Without getting into a big discussion, I spent a great deal of time completing the online paperwork prior to my visit. Provided ID during my visit. It was an out of network referral. I received a call the next day, the young lady asked me for my social security number, I asked why that was needed and she said for identification purposes. I said well I provided my ID when I was there and I explained I wasn't comfortable providing my social security number. She said, and I quote, "ok no problem" and ended the call. That was almost 2 months ago and I still have not heard back.
