She got so tired of me calling her cell phone for things she said were not related to my mom, that she reported me for Telephonic Harassment. She then had the audacity to request a welfare check on me from my local law enforcement. My mom fell for the 4th time in as many weeks yesterday. The message I received from the lead med tech was, "Your mom fell, she went to hospital, she has skin tear on elbow". When I spoke with him (he's practically impossible to understand) it was necessary for me to ask specific questions in order to get answers - "Where was she when she fell?", "What side?", "Did she break anything?", "Did she hit her head?". She was in her room. Right side. Skin tear at elbow. She "maybe hit forehead". Well, she shattered the right side of her face - mandible, sinus, orbital, etc. Her eye isn't entrapped, but she has some compartment syndrome. She has no idea it happened. She hasn't seen herself today. She can close her jaw properly, but she can't eat because of the pain. There's no one available to answer my questions. There's no one to tell what or if she had any OTC meds this morning. There's no one to tell me if she's properly hydrated. Nada. They did not send her back with any pain medication. It took 5 hours before I got a call from Tuality/OHSU Hillsboro. They know I live 1.5 hours away. Fortunately, I recently hired a wonderful woman who is a Death Doula, to provide companionship, etc. She got my mom back to HOLI from the hospital. I've had a broken jaw and lower orbital. I didn't bruise at either trauma. I had NO swelling when I shattered my lower orbital. Hurt like crazy, though. I was given Tylenol3 when I broke my jaw. My mom's quite diminutive. This photo doesn't come close to showing the level of swelling. The hospital could not get her contact out. To this day, HOLI staff at every level continue to lie about the first fall, despite the hospitalist noting "reported fall". We are a culture of greed and mediocrity, if you can call that a culture. It's so deeply imbedded, our chances of recovering any decency are slim. I'm sorry the system is so broken that the staff at a facility where I pay a base rate of $10,400/month, are so undervalued, they've ceased caring. I cared for my mama for seven years without one lick of help, except a woman who'd sit with her when I ran errands. I had one night off in seven years. I'd still be at it, but I couldn't keep her safe. Obviously, HOLI can't either. Jodie R. - Your Regional Director, Ms. Stroud, has been looped in on this by email. The office help was very, very friendly and the person showing me and a friend all around was very kind and with a fun sense of humor. He was very informative about all the activities that goes on here. Showed us a copy of the menu for that day and a copy of the activities for the month. The Ackerly was very bright and clean. Luckily, I had…read morealready received recommendations from family members of two residents of Merrill Gardens and had planned on touring eventually for anticipated future needs. The staff of Merrill Gardens were amazing in making this move both timely and financially doable for us. I really thought Mom would be unhappy with having to change facilities but she tells me almost daily how happy she is. She loves still having her own apartment, how friendly the other residents and staff are plus how good the food is. I agree with her on these things plus I am so glad to have caregiver services available that can increase with Mom's needs. Merrill Gardens was the silver lining to an otherwise stressful situation.

On the top floor they have a beautiful outdoor deck that has fantastic views of the whole town. We stayed for dinner, not only was a food scrumptious, but the service was even better. The staff here really love their jobs and make this place a wonderful experience. Our friend's apartment is gorgeous and even has a washer and dryer. I'm telling you, this place is incredible.

The dining room servers are usually teenagers and they try to make us happy. I ask for something and frequently the answer is "of course". There is always something I want to eat. (The onion rings are the best ever). The building is older and appliances are tired. But they work. I am content.

This time my dad got up in the night to use the restroom but couldn't make it and soiled the carpet in his assisted living bedroom (which happens to the elderly). The caregiver said that there just wasn't enough people on the weekends and in the evening to take care of it right away so they had to sit in their assisted living apartment and smell the feces all morning and afternoon until I raised hell and finally it was cleaned up at 4:30 in the afternoon.....you can have this type of care too for 11,000+ a month!

The food has put weight on our father, so that says good things about it being likeable. The Regional Director, Matt, is another story. When a new resident brought bed bugs into the building this summer, our father's apartment became infested. He was moved out of his room 3 times into a temporary room, not being allowed to take any of his possessions with him to the temp room. In Sept, we had had enough of him being displaced and requested a whole new apartment. We were left to clean the fumigation chemicals off his belongings (for the 2nd time, because their staff was worried about their own health and safety) and replace items. We had to replace his bed and bedding, lift chair and many clothing items that couldn't go through the hot water process, due to being dry clean only. The total bill was $3726. We asked Regional Director Matt for reimbursement of these expenses since their loss was not caused by our resident. Matt has not responded to this request. We paid the full $5580 each month for ALF, with our father living in what looked like a stark hotel room. We believe the multiple moves over the 3 month transition has sped up his cognitive decline. It has been noticed not only by the family, but by Rosewood staff. We picked Rosewood because of it's proximity to family. If it wouldn't speed up his mental decline more, we would move our father to Marquis in Forest Grove or Avamere at Bethany.

My grandfather has been living at Canfield for just about a year. We enjoy coming to visit him…read morehere. The location is bright and smells clean, the staff always welcome us when we arrive and have listened to us when we have had concerns or ideas in the past. Residents are welcomed to join each other at the dinning room tables of four, and Canfield also offers games, activities and exercise. There are groups who come in to the center to play musical instruments, sing, sell small items like candy/cookies, and perform. The trivia games are one of my grandfather's favorite activities to participate in. He tells us about a new game every time we visit. There's often Wii Bowling that is fun to watch as well as participate in. The nurses and medical staff at Canfield have made sure that my grandfather is able to have his medicine daily, on time and the amount that has been prescribed to him. This is an additional fee on top of the monthly rent, but has been worth the extra to make sure that every i is dotted, and t is crossed. We can feel confident in the attention that he's getting thanks to this helpful addition. The staff check with my grandfather's doctors about medicine updates, and are able to order perscribtions to arrive at Canfield instead of family members having to pick up from a local pharmacy and deliver them to the center. The option to have a personal car drive my grandfather to appointments has been another comfort that we're willing to pay for. There is a small fee, but it's personal service and the level of independence that my grandfather feels is huge. We love that he can tell us, "I went to the dentist today," thanks to this service. Each resident at Canfield wears an emergency call button on a bracelet on their wrist or around their neck. When my grandfather found that he couldn't get out of bed from his nap he pushed the button and immediately the staff determined that they should transport him to the hospital. Thanks to this quick thinking and response my grandfather has no lasting affects from this episode. The dinning room offers a rotating menu with options that appeal to my grandfather's tastes. The kitchen has recently been remolded and the dinning room space is open and bright! The residents are also able to pick from standard items on an additional menu. There are always options like oatmeal and bacon or eggs for breakfast. If it's between meal times there is tea, coffee and ice cream available. If a resident were to want to have a meal delivered to their room that is an option for an additional fee. There is also an option to have a staff member walk the resident to the dinning room (another additional fee). All of these options make the care at Canfield just what each resident needs. We have enjoyed holiday dinners hosted by Canfeild too. You are welcome to buy a meal and join residents year round. We loved the Easter brunch they served and my parents were so excited to share Thanksgiving dinner with my grandfather at Canfeild, NO DISHES to clean up! Bonus! My grandfather's room has a little kitchenette equpped with a microwave, refrigerator and sink. We originally picked Canfield because my grandfather was able to bring his small poodle with him (that's right, pets are welcome!). Canfield offered us a room where my grandfather is able to exit on the ground floor to let the dog outside, but they also offer the option of having a staff member let the dog out if a resident isn't able to (for a fee). We all know how important pets are, and how they can really affect our quality of life for the better. This option made Canfeild a no-brainer for us as a family when my grandfather was no longer able to live on his own. We are thankful that my grandfather is at Canfield and always know they will take great care of him!

I've been here…read moresince August 2019, and have come to truly appreciate how sincere each and every employee is in their care for our residents! My coworkers are an inspiration, and I regularly receive compliments from residents and their families regarding the care we are providing. Before coming to work here, Avamere had been a customer of the company where I previously worked. I had the opportunity to work in Care Communities (assisted living, skilled nursing and rehabilitation centers) all over Oregon and Washington. I worked in communities and facilities which were run by Avamere, as well as their "competitors". From my perspective, you could feel a difference in Avamere versus the others. The staff were genuine, and offered sincere compassion for their residents and patients. The administration seemed to be connected with the staff and resident and patients, as well. It was the many opportunities I had to work with the different companies, and in so many locations, which led me to want to work for Avamere. Unfortunately, I had signed a non-compete clause with my former company, and therefore had to be separated from them for 2 years before I could be employed with Avamere. So, I did my time! Now, I'm here, at Avamere (Avamere At Hillsboro) with intention! I love this company and couldn't be happier with the decision I made!! Just want to warn everyone to keep they're loved ones away from this place. I am a former employee…read morefor Avamere. I worked in the dementia unit for residents that need extra care with day to day activities such as going to the bathroom, getting dressed, etc. I only worked in the dementia unit so I cannot speak for the assisted living section. I treated our residents with the dignity and respect they needed and deserved. After working there for almost a year I noticed the manager, Executive, care staff and nurse turn over rate was brutal. Not one person could keep a job there because they kept getting fired, I didn't know why until about a month before I was let go as well. Everyone who tried to report avamere was fired. residents were getting neglected in one way or another from nurses giving expired medications (like insulin for our diabetic residents) to the care staff laughing at residents and giving them terrible nicknames that correlated with disabilities that they had. I reported this kind of behavior to my managers and executives and they made me believe they were doing all they could to prevent it. After months of this vulgar behavior i felt that I could not stand by and watch these residents that I had become so close to be neglected and abused. I called the regional nurse and told her my concerns for the residents because my direct supervisors were taking no course of action after 5 months of this behavior from the staff. She pretended to hear my concerns and as soon as I was done talking to her she called my direct nurse and I was brought into her office and given a warning about "going over her head". A couple months go by and the behavior of the staff seemed to be getting worse. One day my co-worker and I were on break at the same time and I expressed my concerns to her and told her I wanted to get in contact with the state board of nursing and report Avamere at Hillsboro as a whole. I grew very close to most of the residents I took care of and hated the thought of them not getting the best care they could if the managers would just do they're jobs... well my co-worker went to my direct supervisor and told her that I wanted to report Avamere to the state for elder abuse. Less than a week later I was pulled into my direct supervisors office and was told they would have to let me go because I was "not getting along with my co-workers". A report to the state would seem a lot less believable from a disgruntled employee... Please don't put your loved ones here, at least not in the dementia unit. The staff feel like they can get away with whatever they want because residents with dementia cannot speak for themselves in most cases and if they can most people will not believe them. Avamere does not care about its residents or the residents families. They are only here for the money. This truly broke my heart.
