Fremont Hills Memory Care is on top of it.
