My grandma had been hear several times before and her treatment was ok. This past month of January 2019 she was sent here from the hospital to build strength. Not only did she not build strength she got significantly worse. On January 30th she fell from her bed her bed did not have the brakes and it rolled out form under her when she try to sit down. The nursing home never contacted any of my grandmas emergency contacts. The next day when I went to visit my grandma she told me what happened and she said that the people who helped her up told her would be better if she didn't say anything. She had a huge bruise on her left leg from her thigh to her ankle. I spoke with the social worker Annie and nobody was aware of the situation. Annie reassured me that they would do an investigation to see what happened and I still have not heard anything from her. Now on February 9th my grandma fell yet again. She pressed the nurse button to come assist her to the restroom and per usual nobody came. so she went to the restroom by herself and fell in the restroom. The nursing home contacted me immediately this time and I asked them to have her transported to the nearest hospital where I would meet her. I no longer wanted her there as I did not feel comfortable that she was being taken care of. Once I arrived at the hospital the paramedics said they were getting different stories saying that she fell out of the wheel chair.... I will never take my grandma back here the staff has no sympathy to the patients. I hope one day there loved ones never get treated the way they treat people. read more

I am writing this review specifically for the DoN (or I think she is). I believe her name is Elen. A couple months back I went to visit my great aunt and saw this staff member being rude, arrogant, and condescending to other staff. It was pretty uncomfortable and awkward. I ignored it then. But I recently spoke to my great aunt and she mentioned it. Considering such a vulnerable population of folks live at this facility, I'm concerned. Toxicity and workplace abuse are real issues and I don't want my great aunt, or anyone for that matter, exposed to it. I hope this is addressed. read more

I am so glad to have chosen this nursing home for my grandma. We toured other facilities but we chose this one because out of all of them this place was the cleanest and felt very homey and welcoming. This was our first experience with a nursing home and so we were really nervous and confused about how the whole process worked but once we spoke to the social workers, Phoebe and Anie, we were sure that she would be in good hands. They were very informative and very helpful with the whole process. They covered all the bases we needed to make sure my grandma could come back home with the right equipment and services. We especially loved the nursing staff. The CNA's in station 100 were very sweet and were always happy to help. Dro, Narbeh, Jessa, and Lorraine are wonderful nurses. They treated my grandma as if she was their own family and were very efficient in my grandma's needs. I would highly recommend this facility . 10 out of 10 :). read more

My mom was at leisure glen for 2 years, and let me tell you, she went after the hospital to get rehabilitation, within that time my mom fell and broke her hip, and when I asked what happened, no one knew. Then we took her back to recover from her hip surgery, and we had one problem after another. My mom fell a few other times, with big bruises, again no one knows what happened. I found my mom soaked in her urine, and I demanded answers, again no answer. The last one was that my mom walked out of the facility and no one knew she was missing for a long period of time. Someone from the street returned her back, and everyone was told not to tell the family. All the good reviews are written by them, non of it is true. I don't recommend this facility to anyone. read more

My mother has been looked after in this place on and off for 3 times over the past 3/4 years. Every time everybody from the administrative level to the cleaning staff has been extremely caring and professional, I would say over this period the change in the staff has been the absolute minimum which to me is a sign of very happy workplace. The place is still spotless and smells fresh without overpowering smell of fresheners. To summarise the whole establishment, all I can say is that I am very fortunate to have my mother taken care in this place. Highly recommended. read more

The social workers here are TERRIBLE.All they know is have high heels and showing off like strangers. I believe one of the most important skills a social worker MUST have is COMMUNICATION.They are not able to manage the situation at all. My Mom got hit by next door patient who was a sweet lady but mentally ill and they knew she hit nurses before.They did nothing about that .No one came to visit my Mom after the Lady hit her .So I had to call 911.My Mom wasn't able to scream cuz after her stroke she got speech limitation and she was there just for rehabilitation after the leg surgery. When I sow her in that situation and NOBODY cared my heart was crying. Almost more than one month before my Mom leave there I noticed social workers that we need help for lifting my mom cuz she lives in second floor.Social workers knew already and they gave me peace of mind BUT it was a lie.I took my Mom home from sixteen stairs by myself.Also,the front desk Nurse had no idea about my Mom releasing day. Leisure Glen has the most fabulous, flexible, and dedicated staff. Anytime you walk in everything is clean and organized, patients are happy here whether they are there for longer or short term. I've only left with a positive experience. I've worked with quite a few of their patients after they discharge home and everyone has been satisfied and grateful for the care and attention they received here. This place is dirty!! The rooms are dark and gloomy, the sheets are thorn and dirty!! The staff is unresponsive!!! !! It's like a morgue!! You want to burry your loved ones before they die? If I could rate them with a negative I would!!! This purpose for this facility is for elderly care, however, there is a complete lack of care, medical professionalism, and operations. My grandmother was sent to this facility for rehabilitation after being in the emergency room. Our family was assured that she would be receiving the upmost care at this facility and that she would return home in a few days. This has not been the case at all. We have experienced the following: 1. On Thursday, March 28, 2013 the nurse was supposed to bath her and due to her back pain, they did not. She was uncomfortable for 3 days until we had scheduled to take her out of the facility for Easter dinner on Saturday March 20, 2013; my aunt bathed her in her home. Though we had approval by her doctor and the supervisor of the facility to take home for 6 hours, we still faced hostility and trouble since no one at the facility was aware of her chart, situation, and what had already been approved. They have proved on numerous occasions that they don't know their patients needs and are not organized to even be able to find their patients charts. 2. On Thursday, March 28, 2013 at around 7:45pm, we asked the male nurse about my grandmother's progress, blood pressure, and blood sugar level (she is diabetic). The nurse had not checked her levels after her dinner. He proceeded to check her levels, in our presence (three of my aunts, my cousin, and myself), and her blood sugar was about 403, which for any diabetic is extremely high/dangerous. We further asked if he would be administering her insulin and he advised us that the facility did not have her insulin yet but that another patient has the same insulin and that he would administer the other patient's insulin. This was extremely difficult for us to accept; as such action is medical malpractice. Unfortunately, the medication was necessary and we did not have a choice. 3. Upon being admitted to the Leisure Glen Post Acute Care Center, the nurses at this facility where supposed to make an appointment with a specialist in order for my grandmother to receive injections in her back and knees for pain. She was admitted on Wednesday, March 27, 2013 on Saturday, March 30, 2013 we asked the nurse about when her appoint was, we were told the following "we called on Thursday night and they told us to call on Monday." In my experiences with making a doctor's appointment, I have never been told to call on another day to make an appointment. This led me to believe that they had not called for the appointment. 4. On Saturday, March 30, 2013 prior to removing her from the facility for Easter dinner, my aunt had asked if her medication was given and the nurse claim that he had administer her medication. When she asked my grandmother if she had received her PM medication she said no. My aunt continued to question the nurse regarding her PM medication, he answered by stating the wrong patient's last name and when my aunt corrected him he replied "I completely forgot that I gave the other patient her medication, I will bring your mom's medication and give it to her." After that, I received a phone call from my sister asking what CC amount of insulin she should receive, I told her that the facility should have injected her before she left for the 6 hours and she advised me that they didn't. Due to this lack of care and proper administration of medication she was severely ill and weak, we were contemplating taking her to the emergency room. Luckily, we also have a diabetic grandfather that my sister and I inject with insulin and have been trained on doing so. 5. On Sunday, March 31, 2013, I rushed over to the Leisure Glen Post Acute Care Center, as my cousin advised me of the mistreatment. I approached the nurse and inquired about her medication. The nurse was unable to find her chart because they do not have computers and/or operationally effective patient information tracking system. They instead have several different binders that have various patients in each binder. I told them that my grandmother was complaining of dizziness and they were unaware that she was admitted with that symptom. However, she has Meniere's disease and has suffered with dizziness for the past 6 years and has been treated for this disease for the past 6 years. I am highly doubtful that the doctor would not have informed the facility of this. There is a complete lack of communication between the staff. When asked for further information regarding her medication administration, the AM staff and PM staff where both unsure of what the other group of nurses did. 6. The doctor advised us that my grandmother would be receiving physical therapy on a daily basis because she would be relieved of taking aspirin in order to be able to receive the injections for pain. Since being admitted to the Leisure Glen Post Acute Care Center she has been bed ridden and confined to a wheel chair. More problems but I don't have anymore room to type. read more

Not sure where to start but this review is overdue. The staff here was so compassionate, kind, and supportive during a very difficult time in our lives. As my grandfathers health declined last year he was in/out of the hospital numerous times which ended up with multiple admissions to Leisure Glen. He also was very adamant about leaving and being in the comfort of his home as he was so independent his entire life. The staff was extremely patient, their communication was supreme, and we truly felt they treated my grandfather and each of us the way they would with their own family. Ani and Hripsime in social services were extremely kind, resourceful, knowledgeable, and always willing to help, a huge and special thank you to you ladies. Their doors were always open for us, we have a large family and we were all concerned and involved but they were so professional always and again, very patient. Their own administrator & staff, Mary and Phoebe would personally walk around and check in on the patients and facility. I wish we wrote down the names of the nursing staff that helped my grandfather day and night because they have an excellent team and staff that deserves recognition. We feel extremely fortunate that our grandfather was treated with the utmost compassion and are eternally grateful to the entire staff of leisure glen, thank you all again! read more

Leisure Glen is located in a nice green area, alas the staff in Administration Dept.,are not accommodating and have not great attitude towards patients and their relatives. The Ladies in that Dept, are kind of rude and arrogant. My Dad was there for a while and then he got transferred to hospital. Due to the surgery and post anesthesia he got restless and angry and when we wanted him to go back to LG, they literally refused him stating he is not a proper patient for them. Needless to say while my Dad was there, there were lots of patients who were making noises, being restless and yelling throughout the night, but I guess they picked on my poor dad. I heard from the others that this is not the first time they reject patients after they being transferred to the hospital. We transferred him to other place and then when he got fever and hospitalized again, the hospital, without asking us, got in touch with LG for his accommodation. This time they refused him, telling that they have no problem with the patient but they do not like his family. I was shocked when I heard about this. So if you have a parent who lives there, you as a kid, have no right to ask questions relating to their health? You would not get to know the vitals and every other clinical details of them? My dad has lost his angry and restlessness after being on right medications, and now he is literally good. I think what they did to my Dad, should be categorized as "discrimination towards patient" and I really am disgusted by such acts. Anyway I strongly believe in karma, what goes around, comes around. The other issues that we experienced while there: 1- they do not have enough skilled nursing stuff during the night shift and if patient needs help to use restroom, he/she needs to wait forever, even the nurses told us that they are not happy with that situation and budget cut, and asked us, as the relatives, to complain to the hotline by giving us the proper telephone number. 2- They lost my dad's clothing on the very first day while washing it, and left us with the decision not to leave the clothes there for laundry thereinafter. 3- most of the time the premises smells bad and when we asked why, the response from one of the administration ladies was "do you think this is a wedding place"...I was shocked by such a cruel response. She should have explained the case by a very professional attitude. 4- We shockingly found out that while we wanted to choose another place for Dad, the administrative people in the new place were so nice to us at the beginning and one of them also showed us the room that they had assigned for Dad, but after calling Leisure Glen to get his records, they rejected Dad. We heard that LG was bad mouthing about us which lead the new place to refrain from accommodating dad. Nobody has the right to do such an act of shrewdness. I have to admit their skilled nurses were great and caring. Their rehab staff were good as well. read more

I was very impressed with the overall experience my dad had at Leisure Glen a month ago. My first time with an immediate family member needing rehab, however I've visited many nursing homes in the area since I work for a hospice agency as a nurse. It is indeed one of the best facilities in the area. Most of the time I hold my breath when I walk into these places since it wreaks, however at Leisure Glen that was not the case at all. Not sure how they keep that place spotless. Nurses were very attentive to my dad and the roommate they paired him with was a sweetheart himself. My mother has been a resident at Leisure Glen since last November. I have found the facility to be very clean, cheerful and well staffed. The staff is very caring. The only aide who was not up to my standards was terminated before I had a chance to complain. The food is generally very good. And I have always been able to find someone to speak with whenever I have had a question or concern. We are very happy with Leisure Glen and wouldn't hesitate to recommend it. read more

The management at Leisure Glen Post Acute Care Center is the worst i've ever experienced .This facility is absolutely horrible. They have lied on numerous occasions about my mother's condition. She also under their careless management resulting in my mother developing Covid 19 and falling over 13 times in the past few months. Imagine calling over ten times a day, and every single time they tell me to call back. All they care about is payment from medi-cal for the bed they could care less about the patient who is not receiving the proper care. If you care about your loved ones DO NOT send them here. The social worker is a complete joke. They do not communicate with you and allow incompetent patients to make their own decisions without informing the next of kin. Staff is too busy talking outside, having parties, eating and talking to actually do their job. Don't ever call and expect a return call because that doesn't happen. That's if you can even get someone on the phone. Half the time it just rings or you get someone who doesn't understand English. read more

Everyone at leisure glen was amazing. My grandfather was there for some time and they truly made his experience as well as ours amazing. They really took care of my grandfather and they were very attentive with him as well as the other patients. They always had something planned for the patients, for example on cinco de Mayo they did a margarita day (of course no sugar or alcohol in the drinks), 4th of July they did a bbq for the patients and decorated the halls and the rooms. It was very pleasant and my grandfather and our family truly enjoyed it. Ani, Hripsime and Phoebe were amazing! They were very supportive, patient, and their thorough follow ups and constant communication were very much appreciated and kept us all at ease. Another thing I loved was that walking into the building you right away noticed how clean everything was and how happy EVERY SINGLE EMPLOYEE was and everyone had a smile on their face. I have seen a number of nursing homes and have nothing but good things to say about Leisure Glen Care Center. The facility is clean and well kept and the staff are all wonderful to interact with! I especially want to thank Ani, Phoebe, and Mary for always being so helpful and going above and beyond for us. My father has been with Leisure Glen for over a year. From my first hand experience, and comparing this facility to several other ones in the area, this facility is among the best. The staff here are caring and empathetic. My father is happy and well cared for. Many times during the course of this one year, nurses from the facility have communicated with me, at different hours of day and night, about my father's condition, medication, etc. Some of the staff might not be fully proficient in extensive communication in English. But one should not construe that as an insensitivity. These people work hard and have a lot to do. I for one, with a hands on experience, truly appreciate all they have done and continue to do for my father. I could not have done this without their help. read more

Leisure Glen Post Acute Care Center, conveniently located behind Forest Lawn Glendale because more than likely, that is where your loved one will end up. My mom was in this facility for what was only suppose to be a 2 week stay but was only here for about 7-8 days before returning back into the hospital to treat her VERY BADLY infected Bedsores.This alone was a red flag for us because she was suppose to be doing rehabilitation but since the nurses have ZERO patience they would not encourage my mom to get out of bed, led alone move her from one side to the other or keep her clean. The staff/nurses/doctors are completely rude and unprofessional. The staff at the front desk doesn't even bother looking up to acknowledge you when asking for assistance. The nurses are so incredibly careless and ruthless, it's inhumane to see! HUGE MISTAKE to place my mom in this facility under the care of these inhumane group of people who are clearly just there for a paycheck. Zero passion for what they do. Disgusting! I hope these people are treated this poorly in their final days. My mom passed away a few days after retuning to the hospital from this facility. DO NOT BRING YOUR LOVED ONE TO THIS HORRIFIC PLACE! read more

My grandma had to go to a rehab center after being in the hospital for a fall at home to recover and build strength. I visited many facilities in my area and was so happy to have found leisure glen! The facility is very clean and the staff are so helpful and kind!! My grandma was ready to go home after two weeks and felt bittersweet to leave. She made connections with other people at this facility (staff and other residents). She didn't want to leave her new friends but was happy to go home. My grandmother was recently in the rehab facility for about 2 weeks. All the employees at leisure glen are caring and knowledgeable. The care and support they showed her is more than what I expected because she was here 5 years ago and I was hesitate to bring her back. Wanted to write a review on a facility that doesn't get enough credit. People complaining about this facility have not quite had family at other facilities..My mother was sent to a local facility in Glendale for recovery since I could not take her home yet..It was awful and I felt guilty leaving my mom there. I had lost all hope in nursing facilities, until I stumbled upon Leisure Glen..This place is heaven compared to all the other facilities..It was so clean, and absolutely no smell of urine anywhere. Rooms were much bigger than the others I had seen. Unlike the other facility which housed 3-4 per room, all their rooms had only 2 beds per room. Besides the cleanliness, the place was full of life, nurses cared for my mom like they've known her for years, and rehab was really encouraging on a daily basis..there will never be a perfect nursing home, but this is as close as it gets to one. I can't say enough positive comments about Leisure Glen Post Acute Care Center. From the time my sister was admitted on March 7th to her release on June 15th, she had nothing but professional care. She was admitted the evening of March 7th and 2 of the staff members came in to check her from head to toe. The following morning the dietitian came in to ask her was there any kind of foods she was allergic to or disliked. From that day forward NOT once was she served anything that she did not like and she said that the food was quite good considering it was basically hospital food. Another main factor was that the facility ALWAYS smelled clean and fresh. I went to visit her twice a day and stayed several hours. They were always cleaning the halls and came right into the rooms and cleaned the floors, bathroom and windows to the patio and there were always clean linens available to the patients. Her main reason for being admitted to Leisure Glen was that she needed to learn how to walk again. She had sprained both of her ankles and was bedridden. The phylical therapist she was assigned was WONDERFUL. His name was Harold and he brought her to her feet again and she is walking just like before. I can't say enough good things about how he deligently worked with her and how happy she is today because of him. All of the upper management was so helpful and considerate, Ani, Phoebe, Elizabeth and so many more. My sister and I are so thankful to all of them for the wonderful care she received. No one ever thanks the late night nursing staff, but my sister has a big THANKYOU for her CNA Ophelia. She was so caring and attentive. Its been two weeks my mother is in this facility and I can only say great words. I am beyond thankful for all the nurses for taking good care of my mom. Nurses are very patient and nice. Rehab therapist is the best, he is doing a great job with my mom and I can soon take her home. Social worker Annie is very nice and very helpful. I thank her a lot for Helping me to apply my mom for Ihss so when she discharges home she will have a caregiver. Thank you Leisure Glen for having an amazing team. Leisure glen is a very nice place compared to all other nursing homes I have toured for my mother. My mother has been there for over a year and girls at social services have been so helpful with everything. They go above and beyond just to make sure patient and family are satisfied. Nurses try their best too and we all understand how hard it is to be a nurse. Facility is clean, cleaning crew goes around around the clock to make sure everything is cleaned. Took all possessions including customized wheel chair and stored them off site. When asked to return them, said couldn't find them. Finally returned a few clothes and said didn't know what happened to the rest. Went in hospital and on second day room was stripped of everything. Returned from hospital on third day to find everything gone. Did not honor 7 day bed hold. Very humiliating to be stripped of everything when you are sick in the hospital! read more

Great administrator, knew every patient in the facility by first name. Emphasis on short-term stay for rehab and return to home. Culture is very important at skilled nursing facilities, and this one is all about getting patients home, that is literally what they talk about from morning until night. There is also a small part of the facility dedicated to patients who are long-term and don't have the means to return home. Equipment is top notch, facilities are squeaky clean, parking is ample and convenient. Still, most impressive part of my visit was the mantra "get in, get well, go home." read more
