I am posting screenshots of the medicare website for this facility's rating. If you live your family DONOT take them here. Patient care is minimal, office staff unprofessional. At the time of my complaint their executive director either or fired or resigned and so did their social services director. Within 4 months two executive directors were replaced because NO one wanted to take responsibility for facility's poor performance to the point they abused their patients. The RED hand next to their name is Medicare's recognition that the facility has proven history of abuse. We were very happy with our decision. The location is in a lovely neighborhood, close to home and very easy to get to, which made visiting much more convenient. The environment is welcoming and comfortable, and what stood out to us the most was how kind, caring, and empathetic the staff were. Everyone we encountered treated my relative with patience, compassion, and respect. Going through an injury and recovery can be a difficult and stressful experience, so having a supportive team made a meaningful difference. We truly appreciate the care and kindness shown throughout the recovery process and would recommend Verdugo Valley Skilled Nursing to anyone looking for a caring environment and supportive physical therapy experience.

They have, at the bottom a 4.9/5 (see photo) but when reviewing further... yelp was 3.6/5 and google was 3.7/5. That's pretty far off! LIERS! What a horrible experience! Terrible service and help. They act as if they don't receive report from accepting facilities and don't seem to know anything. They can't answer any of my questions right and it appears as if they don't know anything. My mother is at end of life and still, they try to keep her going despite the fact that she's in hospice care. I was told that she was a "full-code" when she's at "end-of-life" and her wishes was to go when the time came. They never bother to verify this information. They shoved my dying mother in a room with 5 other individuals, robbing my mother and family the dignity she deserves and requested. They kept trying to feed her over and over again despite the fact that there's a doctor's order for "nothing by mouth" (or NPO). She's unable to swallow, eat or drink and they were shocked when we stopped them. According to them, they never got report (of course they did because that's how the ambulance EMTs work). According to them they didn't have notes (which they did because the EMT hands it to them and I also received the same copy). Should they have read the notes, they would have seen the (in all caps) "DO NOT RESUSCITATE / DO NOT INTUBATE". When I started asking them questions about my mother... they stated that she was a "full code" which was just not true. According to the DON (director of nursing) my mother is not on NPO and tolerated 25% of her diet (despite the fact that she has dysphasia and hasn't swallowed or eaten in 10 days). Not only did the DON tell me this, he stopped me mid conversation (just after I introduced myself and told him who/were my mom is) to 'correct' me, stating that she was on a liquid diet and not NPO... he was rude and off putting. This shocked me because I wasn't rude nor did I come at him in a rude manner. I only walked in and sat down (and only after introducing myself and asking if I could). He stated that I was robbing my mother of her dignity when I advocate on her behalf... as she has been nonverbal 9+ months. The only one 'robbing' anyone of anything was this facility, the nursing staff and this DON. They almost robbed my mother of her wishes and robbed my family and I of my mother having the death she requested. As we were walking to my mothers bedside to do an "assessment", I stated to him once more that she's not able to swallow and it shocked me that I'm told otherwise, to which he replied "if you don't like the services here, then you can leave to another facility. Unprofessional and rude. Horrible experience at my mother's end of life. Oh... and BTW, I reached out to her hospice doctor, who also was surprised to hear that a liquid diet was provided to her... despite her being NPO. The doctor ended up clearing it up and informed (the then uninformed DON) that she was, indeed, NPO. Before anyone wonders if I'm just a grieving family member... I am... but I am also a registered nurse and have a pretty good understanding of what's happening and going on. I might not know how a SNF functions 100%, but I certainly know how to show compassion to a family member when their loved ones die. Am I angry? Hell yeah I am. But I am also level headed and compassionate. I'm looking to move my mother out of here before her death to not only bring peace to her when her time comes, but to also provide a caring and compassionate hospice for my family members to grieve. This place does not do this. HORRIBLE!!! HORRIBLE!!! HORRIBLE!!! Also, why did the Director of Nursing (DON), Augustine Akioya, place a 5 star review for this facility and not indicate that he is currently the administrator for this facility anywhere? He made it sound like someone who used this facility commented on it (see image below). Why did he do this?? To inflate their horrible star review. Skip this place. Don't put your family through this.

Ok so I'm finally going to share my experience with you all…read more My mom was in their care for just a couple of months. They won't work with you if you have Kaiser Permanente Insurance so we had to switch to Cigna. They transferred my mom to Culver City Hospital after I was told she would go to Cedars-Sinai for a few days to be evaluated. Do Not let them send your loved one to Culver City Hospital ever! My mom was starting to walk with her walker before they sent her there and now she still can't walk. She had a broken ankle from a fall but was 100% healed. Physical therapy at North Valley Nursing were the only one's caring and listening to my mom. They don't record or track the patients bowel movements from constipated to diarrhea, their pain or pain when urinating and they don't track when they aren't eating. The business office asked why I didn't come to them with my concerns...what the F! Why didn't your staff pay attention and say anything to the Head Nurse for my mom! Oh, I know why...because even when I expressed to the Head Nurse that I thought mom had "another" UTI she did contact the Dr. like I asked but just told me thy were giving her Imdiom for her diarrhea. Within 4 days I get a call that they have called 911 and are sending her to the ER, she's not eating, drinking and can't speak. They even told me the wrong Hospital that the ambulance was taking her to. We visited my mom about 5 days a week as my parents live in Sunland. I hope to God that Nurse isn't still employed there. She would never look up or engage me. It was like she just wanted me to walk away. Honestly this place was recommended to us but I'm telling you be careful. My mom was severely dehydrated and in kidney failure with a bad UTI. She spent 6 days in the hospital and was more alert and herself then when she was at North Valley Nursing. Trust me when I say I have seriously thought of reporting this place. I could continue with more but I hope this gives you an idea of how poorly they cared for her. Physical Therapy cared about my mom and would always update me, love them but the people who spend the most time with our loved one's need to pay better attention to them. I can honestly say at the new facility my mom has had no UTI's or health issues. Good luck if you use this Nursing Facility.

After the COVID-19 7days restrictions were completed he was never given a health plan the family was not called to let them know of Physical therapy was needed to help him move around. No special diet was needed. Then why? were they given my dad purred food. He lost a lot of weight and in 1 month time got sicker. He was a active dyeing patient but still deserved respect and well treatment. I am filling a small claim for damages, stolen property, and neglections.
