i was there 4 weeks and i was treated very good by everyone. The nurses made sure I got my iv's and meds on time everyday. When I asked for something different it wasn't a problem. Adminastration was good. When I pushed the Botton for help the help would respond in a reasonable amount of time. the PT and OT was very good, also the activety director does a great job. The doctor and his nurse are awesome. Overall it was a good experience and the people are friendly courteous and helpful read more

My aunt recently broke her hip, and the entire experience was extremely difficult and stressful for our family. Since she was coming from Kaiser Vallejo, we were initially concerned about the commute and whether she would receive the support she needed during her recovery. From the beginning, the team made us feel welcomed and kept us informed about her care. They were patient with our questions and helped make a very overwhelming situation feel more manageable. We truly appreciated the kindness, communication, and attention they showed my aunt during such a vulnerable time. Seeing her make progress and become stronger has brought our family so much relief. We are grateful for everyone who helped care for her and supported us throughout her recovery. read more

If I could give this "rehab" a zero, I definitely would. A week and a half ago, my sister was found on the floor in her room. She was taken to Marin General Hospital ( my niece was never informed, which she should have been). She was diagnosed with severe dehydration and the beginnings of sepsis. Don't you think that since she wore diapers that someone should have noticed that she had no urine output? She was in such bad shape that she had to be temporarily intubated and the doctors were afraid that her kidneys were going to fail, which fortunately they didn't. There are other details which I can't divulge but which may implicate this facility in a legal matter. If you value your loved one's life DO NOT SEND THEM to this facility. read more

I've been given the task of searching for post acute rehabs for my mom who broke her arm and needs to transfer from one rehab centre to another. This is one of those topics that is kind of uncommon to Yelp but injuries, surgery rehab and elder care are a vital and underserved service needing more exposure and review this I share my experience. I was referred to professional post acute, first thing you notice is there's no visitor parking so that's an immediate put off, not a deal breaker but then when I got to the front door, I received a phone call from my mom's doctor so I sat down on the bench outside, again reiterating Outside, and this rude women, walked out the door, disrespecting my call, questioning my presence, which I explained nicely that I was on call with hopes to visit afterwords, but she inconsiderately ran me off. Imagine what's it like to actually be admitted here! This unfriendly portly gal made me feel unwelcome as I guess they are ashamed and too embarrassed to let in a visitor. Based on this un-welcome I presume they have something to hide. I would also add there's was nothing professional about my arrival. I suggest a name change, Unprofessional Post Accute? I probably needn't say, they are off the list. Based on their reviews it's wise decision no doubt. read more

Wow !!! While I was hesitant at first due to negative reviews and Medicare rating. It was refreshing to walk in and see so many workers and families visiting. On the list that Marin General they did not have this facility but I chose to explore the area. I choose for my loved one to go here as a choice and it was amazing experience. The therapy team is amazing and happy to be there ! They personalized every session and it was among the best experience for my mom. read more

I am apologetic I did not mention them earlier Haissel ( Hope I spelled this correctly) whom is the brightest lite every time she walks down the hallway or into my brothers room. I went to visit my friend's mother . Place was very clean and organized. For me it's important to see elderly and disabled patients, living with dignity and respect . Food was average as part of their diets and needs of the elderly patients. Nurses and take care workers were friendly. read more

The management staff is very knowledgeable, and exceptionally kind. The staff is young and engaging, pleasant and genuinely care for the residents who live there. The facility is well maintained, clean and has always felt very safe. I would definitely recommend professional post acute. read more

My father didn't stay two days due to conditions there. I couldn't bear letting him stay. The staff were nice, but the conditions were not. He is on Hospice and had several falls at home, the last one in which he hit his head, so we thought having 24 hour care would keep him safe. He was placed in a bed in a shared room which was fine, but he had no window, no TV, it was poorly lit, he has a bad left arm and uses his right arm to lift himself out of bed, but because of the placement of his bed, this was impossible, so he was trapped in bed. He has dementia and didn't understand their call bell system which was just clipped to his bed and hung out of reach. The bathroom floor was dirty so his slipper socks got wet and dirty and no one changed them. They pushed his slippers under his bed so he couldn't find them. He was just left to stare at old dirty walls. No chair or room for one for visitors either. Our seniors deserve better. read more

Thought I'd leave a review since I was a visitor here & felt welcome & just overall had a good experience. My loved one said even mentioned Victoria as making her being admitted seamless & treated her with compassion, since she has a bad experience elsewhere she said Victoria answered all her questions & concerns. Getting there was easy & honestly the place was nice, I liked the hardwood floors. Something about the place just made you feel comfortable & calm. My loved one was only there for short term care/rehab but I'd recommend it for those who reside there for longer care. All in all, don't believe other reviews I scrolled along to. But like I said I'd recommend, I didn't take any photos as I was there visiting but even the nurses were kind (I failed to remember any specific names) & the place is well kept. read more

I came to this place with my fiancé when her dad was supposed to be receiving care here. The entire staff was horrible..they neglected care which led to an infection that went unnoticed until my fiancé came for a visit. Then they tried to gaslight her and prevent her from taking him to the ER after realizing it could make them look bad for the neglect. They also somehow lost her dad's clothes every time they did laundry, and the rooms have hardly any space. It's appalling that they are still in business and they should be held responsible for their lack of care which is leading to people losing their loved ones. We have heard from multiple other people who have had the exact same experience as us. I'm hoping this helps someone considering coming here to look for other options because this is not the place. read more

Mine was a horrifyingly disappointing experience whilst in the midst of an extremely desperate situation for "long term" care for my father. This place could not have fallen any shorter... There is NO integrity, respectable or acceptable level of care here that I saw or personally experienced. In fact, I witnessed as well as was victim myself to their neglect and abuse. Don't believe these people when they tell you their "new management" completely improved the quality of this facility. It's an ABSOLUTE farce. It was initially quite a convincing façade, but admissions staff are very young, terribly disingenuous, dishonest and unprofessional-- Victoria is terribly impersonable and downright rude. These people jerked me around something terrible. After approving his evaluation and literally promising me my father could move in THE NEXT DAY, even telling me what room he would be in, I packed up all his things ready to move him. Only hours before the move-in and out of the blue, they messaged me telling me they didn't have enough hospice beds and he could not move in. The administrator, Nicole, would not return my calls and completely avoided me. I waited for her in the lobby the next day for 1.75 hours but she would not even come out of her office, too ashamed to speak to me face to face or simply couldn't be bothered-- the same people with whom I shared hugs and tears of joy telling me that he could move right in! I was at my wits end, desperate and helpless, with my father at end of his precious life. They set me into a horrible spiral. Nobody should be treated this way, especially by a facility meant to CARE for people with compassion and kindness. I fear they are actually just the opposite. I should have known when I got the "tour" and the dark halls wreaked of urine and feces. When I sat waiting that day for almost two hours, a resident next to me soiled himself leaving a big puddle on the floor right inside the entrance. I alerted three different staff members in the span of over an hour that he needed help and changing, pointing to his urine soaked clothes and the obvious puddle or urine on the floor. They all ignored it completely, looking away. This behavior is not only unacceptable but vile. If they exhibit their gross negligence this blatantly in front of a visitor, imagine how they neglect, even abuse their residents with no witnesses around! My experience speaks volumes to the level of care they provide. I came to the grateful conclusion that I dodged a huge bullet. By the grace of God I was able to move him nearby into Marin Post Acute where they were SO much better; the staff, care, the personalities, professionalism, the attention, cleanliness, etc. Go elsewhere if you value your loved one's care and well being as well as your own peace of mind. I'm so glad my angels protected me AND my dear father from this place. read more

If i could give this place a negative 1000 stars, i would. Nothing professional about this place. Truly, i've never seen so much neglect in my life and they take no accountability for any of it. My dad was placed here in November 2024, and he passed mid January 2025. When he was checked in here, he was in good health minus his Parkinson's. Within a month, he developed fast growing pressure ulcers all over his legs and butt that were a result of him not being proper changed and treated enough. He then lost a significant amount of weight, and his ability to speak. It got to the point where he got very sick, and we were advised by the nurses on shift to take him to the ER that same day. As we made arrangements to get him to the ER, we were then stopped by the director of nursing, which looking back was them trying to save themselves from probably being reported by the hospital that we would have taken my dad to. We then got an outside opinion from a board certified doctor who confirmed these were stage 3 pressure ulcers, and considered this to be a "never event", which essentially means that they should have never occurred in the first place and were entirely preventable. My point is, this was all due to patient neglect, and it wasn't just my dad we saw levels of neglect happening to unfortunately. I acknowledge this is an incredibly hard line of work to be in, but this should never have gotten as bad as it did if this facility was as upstanding as they claim to be. Especially for a place that "prides itself on its care". Im certain we aren't the first and wont be the last. When my sister and I met with the head administrator, Nicole, to talk about this and about getting some more specific care for my dad, she gaslit us by laughing in our face and asking if we "met with a psychologist" to diagnose my dad's ulcers and not an actual doctor. Implying that the second opinion that we got wasn't credible. It was clearly avoidant of how serious this was for an 86 year old man. In the end, they pushed my dad to move to a sister facility in a very, very sick state because they wanted him off their hands. This new facility that we moved him to is where he ultimately died 1 week later. After everything though, I'm so glad he passed in a cleaner, more professional and caring facility than this hell hole of a place. Anyway, to make a long and sad story short, don't place your people here if you have other options. Especially if you love them. I grew up in Marin and I'm just so shocked and disappointed that a place like this exists. My dad was my first aging parent experience and if we could have done things differently, we would have. That said, i know that not everyone has the means for better care (we didn't) so with that i would say, if you have to place your loved one here, PLEASE advocate for them. Please. In every step of the way. That will make the difference. My final word is that these people don't care about the wellbeing of their vulnerable patients and place more value in operating as a business (speaking directly of the admins, not necessarily all the nurses). Example 1 being the way they reply to these bad yelp reviews doing damage control. Sending anyone going through this situation love and light, and i promise things will eventually get better. read more

The question is what can they do to improve? Close their doors. This is the worst run facility imaginable. The administration is incompetent and the staff turns over weekly. A dear friend had her leg amputated due to their lack of care. Don't be fooled. This business has a well deserved horrible reputation and many, many complaints have been filed against them. read more

My sister, Florence was at this facility on and off for about 7 months since the beginning of this year following 2 hospitalizations. Initially we were hesitant to have her stay there due to bad reviews and the fact that we had to commute in order to visit her. But at that time PPA was the only facility willing to accept Florence. To our surprise, the staff there are all excellent. They were cheerful, helpful, professional and attentive to not just Florence's needs but ours. They always greeted us with a smile. While in San Francisco for vacation, my mom unfortunately broke her hip and was eventually transferred to this location for rehabilitation. However the gentleman running this facility lied to my family and did not provide several of the things he promised to my parents during their stay. My mom was shoved into a room with three other people, it was loud throughout the entire building and was beyond chaotic. Although the nurses did take good care of my mom, the place as a whole was less than ideal. Lastly, the gentleman promised my parents a free ride to the airport but of course it was just a lie to get them there and they ended up paying $200 for an Uber. To say the least we DO NOT recommend this facility to any family. Please go to a different location that will truly fulfill the needs of your loved ones. read more

Horrible place to even visit for 10 mins. My kids Dad was a resident for 10 years and after he passed a couple months ago they would not return his belongings. After 7 weeks we had to call the police to escort us and then they gave us 2 garbage bags with other peoples clothes. All stained and crumpled up. Only one shirt belonged to him. They will not respond and tell us where all his belongings are. The administration is rude and not helpful. Very sad and outrageous situation. Two friends have stayed here after being injured. They have been satisfied with their care. But as a visitor, I am saddened by the people I have seen sitting in the large tv/dining room. Listless, silent, drugged (?). Rooms are depressing with no effort to providing minimal design/cheer. Is there an Activities Director? Thumbs down. read more

My father has been at this facility for 7 months and I have not once had a care team meeting to go over his care plan. He is there to get rehab to learn to walk again but only has rehab about once or twice a month. He complains about the food, today he said he had 5 pieces of dry toast, 1 egg and milk. He said the food is so bad he often turns away the food. He has lost along of weight. He is now so weak that I can't bring him home without getting live in care. He was put in diapers and had never worn them before. I want him out of there so bad. They gave his clothes to someone else and he told me today all he has is a blanket so mailed him clothes. We sent him some high calorie drinks and he told me they never gave them to him. I can go in and on about the terrible conditions there. Unfortunately I can't bring him to my house and because of Covid I can't find a care person . They had 90 patients get Covid and recently 7 staff get Covid, but no one can visit their loved ones. This place is deplorable and I've been working with the local Ombudsman to work through some of the issues. read more

my dad was in this facility for about two weeks after surgery following him breaking his femur. although there were some very nice nurses who were hard working and attentive, the place had many issues. no central AC, so the building is hot and humid in the summer, and the smell is always peculiar. the conditions just aren't great. i get that it is what it is and it's definitely a lower budget place, but there's some things that just didn't sit right with me. food was very unsatisfactory and my dad went days without eating. sometimes he would have to literally have to beg for attention for his needs. patients screaming and wandering the halls at all hours of the night, even coming into my dad's room at times while he was trying to sleep. i think there's a level of care and professionalism you expect when you trust a facility with the care of your loved ones, and this place was traumatizing for my father and i to experience. again, the physical therapists and nurses weren't the issue. they are kind people who genuinely want to help, work hard and care. i think there's a lot of things that need to be fixed at this facility though, and if you can afford a better place for rehabilitation, find one. read more
