This review is for my experience with the rehab section of the nursing home. I found the building to be clean and well maintained and not stinky. Most of the staff that I saw and/or had dealings with seemed to be competent and generally happy. Yes, I did come across some less than friendly and less than helpful nurses. But I was pleasantly surprised by the quality of the support staff for the most part. read more

My father was admitted to Lorien Columbia for rehabilitation, and unfortunately, our experience was extremely disappointing. From the office staff to some of the aides, I witnessed behavior and conditions that were unacceptable for a healthcare facility entrusted with caring for vulnerable patients. I followed the advice given to me and visited on different days and at different times. During nearly every visit, I had to ask for my father's room to be cleaned. I repeatedly saw gloves, gauze, and other medical waste left on the floor. On one occasion, his urine bag had been left unclamped, causing urine to leak all over the floor. A staff member cleaned it using only a towel, without any disinfectant. I eventually began taking photographs because I felt my concerns were not being taken seriously. One day, while walking past another resident's room, an elderly woman asked me if I could please turn off her call light because she had been asking for assistance for a long time. The staff member sitting directly across from her room simply laughed. My father was also discouraged from using the bathroom. One staff member told him he was "pooping too much" and that it was affecting his bedsore, while another told him to simply use the brief and they would change him later. As a result, he developed a severe rash. When I asked how this happened, I was told that "sometimes patients are allergic to the briefs." If that was the case, I do not understand why they continued using them. In addition, my father was placed on what they called a "mechanical diet" for nearly the entire month he was there, without proper explanation to our family. He lost a tremendous amount of weight during his stay and weighed only 102 pounds when he finally came home. He was skin and bones. I even had to ask staff to allow him to brush his teeth because he wanted basic hygiene care. Healthcare is not just a job -- it requires compassion, patience, and respect for human dignity. These are people's loved ones, and they deserve proper care, cleanliness, and kindness. No family should have to witness the conditions and treatment that I observed during my father's stay. read more

I am totally disappointed with the care of my neighbor received. he was discharged home without homecare set up. Patient's insurance wouldn't take medstar He was sent home without home care. the patient is dependent on all ADLs. a family told me a comment made by a staff member regarding feeding him. I Don't want to repeat what was said. the response was totally inappropriate. would they like their loved one sent home without support. unacceptable read more

Lorien columbia rehab- No therapy feedback! Management non responsive to information requests. Social worker developed a plan after 3-4 weeks but never provided any progress. Toward the end - Therapy seemed to disappear without explanation. read more

Care quality; staff was sensitive to the needs of family. Patient care and questions and concerns were communicated throughout his stay.

We've encountered so many problems in this place. Here are just a few: 1) Food is awful. My mother, who is a vegetarian, asked for a vegetarian menu. The menu lists many meal options. Yet she has been served the same meal (over and over and over) - a dried out black bean burger. 2) Some (not all) of the aides have been condensing and impatient with my mom, which is demoralizing to her and her family. 3) There is no written schedule for physical therapy or occupational therapy. It's impossible to know when the PTs and OTs are going to show up. My mom has not had PT for 5 days straight and there is no excuse for that. 4) My mom did not receive a shower for 5 days straight and now only receives a shower 2 times per week. 5) When we push the button for an aide or a nurse, it usually takes at least 30 minutes or more for anyone to arrive. There are some caring, nice and sweet staff. For example, Kena has been kind and responsive. But overall, this place is not good. read more

I found Harmony Hall/Lorien Health in 2023 and thank GOD I did. Out of so many assisted living facilities, how do you even know what is best for your loved one? It was fate that led me to these great people and I have peace of mind that my dad is in the right place under the right care with real humans that care and will do what they can to ensure a level of dignity, respect, and kindness to each resident that is under their care. All the staff have been wonderful, especially Melissa, Natalya and Theresa. Thank YOU ladies for all the support, understanding and communications which is so important to providing that assurance that my dad has the support and can live his life with some normalcy. A friend was recently admitted to Lorien in Columbia. We spoke on the phone and she was extremely upset because after being there a day she told the nurse she had to go to the bathroom. She said "I told her I had to poop." The nurse told her to go in her diaper because that's what is was for. This lady never wore diapers and was appalled to be told to go in her diaper and that no one would help her get to the bathroom. A mutual friend visited her the next day and said her back was in a lot of pain and a nurse rubbed some cream on it which helped. I visited a couple days later and she told me her back was hurting and she wanted that cream again. I went to get the nurse. He said they didn't have any cream and a doctor would have to order it. Isn't a doctor available 24/7? Isn't a pharmacy available? When I asked questions he kind of stumbled over his words. He said he could give her morphine. So - there was no cream but there was morphine. Administering morphine is easy. Just give a couple drops in the mouth. It didn't help at all. No one cared. The entire time I was there this nurse and the other person sitting at the desk just sat there on their computers. I walked around the entire floor and never saw anyone anywhere checking on any of the patients. Another friend visited a couple days later and was very upset by what she saw but I didn't witness it myself so won't repeat what she told me. read more

Please don't send your loved one here! I had to remove my father from this facility after about 17 hours! They are not equipped to care for people with medical concerns. My father, who is a diabetic, was given the incorrect diet, and when asked for different food, was not given the opportunity for an appropriate meal. His medicine was administered by just placing the pills in a plastic cup on his food tray, which, due to partial blindness, he was not able to see so he did not get his medication. When he needed assistance to go to the bathroom, the staff didn't come often for almost 45 minutes. when he needed assistance to go to the bathroom, the staff didn't come often for almost 45 minutes. the staff is rude and disrespectful, and my father had me on speakerphone as the nurse was talking to him, so rudely yelling at him instead of providing the proper care. It was so bad that I had to come and pick my father up and try to take him home to care for him because this place is very unsafe. When I called to speak with the Director of nursing about what occurred during my father stay, his response was don't worry about it, if his sugar was high, we would have just given him more insulin. I have never met a more callous, uncaring GROUP of healthcare professionals. From the doctors, nurses and CNA's. And food is swill. Served the cheapest beverages every meal, coffee and tea, very strong. Slow response times and sarcasm was rampant. Do not take your loved one here. The doctor on duty will tell you straight up, 30% do not make it out alive. It's a hopeless place. read more

You need to go to a nursing home! How Profound is that? When I heard that I was to go a nursing home to recuperate last year, i started to shutter. I have been in several that left me wanting to go home before I finished my rehab. So when I arrived here, I was greeted at the front desk by a lady who introduced herself and herself and then called me by my name, gave us directions to my room and then said "have a good stay". When we arrived at my room, I found 2 nurses waiting to help me to bed, they settled me in to bed and explained that tomorrow would be a very long day. The nursing staff were so efficient and kind it settled my fears. My first full day was amazing. The nursing staff started coming in to get me ready for my day. Besides my nurses, I got a visit from food services, physical therapy, the administrator, admissions, and the social work. Activities staff came in to tell me all of the things that they do on a daily a basis. Then I got all excited. Everyone was interested in me and not just my medical condition. They all wanted to know me as a person and by the time they had left I felt like I had moved back into my old neighborhood. I heard stories about the longivity of the employees and many long-term residents, some of 20 years or more. A couple of the administrative staff when I was feeling better asked if I would want a tour of the faiclity as I got more interested in long term care. For several months I had been praying to the Lord my God to please find me a home for me to spend the rest of my life. After my third day here the more I spoke with staff and long term residents, I asked my family to check into long term residency for me. Long story short, I am long-termed. This was and continues to be one of the smartest decisions i made in my life. I now have an attentional family and friends and thanks to my Lord I have a new place to call my home. read more

recently i was a patient for 30 days , and was treated with the best of care by the entire staff mainly elizabeth,shante,oshee,angela,mevalis meleau and the entire physical fitness group. for the last 3 years i have been under strict medicalattention due to covid in whiich lost a great amount of strength.i was hospitalized 4 times and lorien saved my life with their one of a kind attention. i was ready to give it up but the entire staff on the 3rd floor and rehab made me gain all the confidence i needed to go foward in my life.i will never forget them!!!!!!!!! thank you for everything you did mike hallet read more

I was at Lorien for 99 days. During that time, I encountered the worst of Nursing and the best of Nursing. I don't believe that at any time there was my call buzzer answered Within 5 to 15 minutes. There were several times when it would take over an hour and four times it was 2 hours before I got a response. This is just not acceptable. You feel like you are a burden enough just being there. The lack of response time just adds to that feeling. However, there are a few angels that work there and I would like to publicly state who they are. They go above and beyond and always made you feel good when they came into your room. That would be Minerva, Denisha, Cat, Mercy, Lee (who is no longer there), Octavia, and Allison. I found the food to be acceptable, they usually had a good variety to choose from. However, the chances of your getting it while it was hot, or getting the correct meal in its entirety was not great. OT and PT services were great - Kim, Iva, and Sarah were wonderful. Lorien was very convenient for me and my family and that is why we chose it. I would say overall they are very short-staffed in the nursing area and the aide area which causes most of the problems. It is just not acceptable to have to wait as long as I did for a call button to be answered. An aide would come in and shut the button off (which makes it look like they responded in a timely manner), say that they would be right back and then you could wait half an hour to an hour and a half for someone to show up. Their job is tough, I understand that. Lorien has got to hire more aides so that this is not an issue anymore. Overall, I was happy to have this facility be conveniently located- but you really have to advocate for yourself to get the attention and services that you need. read more

this place is terrible they neglect all the patients. Its short staff. They leave the patients unattended for long periods of time. This place does the bare minimum for the patients it's terrible smfh patients that need to be checked on frequently are unattended for hours. In my honest opinion they try to keep patients there for as long as possible for the money. I would never recommend this place to anyone please do not bring your family members or loved ones here. this place has a 2 star rating for a reason!!! read more

This review is specifically for the incompetent social worker Mike Soliman and Lorien's decision to discharge my Mom, who is an elderly dialysis patient with severe mobility issues and former two time resident at Lorien Columbia in the rehab department. My Mom was discharged on 4/29, keep in mind that her last dialysis treatment was on 4/28. Mike Soliman supposedly set her up to receive dialysis from DaVita Catonsville NorthWest center. Fun fact: during my Mom's former discharge, Mike set her up to receive dialysis at DaVita Catonsville West, only for us to find out they told him weeks prior to her discharge date that they told him there was not enough staff available to take her...after speaking with the staff at DaVita, since clearly Mile didn't know where she was going, they gave me a list of phone numbers to call the different centers in the area...I digress because I let that go since myself and DaVita quickly solved the problem. However, this is take 2! Mom had a late night fall and ended up back at Lorien. After spending a little over a month rehabbing at Lorien, she was set to be discharged on Friday, April 28th. During the discharge exit conference, Mike reconfirmed that he contacted the dialysis center (the correct center this time) and stated that her treatment would begin on Saturday, 4/30 at 9:30 am. Great! Right? He even reassured us that he spoke with the center staff himself due to our prior issue...well, this time when we arrived at the NorthWest center they let us know that my Mom was not in the system and the director specifically told Mike that my Mom couldn't start until Tuesday 5/3. A cool 5 days after her last treatment. Of course this information wasn't relayed to us, I'm assuming he forgot that important detail because WHY WOULD SHE BE DISCHARGED IF SHE COULDN'T RECEIVE TREATMENT FOR 5 DAYS!!??! Why lie and say you spoke to the center confirming dates and times...so of course all of this is found out this morning (4/30) while at the dialysis center that is refusing her service because they don't have her paperwork WTF! I called Lorien to see if they can take her since they're the ones who said she was "good to go" and "all set" but no, she's out of their system and not their responsibility anymore. To top things off, Lorien and DaVita Catonsville NorthWest's solution to her 5 day break in service...take her to the emergency room for treatment. I'm livid. On top of the disaster of today, on her actual discharge day, none of the support staff knew she was leaving and I ended up wheeling her downstairs and putting her in the car myself and when I got her home she couldn't walk or stand properly...what was the point of PT OT everyday for over a month if she came home in the same/worse condition??? Needless to say, we'll be taking prompt action, because this was absolutely ridiculous. read more

I wish I could giv them a lower rating than one ! Please do not place your Love one at this facility on Cedar Lane ! The administration Chelsea is a liar, Dr Gupta is missing in action she does not communicate w/the patient nor the patients Family Chelsea does not follow up on anything that you ask concerning issues neither return calls ! The nurses are never on the same page from one shift to the other ! They list information on your chart thats irrelevant to your medical situation they claim they did things on paper that jus didn't happen ! The staff you meet initially tell you a lot of lies to get you to place your Love one in the facility ! Kena does not know her job or she has the wrong title ! You leave messages concerning your Love one Good Luck on a response ! The aides are there for a check they are from other facilities so they have no interest in their job b/c they may never be back at this location they could care less about the patients ! The staff totally ignore the buzzer they come when they get around to it ! The staff at the front desk will be the best you're gonna it ! There is no organization at this facility they all do not communicate w/each other from the top to the least,mistakes are made daily pay close attention to Everything ! Listen to your Love one chec on them regularly don't let the staff know when your coming or the time jus show up keep your own paper trail of what's happening ! My mom was a patient at Lorien Columbia for 3 weeks. She made tremendous improvements. She was very happy with her stay. The nurses, rehab therapists, doctor, dietician, administrator, social service team all made it possible for her to come home in far better condition than what she expected. My grandmother recently passed away after staying here for almost a year. Once im old, I'd rather die than have to stay here. Grandmother had suffered a stroke and began leaning towards her right side, and as her family, we just assumed this was part of her aging and illnesses getting worse. Even when prodded, workers here would say that it was normal. Unfortunately this was not the case, and it was discovered from Howard General Hospital that she had in fact suffered multiple strokes while under the care of lorien and instead of sending her to the hospital accordingly, what did lorien do you might ask? slid a few more pillows under her right arm to prop her up. This ultimately contributed to her premature passing which really hits home. I've made it clear to all friends and family to avoid this place like the plague. If you care for your loved ones, please don't send them here, I know most nursing homes are probably crap, with workers who hate their jobs, but this place takes the cake. Complete ignorance to individuals that have suffered strokes and losing a loved one the way we did, unforgivable. Although I was against it, my mother would bring goodies and gift cards to employees here but, guess it wasn't enough for them to really care. So in conclusion, if you despise your family, send them here. If you love them, don't send them here. read more

My Mother recently stayed there to transition from the hospital to home. I was dismayed to hear that the food was inedible and the nursing staff never answered calls. This is rather unacceptable given that some are likely on pain medication that needs to be given on a timely basis and in no case should calls to the staff go unanswered!! That plus the mandatory fee of $25 per day for TV is quite expensive given the limited incomes of the elderly. Unfortunately, the residents seem scared to speak up which is awful given that they are old and frail. I really wish we can treat our elderly better and with the respect and dignity that they deserve. And that the staff can be reminded that they too are aging and may find themselves on the other side someday - karma is a strange thing so they better hope they are not on the receiving end someday. read more

My Mom was admitted to the facility after having been in the Hospital, the facility had a much fresher smell than the one closest to me. The problem began with the Doctor, who was also on staff at the Hospital. My Mom advised me that there were days the Doctor didn't come in her room. She would call out to her Doctor as she went past her room and the Doctor would not come back. I had an experience while visiting my Mom. The Doctor walked pass Mom's room and as she called out to the Doctor, she kept going. I jumped up and ran to catch her just before she got on the elevator. I told her that my Mother wanted to see her and she came back. I also told her that I was told that my Mom said she (Doctor) had ignored her on several occasions. My Mom was back and forth between the facility and the Hospital for several months, the same Doctor was responsible for her care in the Hospital. I and my sister meant with the Doctor and Hospital staff several times in an effort to explain our Mom's medical history, to no avail. She didn't make a request for Mom's records until the night before Mom passed. She seemed shocked by Mom's medical history, but it was to late. It took a long time before I could come to terms with losing my Mom. I didn't know I could write a review until today. I wish I could sue her for negligence. This facility is located in Columbia, Md. which is also the State that I live in! read more

Initial impression: It's a dump with bad nurses. Avoid if at all possible. Longer review once I recover. Updated after 2 weeks: OK, not all the nurses are bad. For every nurse here who will do her best for you, there's an awful nurse , and complains about being over-worked, there are varying numbers of nurses who do an adequate job. It's hard getting across to some of the nurses just how painful orthopedic surgery can be. They may think they understand, but having ice wraps available after physical therapy is mandatory because your legs WILL feel like they are on fire. Getting pain meds in a reasonable amount of time isn't just nice, it's the very minimum they can do. Needing help to go to the bathroom is useful, but not if the nurse waits so long to answer, the request is moot. There's not really good communication between nursing/physical therapy/occupational therapy. When you have two non-functional knees, it's going to take more than 5 minutes to get ready in the morning. Or if OT announces that they're going to evaluate one's morning prep, they ought to be aware that the nurses will skip taking that patient to the bathroom, which can lead to embarrassing results. The food is bad. Prison riot bad. And it's not really tailored to patients: my attempts to get a high protein/low carb diet have resulted in supplemental cans of "therapeutic nutrition" being dropped off. Since it tastes horrible warm, it'd be nice if they could remember to drop off a cup of ice with it, also. Edited: a week after I was discharged, I was back in the hospital, this time with a UTI that causes an extremely high fever, chills, and other flu-like symptoms. Given that Angie, the day-time LPN, copped an attitude every time she assisted me in going to the bathroom, and took hours to refill my water pitcher when I drank it dry, can you blame me for restricting my fluid intake? And then she had the nerve to scold me about the dangers of fluid restriction (a UTI, as it turns out)! My biggest regret was not going to the nurse's station in the wheelchair every time she took more than a half hour to respond to a request for more water, with a secondary regret that I didn't whiz all over my bed when she took too long to help me into the bathroom. Fortunately, I've been able to share my experience with friends who needed a rehab unit, and steered them into other places. It's possible they've cured these issues in the last 3 years but I'd want to check first. read more

I have been visiting a relative here for 8 years now. Although, these aren't places we tend to like to visit -- it has served well over the years. I have not witnessed or heard of any abuse, but then families need to designate visitations on a regular basis. Especially if a loved one cannot communicate with you. The first floor is nice after they remodeled. Smells have been under control when visiting. The front desk staff is friendly and my relative is fed well. For those who don't have a lot of money to keep a relative at home with help -- you research, pray, and make the best decision you can. Report problems to management right away wherever it is. My mother was discharged last night from Lorien. I was very happy and excited that she finally was coming home until my mom's social worker at Lorien and the security guard ruined it for my family. We were ready to get the discharge papers in order until the attending doctor says she cannot go home because she didn't have her cpap machine delivered and she is not on oxygen. I told the doctor, I said noway, I was told by the social worker that she is to be out of here by midnight tonight because my mom's insurance only covered for so many days. And if she is not discharged we have to pay out of pocket. I told the doctor the nurse there is no way in hell that my family is to pay an extra day just because a social worker couldn't communicate anything with us. I have repeatedly tried to call the social worker last week, never returned my calls regarding the discharge process. I had to hear about it from the business office which only handles the insurance portion. The social worker and I got into it because so she had to call security. The security guard told me I had to leave ASAP. I said no I am not going to leave my mom. He said you don't have to be with your mom. You cannot pick up your mom tomorrow you have to have someone else. I said who are you to tell me that I cannot discharge my mom. I am the only caregiver to do so in my family. I have no help. He still says i had to leave and if I dont leave I will be arrested. The director of nursing intervened so she finally solved the cpap machine problem by contacting Johns Hopkins. Hopkins didn't get the paper work until two hours before. Luckily the machine came and my mom was discharged. The social worker visited my mom while I was with the director of nursing and her boss in another office. The social worker never said anything to my mom. My mom told her why you had to call security on my daughter. She didn't asy anything. My mom also said to her that you were told by us last wk that my son had to take off from work to set up the ramp for me to get into my house. He can only take a day off. That is why my daughter was upset with you. She didn't say a word. THe director of nursing told the secuirty guard that she can stay. The security guard stood outside while I was planning to get the cpap delivery. I really liked Lorien up until my mom's discharge. My respect for Lorien has really gone done because of this incident. That is really unfortunate because overall they had done a great job taking care of my mom. It is really bad that a social worker cannot take criticism and she lied that I was threatening her. I never had threatened her. I yelled at her but never threatened her. After this incident, I will never ever comeback to Lorien again. I hope to God that my family will never use a skilled nursing facility again. I told the security guy, I never caused any problem and now you are going to threaten me with an arrest. That is cold. Also, I was my mom and I was offended that everyone kept asking her what time she is leaving as if they were in a hurry to fill her bed. That is very rude and inconsiderate. Also, the physical therapy and occupational therapy staff could also use a better method of communication as well regarding time. read more

Awful. My mom was there for rehab and was left sitting in a chair for hours, her feet and legs became swollen. They did not record her seafood allergy after being told on 4 separate occasions and when I was there during admission and was served a salad dressing with a seafood base. Nurses are rude and were not conscious of my mothers fragile condition upon admission and insisted upon tossing her in a sling to weigh her and roll her around to check her skin despite her screaming in agony. Horrible place. Avoid at all costs. read more
