    Review Title: Severe Medical Misdiagnosis & Violation of Rights…read moreI was brought to this ER in the middle of a life-threatening medical crisis caused by a toxic interaction between my daily Wellbutrin (bupropion), Effexor, and a standard dose of DXM. Wellbutrin is a known potent inhibitor that can cause DXM levels to spike to toxic, dissociative levels. Instead of identifying this pharmacological emergency (Serotonin Syndrome/Toxidrome), the staff dismissed my involuntary physical thrashing and confusion as "behavioral issues." I was denied a medical bed and involuntarily committed to a psych ward for over a week. While there, I was coerced into taking Olanzapine under the threat of a 2-week extension if I refused. This was a documented substance-induced episode, not a psychiatric one. The hospital's failure to screen for this well-known drug interaction led to medical trauma and a total violation of my rights. I am sharing this so others on maintenance medications know this facility may prioritize a psychiatric label over actual toxicological care.

    Colonoscopy T minus 22 hours. Woke up and per instructions…read moremixed 238g of Miralax stool softener with 64 oz of Gatorade, which I'll start drinking tonight as part of a liquid diet. 238 g is equivalent to "14 daily one ounce doses." The anticipation is palpable. My friend Bonny sends me a note telling me to "Enjoy your photo shoot tomorrow." Best euphemism ever. I'm fasting on a clear liquid diet and my beautiful wife keeps telling me about all of the good food she's eaten or will eat today. Is gin a clear liquid? Asking for a friend. 8 pm. Time to start drinking the Gatorade Miralax mix. 8 oz every 15 minutes til it's gone. Mmmmm. 9 p.m. Watching a complicated movie on HBO, Now You See It - might not have been the best choice since I can't pause it and I'm beginning to have a few interruptions. 10 p.m. I'll spare you the details, but Miralax works. Bowfinger is on. The scene where Steve Martin makes Eddie Murphy run across the LA freeway always makes me laugh so hard I cry. 11:30 bedtime. 7:15 a.m. Arrive at Carilion. The staff couldn't be nicer. 8:00 a.m. I meet Dr. Olsen, the young anesthesiologist. I don't ask her if it is take your daughter to work day even though she looks like she could have been one of my students in the last two years. Turns out she's my new favorite person. Dr. Mercure stops in. 8:15 IV is in. I roll into the OR. I roll onto my side and the all-business nurse pulls back the gown to reveal my dimply butt to everyone. Dr. Olsen tells me I'll feel something warm in my IV. Dr. Mercure is standing by. 9:15 I wake up. It's done. I don't feel a thing different. Doc says my colon is unremarkable. 10:00 home. So if you've been putting off a colonoscopy, have no fear. Other than clearing yourself out, it's no big deal. Just think of it like a scheduled radiator flush and fill at Jiffylube for your digestive tract.

While there is a lot of…read morenegative press on access to care, Salem does much better than provide outstanding care - they lead the way on how a VA Medical Center can set the standard for care for Veterans in the United States. I've seen other care areas that did not provide care to the level that most Salem VAMC beneficiaries take for granted. My providers care for me and I feel confident in their assessment of anything I come to them with. I have no problem talking with other Veterans who might not be utilizing their benefits there. I would say that my experience there has been exemplary. I highly recommend the Salem VAMC to anyone who served our country honorably for their care. I appreciate them and trust them with my care.

I requested a male provider when I enrolled and was assigned a female provider anyway. During my appointment, I explained years of failed treatments, severe medication reactions, chronic GI issues, chronic pain, PTSD, anxiety, and the fact that I had previously been placed on numerous medications that caused serious side effects. The response was essentially more medications and yoga. I requested Community Care after a complete breakdown in confidence with my care and ongoing communication problems. Instead of addressing those concerns, I spent months going in circles with Primary Care leadership, Community Care leadership, and the Patient Advocate office. I also spent months trying to obtain a basic accommodation letter for my service connected disabilities and PTSD. What should have been a simple request turned into endless delays, excuses, and follow-ups. I repeatedly requested written communication because of my PTSD and anxiety. That request was acknowledged, yet I was continually pushed toward phone calls and meetings instead of receiving direct written answers. The final straw was my chiropractic care. Chiropractic care has been one of the only treatments that has consistently improved my daily functionality and quality of life, even if short lived. I notified Salem VA leadership in advance that my referral was expiring. I followed up again. I specifically requested that my chiropractic referrals remain current without interruption. The Chief of Primary Care, Community Care leadership, Care Coordination, and the Patient Advocate Supervisor were all made aware. Nobody responded. No referral was renewed. When I arrived for my appointment, I was forced to pay out of pocket because Salem VA failed to coordinate a referral that they had been warned about weeks in advance. That is what veterans are dealing with. Endless bureaucracy, poor communication, lack of accountability, and leadership that either cannot or will not address problems even when they are repeatedly brought directly to their attention. Veterans should not have to fight this hard for basic care, referrals, accommodations, and communication. God Bless America   & Semper Fidelis
