From allowing us to do pay-ahead billing to greeting us by name on each and every visit, Forte was fabulous. A few additional items of note. First, unlike most clinics that have a small window overlooking a parking lot, Forte has huge picture windows overlooking a beautiful golf course. Outside the windows you'll see squirrels and birds such as hummingbirds and even brilliantly red cardinals. You might even see a few duffers hacking away and tearing up the turf. Second, parking is easy and plentiful - we never had an issue finding a space right up front. One other thing to mention - their hours. Forte has, at least in my experience, rather late hours. My wife had a couple of appointments where she didn't leave until 5:30PM. The entire staff is amazing. Thank you Sonja for being kind and compassionate to my wife's needs not to mention your patience in dealing with her language barrier issues. Rachel, you often checked in and seemed genuinely interested in my wife's progress. We appreciate that greatly. Diana, the PTA, was also a pleasure to work with and always had kind words of encouragement. Lastly, Grace, sorry for breaking the credit card machine... But seriously, thank you for your kind and courteous nature and in dealing with our insurance issues. You explained things to us which made the process easier to tolerate. In closing, Forte Rehabilitation Clinic has an amazing staff, a beautiful view looking out over a golf course and plenty of parking. They create a very welcoming environment by greeting you by name and checking in on you when another therapist is tending to your rehabilitation. Checking into the facility, I noticed it looked a bit old and had walls being repainted BUT it looked and smelled very clean. Despite this, I has my reservations about leaving my mom. It was the first time that we had been in a situation where we required extensive nursing care outside of our home. Then, I met Lela. She is a nurse manager at the facility. Immediately she sensed my apprehension. I expressed that I didn't want to leave my mom. She encouraged me and offered reassurance. She said, "Maybe the building needs TLC and we're working on it but, I guarantee you, the people here are the best at what they do and you're mom will excel." So, although I was hesitant, I left my mom, and returned the next day and the next day and the day after that.... What I witnessed was truly remarkable. Having been a patient and being a Nurse Practitioner, I can tell you that medicine isn't always kind...except at Sonterra Health Center. Every single staff member that the facility has amassed CARES for their patients. They take their lunch breaks in their patient's rooms if they notice someone feels lonely, they come in with a smile at the beep of every call bell, they gladly change out food trays if the least bit of disappointment is expressed. Every single person has gone above and beyond to support my mother as she heals. They even provided my mother with a volunteer opportunity to do some social service, which lifted her spirits. The testament to the staff was that when it came time to go home, she didn't want to leave! I will never use another facility for skilled nursing. Hands down, the best place!!!

I want to be fair: the CNAs, nurses, and therapists were some of the kindest, most compassionate and hardworking people I have ever met. Many work extremely long shifts doing physically and emotionally demanding work. My criticism is not directed at them. My concern is management. I became especially concerned about an elderly resident who told me she was terrified of her roommate, who has Alzheimer's. She reported being physically attacked and waking at night with her roommate standing over her threatening her. She became so frightened of her room that she sometimes preferred sitting outside in the Texas heat. I personally approached the facility director, explained the situation, and asked him to intervene. He promised to look into it. Before I was discharged, I confronted him again and asked whether this woman's situation would actually be resolved. After leaving, I checked on her and learned that the problem still had not been meaningfully resolved. Sonterra later contacted me about my evaluation. I again explained the situation and was assured someone would investigate. I checked again afterward and was told that nobody had come to see her as promised. This particularly bothers me because this resident appears to have nobody consistently advocating for her. I cannot know why her concerns have received so little attention, and I will not accuse anyone of motives I cannot prove. What I do know is that a frightened, vulnerable resident repeatedly asked for help, I personally asked management to help her, and I still saw no meaningful resolution. During my stay, I also had concerns about the food, insects in resident areas, staffing levels, and what I personally observed as a lack of meaningful interaction between upper management and residents. My advice to families is simple: there are wonderful frontline caregivers at Sonterra, but advocate strongly for your loved one. Visit frequently, ask questions, pay attention, and don't assume management knows what is happening simply because staff have been told. I have written commendations for Sonterra employees who provided me outstanding care because good people deserve recognition. But vulnerable residents also deserve someone willing to speak when something is wrong. Every resident matters, whether they have money, family, influence, or nobody at all. They are human beings deserving dignity and safety. I tried repeatedly to address this internally before writing publicly. My significant other was transferred here for gallbladder surgery, and from the moment we arrived, the stay was filled with safety concerns, poor communication, and lack of compassion. We found another patient's urine sample left in his care area, used medical supplies near the bed, and immediately questioned whether the room had even been cleaned. The worst part came after surgery when he was left alone in severe 10/10 pain for about 40 minutes despite calling for help multiple times. No nurse came until I physically went looking for her and demanded help. (Jamila) 3rd Floor Discharge was just as bad. The nurse openly admitted she had not even read his discharge orders, gave no verbal instructions about medications or home care, and rushed him out only hours after surgery. He ended up having to walk downstairs because they used the wheelchair for our belongings. I did everything I needed to do at the house so that I could follow them to the hospital. Upon arriving at the hospital, parking is bad, at best. Then trying to navigate in the hospital requires a tour guide, breadcrumbs, and a compass. They let me in the emergency room and told me what little room she was in. These are rooms off the main emergency room, and they are small, and not made for visitors. One thing I noticed getting into the area was all the patients in beds lining the hallway. After doing all their testing on my wife, the doctor advised that she had not had a stroke. The reason she passed out was due to a tumor pressing on a vein. He wanted to do surgery the next morning. My wife never got a room that night, and I found out what it was like to sleep in a 5th grade plastic lunch chair to sleep in/on. They rolled us to surgery first thing the next morning, and we were visited by multiple doctors and nurses. Finally, the surgeon arrived to talk with us. He had a list of questions he went over with my wife. One of them had to do about whether or not she had taken any kind of medications in the last week. When he found that she had taken Advil within the last 7 days, he said we can't do the surgery because of complications from taking it. He said we would have to reschedule. That turned out to be a godsend. Will I return? As the doctors and nurses we saw appeared to be very capable, I was not impressed with the hospital itself. They are very busy and crowed with not enough rooms for the people they are treating. I don't know if that overcrowding was just that day, or if it is the usual way of doing business. Maybe San Antonio doesn't have enough hospitals. I would go to this hospital, but I prefer to go where we ended up at Methodist Stone Oak.

The…read moreoffice was clean, I was seen on time, and the physician in training who saw me before Dr. G was outstanding: thorough, thoughtful, and attentive. That first visit was so positive that I even recommended the practice to my mom, aunt, and sister. Everything after that was the opposite. It took multiple calls to get my prescriptions sent correctly after they were sent to the wrong pharmacy and most weren't sent at all, and we'll call you back took days. Then, When my lab results were ready, I called and was told they could simply be emailed to me, and they were. A few hours later, I received a notice that I had missed an appointment and was being charged a $35 no-show fee. I called the next morning, explained the misunderstanding, and was told to come in Friday to review my labs. On Friday, the receptionist handed me paperwork for an annual wellness visit I never requested and had recently completed. When I tried to explain I was there only for a lab review, she repeatedly interrupted, waved me back toward the waiting area and simply repeated multiple times that was what I was scheduled for. I said I did not request a wellness visit. I was told I would be charged another no-show fee despite being there in front of her if I didn't have this procedure done (despite having had it done recently). When I asked about the attitude, she laughed and said "I'll go get my lead", I never asked for a supervisor. The receptionist left and returned with the office lead, whose first words to me were, "What do you need?" She did not ask what happened, only repeated the policy and said I would be charged. I asked if I could briefly speak with Dr. G about the scheduling error. I was told no. I asked to simply leave my phone number so Dr. G could call me later so I could share my feedback, and while I was finishing writing it, the office lead demanded I go sit down to finish it or threatened to call the police. THREATENED TO CALL TGE POLICE, when there is nobody behind me, when I didn't raise my voice or curse, I wasn't asked to leave, simply because I was unhappy paying a no show fee for showing up to a type of appointment I didn't request. There were no patients waiting behind me, and I said repeatedly that I was happy to leave if that was what they wanted. While she had the police dialed she went back and Dr. G eventually came to the front. I appreciated that, but before I could explain, I was told I couldn't disruptive as she was arriving (I wasn't even speaking to her yet) and she referred any feedback back to the office lead. After seeing the same behavior from the doctors as the tone deaf staff, I left because it was clear my concerns were not going to be heard and the staff is one that escalates simple matters. This was a scheduling misunderstanding, not a safety issue. A receptionist or office lead should listen, clarify, and de-escalate, not jump straight to threatening to call the police. Because of how this was handled, I called my family and told them not to become patients. Based on my experience, I cannot recommend this office to others or trust it with my family's care.

I have been extremely happy with PTS. They also hold MOCAA meeting here! So great! Its a support group for moms with children/teens on the spectrum. They also hold storytime here for kids which I have heard is soooo fun and engaging.

She really understands our son and has helped us understand him. She noticed about a year ago that his eyes were not converging and referred us to OCVT for vision therapy. We are so happy at the progress Memo is making. His reading & writing have come so far as well as his overall coordination and overcoming some developmental delays. The room and entire hospital was clean and comfortable. The nursing staff went above and beyond in their care of my husband's every needs. The whole staff of Warm Springs made us feel safe and secure in their care of both of us. I was included in all decisions of his care, and his Threapy. He was finally diagnosed with Dementia and Dr. Patrick Malory was wonderful at treating him and watching him as he added a new medication. That kind of hands on care is exactly what we should expect from all Doctors. I met some truly wonderful people from housekeeping to cafeteria and Nursing. We were always greeted with a smile from everyone and when we left we felt as though we were leaving family behind.
