He's walking with a walker, progressing daily, and finally getting the care he needed all along. The administrator at Mountain View, Joji D. Gervacio, once told me coldly and confidently that "this was his life now," meaning he was literally bedridden and couldn't do anything. She said that was his life and he was incapable of recovery. Two months later, she was proven completely wrong. If we had accepted her words, I truly believe my dad would not be where he is today. This review is to say: Don't take everything they say as final. Don't give up hope. And if you get a call from that administrator, stay sharp, ask questions, and trust your instincts. read more

I left a Skilled Nursing Facility that I resided in for 4 years to move to Mountain View convalescent center after receiving several recommendations from friends and acquaintances in the medical field. it's unfortunate that in life that we may end up needing assistance but having these facilities is a blessing if they're run properly. I feel very fortunate and blessed to have found Mountain View to help me through my journey, so for those who are looking to place a loved one you might consider looking at this facility. Liking business there are those that are successful and some that fail it usually starts at the top ownership, management, employee, dedication it seems this day and age that's very hard to find. It becomes very evident after talking or exploring the history of this facility that you might find that the owners and the management and the employees have this dedication to serve and to care, I say this because it is truly very hard to find, why I don't have an answer to that. Unfortunately we don't always plan our end of life very well and when an emergency happens there isn't much time for planning, where do you go what do you do? I'm only asking that you might consider looking at this facility first because of all the very unique qualities this facility has to offer. Not only do they offer skilled nursing care, but they also offers subacute, and they've worked for years to get licenses in place to be able to do in-house dialysis care. Negligent and Unacceptable Care My grandfather was at Mountain View Rehab Center, and the experience was absolutely horrifying. We repeatedly told the staff that something was wrong with him, but they ignored our concerns and dismissed us completely. It wasn't until he vomited twice--and we insisted again that something was seriously wrong--that they finally decided to call an ambulance. By that point, his condition had worsened so badly that he had to be rushed back to the ICU. This could have been prevented if they had just listened and acted earlier. The lack of attention, urgency, and basic compassion was shocking. This place failed our family in a major way, and I would not recommend trusting them with the care of a loved one. read more

My husband was there on and off for a year. The last week my husband was there was a nightmare. My husband was treated at Kaiser for 3 days, he had pneumonia and covid.. when he back at Mountain View he was put in isolation, next day they wanted to move a sick patient with him, I said no way. They knew I meant business, no roommate for him, he died 4 days later. He died at Mountain View, you would have thought that they could have sent a card or called me. No card or no call. When you have to use places like these it may not be a pleasant time in your or your family's life, but Mointian View does everything in their power to make is as good as they can. The nurses and staff are ALL top tier professionals. They care about the people here and you can sense it... it is sincere. From the receptionist, Sophia; an office person, Nancy Ortiz, to my brother's case manager Miako.. and Lyndsay in finance, they all were there for us. The physical therapy department are really nice and the patients don't really want to do therapy, but they get them to exercise properly.. good job! On a side note, the nurses are all really cute and friendly and care about my brother and us.. Don't think of this place as your grandfather's sniff... this is high quality care by people who care and deal with end of life issues with dignity. The housekeeping staff keeps it clean and the place does not smell and the grounds look well kept. read more

My uncle Dev Vrat came here after 30 days of being bedridden. Of all the care facilities he's been to this year, this is the best. From the cleanliness, nurse care, food, PT, OT, medication monitoring and well-being care, don't let the casual attitude fool you; the program lets patients feel human and empowered, it allows them to get their inner determination back and sense of being. The supervising nurse discharged us with proper instruction for each medication. The PT AND OT team made him strong and the nurses cared for him every step of the way. Pretty much at the bottom of the number of nursing homes I've had relatives stay at. One doesn't get to meet anyone with any authority. The administration, doctors, social services, and even registered nurses hide behind CNAs and LVNs. They must think themselves be too good for the people who generate their salaries to meet! All the day to day concerns are addressed by passing the buck to upper echelons who are incognito, hence nothing ever get s addressed! read more

After almost 3 months, the reality of this place became apparent. There are certainly a few very good caring staff members. But there are also a lot of inexperienced newbies that are not qualified to take care of these people. Just a few of the problems I watched unfold first hand was a LVN who was unfamiliar with the suction machine who actually asked me how it worked, while my friend was choking on his own saliva. That same nurse was taking a personal phone call to her attorney regarding custody issues with her baby daddy. As well as leaving a hypodermic needle with another patients medication in my friends room and forgetting about it. Also there were times my friend was not properly bathed. Feeding tube was not hooked back up after PT. Finally had him transported to another facility and gave them specific instructions. Put his things together for his journey in one bag and they forgot it. If you end up with a loved one here, be diligent about their care. Question everything and keep records. Their life may depend on it! On a plus side, PT was very good. Don the RN whom we found out at left, cared deeply for patients well being. As well as the CNA Jose, whom we loved! He spoke so positively around patients, giving them great care and uplifting energy! A true plus to the facility. Also regarding grounds, the last day I learned of the giant fish tank in the main building. It's beautiful and a nice spot for patients to view. The cockatiels are also nice to watch. I had to ask them to be fed one day as they were clearly hungry, scratching up dirt and feces in ground for any fallen seeds. I know they've been donated but perhaps a little bird care 101 would be beneficial. And the beautiful lion fountain is also a nice place to sit, provided people aren't smoking there (especially sad to see staff smoking there). Parking in the back as well as handicap spots in the front, and street parking, just check signs for no parking days. read more

10/05 First I thought it was an exile sending my dad to Mountain View Convalescent Hospital in Sylmar. Then I remembered my family and I did pray to God for the best care of my dad now 83 years of age and, the night before he was transferred there from Gardena Memorial Hospital, I had a transformational dream that confirmed God has answered our prayers. Mountain View Convalescent Hospital is God's blessings for my family. The night we arrived at the facility, although late (7:30PM-ish), we were warmly greeted and advised on what was needed to be done for my dad. Karima and Trisha (pardon me for misspelling, or missing anyone's names here) were particularly receptive and helpful. Their presence and demeanor were disarming, making us feel at home and that my dad has arrived at the right place for rehabilitation. On the following day visit, we met up with Gina, Karl and John and, to our family's surprise, they spot us out first to ask questions concerning my dad's health status for proper care and rehabilitation. My dad already looked better by now and was able to walk in subsequent days. This recovery was largely a team effort and painstaking executed by the love and compassion of Certified Nurse Assistant (CNA) Anna. I believe that a good healthcare center is good because of its operating philosophy and belief in serving humans with dignity. Talking with Ms. Castro gave me the confirmation that this is true; but this is not just specific to Ms. Castro on the front end as other team members of the center demonstrate the consistent service quality of healthcare and services. Karesa, Rachel, Monee, Ruby and Sophia all embody this very quality that defines human love, compassion and dignity. Our family are still facing the challenges of locating a skilled nursing facility in the South Bay area that fits the coverage, so we may visit my dad more often. We continue to enjoy working with and relying upon the professional and dedicated staff of Mountain View Convalescent Hospital in Sylmar to materialize God's Grace for my dad and everyone in need of care here. First of all, I wouldn't take my dog, even a mean dog to mountain view convalescent place. They are rude, nasty, the most non carrying people that come in their door. My father was transferred to Mountain due to a fall at home. Mountain View was the perfect facility for my father. His room had a sliding door with a beautiful and peaceful view. The facility is very clean, organized and the staff are very attentive and caring. We met with the team to discuss goals for my father and any concern was addressed in a timely manner. The beautiful bright rehab gym looking out to a beautiful landscaping view was soothing. My fathered enjoyed his therapy daily. The rehab staff were phenomenal. My father suffered a fall, went in for rehab and recovered in less than 30 days. I highly recommend this beautiful facility. I can't say how grateful I am for thia place and ita staff. My father was in this location as a Covid recovery patient and Mountain view staff went above and beyond for my dad. From the nurses to the RT's to even the food and cleaning staff. They all absolutely took care of my dad. Vanessa, Ruben, Sevak, Gigi,Runette,Adam, Barbara you all were absolutely amazing and my family and I can't thank you all enough for everything you all put in to bring us dad back home and getting him back to health !! They nearly killed my grandma. The doctor took her off gabapenten so she couldn't use her hands. They left her in a dirty diaper all day to the point where she got a diaper rash all the way up her back! Came to visit and she was in bed in a dirty diaper, no bra, and her shirt on backwards. Told us we couldn't visit on thanksgiving then they had the audacity to ask her where her family was on Thanksgiving. read more

My dad never made it home, he passed away today. I had to pull him out via fire department 5 days after his admission as he was not given the proper care, placement, help and treatments. My father relapsed. Every time I called I felt that I was bothering them, I come to find out they wouldn't check on my father when they told me they would. We are sadden by our loss. I wish I could give this place zero stars as my review. I'm not even sure how they can have a 3.5 with the lack of care my mom received. My mom was there for 1 week and in that week on the very first day she was left more than once for over 2 hrs in her poop diaper. She was able to get up to use the restroom but was forced to use a diaper because there isn't enough staff to help her up. After me complaining she was given a used and dirty bed pan which they would leave on the floor of the very dirty bathroom floor in between use. The sliding door was left opened and unlocked all night and it was very hot in the room. I had to ask for the air to be put on. I saw more than one fly which in that area is not good at all since there are ranch style homes with horses and another animals near by. The staff was hit or miss. I sometimes felt like the nurses didn't know what they were doing and on many occasions it seemed like they were asking me how to do things. Some were nice and others rude. A social worker, Vanessa was extremely rude and Matt had no patience and wanted me to except the care as "low staff" or other excuses. I gave them many chances in that week and I had to complain more than once not only for my mom but her roommate. It's very sad to see that the elderly are treated in such a way. People would be yelling "nurse" or help and my mom had to ring her bell many times for meds. The last straw for my family was when they were 3 hrs late on her IV antibiotic (this happed 2 times) which was her main reason for being there. We were never told home health was an option, we were actually told that it wasn't by the social worker. I brought my mom home with bed sores that she did not have prior to arriving. It is definitely neglect if she could get a bed sore in just the first 3 days. Her staples were not taken out because they said they weren't sure who would be doing that. The only thing I have to say that's nice is that they did let us borrow an IV pole for the first night home. I'm sure if that was out of fear of releasing the patient without the correct supplies to continue the next dose. If you can avoid this place and keep your loved one at home then do that rather than bring them here. read more

This facility has been such a traumatic experience for my love one; and his family for his entire stay. The abandonment, lack of compassion, and the reckless care provided by ALL members of the care team is heartbreaking. My boyfriend has been placed there for rehabilitation and every single day he has experience neglect, delay in medication, and for half of his stay no action was taken to rehabilitate him. Oh man where to begin with this hell hole of a place. Grandma came here for "rehabilitation" supposedly to learn to walk and talk again after having a major medical emergency. This place is a disgusting joke, if I had know what it would turn out to be, i never would have checked her in here. I was told it's a great place and they really care here. Such BS. You walk in and it smells terrible in this place constantly, like they never clean. It seems more like a place where you go if you are put on a 5150. Honestly, people yelling all day and night. They do NOT care at all for the patients here, they do not take care of them at all! My grandma constantly was in soiled diapers which caused to to get a UTI which they probably did on purpose so they could get more money. She told me they barely checked her diapers and never brushed her teeth or anything, when she got home I brushed her teeth and her poor gums started bleeding, her tongue was caked with stuff. This place is purely for profit and should not exist. Just a bunch of money hungry, greedy people working here. It's a scam of a place and should not even exist. I'm so upset at the neglect and abuse she received. She is so much better at home, where she should have come to begin with, already eating normally and moving within the first 3 days! She was here for almost two months and they made no progress with here whatsoever! They deserve ZERO stars and honestly we are looking into legal options against this place. Since we, in part, used Yelp to learn about Mountain View for my father's post surgery rehab, I thought it might be fitting, to kinda, you know, keep a log on the progression of his treatment, and give accolades to those that significantly contributed to his successful convalescence. With that said, there are no other facilities, no competition, no other choices. So whatever we document here will likely not matter to anyone but my family. Day One: So far I've been shocked. I think that, even with all the pre-arrival visits my family has made, even with all the pre-treatment communications from the surgeon, they still seemed stunned that my father arrived. The room is dirty, so we cleaned it; but no trash cans for the wipes. No chairs. No nurse. Just waiting around. Meds not ordered. We need new pain pill prescription because they forgot to order. I think we were supposed to bring IV kits for meds delivery, pain meds, commode, antibiotics, chairs, urinal, trash cans, sharps container, etc. Even the bed is too short. It's been 6 hours now of trying to manage his admittance. The next few weeks are going to be really long. Day Four Exhausted. Note: You must have an advocate at the facility everyday or all efforts at care and comfort will stop! Photograph everything. Bed is broken, can't be leveled. Where is the RN! Something's wrong. Can it be that all rehabs are like this. This is supposed to be one of the best. Getting old or ill is going to suck. So tired. Day 6 Disappointed. Exhausted. Still can not get meds list corrected. Are they doing this on purpose because I won't let it go? I just want them to stop trying to give him the wrong meds! Every day it's the same battle. I have to be there or they go back to the mystery list. Now his GP is calling asking the same question. And why is he sleeping with his shoes on at night? Do I have to do that too? Why is he required to be here, 30 miles from home, when I have to have someone stationed there all the time? I could do this from his home. Finally got him home. This is a dangerous place. read more
