Preferred care is getting 1 star because I have to. They renovated the entry way, hallways and common areas towards the front. Beyond that it is very gross. Dirty, worn, not clean, old, and more. Once you open the hallway door from the common areas there's a bad stench like sewerage/poop/pee. If that's not bad enough the rooms are cramped and you can't even open the closet with a wheelchair & chair blocking it. Dinner was ham & cheese (1piece each on white bread) and the ham was an ungodly color... not edible. I looked in horror that I moved my mom here. My mom asked the aide for lotion and she said "I'll get your nurse". Of course, she's on break. When the aide came in she laid mom down and pulled her up to the top of the bed but so fast she bumped her head into the headboard. Horrible service and I can't wait to move her out. I'm horrified and don't recommend. read more

I just want to let you know Devin .. a Full Time RN at Preferred Care at Wall is the most caring and loving person..she has been beyond kind to my mom who has Alzheimer's..Devin keeps our family updated..you can see her demonstrating unconditional love and caring to all of her patients in the memory care unit..Hold on to Devin..she is a gem..I watch her everyday I'm visiting mom..she is loved by her staff, patients and other families..she explains and expresses how she will help your loved one's! This place is not a place to bring your loved ones. They are short staffed, neglected to shower my mom for almost a month. They didn't give her medications on schedule and it's a dirty filthy place. I'm surprised medicare doesn't get more complaints about this facility and shuts them down. read more

Here you will find a complete lack of management, no protocols and residents who have to beg for assistance. If you are looking for an environment where the care is lackluster at best, this is the place for you. Most of the Nurses and aids are extremely unprofessional and the Social Worker is clueless. The Director talks a good game, but never takes any action for improvement and NEVER follows through. I wonder how they sleep at night. Do yourself and your loved one a favor. Administration is unaware of the service given to the occupants. No one checks the rooms or patients to see if they have been cleaned , fed or dressed by 11 AM in the morning. It is a continual problem that they don't feel is necessary or just unaware or uncaring. This has been brought to their attention and ignored. read more

do not put anyone in your family here, my mom was there for 2 years they started out good but went down hill very quickly without naming names the head nurse and adminaction are directly involved in my mothers passing they gave her wrong medication and completely refused to treat her medical assistance when she needed it when i finally got her out she was covered in bed soars and much more beware of the facility read more

Rehab facility transitioning from hospital to home health care. This is actually a very scary environment to be a patient in. I woke up this morning to have no nurse or aide assigned to my room. Don't be fooled when you walk in the newly renovated door. This place is atrocious. My mother was a patient for nearly one year. Though we visited almost daily, we often found her neglected, dirty, soaked with urine, and at times, not even hydrated. Though wheel chair bound and non-verbal, my mother was inexplicably hurt several times. This always happened during the evening/night shift and weekends, when the staff is the bottom of the barrel. Also, all of her stuff would go missing. We were constantly buying new clothes and bedding. We reported them to the state twice. We were also told by someone in the county offices that, as of August last year (2019), this place was second in the county for number of complaints. The number one facility for complaints was Allaire Rehab...owned by the same company. Stay away. read more

Some of the nurses weren't bad but this place as whole is a complete shit show. We were supposed to bring her to meridian in wall and one of the nurses called my sister and just said "this is "blah blah" from the rehab under the pretense that she was calling from meridian, she then apologized to my sister for making her think she was from meridian. So because of that phone call she was sent to preferred. These people have no idea what's going on with anyone there. I would absolutely not recommend this place to anyone! My Dad stayed for respite care. His clothes went to be washed. Two weeks later they still haven't located them. He is a diabetic and when I brought him home his insulin reading was 425!. They weren't giving him the correct doses of insulin the entire stay. read more

This place does not care about the well-being of your loved ones. It is dirty, and unorganized. No one knows what is going on at all! My grandmother was sent there after surgery for 12 days. During this time they failed to remove her incisions that were suppose to come out, remove a catheter... And worse, she DID NOT SHOWER THE WHOLE TIME. Please avoid this place. I saw my father through a window open 3 inches today... blinds were also down so I kneeled on concrete the whole visit... only one in 6 months. I respect what is going on in this world. THE WORST FACILITY AROUND !! My 82 year old dad was here for about 30 days prior to his passing . First of all , the cna's that I encountered were horrid ! I'd come during different shifts . He looked like they weren't doing any adls when they got him up in the morning . My father has dementia so he couldn't remember to drink . He got so dehydrated he got admitted into the hospital with the starts of renal failure . He hardly ever got changed which resulted in a uti . I went once in my dads room in the evening and his bed wasn't made as usual but also had blood on it . Every single time I went I asked them please shave my dad he always kept a clean shaved face . NEVER DID THEY SHAVE HIM !! His nails got long with crud all under them so that means he wasn't getting nail care at all during his shower . My dad ended up dying at this place and ultimately he did have a Deteriorating illness I believe because of his poor care he died sooner then he should of. The social worker very sweet but just said yes to things or was noted in his chart but never done ( making sure he gets enough fluids , being cleaned and shaved and changed ) ! The only positive thing I came in counter with at this facility was the sweet unit Manager and the lpn . Overall this place is horrid ! read more

If I could give this place a zero, I would. It has taken me almost two years to write about the truly horrendous experience my father sustained here which ultimately led to his death. I wish I had read over the numerous negative comments and feedback from those unfortunate families who brought their loved ones to this "hell". My father was a kid and gentle man of 97 when he was treated like an animal. Staff was rude and overdosed him with drugs to keep him sedated, compliant and consequently needing minimal attention. He was placed in a wheel chair which was placed behind a large wooden table and shoved up against a wall so that he would not move. This restriction led to pneumonia which led to his death. The few good and kind staff are immediately fired because the administrators owners (who are not patient oriented but money driven)see these good folks as a threat to their evil deeds. Recently, I looked at several pictures I had taken while my dad was incarcerated there. I cried and cried to think I was duped by their lies for the miserable last three months of his life. I will live with this guilt forever. They should be shut down, I had the state investigate and , of course, people like these administrators always seem to find a way to circumvent the laws! read more

I CANNOT EVEN BELIEVE that anyone thinks this place is ok. Just the looks of it from the outside, horrid and seedy. Inside, "under construction" falling apart, filthy and uninhabitable. The rooms are dark and cramped, the grounds are overgrown the dining room has dirty tables, floors and trays. The staff are the worst bunch of animals ever to be assembled. They literally hate old people and hate performing any of their job duties. Refused to answer bell to take my grandmother to the bathroom, cut her leg so bad from rough transfers to wheelchair/toilet. Took EVERY opportunity to steal what they could. Hearing aids, tablet, headphones, clothes. You name it, it went missing. I understand nursing homes are notorious for maltreatment, but this one takes the cake. My family nicknamed it the snakepit. PLEASE PLEASE PLEASE heed my warning if your love your loved one. I wouldn't leave a dog there. I beg of you, reconsider your decision if you are set on this place. There are better places out there that employ people who have compassion and integrity. Let me help you prolong your loved ones life. Stay away.
