I traveled all the way from Portland for my initial appointment and had expressed at that time that I require a trauma informed level of care due to having severe PTSD. The provider I met with had absolutely zero understanding of what trauma informed care is or how to provide it. When attempting to explain my need for this specific level of care, it was merely called into question instead of respected and or accommodated. My wife attempted to contact their office as well, in an effort to explain the severity of my PTSD symptoms and request an appropriate level of care and/or a collaborative approach with my primary care provider to ensure my needs were met accordingly. Instead of finding a way to assist in proceeding forward, they told my wife that they do not; or are not willing to accommodate my needs. As my PTSD diagnosis is a disability and falls under ADA protections, I am absolutely appalled at not only their professed inability to provide an appropriate level of care but the manner in which they so poorly navigated the request. The office staff was extremely rude and unwilling to assist in finding a viable and appropriate option for me to recieve the care I need at their practice. As a retired provider, I am beyond disappointed in their lack of professionalism and/or ability to serve individuals with disabilities in an ethical and appropriate manner. As a patient, I am absolutely devastated by the inability of this office to meet my needs or even express any concern regarding my care.

In January I had my routine dental cleaning & check-up and let Dr. Duffy know I had a "lump" on the roof of my mouth. After he looked at it he said I needed to have a biopsy done & he immediately referred me to an Oral Surgeon & got me an appointment with them that day! Long story short, I had a tumor in the roof of my mouth but thankfully it was diagnosed in time & after having another surgery the pathology came back clean on all margins. The staff are very friendly. and professional this office is very clean I'm impressed. Dr. Wallace is very personable, knowledgeable, and he made me feel very comfortable. On January 19th, my bridge broke in the front tooth. I immediately tried to make a dental…read moreappointment and Salem Family and Cosmetic Dentistry. They were not able to see me until January 29th leaving with a hole in my front tooth. They then changed the appointment to February 2nd. I voiced m opinion in a letter written to the dentist. He then called me on February 2nd refusing to fix the bridge. I also asked repeatedly in emails to send me my records and the name of the dentist that would work on my bridge. They sent a document that was encrypted and I could not open. They refused to send me my payments that I have made. They also refused to send me the information where the original dentist is practicing. Within the past year alone, this denture has broken three separate times. This is unacceptable for work of this cost and nature. While I understand that the dentist who originally completed the work no longer practices there and that the office has changed ownership, Salem Family and Cosmetic Dentistry purchased this practice and assumed responsibility for its patients and prior dental work. I have also been told that you will not repair the denture unless I pay upfront, allegedly due to my "attitude." I am upset because I have every right to be given that I have had to deal with three failures of a very expensive denture in less than a year. My frustration comes from feeling ignored, dismissed, and treated unfairly.
