Now provide resounding (5) STAR and recommend them wholeheartedly. Not enough kind words for this team of physical therapy lifestyle professionals. Dan runs a heck of a team; Michelle, Lucas, Claire, Sarah...I wish I knew everyone's name as I would thank them personally. The entire team was helpful every session. Well beyond the PT itself, the vibe and atmosphere at this facility (and with the folks working) was as positive as any place I have been related to sports or medical facilities. Everyone directly involved in my therapy, or just happened to be onsite working with someone else was positive and supportive. Lessons learned in (6) weeks here I intend to use for a lifetime. After stating I would go somewhere else, I was called back and told they found my claim number and I could come in. I had already contacted a different physical therapy company, who did what needed to be done to get me authorization with absolutely no issues.

As with any care center there are some clueless people. I blame the management of this place. from what I saw and after talking to the staff the training is little or non existent. I never saw anyone that appeared to be "in charge". Unless you have a family member sitting with the resident they get likely get ignored. My family member was slipping out of the wheelchair. When I finally found someone to help they didn't know what they were doing. No sense of urgency! The nurses told my family member to drink more water, but the water container was empty. The food is what you expect - horrible. I doubt they have a dietitian. Had to search for eating utensils that could be used since my family member has difficulty in holding a regular utensil. There are 30 patients for 1 nurse.The CNA's are thrown on the floor with little or no training and there is a lot of turnover from what I could tell. There was a day I couldn't find a CNA. I have seen worse and I have seen better. I sat in this place for a week and I hope to God I never end up in a facility like this. UPDATE! When getting my family member out of bed - THEY DROPPED HER!!! Go somewhere else besides Countryside Care Center.

So far, every time I've visited, they were either making popcorn for the residents, helping them shoot basketballs into hoops, and engaging in conversation with them. I feel my mother is much safer here, and better taken care of since it is a nursing home, and there are nurses, CNAs, and workers everywhere. I was told that they even buy little Christmas gifts for their residents. How thoughtful. I notice that my mom isn't as anxious at this facility than she was at the last home. I think because there are so many people everywhere, she isn't left in a quiet room by herself. There are 2 residents to a room. My mom's roommate just happens to be a woman who was with her at the last assisted living facility. So I'm thankful she's with someone she recognizes. The place may not look fancy, but they are well taken care of here and that is all that matters to my family and I.

They stopped my mom's antidepressants cold turkey, they didn't check her blood sugars even though they knew she was diabetic, some of the females were not nice to her, and they lied to me over and over. They said they didn't stop her antidepressants and then I heard someone in the background saying we'll have to get a doctors order for that. Same thing with checking her blood sugars.
