I didn't mind filling out a health history, but when I questioned 10 pages of non-applicable forms she told me if I didn't fill them all out (including an unlimited info release to anyone the doctor chose - and agreeing to pay outrageous collection and legal fees) then I couldn't see the doctor (mind you this was just for a consultation that was going to cost me $125). When I selected a different date, the own sarcastically said, "Was that so hard." I could not believe how rude she was being with me, when I was just trying to schedule an appointment. Also, the owner was incorrect about what the fine print says on the Groupon, claiming it requires one purchase to be used as a gift, when it doesn't say anything like that. Customer Support at Groupon was so alarmed by the owner's behavior that they allowed a full refund for both Groupons. Do not patronize this business. They are extremely rude on the phone.
