The level of incompetence, unprofessionalism, disorganization, horrid communication and lack of ethics is staggering and dangerous, which ultimately puts your loved ones at risk. In point of fact, within the first twelve hours of arriving at Skyline, my mother was not properly supervised and was allowed to fall and catastrophically break her ankle. Her orthopedic surgeon said she will never walk again. I initially accepted that sometimes these things happen, but given all that I witnessed over time, I realized that this may have been easily avoided. Of note, this happened under the more competent previous administration. As a retired clinical social worker with over 40 years of experience in human services with federal, state and local facilities, this is by far the worst I have witnessed. Please note that everything stated here pertaining to the issues and communications is objective, and documented with dates, emails, photos and video. Based on their responses to other horrible reviews I expect management to try and discredit this review as well. In dealing with issues like this, I try not to make it personal, but both Kristy Lavey, the Executive Director (ED) and her completely unqualified "Wellness" Director, Elysia Angala (WD) on many occasions were so arrogant about their uncaring and incompetence that they chose to make it personal, with myself and other family members. Repeatedly! Whenever I politely brought up anything from the fact that Mom was usually lying in her own excrement (frequently noted by multiple Renown medical staff), to the never-cleaned black moldy toilet and piles of dirty laundry on the floor, I was met with DENIALS, LIES AND THREATS of retaliation: "perhaps your mother is no longer appropriate for this facility". I noticed these things in my visits, but they call into question what's going on when you're not there to see it. Skyline unilaterally decided that Mom needed to leave (after being taken OFF of hospice care). They DID NOT communicate this to me. Shortly after the director told me "this is your mother's home. We will do everything we can to keep her here!", I got a call from a nursing home salesman, who had been given a considerable amount of protected personal and medical information about my mother. CLEARLY unprofessional and very unethical. WD later indicated that a call from a third party was supposed to be notice that my mom had to move out. According to the Long-term Care Ombudsman who got involved with the case, Skyline was legally required to notify me in writing, with a rationale. This is one of several State violations she noted. What we got was an email from the WD on a Friday, notifying me that Mom had to be out on Monday, and that if she wasn't they would discharge her to a local hospital - a perfect example, among many, of Skyline's complete lack of caring. RUN AWAY! Current open investigations HCQC: Nevada State Bureau of Healthcare Quality Compliance Federal government for HIPPA violations Carson City Sherrif's Department for multiple theft issues including cash and personal items Ombudsman stated that Skyline had violated several state statutes and should be reported Stolen property (ED and WD took absolutely no responsibility) from cash to an entire new bed, which disappeared when a hospital bed was brought in. When we reported this to the Sheriffs deputy taking our statement she told us "the exact same thing happened to my grandfather when he was at Skyline!" Ball dropped multiple times on prescribed medications The worst quality food you can imagine. Think of a ten-cent hotdog as the main course Front desk person eliminated. Impossible for family or medical providers to reach a human Other family members not allowed to attend meetings with ED and WD Although there were a few really great line staff, they could in no way make up for the truly blatantly NASTY, uncaring management

If you are considering hospice care for a loved one, I urge you, please, look elsewhere…read more My dad was admitted into Gentiva's in-home hospice care in early April of this year. He was dying from Parkinson's disease, a cruel, torturous illness that slowly robs a person of their strength, mobility, and independence. As a family, we knew his time was coming, and we also knew that he wanted to stay at home. The last thing he wanted was to spend his final days in a cold, sterile hospital room. To honor his wishes, we chose Gentiva. Their homepage carries this promise: "At Gentiva, we believe every day is an opportunity to make care more personal and life more comfortable. That's why we offer home health care, palliative care, and hospice care to give people the support they need with dignity, grace, and love." Sadly, our experience fell devastatingly short of that commitment. The medical director who was assigned to oversee my dad's hospice care did little more than sign off on orders. She never visited my dad in person, never scheduled a virtual or phone consultation, and never met the man whose comfort and dignity she was entrusted to protect. My dad was a physical therapist in Minden, NV, from the 1990s until 2018. He served his community with love, dignity, and kindness, a true healer, admired and loved by his patients. To see a man who had dedicated his life to helping others become a prisoner in his own body was heartbreaking beyond words. We had hoped that Gentiva, and the medical director in particular, would help make his final months more comfortable and dignified. While the hospice nurses and staff who cared for him were compassionate and wonderful, the medical director repeatedly refused to increase his pain medication, even as his suffering became unbearable. His nurse, Jesse, pleaded with her several times to adjust his dosage, but she would not. We were given no explanation. Some of my dad's final words to my sister were this: "I am the patient, I feel like shit, why won't they help me?" Eventually, his pain became intolerable. We had no choice but to discharge him from hospice and admit him to the hospital. He was admitted on Friday, July 11th, and passed away three days later, very early Monday morning, July 14th, alone, without my mom or any of our family by his side. My dad did not deserve to leave this world that way. He lived his life with integrity, compassion, and love, and he deserved to die with the same dignity he gave to so many others. It makes me sick to think that the medical director continues to practice medicine and call herself a healer. After some research, I learned that she serves as both Gentiva's medical director and operates her own medical spa. It's hard to imagine how someone juggling the roles of medical director for a hospice and private business owner could possibly provide the level of care Gentiva promises --"with dignity, grace, and love." I'm angry for my dad. I'm angry for my mom. I'm angry for my siblings and myself. My dad deserved better.
