Thank you for took care of my father 2 years ago! However, grief is a funny thing. When I received the news that my father had passed, the first thing I was told after that was I need to be there instantly! Excuse me! I live in hacienda heights, and I had to go to West Covina to go pick up my mother. I could not be there instantly I told the male nurse over the phone. You need to train your staff to speak in a more sensitive way. Especially after a bad news had given to the family. "I am so sorry your dad has passed, however; we do have policy the body has to be removed in x amount of the hours. Please drive safely to get here asap when you can." This was the way, your staff should have spoken to me, 2 years ago. I did not complaint before because I was in total shock! Do Not take advantage of people's grief and depression again! Today I had the privilege, i'm pretty a very amazing person today at Claremont Heights post acute care. When you drive by or enter the building, there's nothing glamorous about it. I see a lot of work as they're trying to make improvements. For a visitors, employees, and residence of this facility. I had a question where my friend was located at. They were very quick to look in the computer and guide you which way to walk closer to the room. Smiles on each of the workers. For they truly care of what type of care they give to each resident that lives there. They work with post acute patients that come out of the hospital. Claremont Heights provides physical therapy so the residence can be more independent so they can go home quicker. The beautiful activity room was wonderful to see as this is a heart of each residence, getting better and stronger and feeling loved. They provide the medication each residents needs. The check-in kiosks is so easy to understand and use. Once you walk in the lobby, the staff is very happy that you were there very kind and always has a smile on their face. The only downfall I noticed is the facility. Some rooms have cracked in the ceiling and in the wall. There needs to be a custodian team to keep the rooms swept and cleaned to whether you'll find trash on the ground. To all the nurses that work there the CNA's that work there the kitchen crew that works there. I am so thankful that every resident has a smile on their face, knowing they are safe and well loved. There are vending machines and some courtyards so the residence can buy their favorite snacks. The roses and the colors of the gardening are just so beautiful. It feels like you were at home. When you walk into the lobby, there is music being played. And there is a TV screen and it shows the most amazing colors of every season like a picture window. read more

Erica was amazing from the start. While my FIL decided to leave, the staff were great. Every single one. They had him up taking steps post back surgery. The facilities are very clean. We had no issues at all. Would highly recommend them for skilled nursing. read more

I was a resident there for almost 4 months. I have to say, that I have good reviews and not so good reviews. 1) the pictures on this site, don't show the true facility atmosphere. 2) l loved my LVN's (Asante,Aden,Emily, Monica and LaTosha ) For my CNA's you girls were my rocks! There for me for everything. You girls do not get enough credit, validation or pay for everything that you do and go through. So thank you so much (Jazmin,Yaya,Micah,Anahi,Tia, and Lauyrn) 3) The food sucks sucks beyond sucks. Eggs every morning and some kind of gravy on almost every dish. Yuck! 4) Staff Admin are hardly there to hear complaints and when they do, they do nothing about it. Also in the 4 months I was there, I only saw a doctor once and that's only because my family keeps asking. 5) It took 2 weeks for them to change me rooms when I complaint about my roommate harassing me and the staff. I become emotional distress because of this. But when they needed my single bed room for an isolation person... Well I got moved within minutes. 6) Don't do your laundry there! Things get lost or given away and nothing is done about but because you don't have the receipt of your clothes. REALLY! BS 7) THERAPY!!! The true reason I was there and let me tell you they give you some tough love. I do wish sessions were really timed or it be a little more one on one to get the better use of what to do during therapy. My OT Lisa, Nancy and Noah helped me gain more range and use of my left arm. They taught me tricks to help assist myself when dressing myself. Plus they taught me big words that they would call the nerves and muscles in my arm, shoulders and back. My PT, man these guys and girl have me the courage I needed to stand up on my own. I had so much fear of falling and hurting myself. They helped me build strength and muscle to be able to walk with a walker. ( Yesenia, Melvin and Jonny) Thank you therapy team for music Fridays, jokes and snacks and all the touch love you gave me. 8) Needless to say, everyone experience here and everywhere is different. So don't judge a book by its cover. If you like weekday you like great. If you don't, we'll do something about it and leave. I gained friends and my ability to walk again plus courage. read more

They are the best. Everyone working there gives 100% to the patient. I read online that the food wasn't great. I found the food to be good. You need to salt and pepper it yourself because not everyone can eat salt there. I highly recommend this facility to anyone in need. read more

The RN's, LVN's and CNA's are all very nice and always want to make sure you are comfortable. The PT's and OT's are awesome! When it comes to the food, it's all nasty. To be honest the food looks like prison food. I feel so bad for people that are here long term. You have to buy your own food or order food from door dash daily. read more

My grandmother was placed at the Country Villa Center a little over a month ago and I feel so blessed about the situation. My mother was her caretaker for almost a year and it began to be too much for one person. We were sad about having to move my grandma out of our home but the Country Villa Claremont Center has been amazing. The staff is the nicest group of people around. Nurses are very attentive and caring. They treat my grandma with respect and love. I'm thankful that there are good people in the world that give their time to help people like my grandma. I will forever be thankful for the staff/nurses. Also, Erica from Admissions is amazing. She helped me and my family out at a time when we were most in need. She was very professional and made the admissions process as easy as possible. I felt that she cared about me and my family, without even meeting us yet. Thank you Erica! I'm forever grateful for you and the team. Before being admitted, my grandmother wasn't able to walk. She was in bed for about six months prior. Less than one month in the facility, I saw my grandmother walk with the assistance of nurses and a walker. The facility is very clean and in a great neighborhood. The food also seems good! Physical Therapists are awesome! The staff is caring and competent. However, the physical plant is shabby and the food situation is terrible. Some things are well cooked but there's a real disconnect about getting the meals right. Bathing was an issue too. I had trouble getting my meds on time, especially my pain meds. read more

I am a disabled veteran, I became a disabled veteran as I served our country honorably earning The Purple Heart for Heroism in a combat situation. Since the late 1970's I have been in and out of VA Hospital's, and lately over the past 10 year's I have been in and out of private hospital's and multiple skilled nursing and rehabilitation facilities in helping me with after care of my surgical procedures. No other facility has impressed more than Country Villa of Claremont in the City of Claremont. When I came to Country Villa of Claremont my stay was to be only temporary as I waited for an opening at a different skilled nursing facility. I was transferred by ambulance from Pomona Valley Hospital in the evening, and Mrs Erica the Admissions Director was fully aware of my intent to transfer to the other skilled nursing facility, and by the following morning I was asking to speak to Mrs Erica, and when Mrs Erica promptly came to my room I asked Her, if, it was too late to change my plans and remain at Country Villa of Claremont, she replied that I could, definitely, stay at Country Villa of Claremont and welcomed me to Country Villa of Claremont, and this has been Home, now, for the past 3 year's. There is No Other Skilled Nursing and Rehabilitation Facility that I would love to be at than here at Country Villa of Claremont for the Nurse's and Administration make this the suitable facility for hospital and surgical recovery and rehabilitation. Ms Christina Arias our newest Facility Administrator has certainly made it Her Mission to Provide and Care for All Short and Long-Term Resident's, and has made a clear and definite difference in the life's of everyone here at Country Villa of Claremont, everyone here has seen and felt the Love, Concern's, and Daily Care that We receive from Ms Christina Arias and Her Team of Equally Caring Professionals. I would like to give this facility the absolute highest recommendation! My husband was in their care for approximately 2 and 1/2 months. This place offers the finest staff in all departments, from the front desk Reception to immaculate Admission and Business office professionals who navigate insurance and financial concerns. Thank you Erika, Marisol, Jennifer and Denise. The Nursing team is exemplary without exception. Marl, Ann, Fernando and Asante addressed my husband's many issues as a result of diabetic wound, multiple fractures and post operative recovery at the highest level of care. Appointments and transport to multiple Physicians for follow up and a continued plan of care which was so necessary were arranged with precision. The many LVN and CNA personnel were always kind and helpful. Finally, I want to recognize the amazing Physical Therapy department whose knowledgeable staff worked with us daily to teach my husband to walk again. I was transferred there from the hospital after major back surgery, the facility was in dyer need of renovation. Patient rooms and bathrooms needed painting, updated beds, chairs, nightstands, bedside tables, drapes etc. The lobby was renovated prior to the Covid Pandemic, so it gave the impression it was a beautiful place. The only thing that was relatively new was the flatscreen tv, although they couldn't find the remote for it, but eventually produced a universal one. My window was so dirty you couldn't see out of it. The drapes needed to be replaced, looked as if food or something splashed all over them. Bathroom had old plumbing, commode, sink, all needed replaced. Shower was disconnected. When I arrived on a Monday afternoon the trash was never emptied, and someone's clothes left in the room. The evenings were very noisy my room was near the nurses station and I could hear them loudly chatting during the shift change. Initially when you're admitted you are placed in a quarantine area for two weeks with other patients who have various types of conditions ie dementia, mental and major physical disabilities as well as surgical patients like myself. The first evening I was there, a man in a wheelchair rolled himself into my room, he roamed the halls without any supervision. It seemed like the facility was not informed or prepared for my admittance. The food was inedible, and I required a special diet, gluten free. They couldn't accommodate my diet initially so it took several days. My medication was not transferred correctly from the hospital and it took a couple days to straighten it out. It seemed they were short staffed, because the last weekend I was there the same LVN worked all the shifts all weekend long. I asked him where everyone else was and he said everyone called in sick. After enduring the place for seven days I asked to be discharged, but they were reluctant to do so. I was given PT everyday, so I felt strong enough to take care of myself. I believe if I wasn't so insistent about leaving , they would've kept me there for another week just for the additional Medicare revenue. I would NOT recommend this facility, it's not a favorable environment to recuperate and recover in. read more

My mother recently stayed at Country Villa in Claremont and we had a wonderful experience. All staff is very friendly and attentive. They always made sure her needs were met and in a timely manner. She received daily physical and occupational therapy which helped her make a quick recovery. I am happy I made the decision to have her attend Country Villa during her recovery. My father was admitted to Country Villa in Jan 2021, with a focus on PT/OT to help regain his strength after his stroke. The rehab staff did an amazing job with him, he always looked forward to going to the "gym". Also thank you to the nursing staff that were always so patient with him, even when he was his grumpiest. I don't recommend to send your love ones at this facility. They are not caring or attentive. The rooms are kept unsanitary dirty, The nurses are not sensitive to there patients or to the family members if you ask them any questions about the patients care or updates. The staff is always in a hurry when they check in with patients while on shifts. There are some nurses that are dirty from other patients germs and body fluids on there Scrub uniforms. Most important there not nice or caring to there patients. I've seen this bad service over and over again. My love one was afraid with the staff and constantly repeated how she didn't like the staff or the facility. Her jewelry disappeared and were missing and never recovered. If your looking at this facility to make it there home it will be a bad choice please look into another facility where they are kind and patient. the administration are insensitive to the patients. First I have to say that any of these places come with their challenges. It's important to stay involved and advocate for your loved one - regardless of which facility they may be in. Having to entrust my most precious thing in the world - my mother - to strangers was / has been the most difficult thing I've done. COVID has complicated visitation but the restrictions the facility has had to impose have been regulatory so not unique to them. However, in the six months my mother has been there I can say that she is being well cared for and there are a number of wonderful people that have been helpful. My first introduction to the facility was the admissions director - Erica. She has been stellar from the start. Many of the nursing staff such as the director Anne and Marita have been attentive. And there are nurses such as Sara, Fernando and Kendall that have gone above and beyond to help me when I've had concerns about my mom. As I mentioned, no place is perfect - but I can say that the new administrator Colleen has genuinely been responsive to my concerns. Marcus and his team in activities are great as are the receptionists - Isela and Madylin that are always helpful The inside of the facility is decent and there is no obvious odor which is a very good sign. The outside definitely needs work all around. There is a lovely gazebo but it's in very poor shape and not useful. The back of the property is most definitely in need of attention and keeping the huge facility garbage cans outside of the dining room and patient rooms is certainly not ideal. But these observations are secondary to what is the most important factor - thus far my mother is generally well cared for. And for that I am grateful. read more

Took in my father for a blood transfusion. Waited 4 days to do a blood test even though he had one the day before. They then pushed him to the hospital where he died. Could not get any communication. Had to go to the facility and wait for hours to talk to someone. read more

I felt hopeless, that I was never going to get my grandmother out of a horrible facility. It was a blessing when I finally found a place that really cared and did everything they could to help me. My mother was recently discharged from Pomona valley hospital and transferred to Country Villa after fracturing her pelvis. She then received physical therapy and care. The LVN, CNA, and physical therapy team were all very kind and attentive, always making sure she was comfortable. My mother enjoyed the food and the overall service was great. My family and I were in search of a facility that would provide the highest quality of care for my sister. The location of the facility was perfect and I found it to be immaculate. It was imperative for me to partake in the tour asserting that I was able to view all areas that my sister would be utilizing. Upon completion of the tour, I was able to meet with the Assistant Administrator Ms. Arias, who was very professional and made it a point to go over my concerns in order to assure me that my sister would receive the highest quality of care from her staff. She also expressed that she would make herself available to me and my family in the event we needed to address any concerns regarding my sister. I was extremely pleased with my meeting with Ms. Arias, so I made the decision to have my sister transferred to Country Villa, Claremont. I am very happy with my decision, as my sister is adapting well to the facility. In fact, she appears to be improving each day and she is making the effort to be sociable with her roommate and the staff. I take care of lots of different elderly people. I've been to many different rehabilitation centers, and this Claremont Country Villa is by far the very BEST!!!! The staff there is just so friendly.. Came here to visit a long time friend of the family. We were so appalled with the lack of care and demeanor of the nurses. The first nurses we encountered in the hallway were very impersonal and rude. We noticed our friends (she was in the activity room) enteral feeding had spilled on the floor & let the nurse know. She said she would get it cleaned up. When we returned to the room, there was a "wet floor" sign in front of the room, so one would think it was cleaned. But when entering the room, the feeding was still on the floor along with Kleenex pieces from the other roommate. Asking another nurse about the spill on the floor and her response was "this spill was from last night and it's all dried". WHAT??? Did she say LAST NIGHT ??? Ugh .... so I asked "Last Night?? You guys don't have paper towels here to wipe it up after a spill?" read more

I've been with the company for over 15 years I have also had 2 of my relatives at Country Villa My husband's grandmother who was on hospice the nurses took very good care of her and kept her comfortable till the end have to say thank you to Asante LVN her last days he went above and beyond to make sure that grandma was as comfortable as possible till hospice came to be with us. My uncle was also a resident as well for about 7 years I special thanks to Alejandra Glenda, Alvaro, the list could go on you guys became like family You guys are awesome nurses my Uncle greatly appreciated everything. This Facility is a wonderful place. The director of marketing, Bryce, made sure all of our needs were met while my grandma was being admitted to their facility and made sure she was well taken care of during her time at this facility. Also Erica in Admissions was also very helpful getting things done and making sure she felt welcomed to the facility. The cleanliness I would say is excellent and the nurses like Mario(RN) I would say treats their patients as if they were their own family members and thats what we were looking for my grandma when she was getting out of the hospital and getting ready to go to a transitional home so they can help her get back on her feet to be able to come back home with us. Over all I would say my grandma's stay at this facility has been great and all the staff continues meet our expectations and of course most importantly my grandmothers. Will update this review as soon as my grandma is ready to come home and feels better. With everyone's help at that facility its looking a lot soon than the doctor's has interpreted. read more

The only reason this review is getting one star is because there is not a zero star option. First, when you call and staff transfers you to the nurses station, you are on hold for a minimum of ten minutes and up to an hour for a nurse to pick up and sometimes you hold for a long time and they hang up. Second, the staff lies constantly, it's hard to believe if there is any truth in what they tell you. They do NOT watch residence take their medication(s). I was a health care worker for ten years and worked in a dump facility, Claremont Country Villa makes the place I worked at look like a 5 star facility. Believe me when I say I didn't think there was a worse place than where I worked. Lastly since covid-19 the staff does not wear face coverings. My loved one was sent to the hospital during covid-19 and when my loved one returned to Claremont Country Villa my loved one was not quarantined. This place needs to be investigated and shut down! Please do NOT let your loved one(s) live here. read more
