The care this rehabilitation facility provided my loved one was minimal. On the second day, the nurse could not confirm whether the last shift had dispensed her medicine. She rang the bell to use restroom, but no one came for 20 mins. I saw three bugs in the building, two alive and one dead. She was not given a shower for two weeks. Soiled sheets were not changed despite our requests. I'm sure it could be worse - it could always be worse - but this place should only be used as a last resort. read more

Very poor. Granted it appears to be clean; however, patients are largely ignored and left to fend for themselves. Folks for 14k a month a nurse or CNA should see the patient at least 3 times a day. We just removed my father from this facility after 30 days and a week of observation by the family. He fell no less than 3 times while a patient. He was never clean or cared for. A veteran and a first responder deserves way more than they can get from this facility. read more

Kept husband in diapers so didn't have to assist him to walk to bathroom. During 1 visit, my husband sat on toilet 15 mins after pulling assist chord. I finally had him get up n took care of him. They spilt a cup of cold water on him while bringing his food. Whn I came, he was still in wet bed and wet clothes. They never shaved him until I had to get nasty. His brkfst was always cold. They were only wrapping one leg & was supposed to be wrapping BOTH legs as directed when admitted. I wouldn't take my dog there!​ read more

Signature health care has not updated this place in many years. Rehab program is awful. My mother had a hip replacement and went there. One good nurse in 7 days. (Stephanie was the nurse). A rare good MA. Most of the MAs were sitting doing nothing when I looked for them to help with something. The grounds were so beautiful years ago. Now unkept and look awful. Food was just gross. Physical therapy department barely passable for helping mom to improve. There must be better places in town. Would never go there again. read more

Very dissatisfied with the care my Dad received at this facility. My Dad was transported from the hospital to Jefferson Place for physical therapy. We asked them serval times to put guard rails up on the his bed and they responded they would have their maintenance department install small ones on the bed, but never did. Every time my Dad or one of us pressed the call button it would take on average 20 to 30 minutes for someone to come to the room or for us to go out to the nurses station to get one of the nurses to respond. The fourth day into his stay at this facility I received a phone call from Jefferson Place saying my Dad had fallen and was taken to the hospital. When I asked which hospital he was taken to they didn't know which one. When I asked which EMS took them they didn't know that either. It took them an hour and a half to tell us which hospital he was transported to.. When he arrived at the hospital his temperature was 93 degrees which told us he was lying on the floor of his room for a extended time before the staff at Jefferson Place found him. This facility is not equipped to take care of patients with memory issues. In addition we feel the facility is severely understaffed. Craig S. read more

Visited a friend who was recovering from a broken hip. The facility has dual functions as a rehabilitation center and as an assisted living for seniors. The different functions are in differ sections of the building. The staff were friendly and helpful and the building was clean and odor-free. I thoroughly enjoyed my visit. read more

A shout out to Angela(OT) and Angel (PT). I was at Jefferson Place for 10 days rehabbing a broken arm and working on mobility. These two really took good care of me. I miss seeing them every day. I am still playing my APBA Baseball game. I just completed game 65 for each team. I will play 160 for the season. I will finish in December. The new cars for 2018 season will be shipped to me in January 2019. read more

Horrible experience.:-(( mom had knee surgery.. the day after she arrived she had to beg for pain meds.. they gave it to her at 12:15pm and not again until 9pm when it was supposed to be every 4 hours.. the day after surgery.. then she begged for an ice pack and it took over an hour to get one to her.. day of discharge, they didn't call in her prescriptions and late getting her ready.. we just feel like nobody cares at all.. horrible place to take a loved one.. read more

VERY dissatisfied with this facility. My father is 86 years old and retired Lt. Col from the Louisville Fire Dept after 30 years of service to the community. He is also a Veteran of the United States Army. Unfortunately he has progressive dementia and is struggling to ambulate. After being discharged from the hospital our family chose Jefferson Place Rehab for him to receive physical therapy. The first 9 days he was there he only received physical therapy 4 days. We were then informed that he was going to be discharged because of lack of improvement in therapy and an inability to ambulate. They then tell my mother that she has to transport him home. In order to get in the house you have to climb stairs. How is an elderly woman supposed to get a non ambulatory elderly man up stairs? Unfortunately, I reside out of town and was unable to assist. I contacted the social worker of the facility and verbalized my concerns about his lack of rehab as well as the lack of arranging transportation home. Instead of the social worker doing her job of contacting a stretcher transportation company and arranging transportation home, she gives my elderly mother a list of ambulance companies to contact. My mother did as instructed and paid over $300 + $8/mile for my dad's transportation home as the ambulance company informed her that they didn't accept insurance. I contacted my father's insurance company who informed me that the proper way to have arranged transportation is for the facility to contact ambulance companies that accept insurance. Moral of the story is....don't judge a book by it's cover. Jefferson Place looks good on the outside but their Physical Therapy and Social Services departments are the WORST! Shame on you Jefferson Place for mistreating a community servant and U.S. Veteran. read more

Both my parents have stayed here for rehab and they had great therapists....and the food was good. That is the only good things I can say. If you are need of alot of personal attention from nursing and aides, this is not the place for you and that's putting it nicely. I will not let my Mom ever return there. read more

I have to agree with the other reviewer who stated that while the rehab therapists are good, if you need any sort of personal attention from the nurses or aides this is definitely not the place for you. We had one wonderful aide, Alba, who was competent and caring. Otherwise, any concerns I had were dismissed and unaddressed by staff. Overall such a disappointing experience I would never have a loved one return nor recommend to anyone else. read more

This review pertains only to rehab services provided at Jefferson Place, not long-term care. My son went to Jefferson Place for rehab following a lengthy hospital stay. While there, he was sent back to the hospital twice. The first time, they clogged his feeding tube by not following protocol. The second time, he was sent to the ER from his doctor's office due to a life-threateningly low level of potassium. I had tried to tell them at Jefferson Place that he wasn't doing well, that he could barely get around, but they blew me off. Thank God he had a scheduled visit with his own doctor. Otherwise, he might have died. Phone calls were not returned, soiled bed clothes were left lying on the floor, and it took an act of congress to schedule a shower for my son. Jefferson Place was not my first choice, not by a long shot, but my son's medical condition is complex, and the better rehabs turned him down. When researching Jefferson Place, I discovered they had been fined over $150,000 during a recent three-year period. I asked why, but never received an answer. There is one thing this place excels at -- having meetings. The people in charge of this place are ALWAYS 'in a meeting'. I will admit that the physical therapy staff seemed competent. I will also admit that there was one nurse -- ONE -- that made me feel like my son was in good hands. And there were two nursing assistants that were reasonably responsible and caring. So, Jefferson Place may not qualify as the worst rehab facility in the city, but if you have a choice, go elsewhere. read more

Absolutely horrendous! Had to make numerous calls just to get my mother her medication and for someone to come to her room. They gave the wrong medication, which she was allergic to, that put her in the hospital. The nurse then tried to blame her for taking it. No communication, no oversight and absolutely apathetic. This place should be shut down. read more
