I liked that it had recently been renovated and had an Updated PT Department compared to what I had seen. My mom recently stayed at Creekside 9 weeks for rehab and she is a complex case due to severe osteoarthritis. Upon arrival she could barely stand up and walk 50ft. - she required 2 therapist during her PT sessions at first. It wasn't long before she was walking further and progressing in both OT and PT. The therapy team is Great! - Angelique, Anisha, Kiko, and Natalie worked with mom often and made amazing progress with her. Aloha Randy! Watching the entire therapy team in action everyday was impressive as to the range of skills they demonstrate patient to patient. Mom is home now walking around the house and outside. This was the primary goal for her being at Creekside. The CNA's are truly Special! Most of them treated my mom very well and kept her clean and if she needed something they took care of it. She had a couple favorites I would like to mention for well deserved recognition - Sauli - Tirhas - Carolina - Jonjie - Thank You for everything you did for my mom!! I really Appreciate YOU! Big Shout Out to RN's Sheri and Hafsa for getting mom her pain meds On Time! The kitchen Boss - Nakea checked in with mom to see if she wanted the menu item for the day or an alternative for Dinner - Very Nice! Annie is Awesome! She was moms Case Manager - All of the issues I brought to her attention were resolved quickly and professionally. I completely trusted her and valued her advise when needed. Emil and Annie make a Great Team and seem to run the place very well. Emil was moms Care Coordinator - Very Hard Working and easy to work with. We had weekly meetings regarding important details about mom's care. My mom had a private room during her stay at Creekside and I was with her most of the time to keep her company. I felt comfortable leaving her there alone at night. Her only complaints were the room was cold and the food not so good but we brought in outside food often. As far as Nursing Facilities go Creekside does a Good Job and I would let them take care of my Mom again. Thanks Again Creekside! Vince M.

The responsiveness of the nurses and CNAs insured I was well taken care of since I could not get up to do anything on my own. The Occupational and Physical Therapy teams were top notch - they really know their field and made sure I was well educated on my own care once I was released home. I highly recommend the facility, from the administration to the nutrition and housekeeping teams and everyone in between, you or your loved ones will receive excellent care!

My dad had to stay here for almost 2 weeks for rehab after being transferred here from Kaiser. The place was clean and is in a really good neighborhood and is within walking distance to restaurants and shopping. The CNAs and nurses were very nice. I forgot one of the nurses names, I think it's Helen. She brought my dad a packet of hot chocolate for later and always said hello when she saw him. Another CNA or Nurse named Star was also great with my dad. My dad was well taken care of here. If you ever need to put your loved one in a care home whether it's temporary or permanently, I trust this facility.

Everyone, and I mean EVERYONE that I interacted with was so kind. My mother was treated with dignity and respect by all the staff assigned to her case. I visited my mother every day and witnessed a wonderful team effort-Nurses, CNAs, LVNs, OTs, PTs, case managers, kitchen and custodial staff-everybody that I interacted with were helpful, patient, and did their jobs with a smile. Even staff not assigned to my mother's case would pop into her room and check on her. The Director of Admissions spoke with me several times and each time I felt confident that my mother was in the right place. I cannot state enough how happy I am with the care that my mother received, the expertise with which it was given, and the wonderful attitudes displayed by everyone. The facility itself is quite clean, and there is ample parking provided you visit after 3:30 (when staff start departing). There is a large physical therapy room that is fully staffed and equipped. Residents have the opportunity to participate in daily group activities (which my mother did). Dietary requests are also given consideration. Communication between the staff and me was excellent. I was kept informed more than adequately. When it came to discharge my mom I was so relieved that many after treatment details-prescriptions, follow up appointment, arranging of home health care-were handled for me. My mother is very happy with how she was treated by everyone here and I am so glad that Canyon Springs was our choice.

Despite the natural challenges of recovery, the experience was genuinely positive. The facility is bright, clean, and thoughtfully designed with lovely gardens where both patients and families can relax. The staff were consistently professional and kind, and the management took the time to reach out to us as family members to ensure we were comfortable and heard. Overall, I'd give the experience a solid 4.5 stars. The only small area for improvement would be the food options. My mom is Asian, and we noticed there weren't many Asian-flavored meals available. With more and more Asian patients enrolling, adding a bit more variety in that area could make the experience even better.
