My mother was sent here to strengthen the bacteria balance in her gut. She was forced to wear a diaper and was not permitted to use the toilet, and she is not incontinent. She would tell them I have to go to the bathroom and they'd say well then go in the diaper and I'll come right back and change you, and they wouldn't be back for hours leaving her to sit in her own waste. The food she was served was so bad sometimes she did not know what she was eating, but she had to eat because she was so hungry. I sent her food from Instacart so that she would have some healthy food to eat in her room. They would limit her water so that she would not urinate which resulted in delirium. She called me in the middle night and her mouth was so dry. I could hear her lips sticking together because they were refusing to give her water. They ignore the call lights. They would leave her in her poopy diaper for hours at a time, causing her to get UTIs. The delirium that she acquired at this place from their inadequate care was so bad that my California family thought that she was in her end times and was going to leave her there. When I called the office and kindly asked them to change her diaper for three hours to change one diaper, they hung up on me. I heard the nurses with unkind angry yelling tones yelling at my mother while I was on the phone and they did not know the phone was open. I texted the management of the facility trying to get my mother help and 90% of the time they ignored me. When company is there or they know the phone is open they are not hostile but as soon as there's no one around, they are. I've already called Ombudsman's to check out this facility. It should not exist; It is that bad. I drove 1800 miles to rescue my mom from this facility two weeks ago. The emotional abuse is one of the hardest hurdles to help her over. Do not take any loved ones to this facility. I will be reporting them to anyone who will listen. My son sent this up accurately when he said "that facility is nothing more than a pound for old people."

Would never send a loved one there. beyond the lies and staffing shortages and bad experiencesread more

My 94 year old aunt was having a painful transition and they took her with hospice (Cambridge). I was there for the few days and she was clean, dry and comfortable. They are staff that is invested and have spent awhile with Villa Mesa, this should be a continued goal of the management's part--there are residence that look better than many years ago from the last time I saw them when my employment ended and that is a huge testimony and the way it should be. The residence at Villa Mesa are thriving and well cared for. And even in a transitional period, my aunt was treated so well and so was I. Thank You all. Also, it really is the cleanest place too. Take it from another nurse who has instructed all around the Inland Empire, this place is a good, safe place and it's clean and their nurses are smart. The people in this rehab do not care for these patients at all . My husband also took out his catheter and he tried to escape, he wasn't supervised during this they found him on the floor. he'd always be drugged up but always tried finding a way to communicate so one day my daughter and I visited him and we were just about to leave and once we were saying going to say bye my husband was begging me to take him out of this place and to help him but I thought in my head that he just was tired of being in this rehab. I got sick and tired of him going back in forth to the rehab to the er room, so I do home care and I became his care giver, I had this guest room in my house and made it his own room and ever since he's been home he hasn't got sick and moves and talks more than he did at the rehab. Two days ago we put my husband on the wheelchair and my daughter was talking to him and my husband said when he went to this rehab people would be hitting him in the private area and my husband would try to defend himself but he was scared and drugged when they did that, he told us that a girl would hit him in the private area. Finally when he gets out of these medications and once my husband talks better we're finding out who hit my husband and suing this rehabilitation. I do not recommend this place at all 0/10 The nurses you meet might be nice but they do not care. This is my husband now.

Carmen also took the time to alert us to a potential issue with my mom's medical insurance that could have impacted her continued stay and care at Heritage Park after the first week. Understanding how sensitive and life changing this situation was for our family, especially concerning my mom's overall well being, Carmen approached everything with genuine care and professionalism. She helped us clearly understand the potential impact and guided us through possible solutions. Her knowledge, experience, and willingness to advocate for what was best for my mom truly became the calm in the storm we needed. Thanks to her guidance and expertise, we were able to resolve the issue within 24 hours. Carmen has been an absolute pleasure to work with, and we truly appreciate the care, attention, and support she provided in making our transition into Heritage Park seamless. My mom has been at Heritage Park Skilled Nursing for a little over a week, and the professionalism, kindness, and compassion shown by the entire administrative and healthcare staff have made her experience extremely comforting. The level of care, attention to detail, and commitment to providing the best service have been very reassuring. We feel extremely confident that my mom is in the best hands here. The entire Heritage Park staff has been amazing and a pleasure to work with. Working with Anna Alcaraz, Director of Social Services, has been both a pleasure and very insightful. She has played a key role in helping our family obtain resources for legal authorization such as Power of Attorney, as well as guidance for current and future healthcare support and housing or placement services tailored to my mom's needs. My mom has also shared how much she enjoys her physical and occupational therapy sessions and how friendly and supportive the staff has been. The RN evaluator is also caring, friendly and very capable. If family is involved and communicates they are willing to work with you for the care of the patient. The housekeeping staff is friendly and kind and always want to help. My mom needed doctor appointments and transportation and this was all arranged by the hardworking social worker. She handles the care meetings and coordinates care as if she was a case manager. She handles the hair salon and even additional nursing attention as needed. She walks around and checks in on patients. At discharge she handled all the needed items for home care and worked hard to advocate for my mom. Admission staff was awesome. They were very welcoming and made sure she had coverage with her insurance. They are great to work with. The doctor that oversees the facility was not easy to communicate to in part due to her availability. She is a full time doctor seeing patients in her office during the day. She made an effort to call back after hours but my own schedule also got in the way. It would be best if the doc attended the care meetings upon admission so goals and care can be discussed clearly. It was difficult to pinpoint care plans. Staff would send message to doc that family want to discuss care and it was hard to receive info back quickly or at all. Maybe if the doc was present at limited but fixed hours on site it would help. PT/OT was good and made an honest and persistent effort to assist recovery. It helps if family is involved and asks questions regularly. My mom was independent prior to her medical issue so the goal was set to return her as close to that as possible. You have to make it known to them where she was before very clearly. Otherwise she may be released too early and becomes a fall risk. The food staff is very nice and concerned. They made an effort to give her Greek yogurt often for her digestion. The food is otherwise not very tasty or presentable and comes from the main facility. I'm sure the quality in Claremjnt Manor assisted living is better than in the rehab unit. My mom ate mostly in her room and so could not ask for extra items from the kitchen as those in the dining room. The dining room was filled with behabing persons that need assistance eating and this was hard to see for her to see while eating. She is a social person otherwise. I would definitely recommend this facility. They have helped her recover through 4 events in the past year and a half. No place is perfect but Claremont manor care center does a great job. My grandpa was given a necklace with a device to press when he needed aid to use the restroom or other needs and they wouldn't reach him for 30ish minutes and he would then sit there in urine and feces, when all he wanted was for someone to help him to get up and use the restroom. What if it was an emergency? What if he was desperate for help? They wouldn't get to him for half an hour or more! When he would go eat in the dining area they would take the same amount of time to take him back to his room, he would sit there for 20-30 min after he finished. I timed it a few times when I would go visit to see how long they would take. One time I saw a puddle under his bed and I told the staff he needed to be changed and they told me his cup of water fell, which was a complete lie. The staff is very heartless with zero patience. He was there not even more than 3 months and declined quickly while at the facility before passing. When I visited him he was scared and crying just wanting to return home. This was a place to help get him get strong and back on his feet but in turn the complete opposite happened. One time I over heard a staff member yelling at an elderly patient in the hall way. Once they realized I was there they quickly apologized trying to justify. His restroom would go a week without being cleaned, to the point that we started doing it ourselves at times. I was warned by a hospice nurse from the start to be careful there because they will try and get away with as much as they can unless you visit your loved one all the time and they are aware you're watching. Please beware sending your loved one here, do your research. If you do please visit them! I read in the comments that people were told to leave good comments so don't trust everything you read.

However, we had 1 staff member who came and decided she needed to take pictures of my incoherent fathers private parts. We did not sign any paperwork giving consent for that to be done either. My wife walked into my fathers room to the sight and sound of their staff member taking pictures on her personal phone of his naked private parts. My wife immediately told her to delete the pictures and to never do it again. I messaged the management about it and they apologized profusely & promised me the problem had been rectified and that it would never happen again. We never saw that staff member again either. Outside of that specific issue we had no other issues with any of the staff who cared for my father. The rest were amazing & caring. My other issue was when my father passed. We called and had to wait like 6+ hours until someone came to pick up his body. The real kicker was the staff member who came out to make sure my father had really passed. She was disturbingly more focused on making sure she got a count of all my fathers prescriptions that they had issued him. He's laying dead on the bed and she wants us to run Around the house and track down his medications. She could have cared less about the 20 other medications that he was issued by other doctors; she only cared about the meds that they issued (many of which are simple over the counter meds). She then proceeded to take forever individually counting all the medications and then she poured them into cat litter & mixed with water. Then she left the bag for us to dispose of. Biggest distasteful professionalism in my life that I've seen. My father just passed and I got to sit at a table while this staff member counts medications. Like for real! Horrible method of assessing when something is appropriate to do; timing! Outside of these 2 horrendous issues (of which, I have not sued them over for illegally taking pictures of an incapacitated mans private parts) - I will say that the Visiting Nurses who come to care for the Hospice Patient - they were 5 stars Hands down!!!

Don't think twice if you are put into a position to have to put a loved one on hospice. GHC hospice is AMAZING!
