I Called the owner, and he said oh yeah, we have been having trouble, just bring it with you. When I got there, I gave them the card while I completed an intake form. When they found out I was on Medicare, they wouldn't honor the promotion even if I paid cash for the treatment. They don't want you as a patient unless they can really bang your health insurance.
