If you are looking for a place where the employees are compassionate, then this is the place for your loved one! The facility is well-maintained and has a beautiful courtyard where families can enjoy time together. The caregivers take extraordinary care of the patient, and also provide a lot of support to the families. We were blessed to have been referred to this facility through Dr. Papukyan for my father. A few people I would like to recognize ---Allina, thank you for taking such great care of my Dad and helping me get through the sad days. This place was absolutely horrible. My mother was supposed to get rehab at this place and instead was left unattended and ignored for the 7 days she survived at this hell hole. This was in April of 2021 and they really took advantage of the fact that family had limited access to the patients. She died of a heart attack and was not even noticed for hours. I had been vaccinated and was bringing COVID tests and they wouldn't let me see her. When she passed, we walked right in without any vaccination proof or tests. They didn't care that we were walking into a room with another patient. Their inconsistency shows how they were only following the rules that they found beneficial. Please don't send your loved ones here. They don't care. read more

The care is excellent, for a Rehab and long term care, The office needs to improve with their follow up and customer service.

They could not even cook farmer John sausages for breakfast (only time I tried some of their food), I would leave everyday to go out to eat. Found bed bugs 3 times I my bed. Oakpark is by far the nicest, cleanest, and most welcoming facility I've been to. The staff is extremely attentive in regards to the care for the patients, you'll definitely feel safe having your loved ones here. They take care of everything from A to Z, with passion for their job and for the residents. The rooms are extremely tidy, as well as the dining and activity rooms, and they also have a beautiful patio outside that is very nice for the residents to go outside and feel like they're part of the community. This facility is one of the worst facilities I've ever experienced. I'm so happy my family member will never be coming back again. These nurses/cna verbally abuse and neglect all patients. There were countless times I heard one morning cna verbally abuse patients and nobody cared. Even when I notified the administrative staff they didn't do anything about it. They left my family member soaked in pee and poop for hours and didn't change them once for 6 hours plus. This happened multiple times. I don't know what the nurses there do, bit it's definitely not work at ALL. Never takes your loved ones there, PLEASE. read more

This facility has outstanding therapy! I entered this facility too weak to even get out of bed and now I can walk home. They are extremely accommodating especially with the patient food preferences. Some of the most extraordinary patient care I have received from various staff members. My aunt Mary who turned 100 last march has been at Oak Park Healthcare Facility for almost a year now. I can't say enough about this fabulous place. How could she be so lucky to end up in a place like this. Besides being super clean, they employ the most caring and loving people you'll ever meet. It is truly a hidden gem in Tujunga! It's obvious that Narek, the Director of this facility, is doing an awesome job of managing this place. He's friendly, efficient and respectful towards the patients and their families. Talin also is very knowledgeable, great at managing day to day issues, very caring and puts a smile on everybody's face. Suzy, the entertainment coordinator is the social butterfly, floating from room to room, singing and making all the patients feel important and most of all loved. All nurses and caregivers are very friendly, clean and treat all patients like their own family members. Daniel, my aunt's nurse, is the most amazing and humble human being ever! Oakpark Healthcare Going to Oakpark Healthcare was definitely one of the best decisions I've ever made. I feel that I received excellent care. The physical therapy that I received daily was amazing as shown by the results of my knee replacement surgery compared to others I've known recuperating from similar surgery. Everyone had a great attitude and made me feel that they cared about me and wanted me to get well. Very attentive nursing staff, efficient office and administration staff answered any questions we had. The food was good and the exercises from OT got me on the road to good health. I was impressed with the cleanliness of the facility and constant daily upkeep. The gardens are beautiful with walking paths, tables and chairs, bird house, lovely to sit and relax especially with visitors or just to lift spirits. Felt like I was at a spa. I understand they will be remodeling this facility which will make it even better. I would recommend this place to anyone needing a rehab hospital. The staff at Oakpark is excellent and I have been really pleased with the care my grandmother is getting here. My grandma was recently brought to Oakpark after a major fall and hip replacement; the rehab team was really friendly and took the time with me and my family to answer all of our questions. She has been doing really well with her rehabilitation and Christian and his rehab team is supporting a speedy recovery to get back home. Also, the facility has a really homely feel to it and is comfortable compared to many of the other facilities we toured in the area. I am very familiar with skilled nursing homes because I have been involved in finding suitable placements for three ailing family members. Oak Park is by far the best facility I have ever found. The communication between staff and family is excellent, as is the communication among staff, and between staff and patients. The atmosphere feels more like a family than an institution. The physical therapists are very patient with my brother, even when he is hard to handle. I can't heap enough praise on the nurses with whom I am in almost daily contact, Donna and Lisa. The problem in this facilities , the rooms are not permanent, they keep moving the patient from one room to another. My mom is only been there for 1 month and already been to 3 different room. It is very stressful moving things around and reorganizing. Both me and my mom can't feel at home because she keep having different room mate and different room. read more

Three weeks earlier I was transported to OAKPARK Health Care Center from my hospital of Surgery as it was located close to my house. After a few days of living there and looked after by nurses I was fed up and wanted to leave the facility and go home. But as I had no one to look after me at home the Administrator Mr.Rodolfo S Cosme Jr and Director of Nursing M/s Samantha convinced me to stay back and get treated 100% before leaving early for home. Had I not listened to these two individuals I would have stubbornly left the facility and endangered my speedy recovery. Thanks to them and a wonderful caring loving staff I was able to pull through two weeks of stay there. I would definitely recommend this facility which is staffed by complete professionals who are dedicated to their honorable task of serving the sick and needy people. The whole atmosphere is friendly and family oriented and one does not feel far away from home. Special Thank you-s to nurses Lisa - Claudia - Angelica- Josephine - ERIKA and event facilitator Maria. Our aunt had a stroke about a year ago and after her hospital stay, our uncle chose Oakpark for her nursing home. I can't say enough about the tender loving care she receives there from the entire staff. They are gentle, friendly, and upbeat. This is true of every department -- nursing, physical therapy, activities, food service and maintenance. The facility is always clean and the grounds are really nicely kept.
