This place is not where you want to leave a loved one!! The staff doesn't pay attention to the patients ! I wouldn't take a dog here...

They don't deserve one star. Facility is old and in need of replacement. Bedding is worn. Staff is no response, my help light was on for 5 hours, I had to get up and go to the desk because I was in extreme pain and was denied help! What a nightmare! The absolute worst evening and overnight care. Overnight nurse refused to change canister on wound vac machine. Second night again refused treatment and instructed others to not give required care and medication. Roommate had fallen no response to call light. My son had to get out of bed into his wheelchair and go to the nurses station. Where 3 where sitting. Call light not answered for hours at a time.. Almost all issues occurred during the evening and overnight. If you absolutely have to place your love one into this facility..please.. please check in on them at all times of the day and night. I was there in April and late at night I asked a CNA for help transfering out of the wheelchair to the bed. My left foot has had 3 podiatry surgeries and I am supposed to be careful about putting pressure on it. When the CNA saw that I was having some problems balancing on the walker he grabbed my left leg and slammed it into the ground to get me into the bed Soon intense throbbing pain caused me to believe it was broken. I pressed the call light fpr help and nobody came despite my cries for help. In the morning the new found me with a fever of a 100.8 temperature and the chills an extra showed my foot was sprained and not broken. I was sent back to Dignity Health hospital in Folsom where I given morphine for pain. The next day the podiatrist performed surgery and when I was to be sent back to a skilled nursing facility for recovery I asked to be sent to a different place. read more

I threw a fit and they gave them to her. Then she went back to the hospital, I got pneumonia at the same time and lost her. Just found out they sent her back! My dad is now in the middle of his second stay at Rosewood. He has 20 more days to go and has been there for 3 weeks so far, this time around. Food still sucks, and the facility hasn't moved, so it still has no nice places to go outside and no where nice to really go within the facility, but besides that, we have been very pleased with my father's second stay. The therapists are very good, more the most part, the staff are incredibly accommodating, attentive and very involved with the patients. My dad is doing very very well there, and I'm happy his experience has been so good! read more

WORST POST-ACUTE REHABILITATION FACILITY I'VE EVER VISITED! I would rate it ZERO STARS if I could!!! My 90+ year old grandmother was admitted this past December (2014) after suffering recurrent episodes of ventricular tachycardia secondary to her recent atrial fibrillation diagnosis followed by additional findings concurrent with that of left diastolic heart failure--confusing, I know! She was first admitted to Mercy General and then taken here for cardiac monitoring and rehab. First complaint: Nursing staff does NOT know what to do in the event of an EMERGENCY!!! A few days after my grandmother was admitted she did NOT respond well to her cardiac medication (Flecainide) and started experiencing 15-20 second seizures followed by acute episodes of ventricular tachycardia which later developed into TORSADES DES POINTES (AKA FATAL--Her heart rate was 160 beats per minute versus a normal heart rate: 60-90 beats per minute). INSTEAD of calling 9-1-1, the nursing staff decided to wait for the doctor to come check on her...!! What nursing school did they graduate from?!?! I can't believe how incompetent they were!! My grandmother is lucky my mom was there to A. quickly notify the nursing staff of what was going on and B. take initiative and make them call 9-1-1! My grandmother was admitted to the ER at Mercy San Juan, located down the street from this facility, and continued to suffer from acute seizures followed by short episodes of ventricular tachycardia. Second complaint: ...WHERE WAS THE CARDIOLOGIST THROUGHOUT ALL OF THIS?? Why wasn't he closely monitoring a 90+ year old female patient with new onset heart failure who recently started Flecainide--a medication that bears a BLACK BOX WARNING for PRO-arrhythmic effects and also meets the BEER's criteria for its potentially harmful effects in geriatric patients! Overall, NEVER EVER LETTING MY GRANDMOTHER COME BACK HERE!! read more

This is not the best facility. My mother in law went there for rehab and they are awful. She lost a significant amount of weight because they refused to help her eat. They left her in her bed in her night clothes on multiple occasions until 10 or 11 in the morning. She recently had an incident which they claim is from an wheelchair fall, which I don't believe. She was taken to the hospital and returned to Rosewood on Sunday night. When I went to visit her on Tuesday afternoon she was still in her bed in the same hospital gown she was discharged in on Sunday. They left her for two days in her bed. Their idea of providing assistance to help so one eat is sticking a fork in the food and leaving them for a significant amount of time to see If they figure out how to eat in their own. When they don't figure it out and refuse to eat Gracie they are so frustrated they say the patient refused to eat and they take their food away. Some of the staff need to evaluate if they still have a passion for their career or job because their actions show differently. I complained to Mike the director, all he did was make excuses for their behavior. He also needs to evaluate if he is in the right job. Mike, can you go home at night knowing that you have done everything in your power to make sure you are providing people's loved ones with the best possible care? I doubt it, you have also lost your passion for caring for the elderly. On multiple occasions my mother in law has soiled her clothing and I have left notes for the staff that it is unacceptable to leave her like this for any period of time. Why should I have to remind a care facility of such a basic function of keeping their resident in clean clothing. It's degrading. read more

Not horrible but ok. Super overwhelmed staff with the number of patients. My mom was there they treated her super nice. However it took over a week to get her meds situated. Since she didn't speak English they didn't seem to care about her participating in many group activities even though she was there for over a month. But if your looking for a place where they take care of your loved ones with a good attitude this is a decent place just not close to freeway access. read more

Do not take a loved one here! My poor grandfather has tried to get help to use the bathroom and never gets it! He gives them a buzz when he needs to go and no one shows up until an HOUR later! My grandfather is so unbelievably embarrassed because he's FORCED to wet the bed! The won't check on him at all or do his physical therapy! They are ABUSIVE to the ELDERLY patients! When we called to get to the bottom of this issue the head nurse was unbelievably rude and HUNG up on us! read more

If you want to know my mother's real experience then keep reading. My mom started her one month stay here after recovering from a fall averaging a two hour wait for water and diaper changes. I complained to a supervisor and he said he met with the CNA staff after my mom got diaper rash from not being changed for over six hours. After approximately two weeks my mother contracted a C-Diff infection. I was told this is contracted by unsanitary conditions. She was supposed to be placed in isolation however shared a room with someone else who supposedly had this infection as well. While in this room a wandering patient crept into my moms bed and scared the crap out of her. He also touched her in a inappropriate way possibly on accident, who knows? I was told this patient had dementia and they were not equipped to have a patient in this mind state. I was also told this patient wandered in other rooms as well. Wouldn't you put a system in place to handle this the first time it happened? Why would they allow this as someone could have a heart attack or get hurt from this patient who creeps into other beds. Finally at the fourth week I was told she would be discharged on a Sunday if her stool was solid by Saturday. I asked on the Friday before if someone would call me to confirm she qualified for discharge. Of course no one called until Sunday when the weekend nurse called to tell me my mom was being discharged and wanted to confirm someone would be home to great her. I asked numerous times to let me know what time the discharge process would begin so I knew when to be there and to receive discharge instructions. Nevertheless my mom got put back to bed and they rescheduled transport as I was not home when they wanted to come. They had a transport set and my mom dressed without confirming I would be home to take her in. With respect to discharge, I was told they would order the Medicine Approved Durable Equipment like a wheelchair and slide board so I would have it when my mother arrived. Of course this didn't happen. So many oversights! Finally after being at home while taking FMLA I asked the social worker to ask the doctor to sign my Paid Family Leave form. The doctor refused to sign because he wasn't "comfortable" signing since my mom wasn't there any longer. The doctor is the one who prescribed 24 hour care for 4-5 weeks after discharge. Now he wont sign so I can receive 60% of my pay so I can take care of my mother. After communicating with a supervisor he apologized and said he would speak to his director or administrator to see if anything could be done. Do you think I have heard from them? NOPE. This place is unbelievable. I Definitely Don't Recommend This Business. read more

What nightmares are made out of!! I wouldn't suggest this facility to my biggest enemy. No one who works here seems to give a sh*t about about the patients. The patients are left to fend for their self the majority of the time. I have witnessed several nurse calls go unanswered for up to an hour, leaving patients to sit in their own defecation and urine while the nurses talk amongst each other. I'm appalled this place has not been shut down. My father told me he would rather die than spend another night here!! My heart goes out to anyone who gets sent to this sh*thole! read more

My mother has received good care from many considerate nurses and attendants at Rosewood.

My father-in-law was a patient at Rosewood for over a month and we had an excellent experience there. He was well cared for and left Rosewood after a month able to walk again and much stronger and almost back to his own self again. The staff was excellent; they were kind and kept a close eye on him at all times since he was a flight and fall risk. They even kept him sitting in the nurses station so they could keep an eye on him at all times. The staff kept us well informed and in the loop on his progress, medications and activities. This is not a fancy place but it's a place where the job gets done, and they know what they're doing. I highly recommend Rosewood for post acute rehab. read more

Our father has been at Rosewood for 4 days. He is there for physical and occupational therapy. He seems to enjoy his interaction with all of the staff he has encountered to date. The rest of our family is extremely happy with the level of care and attention he is receiving. The staff aknowledges everyone and there are always patients out and about the halls or in the group areas participating in a game or other function. We only wish there were more parking spaces and nicer outdoor areas for the patience to enjoy. read more

My father had a several weeks stay following a serious infection and hospitalization. We had literally 36 hours to choose a facility for him to be rehabilitated. Rosewood staff gave us a tour on a Sunday and were very professional. The facility smelled very nice and clean. Food was very good, however they didn't have a menu selection that was very diabetic friendly...the meals were really high carbohydrate choices and my dad required a lot of insulin during his stay. I give them four (4) stars purely based on cleanliness, food quality and professional staff. The are two fairly big things that I found disturbing. 1. They have a patient smoking area that is literally next to the entrance that is most preferred. I understand the need (I guess) but it was kinda nasty having to walk through smoke coming and going. 2. Rosewood told Sutter they would pay for his transport from the hospital to the facility. Months after his stay and at home my dad received a letter from Medicare that they would not pay for the transport. I called Rosewood and asked them why they were not paying for it. I was told they did not say they would. I called Sutter discharge planning and they said that according to their notes Rosewood said they would. Rosewood again said they would not pay it. Needless to say The ambulance company immediately threatened my father with collections and he ended up with a $2,000 ambulance bill that he can't afford to pay. So be aware...it they say they will transfer a loved one, get it in writing. Other than the two items above it was excellent. Oh and the physical therapy department at Rosewood was top notch. Loved what they did for him...he left Rosewood better than I have ever seen him. read more

My 86-year old Dad was sent here by Kaiser to recover after breaking his hip. The most important thing is that staff are very friendly and kind. If I had a concern, the staff was responsive. The halls are carpeted which cuts down on the noise. It does not smell! They have so many nice pictures hanging in the halls, it would be nice for each patient's room to have a picture for some kind of visual stimulation as inside the rooms, as the wall patients look at is blank. The lighting in the rooms is awful (flourescent), and the cord for patients to turn on/off the light is like dental floss - hard to grasp, and easy to break. My Dad's lighting cord was broken when we first got there. But as mentioned, as soon as you point out a problem, the staff is responsive, so we got his lighting fixed. His phone didn't work when we first got there - when we pointed it out, a working phone was quickly provided. There is a small sink in the room, but in my Dad's room the hot water did not work. Each patient has a TV, but if your loved-one has a roommate, it can be really awful if they each have a TV on (dueling TVs), or if your loved one wants to rest and the roommate is watching TV there is no way to contain the noise - headsets/earphones would be so beneficial!!! It really helps for a patient to have an advocate to ask for what they need, as in my Dad's case, sometimes he was too sick or too tired, or not able to use his call button to get what he needed. You really need someone to be with your loved one as much as possible to help ensure they are getting their needs met! You can't assume everything will be taken care of - some things just didn't happen until we specifically asked (ie: getting him dressed daily, getting him out of bed and into a wheelchair daily...) You have to be proactive and ask! There was a quarantine while Dad was there, so he couldn't participate in any activities, but there was a schedule of "events" if your loved one is able to play cards, watch movies, bingo... The food was not very good, but patients can request specific items - this is something we learned after Dad was there a while - we didn't really get any kind of orientation, so we learned some things well into his stay, which would have been good to have known from the get-go. No one ever wants to end up in a rehab facility, but if you or your loved one has to go to one, Rosewood is not a bad choice. read more

My mother was taken to this place based on the website looked like a good place to stay for wound care. In less than 24 hours she was calling me frantic to come pick her up because the toilet was clogged and they had not cleaned up the urine and feces off the floor and they were not treating he wounds correctly at all. (Changing bandages until the next day when they should be don't twice a day...Ripping off the bandages and not completely covering them back up) and she says that a man that was claiming to help her was inappropriately excessively touching her. But she never did find out what he actually was helping her with. She has always been a strong willed person and has never had any side effects to any medicines like the nurse started to claim when I asked who was this man and what was going on. They had not even her any medications before this happened. Notice that there are no pictures of this place or the staff? I would not recommend this place to anyone you love. read more

My grandpa had stayed at this facility for a couple of months and every time I visited, they never failed to make both me and my family feel welcomed, comforted, assisted, and reassured. The nursing team was also fantastic! Very on top of things when paged and asked for assistance to the bathroom or when eating. As far as I could see, they had clean, quick, and effective protocol to follow as they go through the task. The rehab team also helped produce fruitful results as they were able to help him walk, eat, and talk again very quickly! I had had a conversation with the rehab director during my visit there, and I don't believe I've ever met anyone as professional, reassuring, and articulate. He definitely made this facility memorable by his passion alone. Great facility and I had peace of mind to know my grandpa was in good hands! read more

The Kitchen staff is wonderful, they ask what your food preferences are and are willing to accommodate any diets. The physical therapy here is great as well. DO NOT GO TO, OR ALLOW ANYONE YOU LOVE TO GO TO ROSEWOOD REHAB IN CARMICHAEL. I was transferred to Rosewood Rehab last Saturday following surgery. First of all, they knew I was coming, and yet the bed was not made. I had to lay down on the plastic mattress until they got organized. I had just had 2 surgeries on my knee. Although it was the long holiday weekend, they refused to provide me with a walker to use to get to the bathroom until I saw physical therapy after the holiday. But what happened next was horrifying. When you enter a rehab facility such as this, they need to look to make sure you don't have bruises or bedsores. LOOK, is the operative word. But they did so much more than look. The nurse, her name was Story, took it upon herself to grab me, with both hands, in a very, very personal & private area. Feeling violated, I told her to get her hands off me. I wasn't even aware until the next day that she scratched me with her nail. It caused an abrasion that was not there previously. When I told her to get her hands off me she threatened to evict me from the home. She said she was going to continue what she was doing or I could not stay. Another nurse came in am& asked me if I was going to comply. I explained to her that they could look, but not touch. She told me if I didn't do what they wanted they would kick me out of the home & I would have no rehab. Then one last nurse came in. He said that he was the head of Nursing & asked me if I was going to comply. I told him I was more than willing to show them that I had no bruises or bedsores, however they do not have the right to put their hands on me, especially in the manner that the nurse, Story had just done. I told him that I was more than willing to sign a waiver verifying that I had no bruises or bed sores in the region they were trying to look. I felt as if they were about to start a body cavity search. Along with the first two people claiming to be nurses, he also told me that if I did not do what they told me to I would be removed from the nursing home. Next thing I knew the ambulance company arrived to pick me up and take me away. I am a 58 year old woman & I am capable of speaking up when something is wrong. It horrifies me to realize what I went through, (sexual assault) & also medical malpractice. If they would do that to me, then what do they do to the elderly people who cannot speak up on their own behalf? What do they do when the families are not around to watch and protect them? I'm gone. They cannot continue to violate me ir my rights. They cannot physically assault me anymore. But what they did is just as damaging. They have listed me as non-compliant. This means I am lusted as a difficult patient which is totally a lie. And this also means that the other rehab centers that my doctors and social workers are trying to get me into have rejected me for being non-compliant. So I was sent home over the holiday weekend and could not get a hold of my doctors or anyone else to advise what was done to me. My knee has already Frozen again and is not bending. And the worst part of it all after everything they put me through, is they are capable of stopping me from getting the rehab I need and deserve. I reached out to the director of nursing but she was not there and the cell phone numbers they give are non-functional. I have reached out to the administrative director Michael Moore. I have left several messages for him with absolutely no response. I'm sure that they would respond if it was regarding money owed, but the moment they have done something wrong, they ignore you like the plague. I have done everything possible to resolve this issue with the facility and their administrative staff but you can't resolve a problem when they ignore you and won't return your calls. Nobody seems to know who the owner is or how to get ahold of him or her to discuss the issue directly with them. So that left me 1 option to register my complaint, to Post online how horrible the nursing staff is at Rosewood Rehab Center and beg you not to go there yourself or let anyone you care about be left alone there to be violated. I don't take sexual abuse lightly and I don't take elder abuse lightly. During the very short time I was there, I WAS VIOLATED. I have filed a police report against them because the nature of what the nurse did to me was definitely offensive and Grievous. Maybe I'll get lucky and the owner will hear about the reviews I've posted and realize I need to be contacted. In the meantime I will continue posting the reviews necessary to get their attention and hope they make the changes that they need to make, for the betterment of the patients they care for or claim to care for. The staff were compassionate and caring. They were prompt in responding to the needs of both my mom and dad, who were there at the same time. As a Registered Nurse myself, I feel they received quality care. I also think that the administration and staff were receptive to my input in regards to facilitating and implementing care plans that were appropriate for their specific needs. The place is clean and the food is good with a nice variety to choose from. This is by far a 5 star hospital...They have treated my mother with the utmost dignity. Any requests we have for her are met within the first hour of requesting--Every staff member greets her by name and never fails to ask her how she is doing....The administrator Michael went out of his way to allow me to do my mothers hair( there is a salon available) however I am a stylist and have done my Moms hair since 1968--this was important for her and for me---I have never walked in and been assaulted by any orders other then freshness....Food,, cleanliness, caring staff and a team of professionals who are there to answer any questions---BY FAR THIS IS THE BEST NURSING HOME IN SACRAMENTO-- read more

This facility is very clean! I have been in many other facilities before visiting family members and they all were dirty and smelled of urine. This place was spotless. They obviously take great care of the facility. While my father was there for his rehabilitation the staff was very attentive to his needs and wants. They were welcoming to our family when we came to visit. I felt at ease when I had to leave, knowing that my Father would be well taken care of and watched over at all times. read more

Choosing a place for your loved ones to be cared for is a difficult decision. After visiting several facilities that I would NEVER have my parents stay at, I found Rosewood. My Dad was cared for there in 2011 and my Mom was recently there for 4 weeks while recovering from a major surgery. They both received excellent care. There were a couple of minor issues such as the room got warm on a rather hot day and Phil who was the charge nurse immediately found a fan and got it set up in her room. I was impressed by his quick response. Another was my Mom did not like being by the window as she felt isolated, Elaine (CNA) and Cynthia (charge nurse) made sure she was moved ASAP. Elaine provided not only great care but treated my Mom as a friend. Being that she was there for several weeks, Elaine brought great comfort to her which in turn brought great comfort to me. Raj, Zena and Doe are other employees that assisted in making me feel that I made the best decision in having Rosewood care for my Mom. read more

My Mom Recently stayed at this Facilty.For about 1 1/2 Weeks.From the Start to the end of the stay was a nightmare.The first day she was admited they gave her something that Knocked her out for 2 days.Im in Oregon and when i called her she had no idea what day or time it was.My mother Is intellegent.I called and told them to please stop giving her what ever is making her act like a zombie.The Nurses said she had NO pain killers?The next day my mom was back to normal?I decided to move her close to me.So the Social worker...Found a Med Transport Company to move her at my Expence.After a week notice.They Failed to Order Oxygen.for her trip.The med transport.Flaked.The Nurses.Lost my moms Wheel chair.at the last min.My moms Breathing treatments were always a hour late.She never seen a doctor.She would call me all the time and ask me to call the nurse's Becuse she has been waiting over a hour with her room light on.Gasping for air.This place is a Joke 100%.To be honest.after my moms experince.I Hope feel for the patients that are there or will be there.And yes im seeking a lawer.Please Think twice about this place.Yes the nurse's are sweet.But if you love your loved ones.Dont Let them stay here!!! read more

I would like to warn everybody. About the lack of real healthcare at this facility. Dr. Kumar was the on staff Dr. assigned to my aunt. She was there for 7 days and only saw Dr. Kumar once for less than 5 minutes. My aunt was unable to keep food down for 7 days. Dr. Kumar said she just had upset stomach. Which turned out to be an intestinal blockage. My aunt left the faculty on the 8th day against there medical advice. Thank God she did. Because she was immediately admitted to Sutter General Hospital. Her white blood cells were dangerously low along with her potassium levels. Once we signed the forms to leave against medical advice. Rosewood nursing director Teresa basically dumped us in the room. Refused to call for transport to the hospital until I said I will call 911 if I have to. Then she reluctantly call for transport. After my aunt sat in a wheelchair for almost 2 1/2 hours. The nurses and aids the run the floor were great. Sophia in admissions was outstanding. If you have any health issues this is not the place for you or your loved ones. Dr. Kumar should be ashamed to call himself a healthcare professional. read more

This place is NOT what they make it seem!!! My father is sick with cancer and did not respond well to his first chemo treatment, which landed him in Rosewood during August 2016. Firstly, he was unable to swallow food or liquids and within the first week lost over 30 lbs. We were very concerned and they assured him that they would send him to Mercy San Juan to have a feeding tube placed. It took over 6 DAYS to get them to make the arrangements, the whole time he was starving and unable to drink anything. He was also in a lot of pain and discomfort, but the lack of staff caused his meds to be delivered to him HOURS late. When he would turn the light on to request assistance, they encouraged him not to do that, saying they would get to him when they could but to not turn on the help light. When he was brought into the facility he had a prescription from his doctor for pain medication, obviously he was not able to take them due to his swallowing difficulties, but that did not stop Rosewood's staff from stealing almost all of his pills, leaving only a few in the bottle. (I am aware patient's should not have these with them, but sometimes things happen unexpectedly and it does not give staff free reign to take what they like from them.) When I first came to visit him, nobody stopped to ask who I was, no one asked me to sign in, I was able to walk right through into his room without even a glance. Although It was convenient to walk right in, it made me feel uneasy about who else could so easily have access to the patients, as well as their belongings. The final straw was when my dad requested to transfer to another facility due to Rosewood's shortcomings. He spent a few days in Mercy General and then transferred to the new facility. Family members requested to pick up his belongings, which were already packed by the Rosewood staff. The day before this a family member had given him $25 in cash, which was left in his wallet. Of course that was MYSTERIOUSLY gone when we picked his things up. When we contacted them, they said they would look into it and get back to us in a few days...Still NOTHING. Overall Rosewood was a HUGE disappointment. I do understand being short staffed and having longer response times than are desirable, but to have things STOLEN (ESPECIALLY MEDS!!!!) from someone who is extremely sick is just horrible. Taking their sweet time to respond to serious medical issues is ridiculous (ISNT RECEIVING PROMPT AND NECESSARY MEDICAL ATTENTION THE WHOLE POINT OF A PATIENT BEING THERE?!?!) There should be way more monitoring going on of their staff and those who are visiting obviously, as well as better training of these staff. read more

I have visited Rosewood on multiple occasions and have always had good experiences with their staff. I feel the grounds and building are kept up nicely and the staff are caring. The therapists and nursing staff I met were all very attentive and responsive to my questions. read more

I must say I was scared about going to a skilled nursing facility for the first time. I definitely did my research and touring before I went into the hospital for my surgery. From the moment I walked in the front doors of Rosewood I was impressed with the fact that there was absolutely No odor in the building! Rosewood was the 4th building I toured. After admitting to Rosewood I was as happy as I was when I toured. The staff was so nice and provided excellent care. The 5 star rating that Medicare gives that building was surely accurate. I was ready to go home sooner then I had anticipated and I have been doing great since. I spent 3 weeks at Rosewood and I miss all the staff that I had gotten close to during my stay. I recommend this facility to anyone in need of skilled care or physical therapy. read more

Please Please Please do not bring your loved ones here. Although it may look like a nice place when you first walk in, it is far from reality. The staff lies about the CNA to patient ratio. They will tell families that it is 4 patients to 1 CNA when in fact it is more like 10 or 11 patients to 1 CNA during the day. And sometimes up to 25 patients to 1 CNA and night which leads to a slow response time. When the state comes to inspect the place they make sure they have plenty of staff if not extra so they can keep the call light response time to a minimum. But as soon as state is gone they are back to their old ways. Although most of the nursing staff seemed liked they genuinely cared about the patients, it was obvious the management team was only there for a paycheck. read more

If you have a choice, NEVER, EVER, go to this facility or have a loved one go there! My mother was there for rehab after hip replacement surgery in April, 2017. She was 75 years old and was suffering from Alzheimer's Disease. After a month of "PT" Rosewood determined that mom would not rehabilitate due to the Alzheimer's, and gave us 2 days to relocate her. During the month of "PT" the Rosewood staff left her in her pajamas until mid-day, left her in soiled clothing, and wouldn't help her eat causing her to lose a significant amount of weight. They had also sent her to the hospital for being deemed "combative" because they were unkind and rough when they were actually changing her into clean clothes or trying to administer medications. She was simply expressing her discomfort and pain at the hands of the staff at Rosewood. Due to the classification of "combative" and "non-compliant", Rosewood effectively sabotaged mom going to any other facility. From June to October 2017, mom was at this horrid facility. She (supposedly) fell out of her wheel chair, IN FRONT OF THE NURSES STATION, and broke her nose. She also (supposedly) fell out of bed and hit her head on her roommate's bed. Both incidents sent her to the hospital for treatment. Finally, at the end of September, mom was sent to the hospital for a UTI (Which she had MANY of due to the poor care received there). She was sent back to Rosewood on September 26th, had failed a "swallow" test twice at the hospital, and we were told that she would only have 1 to 3 weeks to live. The staff at Rosewood REFUSED to give mom medications according to the Dr.s orders, saying they didn't think she needed them! They continued to bring her food trays knowing she couldn't eat. They left her in the same position in the bed for 24 hours! All I can say is, THANK GOD for Vitas Hospice! ALL of the nurses were kind and caring. We had to involve the Vitas nurses to get the Rosewood staff to give mom meds, re position her, change her clothes, etc, because they wouldn't even do these things when we asked them to! Mom passed away at Rosewood on October 5 very early in the morning. Our family was told "So sorry for your loss" by the nurse on duty the day she passed. Other than that, there were no expressions of condolences whatsoever. The staff (with just a couple of exceptions) and administration of this facility need to re-evaluate their dedication to this career choice. They are burned out and mean to people who cannot defend themselves. read more

My father was a patient at Rosewood Post Acute Rehab and it was a nightmare from the beginning. They were specifically asked not to change his behavior medicaitons as it was a two year process with a geriatric psychologist to get the correct cocktail of medications to manage his behaviors. They changed them without telling me immediately. Then they began calling me and telling me he was acting out with his Dementia and I needed to come in immediately. I explained this was not possible since I worked full time and lived 2 1/2 hour drive away not to mention I was in Southern California at the time on vacation. I had previously notified them I would be out of the area during the vacation period. They insisted I come and I refused. At that point they told me they would contact me for every little item. I said I worked in care and that was not necessary. They kept their word and called me 14 time in the first day with trivial information and often incorrect information. During my dad's stay there, he fell and I was not notified, he had a systemic infection they did not tell me about, he was treated for medical conditions that were not his and their stories never matched. Depending on who you talked to depended on what story you got thus which was the truth was unknown. I was very relieved when he was able to return to the wonderful Board and Care home he lived in. The week of August 18, 2014, I received a call from them telling me I needed to come in for a care conference very soon. I called back and it took me three phone calls and into the afternoon to reach the administrator, MIchael, to ask why I received the call. My dad died in January of 2014 and had not been there for over a year. I was told by the administrator there were others with my dad's last name in their data files and they called the wrong one. He said humans worked there thus explaining the mistake. Really??? What is my dad had just passed and it was unexpected and that call had been devastating? What about the people that needed to attend the care plan meeting and never made it because they called me and left a message instead? As I said, I work in care and this is unexceptable, uncaring and not in the best interest of the patients they supposedly care for. I would nver recommend Rosewoodfor any person's loved one! Go elsewhere and know there is real care available. read more

My father spent 3 months in this facility. From day one to the last day spending time there his visit was not a good experience. The food was delivered cold. The response to his needs was very bad. It took my father calling my mother on the phone to get her to call the nurses station to come to his bedside for assistance. When he was admitted to the facility they stated it may take 10 mins to come but reality is it took at times over an hour. They are understaffed and poor communication within their staff regarding the needs of their patients. Overall the place is clean; however, there were times when the room needed cleaning with a simple washing of the surfaces. Most of the staff that worked directly with my father were very friendly and good to my father. However, again the communication between staffing member such as Nursing to physical therapist, one did not know the goals of the other. I would not recommend sending a family member to this facility again. read more
