I went to Peak Form for a pre-employment physical and since then it has been a paperwork processing disaster. The office staff in the Boulder facility is completely incompetent. I have had to hand carry documents, hand deliver them, track down documents and request they process them to no avail. Every time I request an update, I get misinformation back. I finally went to my prospective employer and requested they step in as I cannot seem to get Peak Form to make any progress. Do not use the Boulder Colorado facility. The office staff is clueless. read more

I'm only giving this review one star because I have to so I can post this but they don't even deserve a half of a start rating. I'm a local bus driver for Boulder and I have to have a DOT physical for my required CDL. I've had to go to Peak Form clinic in Boulder for as long as I've been driving. Every time I go for my DOT physical there's always some sort of problem, it could be lack of paperwork or medical information they need from my primary doctor. I had a DOT physical last October of 2021. I recently received a letter from the Department of Revenue saying my DOT certification could not be processed because the box on the for was not checked that I wear glasses. The doctor did not check the required box. The assistant who conducted the vision test told me to take my glasses off and tested me as well as testing me with my glasses on. So because I passed the vision test without my glasses they decided not check the required MEC box, I spoke to Dr Parsons of the Boulder Peak Form Clinic and I was told I had to take the DOT physical again to get a new Certification and I was told I would have to pay for the physical out of my own pocket. I highly recommend if your job requires a CDL and you have to take a Department Of Transportation physical DO NOT GO TO PEAK FORM CLINIC. No one there knows what they're doing ! read more

I had a total knee replacement at the end of October. I chose Peak Form because it is close to my house. I e had a fabulous experience. I've seen both Halli and Lorinda and they are both excellent physical therapists. They knew exactly what I needed to regain function. I used the 8 am time slot and was often the only patient. Their Covid protocols seem fine. Both the exercises and the tissue and scar work were just what I needed. I had great on line homework every week. I was offered a couple virtual appointments but declined since I wanted hands on. Lorindas therapy dog Newton was wonderful when it hurt a lot, which a new knee does. I am now graduating with almost full function after 6 weeks and they will give me a home program. I recommend them and this location. read more

2 stars because I appreciate that the front desk PT and NP all wonderful. I absolutely did NOT receive honest answers about my injury and long lasting effects and possible reoccurrence which did happen. Instead I feel that my last exam was deliberately and intentionally rough, causing pain to areas that were targeted by the examiner to be able to say I had a completely new injury. The physician was condescending in the way he spoke to me, And talked repetitively about my health history, I am already well aware of what it is. My MRI was unchanged from the original. I sought second and third opinions. My personal dr and neurosurgeon disagree with this place and told me that this is in fact the original injury. I was not like this before the original injury. I was extremely fit, athletic with zero symptoms before I was injured. Though there is normal wear pathology in MRI, because it's WC, they have to do whatever they can and it seems they will even physically hurt you in an exam to get you off the bill. I'm never recommending this place, I sought my own care and with a simple rx of pred I got better in just a few days. Each time it's a long repetitive story from the doc, blaming history of other activities etc. I'm not sure it's different anyplace else for WC, I've never used it before this and don't intend to again, it felt so abusive in every aspect. read more

I have been so impressed by this PT clinic. I was referred to another by my neurosurgeon and they didn't have appointments for weeks out. Peak form got me in right away and scheduled me out weeks in advance. I had a herniated disk that was causing me extreme pain, I have never felt anything like it. I had an epidural steroid shot injected into my spine and sought peak form out because the place I had been referred (Altitude PT) couldn't get me in until weeks later. My neurosurgeon told me that I may never be able to end the numbness and tingling in my thumb that I was experiencing, even after the initial procedure she prescribed. After one session with Kourtney I was was able to feel my thumb again!! She and the entire staff at Peak has been extremely comforting and helpful. I would highly recommend this clinic to anyone needing physical therapy. read more

Back in August, I ran my left hand over with a truck. Yep, a truck crushed my hand, and I did it all by myself. Trust me, it takes years of practice to achieve that level of stupidity. After surgery, I was in a cast until the first week of October. There wasn't much left of the muscle tissue, and the tendons in my hand weren't very flexible. When they took off the cast I had no range of motion in my index and middle fingers. My hand was a mess. The week after I got the cast off of my hand I was frantically trying to coordinate a physical therapy appointment for my hand. Having to coordinate with the insurance company and find an appointment that fit my schedule, was proving to be a tough task. The day I was at my wit's end, one of my customers had the solution. She noticed me favoring my hand, and asked me about my injury. After I got done telling her (Stephanie Blankemieier) what happened, told me that she works as a PT at Peak Form and had me fax my prescription for physical therapy over to her office. She also told me that she really loved working with hands in particular. Not to take away from Stephanie's efforts in any way, but the real MVP of the first week of my treatment was Pat, at the front desk. The reason she was the MVP is because after I tasked her with getting the insurance company on board, it was done. I was officially done dealing with my claims adjuster, she handled all the phone calls and correspondence with them. Hallelujah! Their schedule was very accommodating and their location is very convenient. During the first appointment, Stephanie sat down and listened to my concerns and goals for my recovery. After that, my goals became her goals as well. She never once told me that my hand, "probably wouldn't be the same ever again." (Something I had heard from just about everyone else at this point.) We just kept working, and still work on regaining full function in my hand. I got a treatment packet with exercises to work on at home, and a full treatment plan that ends with me comfortably being able to swing a golf club. I am happy to report on February 28 (less than 5 months out of the cast) I just hit 100 golf balls at the driving range and have regained almost all of the strength and range of motion in my hand. I am doing my normal workout routine at the gym that includes deadlifts and bench exercises. Before PT I had a lot of expectations of what might become of my hand as it recovered, but this has exceeded all of them. More importantly: Stephanie Blankemeier and the team at Peak Form physical therapy in Boulder, exceeded all of my expectations, and are responsible for the majority of my recovery from what could have been a catastrophic injury. I am sharing this because I believe that silent gratitude is not much use to anyone and their hard work shouldn't go unnoticed. Thank you for all of your hard work, and I will always recommend you guys to anyone needing physical therapy in the boulder and Brighton areas. -Nick read more

I first wrote a review of this business because I was very disappointed with the billing. My daughter broke her arm and had a 15-minute evaluation. We were eventually charged $70 while our insurance company was charged over $200. As a consumer trying to provide transparency in an effort to make a small impact on reforming the health system. I felt like $70.00 for 15 minutes was a poor consumer experience. Note: After revisiting our surgeon, he suggested that she did not need physical therapy because she was healing well and was young/active. So, in our case it was really $70.00 for a 15-minute experience. However, in talking to the business, this experience is not unique to Peak Form. This billing arrangement is standard practice throughout the industry. Whether you have one block of 15 minutes or multiple blocks, the price is the same and determined by a prenegotiated rate between the clinic and your insurance provider. In the case of Blue Cross and Peak Form, the rate for consumers is $70.00. For other providers like Cigna, Aetna this might be more or less. For out of network providers it's more like $128.00 per a block (whether it's 15 minutes or several hours). My other initial complaint was that we were also billed 10 months after the service. Again, I initially blamed this on Peak Form, but this was an internal error that was Anthem Blue Cross Blue Shield fault. Apparently, the Peak Form owner is an outspoken businesswoman who has been trying to reform the insurance situation in an effort to improve patient care and gets frustrated with the status quo. Personally, I think this is great and wish her luck! I also appreciate the time Peak Form customer care took to explain the situation. For me, I hope consumers continue to publish the real rates they pay for services on consumer advocate services like yelp, in order help provide other consumers with information. This is the only way folks are going to be able to make market-based decisions and support reform in health care. Btw - As far as the care that was provided at Peak Form. Everyone was nice and polite, however since my daughter only had a 15 minute evaluation, I cannot speak knowledgeably about the quality of care. ***So here's an update as of 1/21/2018. The billing for this business is really messed up. I paid the bill and then continue to get billed for the same service...This might be a place to steer clear of especially since there are a lot of other providers out there. read more
