She loves her nurse, the food and she's very happy. When she did research on recovery places this one came the highest rated for her insurance. Nana keeps saying she's surprised at how quiet it is here. We are very happy with the people here and the care she's getting. They all deserve recognition. Facility does not handle money for residents. The following residents' files were missing appraisals/needs and services plans: resident 1 (R1), resident 2 (R2), resident 3 (R3), resident 4 (R4), resident 6 (R6), resident 7 (R7), resident 8 (R8), resident 9 (R9) and resident 10 (R10). R7 and R10 have incomplete physician's reports that do not include information regarding residents' TB (tuberculosis) status. LPAs reviewed medications for the same ten (10) residents. LPAs observed missing medication orders from physicians in the following residents' files: R1 and R2. LPAs observed missing or incomplete centrally stored medication records in all 10 residents' files. LPAs observed the following residents missed medications: R2, R8 and R9. LPAs noted that the medications were left in the bubble packs, however staff had noted that the medication had been given. LPAs also observed that R3 requires injections and R3's physician indicated that R3 is unable to administer the injections. Staff at the facility indicate that they test R3's glucose levels and assist with giving injections. Staff that administer the injections and test the glucose levels are not Registered Nurses. LPA reviewed files for four (4) staff regularly scheduled at the facility. Staff 1 (S1) and Staff 2 (S2) have expired first aid certifications; S1's expired in June 2015 and S2's expired in February 2015. Staff that distribute medication have appropriate training. All staff have required training. Upon arrival to the facility LPAs met with Registered Nurse (RN)/Staff 5 (S5) who informed LPAs that he/she has worked at the facility for two weeks, four days per week and oversees facility caregivers. S5 indicated he/she assists all residents as needed. LPAs noted that S5 does not have a criminal record clearance and is not associated to this facility. Pursuant to Title 22 Division 6 of the CA Code of Regulations, the following deficiencies were cited (refer to LIC 809-D): The licensees representative was provided a copy of their appeal rights (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights. Information received from interviews revealed that the facility activity area was moved from a former activity room/area to a section of the dining room. The former activity room measured approximately 30 feet by 30 feet and the section of the of the dining room being utilized for the new activity room/area measured approximately 23 feet by 21 feet. Information received also revealed that due to the layout of the dining room, the view of the resident(s) that participates in activities is obstructed and/or is hindered causing delays and/or interference with other functions. Based on information received, the allegation is deemed SUBSTANTIATED. The other person working with Crystal, Ed Hahn, gave me completely misleading information that led to another few days wasted on a patient who does not have much time to waste. One said the patient was accepted, the other said he was rejected. They need to start speaking with one another, get their stories straight, and coordinate internally so they don't give people misleading information. They also need to be empathetic to people whose relatives are sick, rather than give attitude over the phone for no reason. Do yourselves a favor and find another place for your relatives if you care for them.

They had many great activities. I even got a great manicure. The most pleasant surprise was the Chef. Retired military and one hell of a creative cook, especially the deserts (even though he is a 49ers fan) vs my Rams. When it was time to leave they helped me pack as I had brought way too much stuff (clothes). A special shout out to Michelle and Mark for taking such good care of me. Sorry it took so long to post but I've been working hard at outpatient PT.

I requested the Medicine for Covid before they sent me home, but they did not even get me the prescription. They sent me home upstairs with a numb right foot, Covid and no help! BEWARE!!!

Unfortunately, the reality was very different. The facility appeared outdated and poorly maintained, and the level of care was unacceptable. Throughout his stay, communication from the staff was poor. We repeatedly had to call and ask for basic information. During the last five days of his stay, my husband became increasingly confused and incoherent. I contacted the facility multiple times, expressed my concerns, and specifically requested that a doctor evaluate him. My concerns were ignored. The most alarming incident occurred when my husband fell on a Sunday and was taken to the ER. The facility never contacted me to inform me of what had happened. Instead, I learned that he was in the emergency room through a message from my husband himself. At the hospital, we discovered that his infection had significantly worsened. Given the symptoms he had been experiencing and the concerns I had repeatedly raised, this was deeply troubling. The problems did not end there. The facility failed to send his belongings to the hospital, and when I later returned to retrieve his medications, I was given the wrong medication. When you place a loved one in a care facility, you trust that they will be treated with dignity, compassion, and proper medical attention. We did not receive that level of care. I cannot recommend this facility to anyone. Our experience was stressful, frightening, and completely unacceptable.

I know places like this can sometimes get a bad reputation, but that wasn't our experience here. The Nursing staff are fabulous Erica the charge nurse she is so full life, sweet , and she is on it she takes care of the floor as if it were her own.
