It's hard not to notice some scary one-star reviews for Queens Boulevard Extended Care (QBEC). But I see similar reviews for other hospitals, rehabs, nursing homes, etc. Our sad national reality is that these facilities are being horribly squeezed by insurers, especially Medicaid and Medicare, and at a time when our aging population is boosting patient census. So yes, there are times when a patient will ring for care and wait a long time for response. I experienced some of this during the early part of my one-month stay at QBEC before I became ambulatory. It's horrible. But until our societal priorities change, these facilities will always be strained and need more staff than they can afford. I'm angry, but not a QBEC. I'm angry about the societal values that limit what QBEC and all other such facilities can do. But as for the tings QBEC can control . . . I'd give more than five-stars if I could. This facility's strength is its people. All patients have a choice in how they will interact with staff; either respectfully or with disdain. Those who choose to treat staff with respect will find that the nurses, CNAs (aides), and transport people, are amazing. With all the horrible things we see in the news today, it's so uplifting to experience the kindness, compassion, and just-plain niceness staff gladly delivers. I started on the second floor and eventually was moved to the fourth. Interaction with staffs on both floors was always delightful. No matter how messed up I was physically, my interactions with staff always made me feel much better. I especially want to call out the physical and occupational therapists. I worked with Marriel (I hope I spelled her name correctly) for PT and Luciana for OT. Thanks to their knowledge, kindness and supportiveness, I can now walk unaided (I came home with a can and walker, but both are now optional)... even though I entered the facility as a dead-weight blob that could barley turn over in bed. I was told by others at the outset how hard therapy would be and folks around me wondered if I would really embrace it. Yes, it is a challenge. But Marriel and Luciana made it easy to embrace. They were never soft on me. They pushed me, often hard, to the point where I needed a nap after each session. But they never caused pain. They always knew how far they could push beyond what I had previously done, and that what their aim. And, of course, they always watched with eagle eyes and took appropriate precautions to make sure I was safe. And by choosing to always do therapy in the gym, as opposed to in my room, I was able to observe other therapists and patients. I saw other therapists work effectively with their patients at whatever condition the latter were. All were different, but I saw all make gradual progress each day. I'm happy to be home now. Who wouldn't be! But there is one bittersweet aspect to this. I have to say I think about and miss the many wonderful people at QBEC that showed me so much kindness and support. read more

I recently was a patient there and the experience was beyond belief. The facility had no towels and when I showered I was given bed sheets to dry myself with. The hot water was non existent after a few minutes in the shower. The food was disgusting. I told them I was a diabetic but that did not prevent them from giving dried pasta twice a day. I spoke with the kitchen more than once but the diet remained unchanged. One evening when dinner was served, pasta again, there was no fork on my tray and when I asked for a fork I was told n o one had a fork. They then distributed plastic forks to everyone. My bed was never changed but instead was straightened up each day. One evening I asked an attendant for water, she said she would be right back. Right back was two days later and when I mentioned I was still waiting for water she just laughed in my face. I expressed my feelings to those in charge and they said they would compensate me. I advised them if I was not properly compensated I would put a review on their web page. read more

The care was sub standard. They have staff that are under experienced and skilled. The place is run down and needs a good overhaul. The food is barely edible. The kitchen does not pay attention to dietary restrictions: e.g. diabetic needs. Time they raised their standard. read more

My mom was very reluctant to go to another rehab from the previous nightmare rehab at Cliffside which has a false Medicare 5 star rating . QBEC had a low Medicare 1 star rating and surprisingly provided good decent service. The Medicare CMS rating is inaccurate so don't rely on those ratings. My mom stayed on the 2nd floor for sub acute rehab so review is for the 2nd floor staff and facility only. Most of the staff were friendly,nice and explained things. The staff and social worker were helpful if you can find them. There will always be a few rude staff. Facilities are a little out dated but it was ok. I gave 3 stars because it was very hard to contact anyone to get a status and needed to follow up with them to get things done. You had to rely on them calling you with the status. The location is not near any subway station but there was a bus stop nearby. There was a Citibike across the street which I used to go visit my Mom. read more

Terrible. I was physically abused by an aide. Have bruises to show for it. Talked to supervisor but she was here again today. No apologies by anyone. Today there is no hot water. Sponged bathed in ice cold water. It's the weekend so can't be fixed until tomorrow. I heard several residents screaming as they were told it was their shower day so they had to take it. read more

I remembered my grandma coming here after she had cancer due to the stress after my grandpa passed away back in 2010. This place isn't glamorous but was a few blocks from the 7 train station. Please don't leave your love ones in this house of horrors!! Unfortunately, we had to leave my mom here for a period of time, till we were able to bring her home, thank God!! One week of her being there, my brother found out that they had put her on anti psychotic meds without our permission!! Mom couldn't feed herself. Numerous times, we would drop by to see plates of food by her bed untouche! One time i came in to bring her food and found my mom in the dining area on the floor!! That did it for me it was so disheartening to see this, looked around, and some of the staff were doing whatever they do, felt like exploting!! My loved one was there a week before pt started they had no wheel chair for her. She came home and still needed more therapy. This rehab is a bit old but the spirit & friendliness of the staff makes up for that I have been in two other rehab facilities since my last visit for non Ortho rehab - This place has THE best post orthopedic surgery PT around. Eva is the heart and soul of the 2nd floor Orthopedic rehab. She is the best ortho PT & the only reason I return here. She treats every person individually with great care & respect. She's so hardworking & inspires everyone to exercise hard & do their best. Jason from dietary visited me frequently to make sure my GF celiac disease was being met by the kitchen. There was GF bread this time - a real treat. When no GF option was available for a meal the kitchen supplied a GF Amy's dinner A real treat too. All the staff was amazing & too many to mention Wonderful caring RNs & nurses aids Joy the head nurse- always available for any issue I know that I will be needing another hip replacement in a year or two. I look forward to returning to Queens Boulevard Extended Care Facility again - scheduling my surgery at NYHQ for when I make sure Eva is not on vacation read more

This place is horrible, I worked for only a week being employed in this facility being hired as a kitchen worker. The kitchen staff are very unprofessional and behave as if they still belong in junior high school. The manager's break Covid 19 standards as well as the kitchen staff. I left the job because of an overly aggressive employee that was giving me order's for very simple things to be done. If you have any special dietary needs, DO NIOT COME HERE. The nursing staff is great but the dietary department needs to be traded in for players to be named later. Not only do they constantly mix up the food orders but are incapable of following dietary needs, suck as those lactose intolerant or those who require a renal diet. The facilitators of this establishment are too busy spending patients funds om failed political campaigns., when the residents are not even offered ways or the means to receive absentee ballots to vote. Our lovely kitchen cannot place the proper orders om the proper trays. They might as well employ grade school children, of the wave of new refugees who have come seeking better lives. OMG this place doesn't STOP sending me freaking invoices. My father recently passed away NOV 2019 and to this day even today being Valentines day Feb 14 2020 I received another one. I sent them the death certificate once twice and still send invoices? do they even understand that my father passed means he passed away. I am so angry with this s---t from them they are driving me up the freaking wall, then again I am dealing with incompetent people who's IQ is 2% below plant life. The rudeness they have especially Beena or the other one I forgot her name it's ok she's not important either . Neither of them have respect. I told them 50,000000 times my father has Medicaid, they said it's different from medicare I said I know im not stupid. He has Medicaid 40x I told them & they think im lying. I gave them the Medicaid id# number my father has and they felt stupid. They wanted other information personal about how much money in the bank etc.. I told them I dont know and I actually do not know and wanted his statements. like I said they are incompetent and only care about MONEY and not well being of others. They told me without knowing how much money in the bank they cannot pursue getting Medicaid for my father. I said I dont know how much money is in their account and where he keeps it and again I told you he has Medicaid already I got it for my father can you please stop with the b.s. well, what happens they send an outline in end Sept 2019 break down my fathers social security and want payment plan of 700 a month to pay off 13,000 from Aug 23rd to Sept 12 2019 my father was there etc... I told them in a letter where do you get X amount of money he doesn't get that much the other half you are counting is my mothers social security which doesn't even count. Supposedly you are in the finance department and you dont know your ass to your elbow. My mothers social security should not even been accounted for and no way in hell are we paying you 700 a month to pay off that bill?? My father even told them we dont have any money how are we going to survive we have rent phone cable electric food etc.. you are insane. The other thing that pissed me off besides the two retards in finance department is Anna social worker. The liar that she is told me that after the 21 days the payment will be 300-500 day depending on room and other factors. After she told me that a friend of the family (aunt) was on the other side of the 4th floor and her nieces and nephews visited and I told them that social worker Ana said 300-500 and they said wow that's a lot. I approached her again and she tells me in front of my parents and my aunt Anna says " oh no I didn't say that I said 650/day I said oh no you didn't u told me 3-500/aday where do you get 650 this is not the Taj Majal???? like really. I wish I could've record her telling me it was 3-500/day yet too late. I know what I heard and I dont make up stories and that is what I told Anna unbelievable. With 4th or 5th invoice received today 2.14.20 I am to the max pissed off with their stupidity. Only I can say very few people are nice there and caring from the staff everyone else is crap. I would never have anyone go there at all they are low class, liars and also MONEY HUNGRY jackasses . They dont care all they care is the Benjamins in your pocket. I had a family member staying there over the course of a few months after a major surgery. The facility was wonderful and clean. They took proper precautions during the pandemic assuring that all patients were safe. I felt very comfortable leaving my great uncle there. First off let me say this as a former worker there. The veteran workers don't give a damn about the residents. When you have good workers they try to bully you. I was working in 2020 giving out trays for lunch. One particular resident keep ringing the call bell. As I was the only worker could be seen passing by his door. Everytime the bell goes off I would go in to make sure he was OK. Not even my patient. I told the patient it's lunch time is there anything I can help him with. He wanted his nurse, told the nurse in charge of him, patient room number need him. Continue doing my work. The man rung the bell continuously in a space of a minute. I stand by the door with a tray and told him the nurse will be with him uno momento. Exact words. He report to the dns, I abuse him. I was in awe when called in the office to speak about him. I was gonna leave the facility anyways. I didn't want to deal with the conditions I was going through getting sick all the time. Feb 2020 covid came next. Anyways, I wrote a letter giving it to the Dns in hand I quit effective immediately. She told me if I leave I look guilty. I ask guilty of what I was gonna leave to become a correctional officer. They didn't know that. You know what queens blvd extended care did? Report me to department of health for neglect an abuse. I did nothing wrong. All was clear with no wrong doing find. I was assigned there after my knee replacement surgery for four weeks and it was hell. Because of my age and being the fact I were the youngest person on the floor the therapists try to put more work on me than the other patients and when I complained they got an attitude . Don't ever try to talk to them cause comprehension is not their strong suit despite some good people working . They need to have better communications and people especially some members of the daytime staff. To me it is like if you are under 60 the staff don't know how to deal with you cause they all about control like they do a lot of their patients. I wouldn't send a dog there and with the exception of a few people( mainly the night and weekend staff) they need to replace a lot of the daytime staff. read more

I have already reported them to the department of health but have no clue what they did to help. Today I brought my mom pizza at 12:30pm and at 3:20 my mom still had not gotten her pizza. The person that delivered it left it in her room and never told anyone. Then the supervise Mrs. Walia who does not help at all said she would give her the pizza. I asked her how can you give an 87 year old person cheese that has been sitting out for 3 hrs? So instead of making things better she hung up on me. Please avoid this place at all cost, they don't care about the patient they are just money hungry. Check carefully their information, Their website is not accurate. the photos are from at least 15 years ago and very far from reality, most the staff shown on the website are NO longer there, their testimonials are made up to their convenience. Unfortunately their staff are very arrogant, irresponsible and dishonest who have NO respect for the patient or their family specially Jhonatan Mawere, Anna Bernabéu, Lindsay Sherman and Nursing Supervisor Ms Tan. My father was in that place since October 2019 due to a hip issue, which ended being a DEADLY malpractice done by Dr Mohammad Jawaid and his colleague Dr Stephen Johnstone. My father was being treated by Doctor Mohammad Jawaid, who also is a doctor at this nursing home. It all started After my father complained of a hip pain and decided to make an appointment with Dr Jawaid. He referred my father to doctor Johnstone who performed a procedure that according to them it was necessary to help my father get well and walk normally again. Unfortunately three days after that his leg got swollen, and infected, with a bacteria. Dr Johnstone did not take any action or responsibility about this. After so many months of treatment, my father went through a series of procedures that eventually worsen his condition, it seems that the infection was created due to the lack of higiene at his clinic, after so many days of treatment my father was send back to Queensboulevard Extended Care for recovery and therapy. He didn't recover at all. Ever since he was there the attention was lousy and the nurses didn't have much idea of his condition. My father was not able to move by himself very much and he was left without food many times, including one time while we were there and he was in the dining room waiting, he didn't get his lunch. Despite of being there for a hip issue, the therapy done was non-related to his condition. He was sent to "therapy" just to do some arm exercise and other nonsense. Nobody wanted to deal or had the patience to deal his walking therapy. Therapists only focus on the patient that will be there for a short term, everybody else won't get the therapy they are suppose to get. After being there for a many weeks they realized that he needed a procedure. According to them, the surgery was essential and will give him the ability to walk again. It didn't help at all but instead it depleted his insurance budget On march 25 he was discharged because his insurance ran out. After being there for almost 4 months he never actually recovered completely. They used all his insurance budget and the recovery was never successful . He was discharged without his equipment. They ask us to buy a shower chair that will cost a few hundred for him to use while being at home, but it will not be cover by the insurance anymore. He was discharged without showing any concerned while NYC is going through quarantine due to the COVID 19, they didn't care about that. On the day he was discharged the Social Worker Anna Bernabeo brought him down to the lobby along with his stuff, Stating that he was ok and capable of walking. She lied about his condition, all she wanted was to get rid of him, We tried to get in contact with her but she is nowhere to be found and she doesn't return any of our calls. She is a very irresponsible and unprofessional social worker. After a few days of being discharged my father started complaining of pain, he was sent back to the hospital due to the fact that the infection came back, and eventually he went back to Queenblvd Ext Care 8 floor as requested by Dr Jawaid to finish the treatment and then another surgery. After that surgery my father didn't recover At all, the doctors in charged Dr Jawaid and Dr. Johnstone didn't follow up on the procedure as they are suppose to, they are just irresponsible. On July 11, my father was under the supervision of Nurse Supervisór Ms Tan, when she called us stating the my father was vomiting blood during the process of transfering to Mt Sinai hospital his wedding band of 58 yrs of marriage got removed. Upon asking about his wedding band Queensblvd Extended Care denied its existence despite of showing pictures taken on July 5, 2020 in which his wedding band was visible. Dr J Mawere questioned the validity of the pictures and obstructed an ongoing Police investigation about this matter, advised by his IT guy who claimed the pictures were not legit. Shortly I requested to speak with Dr Mawere and Lindsay Sherman but they refused to see me. Please be careful, they only care about the money, the administration and owners don't. Care about the patient, money is the main reason they are running this place. Please be careful with Dr Jawaid and his colleague dr Johnstone, they are very irresponsible. read more

Would give no stars if it was an option. Definitely don't recommend this place to put loved ones in care. Most of the staff, if not all, neglect and ignore the elderly. A relative of mine was here for some time and constantly had bed sores and wouldn't be taken out of bed unless we were there and spoke to somebody to do it. This place should be shut down and filled with responsible and attentive staff that actually care about the sake of the patients there. read more

Unfortunately, I don't have good things to say about this facility. There are a few employees that were kind and helpful, but the health care workers were insensitive and don't give you clear answers. They not only lie, but they neglected my grandmother at times and failed to inform my mother and I she had fallen previously. Since COVID 19 hit the US specifically NY, my mom and I received a lot of confusing information regarding my grandmothers condition especially when we questioned them if she has Covid. They never bothered to test her for COVID even though we were told she had symptoms. We were never really given clear answers except for that she was being treated as if she does have it. The week before my grandmother died in this facility in April, my mom asked to speak to her mother and no effort whatsoever was made to make it possible for my mother to speak to her. The excuse was that she was in a room getting antibiotics. Apparently the cause of her death was due to her heart stopping. My family and I believe she was neglected based on our past and recent experiences and it is truly heart breaking that we will never know what really happened during that week she was sick and suddenly passed away. We are grateful for the few that showed kindness and compassion, but we cannot say the same for the majority. read more

If one is high-functioning and only needing short-term therapy after surgery, this facility may suffice. The physical and occupational therapists I encountered were mostly topnotch. Although many hard-working well-intentioned staff members work here, overall it is poorly run, has low standards, very poor nursing quality, and no system whatsoever for managing patients' oxygen needs. In my opinion it is not a safe environment and aides are paid and treated poorly and mostly unhappy with their work, which makes for an unpleasant atmosphere. RNs may be busy with paperwork as they do not report changes in health, even imminent death, to family members. There are so many substitutes they don't know who their patients are and take no time to introduce themselves. Medicine mistakes are common, and staff so overworked they don't like any questions. On a longterm floor, the longterm charge nurse badly scolded a woman with dementia. Showers are rarely offered to patients. I could go on and on but it brings back too many bad memories. read more

if i have a choice i will rate this place a zero. the service is horrible. you cant get any help at all from the nurses. all they like to say i dont know ask her and then they walk away. nurses doesnt give any information to the family members such as "oh her appetite is not so great, or she is not moving too well, need to motivate her, and so on" as a family member of a pt, we do like to get involved and help with the care. calling the facility is so difficult, no one answers the phone, they always like to ask you to hold and transfer you to somewhere else, pretty much playing phone tag within the system. dont go to this place. awful, and not kind to the patient. all they care is to get there work done. read more

If I could give this place zero stars i would! This place is a disgrace!!!! My grandmother had, had an accident and was placed here for 3 weeks and all I can say is this place is trash. Lets start off by saying how filthy this place is. Bathrooms? cleaned? HA, if you want who ever is staying here to use a clean bathroom, I hope you'll be there to clean it because they wont. And service!? No such thing here. My grandmother would press the call button for assistance for hours and not one person would show up. Nurses? anywhere? Nope 1 nurse per floor and you're lucky if you get anyones attention. My grandmothers things would fall on the floor the blankets and pillows would fall and not one person would come in to check to see if she needed help. Oh! and the attitude these people have when you ask them "What the F*** they are doing!?" my goodness. how they take offense and act like they work so hard. Trashy people working in a trashy place. This facility is one that prioritizes patients over each other, and does not give all the same treatment. My mother was admitted 2 weeks ago into this facility. She has a hip fracture, but I've learned that she only gets 30 minutes of physical therapy a day Monday to Friday, and the rest of the day she's left alone in bed. She receives no therapy on the weekend whereas her roommate has longer therapy sessions Monday-Saturday. When I asked why my mother receives different treatment I learned that patients who get longer therapy sessions are the patients of a specific doctor (Dr. Besser I believe is his name) and they receive overall better treatment in terms of care and bed service. I am shocked and honestly disgusted at this situation, and its unacceptable that certain patients are prioritized over others. This is definitely discrimination and mistreatment, and I truly don't understand what progress the physicians expect to happen over the course of 30 minute therapy sessions. This is a horrible place, and I urge you to not bring your family members here. read more

I currently have a relative staying at the facility. They are SUPER under staffed and employees are not helpful and rude. There has been numerous amounts of times that I've assisted a resident at the facility because they would ask for help repeatedly and no one would assist them. The house keeping staff is LAZY, they have no respect for the residents clothing and just throw their clothes into drawers. They've even made a hole in my relatives clothing because putting a shirt on a hanger correctly is too complicated for their simple minds. I constantly have to go through the LABELED clothing and give the nurses back other patients belongings. I've even received a note back from the staff saying "boo whoo" in regards to my note asking for them to simply check the labels before throwing clothes into closet which also states family will do laundry. Doctors take their sweet time returning your phone calls and nurses can't explain what's going on because they're not "trained". It's a shame because it is a beautiful facility and well taken care of but the staff make it horrible. I watch these nurses chit chat while the bells are going off because patients need their assistance. Telling them they'll be back shortly while in reality an hour or more has passed. My family members roommate was cold and asked for a blanket, I overheard a nurse saying ok one minute; I left maybe 3 hours later.. He never got it. On my way out I reminded the nurse who was watching tv. She responded with an oooh yea I forgot. It's a shame!! If you're going to have family stay here make sure you have the time to check in on how the treatment is, don't just take a quick tour, do your research! This is someplace we trust to take care of our family members and they barely care. read more
