I had a appointment to come in for alcohol detox, they got me all checked in and had my things…read morelooked through. I was there from 5-9 and they never once medicated me which they should have. They looked at my history and saw I had a seizure due to not having medication for withdrawals, instead of giving me medication and realizing my vitals were fine, they wheeled all of my belongings to the front office and told me I needed a higher level of care. The staff that dealt with me I forgot his name he was some sort of fake doctor. Said I was going to the hospital. I'm sorry but if you call yourself a detox place and you can't medicate to make sure I do not have seizures it's so unprofessional. While the manager just stood there staring at me saying nothing. Then they go on to say that I can't even do their inpatient treatment after I detox at the hospital. This place seems like they have mediocre nursing staff and "doctors" I would rate this place a 0. They refused to ship my stuff here that wouldn't fit in the ambulance and now my mom and parter had to go pick up my belongings. Absolutely ludicrous. I've been to some terrible places but this was by far the worst. What kind of nurse doesn't Medicate someone withdrawling after seeing the doctor. While I was there for 4 hours. I had to fill out a bunch of repetitive paperwork for no reason. I will never even drive by this shit building and I'm only putting one star because I have to. Id fill out none if I could. The only person I want to praise is Jaqueline

The lady forced herself between me and the patient and proceeded to abscond with him. I informed her that it was my wheelchair that the patient was in and I would like to operate my own wheelchair to which she abruptly said "no" and proceeded to tell me that she needed to do some of her own duties before I could take my chair with me. I informed her that I was merely transport and I was short on time and did not have time to run around the facility with her to, she attempted to proceed anyway. There are further considerations to mention regarding liability in the event of incident. The longer the patient is in my equipment likelihood I could be held liable for any incident. As a courtesy, I try and accommodate them as much as I can, but if I am short on time, they need to stop trying to intimidate me into accommodating their schedule. It would be different if they had requested my services and were paying for them, but they are not my customer.

I got excellent care and occupational therapy (OT) and physical therapy (PT) 5 days a week. The staff treated me very well and I learned a tremendous amount in PT & OT. There will always be times where a wait feels a bit long no matter where you are, but you must realize the nurses know how to prioritize the severity of the requests. I feel that if my other knee ever needs replacing, I will certainly plan to stay at Evergreen Windsor the next time to recuperate as well. Thank you Windsor Staff, for the great care! (Colorful Resident from room 28) 9/23/16 Insurance only covered 20 days of inpatient rehab, so I have 6 more PTs scheduled at OHSU-as outpatient. My first OHSU evaluation therapy was yesterday and the therapist said the progress I made from the therapy at Windsor was excellent, and this is after having no formal therapy for 9 days. I have been exercising at home the way Windsor PT and OT taught me, but wouldn't have scored so well at OHSU had it not been for my Windsor stay. Thank you Windsor staff. My new knee and I will always be grateful!

They were very kind and helpful to me but I just didn't need to be there. If one has someone they love in a SNF (I know, it's more than a SNF) someone needs to be there with them as much as possible. Like I said, they were very kind and helpful to me. One suggestion though. When the Noc shift went to laugh and yack it up, do it in the break room. About 3 staff were laughing and talking right outside my room at 4:00 AM. But I did receive good care.

Never consistent on meal time or medication. Lots of extra staff as it appeared to me just wandering around and blocking the hallways. A couple of the staff and the cleaning staff are very good and caring, and a special thank you to them (and I'm sure they know to who I'm referring ). As far as front desk, office staff, PT and medical staff it was a sorry excuse for a rehabilitation facility. The doctors on staff came by the day of admission for short visit and then for about 5 minutes on discharge. I would not recommend this facility to anyone looking for an even average, let alone better or exceptional care facility.
