    Be careful being nice to the staff, they don't take compliments well.

    My first experience here was trying to get someone to answer the phone, I called three times and finally got someone who was mumbling and talking to another person at the same time. I got a little information, but nothing concrete. I was told that I needed to go meet my loved one there after they were transferred from the hospital, yet no one could tell me a time. It was very frustrating. From the moment I walked in, the staff were not helpful or friendly, and the strong, pungent scent of what smelled like those blue disinfectant disks in urinals permeated my nostrils, made my throat hurt and my eyes burn. It was a little too late to say no to transferring a loved one here. Once I managed to find the front desk, I stood there waiting for one of the three people to greet me or even look at me, not one of them did. I finally said "Excuse me, are you able to assist me," and I got a response "I'm listening" with no eye contact or even a glance up at me. I explained why I was there and he motioned to go a specific way. I found all of the interactions very rushed and unprofessional. That was day one! I located my loved ones room. There was no staff to greet her, after about twenty minutes a lady came in and was rushing me to sign "paperwork" on an iPad. The welcome bag was a nice touch. Over the 13 days that my loved one was there staff did not communicate with me or other family members, unless I tracked someone down. The interaction with most staff was rushed and I felt brushed off. Four days prior to me asking to remove my loved one from the facility a doctor did call me, because three days before that I insisted that they check my loved one for a UTI. That was the only discussion that I felt heard. The doctor was very informative and ordered a mobile scan for my loved one and then no one provided results to me. After multiple discussions with family members about the lack of communication and unprofessionalism, we decided to bring our loved one home. This facility is not equipped for memory care. The windows are old, the screens don't fit properly, the blinds are dusty, old and don't raise. There are areas that need repairs. This building is neglected. Their website states that we are able to send e-cards and the staff will deliver them to the residents. That's a lie. My loved one never got any of them, and when I inquired about it at the front desk, the man had never heard of this, the social worker also didn't know about it. Why have that on the website and then staff is clueless!? The director didn't answer my email and does not have a voicemail. There is so much that was not great here. I also had to ask four days in a row for my loved one to have a shower. Gosh, there's so much to complain about, so I'll stop here. EDIT: The two stars are for Nancy; because every night that she was there, we were greeted why a smile and light hearted conversation! Nancy also stopped someone from visiting our loved one when we said that he was not allowed to visit. Nancy deserves a raise.

    When can Kevan be reached if he is gone by 2pm every day, M-F? According to a front desk receptionist who is there at 2 every day he is already gone as of this time.

    Answering my own question - he's no longer the administrator.

    How many people in a room? what is the ratio for patient staff?

On the list that Marin General they did not have this facility but I chose to explore the area. I choose for my loved one to go here as a choice and it was amazing experience. She was taken to Marin General Hospital ( my niece was never informed, which she should have been). She was diagnosed with severe dehydration and the beginnings of sepsis. Don't you think that since she wore diapers that someone should have noticed that she had no urine output? She was in such bad shape that she had to be temporarily intubated and the doctors were afraid that her kidneys were going to fail, which fortunately they didn't. There are other details which I can't divulge but which may implicate this facility in a legal matter. If you value your loved one's life DO NOT SEND THEM to this facility.

I've been to a lot of nursing homes and this is overall the best one I've stay in. I can honestly say I hate nursing homes I've been to at least 10 all over the bay and they all sucked I saw a lot of abuse going on it so sad to see how some treat their patients. I have so many horrible stories. Also I noticed that the nicer the nursing home looks the worse the CNA and nurses are! This one on the other hand isn't the nicest looking one or that good smelling but one thing for sure your love one isn't going to be laying in a dirty diaper. When I press the call light to get my bed pad change they come right away, they also use soap and water when they clean you so you don't smell like pee. The CNAs and Nurses are such hard working people I get a bed bath every day in the morning. Oh and something else that I loved is that if I had a problem with a CNA or Nurse I just needed to speak to the people in charge once and they would speak to that said person and I would never have to deal with that said person again, no matter how big or small the problem was they would NEVER make me ask twice that problem would be fix ASAP! I also got PT his name is ALEX. He is very friendly, funny, and caring. He spends time explaining and making sure you understand what you're doing and why you're doing it. He is amazing! They're all amazing but not Perfect! The night shift or 11-7 crew can use some work but you have to be patience they deal with a lot sun down patients. Just remember this place isn't a 5 star hotel or a palace, the CNAs and Nurses aren't perfect, Yes it will be stinky sometimes and most importantly be PATIENT! I promise you will have a smooth ride!!! Oh bring a book!

The Center provided a smooth transition from the hospital to rehabilitation. The nurses are skilled and are supported by a team of doctors who provide quick response to any care issues. The Center also can provide ancillary services such as Podiatry, Dentistry, and Optometry. San Rafael Healthcare and Wellness Center is a smaller center, but it punches well above it's weight in services like physical therapy and food services. At the same time, it's small enough that a patient can know the whole staff by name.

During COVID, she became very ill and was placed on hospice. After caring for her at home for 9 months, we were advised to find a long-term care facility. I briefly placed her at another facility in San Rafael, but knew within hours it wasn't the right fit. Finding South Marin was a turning point. My mom was diagnosed with terminal cancer, kidney failure, and dementia. The care, compassion, and respect she's received here have been exceptional. A special thank you to Kelly, the social worker, who is knowledgeable, supportive, and truly cares. Carina, the activities director, is also wonderful and brings so much kindness to the residents. All the nurses, assistants, and staff have been wonderful. I never imagined putting my mom in a SNF, but I feel very lucky she's at South Marin. As a former long-term care volunteer ombudsman, I also want to add some perspective. I've seen reviews mentioning delays in care. The reality is that staffing shortages are a widespread issue across the industry due to policy and funding challenges. That said, the staff here consistently do their best under difficult circumstances. Based on both my personal experience and professional background, I can confidently say South Marin Health and Wellness is a top-notch facility. Thanks team, I appreciate all of you!

Their PT (what she tends to need most) is terrific - the PT therapists seem to experience little turnover as the same individuals have provided PT for my mom over several stays. Despite being very busy, the social workers are also very helpful. And Dr. Susan Nguyen (the hospitalist there) is wonderful - caring, smart, and on it. Rehab isn't fun, the facilities are all really busy and frustrations are inevitable. But we will keep requesting South Marin when my Mom needs it b/c it's the best around.

He had been quite resistant to moving from his independent living facility in the south bay, but it was clear he was not getting the care he needed there, even though we were paying for extra services. After an illness and 10 day hospital stay, we knew he couldn't go back there and are delighted and grateful that Creekwood had an opening. They were flexible about his recovery needs and the transition went very smoothly. He is very content and calm and is actually showing signs of cognitive improvement now that he has all of his needs met at Creekwood. The facility is always clean, and the kind staff is wonderful! Everything is included in the monthly rate: help with showering, dressing, shaving, bathroom help, medication administration, incontinence supplies, so you don't have to piecemeal individual service costs, which makes things so much easier. The best part is I have the utmost confidence that all of his needs are being met and he truly feels at home now.

So when Kaiser recommended that my 78 year old dad come here for a week after knee surgery I was skeptical. He hates the very idea of rehab and has seen both my mom and grandmother spending time in some truly hellacious places. So we took a tour with Nico, the admissions director, who was charming and very sympathetic. The place was in perfect condition and the staff seemed sharp. Kaiser had recommended 7 days but we managed to talk my dad into going for two days. After the end of his second day I thought I might have to battle with him to get him to stay, but amazingly he was happy with the care he was getting and told me he wanted to stay the full time! The staff here is outstanding. Every single person from orderlies, to nurses, to social workers are friendly and attentive. The grounds have beautifully maintained gardens, and the rooms and public areas are freshly painted with nice curtains and newish carpets. I'd originally planned to spend two weeks with my dad at home helping him rehab his knee but after watching the amount of work these people do it's clear that I wouldn't have been able to even come close to the kind of care he got!!!!
