New management is pushing 2 & 3 patients on one therapist at a time and the quality of care has drastically reduced. The patients are only a number now. The equipment is out of date and the staff always seems harried. The billing dept. double billed me more than once and I have been trying to get my medical records from here for more than 5 weeks.

    I'd give five stars if the billing process hadn't been a total cluster lasting five months &…read moreculminating in me (a medical billing rep, myself) telling the billing supervisor exactly how much of a mess their process was, how it was billed incorrectly, how the attempt to correct this (I called in January to inform them that they had billed incorrectly & they stated then that they would refund the money to Medicare, reverse the charges to Medicare & would bill Veterans Choice like they should have in the first place.) was done incorrectly & how rude the customer service rep & the rep's supervisor were. The first 2 claimed the issue stemmed from their software limitations but were, in fact, errors made by humans. The statement we received also mentioned sending the account to collections even though 1) the account was marked current and 2) you can't collect from the patient if you have screwed up the billing - that's not how it works. Statements are a mess. Line items do not match up with the dollar amounts & actions attributed to them. (Ours says VA paid & was refunded but it was actually Medicare that paid & was refunded.) The date of payment was stated as two days after the refund - not sure how you refund money you hadn't received yet. After a total of 2.5 hours on the telephone trying to clear up the mess the billing supervisor conceded that the process had been botched & the attempt to correct had been botched (tho the rep had good intentions) & that it did, in fact, need to be addressed. Several hours after the call ended the billing supervisor called me to tell me that she had actually called Veterans Choice office personally & that they had received the corrected claim. I wish the gal in January had asked her superiors for help in correcting the errors. This all could have been avoided. The experience that we had in the office was stellar. Great consult with the doc. Friendly, knowledgeable staff. Helpful referrals person. (Denise was AMAZING at helping us navigate the Choice Program for the first time.) The surgery itself was great. The surgeon was great. PT was great and very caring considering he's an amputee & cannot ambulated as their average patient can. Hospital nursing staff was great except for the person "assisting" my husband out of his room upon discharge on Christmas Day. I was attempting to gather all of our belongings and she was barking at us to get going. We literally had 15 minutes from the time we learned he'd be discharged that day to him getting in the vehicle to head home. It felt like it was a mass exodus because they were short handed due to the holiday. (They discharged 90% of their patients at the same time as my husband.) I had been documenting his output of fluids and blood for two days already so it's not like they were overworked with him. They came in to give meds 3 times a day &checked his vitals at the same time. I helped him w/everything else that they normally would have. My husband is an amputee who had just had back surgery so I needed to make sure he was safely getting prepared to go (getting him dressed, getting his brace on, making sure he used the facilities before the drive home, etc.) while making sure I had all of our belongings. I've never experienced such treatment from any hospital staff before at any hospital discharge. (Including the VA!) To say we felt rushed is an understatement. Our first night home he spiked a fever so I called the hospital who told me to give him Motrin. (One, he has Addison's Disease so a fever can be a sign of adrenal crisis and two, Motrin is not recommended after back surgery as it can hamper bone healing.) I feel like if it had been any other time but the holidays none of this would have happened. Bottom line, the docs are great and know what they are doing. Don't have surgery around holidays and stay on top of the billing to make sure it's done correctly. (And hope you don't get the same lady that we did to bum rush you out upon discharge.)

The communication with patients is not acceptable and leaving a voice message which is the only option available, does not mean you will get a return call. This was my experience more than once. At the present, I have been trying to have my records transferred since May 12. Again, speaking to a person is not an option and saying they will call back in 72 hours is not true.

Years later our mother was suffering from dementia and we were very pleased to learn that Richard Tyer was a neurologist and was moving his practice to Tyler. Mom had dementia for several years and eventually passed away a couple of years after seeing Richard Tyer. In appointments with other doctors we had always found it difficult to have frank discussions with the medical professionals as Mom was just enough with it to disrupt such discussions by arguing that she had no problems. So, when we were making arrangements for her to be seen by Richard Tyer, we spoke with Tyer's sister who was managing his office at the time. It was agreed that we would have two initial appointments with Richard Tyer. The first would give him the opportunity to evaluate our mother and the second would allow a frank discussion between my brother, Richard Tyer and me. We agreed to pay for any costs required for the appointments. I had my mother's durable medical power of attorney which was in effect because of her much diminished mental capacity. All HIPPA, medical, and legal points were addressed, and arrangements were made to have her files transferred from Mom's prior neurologist. We had our first appointment with Richard Tyer and it went as expected. She did poorly on her cognitive exams and nothing was said to contradict the prior diagnosis of dementia. However, prior to the second appointment I received a call from Tyer's sister during which she apologized but went on to say that Richard Tyer would not meet with us for the second appointment. NO EXPLANATION WAS GIVEN. We were upset that we had gone through the trouble for nought and Tyer's sister was clearly upset about the position her brother put her in. I expressed our disappointment that Tyer would treat his patients this way, let alone old family friends. Shortly after, I received a second call, this time from a yelling Richard Tyer during which he cussed me out for upsetting his sister, threatened me if I ever set foot in his office again and then hung up on me. Nothing that was said to his sister warranted any of this. Perhaps there is some law or code of ethics that made Richard Tyer decide not to go forward with that second appointment. No medical professional who has heard this story has come up with any explanation. In any event, if there was a legitimate reason for going back on the arrangements, neither Richard Tyer nor his sister ever communicated it to us. Not long after all this, my cousin ran into one of Richard Tyer's oldest and best friends at a T.K. Gorman sporting event. This fellow was at the event with Richard Tyer and his family. My cousin told the gentleman about what had happened and the response was telling. Richard Tyer's friend said words to the effect of "That's Richard. He's an **shole." No doubt. The man is untrustworthy professionally and he's a hothead. Shortly after seeing Tyer we went to see Dr. George Kariampuzha. He was excellent and we cannot recommend him more highly.

I would honestly rate them 100/10. They were attentive, kind, and made a very difficult situation much easier. While my overall stay had some challenges -- particularly with response times and delays in assistance at certain points -- it seemed like this may have been related to staffing levels rather than the individuals themselves. I also experienced some concerns around discharge timing while I was still dealing with significant pain, weakness, dizziness, and fluctuating blood pressure, though I ultimately stayed longer due to a heart-related issue. One small note: food delivery was often delayed, so ordering ahead may help. Overall, the standout staff I mentioned above truly made a difference in my experience, and I'm very grateful for the care they provided.

The Good: Nearly every nurse I had was personable, knowledgeable, and willing to answer my questions. I'm not great at remembering all the names, but almost the entire staff on the 4th floor of the Houston tower were just phenomenal. Also, when I was admitted, I was on a "cardiac diet" that was from an incident from many years ago, but I was also cleared for outside food (imagine not being able to get a packet of salt from the cafeteria, but I could order Whataburger if I wanted to.) I appreciate that the lady working at Food Services took the time to call around and find out what was up, and get it fixed so that it reflected my actual health needs. Also, the paramedics who transported me were kind, skilled, and calming. The Bad: I had one nurse by the name of Annabelle who did not flush my IV for a full day, leading to it needing to be replaced, and who would not get the charge nurse. During my last IV antibiotic infusion of her shift, I told her it was painful (it shouldn't have been) and she told me the IV was probably just tired, then left for an hour and didn't come back to check on me, just left it going. She also basically manhandled me, rather than allowing me to move my own pajama sleeve out of the way, pulling on my IV and causing additional stress.
