After a leg amputation, spent 2 weeks at Sunnyvale Post-Acute Center for continuing recovery, physical therapy and pain management after Kaiser stay. The staff was caring, respectful and committed. Special hats of to physical therapy staff Miranda and Melanie who made daily workouts fun and productive, with special emphasis on safety and independence and getting home! Also grateful to nursing staff who did everything possible to make stay as comfortable as possible. The main issue was with older facility where all restrooms are not ADA compliant, making it difficult to manuever with new, unfamiliar mobility equipment. read more

My mother was recent here for recovery from a fall. The care she received from the staff and therapy staff was exceptional. Everyone we worked with were compassionate, caring and helpful. Recoveries and care of our loved ones is complex and multi faceted. This can be a stressful time for patient and family members. Do not send your loved ones here! Their negligence caused my grandmother's death. If I could give zero stars, I would. My grandmother was admitted to Sunnyvale Post Acute Center after being discharged from the hospital. She was an elderly woman with a history of stroke (right-side weakness), atrial fibrillation, diabetes, and kidney failure. When she arrived, her kidney function was already fragile at 14%. At the facility she was overdosed with diuretics and her kidney health was not checked regularly as it should have been. When we brought her to this facility, there was no one there to greet her -- she was simply dumped into a dirty room with dusty floors and a broken drawer held together with tape, as you can see in the pictures. Unlike other facilities where COVID-positive patients are placed in a separate, controlled area, my grandmother was placed across from a COVID-positive patient with no separation or precautions, putting her at unnecessary risk. Instead of receiving the attentive care she desperately needed, she was neglected. Only after weeks of being dehydrated by the diuretics did they finally start her on IV fluids -- but at that point, her kidneys had already taken severe damage. I demanded that she be sent to the hospital immediately, but Michelle, the Nurse Manager discouraged me, saying she would "just wait for hours at the hospital" and that the facility's doctors were doing the same as hospital doctors would. I even threatened to call 911 myself because they were ignoring my grandmother's condition. Still, they reassured me that hospital care wasn't necessary. At one point, the staff painted her nails, and I initially thought it was an act of kindness. Sadly, I came to realize it was actually a way to cover up the fact that her hands weren't being washed and she wasn't being properly bathed -- her nails were filled with dirt underneath the polish. This was not care, it was neglect. I repeatedly voiced my concerns, but Matthew, the Social Worker Director, never responded to any of my requests. I don't understand how he is in charge of running this place -- he is obviously not qualified for such a big responsibility. The staff refused to take my concerns seriously until I threatened to call 911. Only then did they send her to the hospital -- but by that time, her kidney function had already deteriorated to 8%. She passed away two weeks after. To make matters worse, they even lost my grandmother's cellphone by throwing it in the trash "by accident." This added even more stress to an already devastating situation. The lack of compassion, communication, and accountability at Sunnyvale Post-Acute Center was unacceptable. Families trust rehabilitation centers to provide care, dignity, and safety for their loved ones. This place completely failed in that responsibility. There are SO many other examples of poor care. She was made to wait over an hour in the lobby for a doctor appointment that I had told them the day before had been canceled. Emails to staff were ignored. Instead of speaking with a doctor, a case manager would read the lab results over the phone to me-- not once did a doctor call to discuss her care, nor a therapist. Despite my phone number being posted next to her bed! I was told to bring my own extension cord so she could connect her phone and charge it. I strongly do not recommend Sunnyvale Post-Acute Center. Please think twice before placing your loved one here. No family should ever have to suffer the way ours did, and no patient should be treated with such neglect. Be aware the place looks nothing like the pictures they posted, I recommend you visit it in person after 6pm so you see the reality! read more

As Hispanics, it's not our custom to leave our parents at facilities due to their stigma of neglect & making people feel uncomfortable. My mom is very hardheaded and picky, but she was comfortable there during her 3 weeks stay. The facility was very clean. The first night there, my mother's neighbor was allergic to flowers. We requested to switch rooms and the nurses there honored our request. There was only one nurse that was very rude, but thankfully she switched jobs, and no longer there. The team as a whole accommodate all my mother's needs & were helpful with transportation for her medical appointments. 4.5 STARS if there was one. I had an overall good experience at this rehab facility referred by Kaiser where I stayed for more than 2 weeks. Most of the nurses and all of the PT staff I interacted with were specially fantastic. Nurse Jeneley and Vanessa were my favorites, as they were on top of my shifting circumstances instead of sticking to old orders that needed updating. Also, Angelica my nurse case manager was great and managed disconnects caused by kaiser well. Most CNAs were also fantastic including Alvin, Meiko Mahgol and Evette. I cannot say enough good things about the physical & occupational therapy team. Samantha, Joe, KK, Kajal, Angie and Miranda each had different styles and taught me new ways to function and walk again. The facility is clean and well maintained. I would have given 5 stars but on occasions there were issues with my food as I had dietary limitations (eventually mostly fixed thanks to Trixy & Jessica). Also, they had some problem dementia patients. I think those kinds of folks who tend to get into yelling matches with the staff or walk into other people's rooms should be in a closed section or other kind of "memory" facilities. I repeatedly had to ask my door to be kept closed as a result and in the end it worked out for me. I was lucky that after changing my room I had a great room-mate but the first one kept waking me up at night. Hats off to the professionals that work in SNF settings and make it great for some of us. I chose Sunnyvale Post-Acute Center for my father to be transferred to for therapy after having surgery at El Camino Hospital. I live in another state, so things were a little rough at first, and definitely an adjustment for my father, but after the Admissions Director, Jessica Tieu stepped in, everything started to fall into place. She went above and beyond to make sure my father received the care he needed/needs. She made sure to call or text me any chance she got, and reassure me that my father would get taken care of. I couldn't thank her enough. Any time my father needs something, she is always there each morning to assist him. A couple of other individuals that were/are amazing, and so patient with my father are Joseph & Robert (Administrators), Matthew (Social Service Director) and his assistant, Carla. Lastly, Michell (Director of Nursing), and Blanche (Director of Activities.) You all have been so wonderful to my father, and I appreciate you all from the bottom of my heart. I trust these individuals with my loved one, and I highly recommend this facility, as they have treated and continue to treat my father so well, for the next several months. Some of the nicest, kindest people ever work here! From the second you enter the Center you are greeted warmly by the wonderful, kind and knowledgeable front desk personnel.   We put our Dad here after his stroke, because Sunnyvale Post-Acute Center was highly recommended. What did it for me initially is when someone I trust from the previous care-facility said to me that "if it was my Dad, I'd send him to Sunnyvale Post-Acute Center".   The place looks GREAT! They keep it clean and well maintained on a regular basis. Even has a nice piano up front (that one of the patients plays from time to time to set a nice mood, she's an excellent pianist).   The Physical Therapists, Speech Therapists, Occupational Therapists do an amazing job bringing patients back to normal functioning!! - They did wonders with my Dad. The Case Managers as super knowledgeable and really know their stuff and can guide the family towards the best possible route for the betterment of their family member's health. All the nurses here are super kind, caring, empathetic, and go all out for their patients; it's not an easy job tending to so many different patients with varying needs, but they manage really well, with their super-human skills and caring nature. The food staff always makes sure food is prepared to each patient's preferences and dietary restrictions and is delivered warm. The nutrition department, social department, laundry department, fun-activities department, planning, management and all the other departments here (which I may have missed, forgive me) are all super excellent at what they do, what's most important is 'how' they do and 'why' they do what they do, they do it to make a difference and work very VERY hard at it too.   I love that they encourage group activities for their patients, something that my Dad enjoyed as he got to know more people and felt the love through interactions with everyone. One of the patients there told me that when she had to come back for a second time (due to her diabetes situation) she made sure it was Sunnyvale Post-Acute Center, because of how wonderful everyone is here!! Over the 2+ weeks, I've gotten to talk to more of the other patients, and they love it here because they've been looked after so very well by the amazing staff here!   My hats off in the highest honor to everyone at Sunnyvale Post-Acute Center for doing what they do with so much love, and for making a difference not only to their patients, but also to the patients' families too!!   Sunnyvale Post-Acute Center - You all rock! read more

My father stayed at Sunnyvale Post-Acute Center for the last two weeks of his life after he suffered from stroke and was on comfort care. This facility was the best compared with other two post acute centers that my father had stayed prior to coming here. Front desk people were very nice, they were quite flexible to even let me come in outside of regular visiting hours to stay with my father. All of the nurses and most of the CNAs were quite professional, they took care of my father well, and were always willing to listen to my suggestions to ease his pain and make him more comfortable. Case manager and director of this facility were very kind, they managed to find a room with window view for my father despite almost all the beds were occupied, they helped me and my wife to find the right contact to be prepared before my father's passing. Overall, this is a great place, excellent facility and wonderful people. Thank you all for taking care of my father! This place is an absolute MESS. This post is well overdue. My grandma is currently housed there for medical reasons following a stroke. Let me start off by saying that I went to visit her over a month ago and went inside and she was covered in dry pee and her legs were fully exposed. (I have the pictures to prove this) when I sat next to her there was damp pee all over her sheets and it looked as if no one had come to check on her in awhile. There was also her breakfast untouched and she was drinking warm milk. To me this is unacceptable. I called to file a grievence complaint which no wasn't followed up and never happened. You have the most important person to me and this is how she's being treated. This isn't the first time I have had an experience like this. She has wet her before and needed assistance multiple times while visiting and getting someone to help is like pulling teeth. It's as if they do not care about their job and I am bothering them. Specifically the CNAS (not all but most) one time we came in to see her and the CNA came in and told us to "get out" The reviews really reflect on what it's like in that place. One staff member in particular was extremely rude to us today when we came to see my grandma. She was a wearing a yellow sweater and looked to be some sort of admin. The communication is so bad we drive from Tracy just to see her and we never got the new protocol but it was "emailed to us". No apology no nothing. The lady was cold and made us feel so bad. She told us we could take to a cordinator and when my mom went in there to see why we never received an email he was very disregarding and rude. I think when working in such a vulnerable place you have to have patience and empathy and this staff lacks both of those things. Sadly due to insurance purposes this is where she has to be but i will never ever stop advocating for her especially because she does not speak English and can't speak for herself. This is the best place you can take your elderly parents for recuperation. They took excellent care of our father. Their service was above and beyond our expectation. The staff are professional. My brother has been in this facility for almost 2 weeks and what he tells me about the care is atrocious. He is supposed to be getting physical therapy. If you call it therapy he is recovering from surgery on his foot. I wouldn't want my dog to get care there. His name is Richard Bsharah and is in room 40 A, if any manager is reading this you need to talk to my brother. Also I called to talk to a nurse last week and was told they were handing out meds. And was told I would get called back but was never called back that was Monday and today is Sunday. I just hope he can get away from this place. read more

This place really don't care about patients. We understand that there are shortage of staff, but we can tell the difference of not caring and not able to care. There were many issues during my dad's stay. One thing particular disturbing was that they kept trying to make him to stay longer. We first requested on Saturday. The nursing supervisor said they can't do normal discharge on weekend. They'll discharge him on Monday or Tuesday the latest. When we talked to the social services on monday they said they were not aware of it and they require three days to do the discharging. So we agreed on discharging on Friday. Then they said because we did PT on Friday then the discharge has to be the next day. I made sure that Saturday was the last day dad will be there. We checked with social services many times and signed paper work and confirmed multiple times that dad will be able to go home on Saturday. But sure enough, when the faily arrived on Saturday morning, they said they couldn't let dad to leave. They said they didn't have the paper to discharge him, and they couldn't find the social service director. I was determined to take dad home no matter what. I reasoned with them and finally dad was able to go home after 3 hours. Dad's thermal pants and a pair of sneaker pants were missing. They had us to write a note and promised to find them but had never contacted us or returned our call. I've noticed that the owner of this facility commented on some posts to contact theie administrator at (408) 245-8070. I've called that number many times. There were a few times I was lucky that someone answered the call and took my messages, but no one had ever returned my call except the first call to request touring before admission. read more

Looooooooooove this facility!!! Did such a great job with my grandmas total knee!!! They have a wing just dedicated to ortho patients which is nice. They're not all mixed together with long term or dementia patients like all the other facilities. The facility was highly recommended by the hospital which now I see why. Update since April 8th since we took her out of the facility we had an appointment with her primary doctor as well as the pharmacist and they were very surprised why the Dr. At the Sunnyvale post acute facility would prescribe medication for her, but not actually have the medication sent out. When she was discharged they sent 3-4 medications for me to pick up when on their "list" she was supposed to be taking 8 medications in total. Her medical equipment that was supposedly ordered still hasn't been delivered and don't think it ever will. I wish you all the best in finding a place for your loved one just not this one, you will be very disappointed if you do. read more

So, this is an update for my earlier review. While I did have a not so nice an experience with one person, I have been fairly impressed with others. Matt from social services has been particularly helpful in arranging appointments and transportation for my wife's doctor's appointments. He has also kept me updated on any changes to my wife's condition. Several of the ladies who man the phones at the front desk have also been very helpful and courteous. This has changed my overall view of this place, you could do much worse, I'm sure! Considering what nurses and associates do for us in regards to our loved ones, I'm extremely grateful. read more

My aunt had to come here for some rehab after an illness. First of all, the staff don't follow any kind of consistent COVID 18 safety protocols. Some staff wore no masks, some wore them incorrectly, some in n95 masks, some just surgical, som with face shields. Every visitor has to wear the mask the facility requires and hand you, plus a face shield and have your temp taken. Every time. Why don't the staff? Next, my aunts shoes went missing, she was only there 9 days, how do shoes go missing? A night gown is missing as well, we even had a sign saying her family would do her laundry. On the weekend we had some concerns and were told that we had to wait until monday to voice them as there were no supervisors who took complaints on the weekends, we were also told that each staff member knows their job and that was that. Lastly, my aunt needed help to use the bathroom, pushed her call button and no one showed up for a hour and in that hour she had an accident. What the heck? An hour? Avoid this place if you can. read more

Mom was told she would not be helped nor assisted to use the restroom, they told her on 4 separate nights to use the diaper they put on her! Seriously WHY would you humiliate a fully capable person to force them to soil themselves! Called repeatedly, no answer in the evening, left about 3,4 voicemail messages NO ONE ever replied. Her Pain prescription expired today and no one on staff thought BEFORE running out of it to CONTACT the doctor. So knee replacement, they gave her 2 Tylenol! My dad was sent here from hospital after having a stroke. First impression was not great actually as it's a busy afternoon and we thought he'd be sent to a place that's walking distance from his apartment so my mom can come to see him as needed. Don't know where the process was broken and we'd been then working on having him transferred. He'll finally transfer today after 9 days staying here. But the initial impression was quickly reversed and we love it now except it's not the closest to my parents home. Everyone here is very friendly and helpful. They have dedicated care teams including nurses, physical therapists, cleaning ppl, admin, social workers, nutritionist etc. Cleaning routine was great and my dad was kept clean and comfortable. He's quickly recovering as well to our surprise. Even on transferring, they're very helpful and supportive, provided needed documents timely to the other place. Both my dad and mom are satisfied with the care my dad got here. We definitely recommend it for their caring and friendly teams and great service. read more

First off we gave this place a second chance now with my dad's recovery/PT, first was with my mom. With my mom it was long periods of time for help with the use of the restroom and finding out when we got home missing clothes. Now recently with my dad I was visiting and they were going to give him double dosage of meds luckily my brother was still around when I was visiting and questioned it to the nurse who just started her shift there was no communication between the two nurses about the medication being given!!!! A different evening my dad needed to use the restroom he obviously needed help from the nurses cause he was weak but was willing to get up and use a commode. The nurse said oh no he hasn't been evaluated yet so he can use a bed pan. I then told her Yes he has I was there when it was done by OT....the Cna/Lpn was asked to help my dad with his restroom needs. At this time it was closing time but I told them I'm not leaving until someone helps him because he has been waiting for a half an hour to get help. Once he arrived I then witnessed him starting to take his vitals, I told him wait a minute my dad has been waiting 1/2hr he has to use the restroom the nurse said he doesn't know how to take him to the restroom!!!! I then asked the front desk lady who was telling everyone to leave cause they're closed,that I needed assistant from the Occupational Therapist who was still there at the time to help. OT came and showed the CNA how to do it after she left the cna nurse said he can go in his diaper and then he will change and clean him up wow. My dad unfortunately was going into sepsis which they called to see if we wanted to send him to the hospital??? If we weren't able to receive the phone calls or some reason what were they going to do let him code (Die)there??? My dad is elderly but once you start going sepsis you can go quickly code, so please be aware to answer your phone from a Sunnyvale phone number not stating that it's Sunnyvale Acute Center !! After my dad being transferred back to the hospital he told me that they had just left him being soiled in his diaper not properly being cleaned he ended up worse being RAW in his genitals. Since the was been back at the hospital I came to pick up his belongings clothes and shoes were MISSING!!!! Only received partial of his belongings no one was there to give me an answer on how items went missing WOW!!! Never come to this place also cause if you have a loved one and you work they close at 7pm M-F the weekend they close at 5pm!!!! Ridiculous hours to limit you on visiting your family and friends....who knows what goes on when they're closed I've seen what happens when they're open....I'm only one person who is probably just taking the time to speak my mind and tell the public of my frustrated experience. Nothing against the staff but everyone needs to be well trained from the bottom up! read more

My mom was at this facility for about 1 1/2 months. I will start with the positive, and the only reason I gave two stars instead of one. The day-time nursing staff is excellent as well as the physical therapists and occupational therapists. Loved all of them, very attentive and friendly. The administration, particularly the social workers, is a different story. They were significantly lacking. The main reason was, my mom was checking out of the facility and it was offered, by the social workers, to request medical equipment to be ordered through my mom's insurance. Here we are, three weeks later, and the insurance company confirmed an order has still not been placed. We have since made the decision to purchase the equipment on our own. We had followed up multiple times via phone and email requesting an update on the equipment and when we did receive responses it consisted of us being told that they needed to follow up with the DME company. But, we never heard back, we always had to reach back out for updates. Additionally, after I had called and left two messages, my sister spoke to someone on the SW staff and they informed her that they don't return phone calls. It took an email from me pointing this out and expressing my disappointment in order to get a phone call and email back. Lastly, not that we expect 5 start restaurant service, but the food was always cold, they would forget to bring some items that were listed on the menu and they didn't seem to keep very good track of what patients didn't eat certain things (i.e. my mom doesn't eat fish but she was served fish a couple of times). Again, not a huge deal, just a little side note! In summary, the main reason for two stars was the care shown, or lack thereof, from the social worker staff. I know, generally, social workers are very busy, but we were disappointed in the lack of follow through on items that THEY suggested to us, we didn't ask for this assistance, it was offered and then neglected. My grandmother had to stay here twice, this one specifically because Kaiser only partnered with them and it's located closest to my home out of the other providers I selected. 1. Parking sucks 2. The aids are not friendly and they weren't adhering to the schedule that the nurses told me would happen. I was suppressed at the plan of treatment- it seemed great, however they simply didn't have the man power to do all they promised. 3- the doctors and PT were great! probably because they are Kaiser doctors. But they weren't on site 24/7. 4. Billing- this is one of my biggest complaints. On my grandmas first stay they tried to bill us $50 a day for the first 10 days but as my insurance was explained to me- I should have had no bills. Obviously I didn't pay it- and received a second notice. So I took it to their office and they acknowledged the mistake. A year later she had to stay again and the same thing happend. I told them again and they told me to disregard the bill. She left the facility in July to come home on hospice and passed in August. NOW DECEMBER 1st- 4 MONTHS LATER, I received a bill for $2,300 for the stay for the month of November!!!??? How is that? I called them to try and handle the situation because I want them to be aware of their mistake, the lady on the phone took down my name and number and told me their manager will call me back. Be careful if you or your loved one stay here. They will bill you incorrectly. I'm sure they do it on purpose to try and find "suckers" that actually actually pay. It's sad. read more

Family members of patient bring food in for patient at Sunnyvale Post-Acute center , the food was mixed up for another patient. Since i do not have teeth to eat food served at this rehab center ( fried chicken, pizza) every day my family members will bring me food they carefully Marked name room number on every container, Nurse at Sunnyvale Pist-Acute center givens my food to another patient. On the day the patient was discharged from this rehab the nurse could not find clothes, shoes and other belonging to patient. read more

Outstanding in every way! My aunt was transferred to Sunnyvale Post Acute for therapy after having surgery. Our family chose this facility because of their 5 Star Rating and when we toured the facility, it was very clean. Upon admission, my aunt was greeted with the warmest welcome. The CNA and the Admission's Nurse introduce themselves and explained step by step of the admission process. The CNA even took the time to take my aunt on a tour of the facility. During my aunt's stay at Sunnyvale, she mostly enjoyed her therapy sessions.The therapists gave her additional therapy when they saw that my aunt was motivated. My aunt bonded with many of the staff members at this facility. Thank you Sunnyvale Post Acute for helping my aunt safely discharge back home! read more

Be careful in having your family members sent here. The nurses don't respond. They don't have charts displayed to see if your loved ones have drank any fluids. When you asked questions, they bully you and say..."Oh, she/they/he is on lunch"-----They don't clean the facility, it's very dirty in their rooms. My father has fallen multiple times due to Stroke and being paralyzed. He has been there for 5 months. WARNING........ TAKE A TOUR AND YOU WILL SEE, what I mean. I am hoping the issues are resolved. I have not posted lately what has been discovered. I am very upset as to how they treat their patients (especially ones that can't communicate). read more

Be careful with patients in the dementia unit. A few months ago a resident called Peter wheelchaired himself into my mom (who got a stroke and has been paralyzed) so I tried to stop him. As soon as I walked by his wheelchair he hit my upper arm twice that I still have pictures to prove it. There were many times he went into my mom's room uninvited. Since I filed a complaint with the nurses, he seemed to behave himself. I heard he touched women inappropriately before. Not only was he scary, his daughter was very rude too. Tonight I went to the kitchen to wash my hands. As soon as she saw me, she jumped to me and screamed at me to go away so she could get something from the cabinet. Don't send your loved ones here, it's a nightmare. The staff are overworked, the physical therapy they offer is minimal, the food is the worst I've seen of any facility and the care seems sub-par. It's nearly impossible to get staff to return calls, and they bully patients and families who are trying to get better care for their loved ones. My mom has had to be in sub-acute facilities a number of times over the past 10 years, and this is the worst one I've ever seen. read more

My friend, Marie, was admitted here three weeks ago. I picked this nursing home because I met Daniel the administrator at the hospital and he was so helpful to all questions I had as this was new for me. He gave me his cell phone number in case I had any questions or concerns. When I checked this place out my friend was treated with immediately concern. My friend doesn't like hospital food. Pros: * Close to 85 and Fremont * Rante and Roy are excellent Cons: * Short staff resulting in long 45 to 90+ minute waits * The staff sometimes yells at patients. Josephine, CNA, has a bad bad bedside manner. The way she talks to my friend is horribly unprofessional. * The food is, at best, ok. * Posted in the hall is a sign requesting you to "be quite" (sic), yet the PA system frequently blasts announcements in the halls.
