I'd like to give a review of the Sacramento Post Acute facility on Hemlock Ave, in Sacramento. The people are what make this such a good place to recover in. Julie Swift on intake, Celly the CNA was available at the push of a button. Other staff were always there and very helpful. PT folks, Morgan, Adriana, Kimi, Stephanie, and Gwen were there everyday to help me gain the usage of my left knee. Excellent folks and they followed plans to get me on my way as soon as possible. I can't think of anything that would make me change my mind on the quality of the care I received there. read more

Everyone, administration and medical staff at Sacramento Post-Acute were absolutely stellar. Their caring, kindness, dedication and attention to detail was both impressive and inspiring. If I needed something, they were right on it, even if that wasn't the person's job, they quickly found someone who would be able to do it. Their communication is fantastic. Any questions/concerns I had were promptly addressed and/or answered. Anytime there was an issue I received a phone call. A huge thanks to all the fabulous staff! I would never bring anyone here the staff are rude especially those CNA's some are nice but most are very rude with the elderly residents they talk loud and if they ask for something more than one time the CNA's ignore them or do the thing in a rough and very rude way the state has to do something about this place idk why these people got hired. Please don't ever put your elders here they get treated worse. And if you do put them please check up on them every single day and make sure check what she ate or did or not some CNA's bring the food trays but ignore feeding that patients who needs help and they would either feed the patient one or two sPoons and report that he denies eating that's a lie. From what I've seen here and experience with my relative this place has to go through some serious trainings. Also management is worse if you report something about their CNA'S or nurses they take their side instead of doing something about it and making it right. read more

Make sure you get all your clothes before leaving. If ordering a transporter upon discharge, make check the address is correct. Sent my dad to his home which he was removed for elderly abuse. Dad was suppose to come to Stockton and Facility knew that plus admitted their mistake. Bill from when household rejected him was $200. We had to pay. read more

This rehabilitation facility is not recommended. They are understaffed and some staffers don't care to work with elderly patients. My mother was there for 4 nights, her estimated stay was 14 days. She was there for physical therapy and to get the stronger after a hospital stay for a fall where she sustained a fractured cervical vertebrae. While at this facility she didn't get help to use the toilet facilities. She had to take herself or soil herself. On the 4th day she fell due to lack of assistance, then she couldn't get assistance to get up. A nurse was supposed to help her up was not available. Protocols were not followed after her fall. Fortunately she was able to call me and immediately tell me what happened, then her grandson was able to go the facility immediately because he was closest in proximity. Needless to say we removed her from this facility. Due to the lack of care for patients, lack of consideration for patients needs and lack of doctors. I would not recommend this facility. This is just one of the reasons why I would not recommend this facility, there are many more promises that were not met. read more

Please do not send your loved one there unless you are willing to be an advocate several times a day all the time they are there. They have people but it seems like they are short handed and over worked even with a lot are students. Not one time did any one ask if you needed something or could help. You will use your call button and most of the time no one answers and when you have to go to the bathroom it a hell of a long time. The food is terrible and some time you get skipped. The hygiene is lacking , like you have to ask for a wash basin to wash up or wash you teeth. Standard cleaning. My husband was bed ridden and had the use of only one arm and of course every thing was out of reach or food tray plopped down with out any help opening things or cutting up so your food so you can eat so this is another thing you need an advocate. On tour they tell you all the right things but once your love one is admitted good luck. This place is rated five star by Medi-Care but if I were you stay as far away as you can. Immediately as you walk in there's a sign that says STOP and wait for assistance. OK COVID is still a thing so I wait and several staff walk by me and do not address me. I can see what looks like a nurses' station a few yards ahead to the right. Further there's a closed big binder on this stand and it says employees on the cover. Like is that a sign in binder? I poke my head into the office to the left and the woman behind the desk directs me to the receptionist a few yards away that you can barely see because her head barely clears the counter where she sits. How is she supposed to see when visitors come in? Walking down the hallway it feels chaotic and loud from all the people talking. I get to my mom's room and it is dark and she is completely out of it! I had just seen her two days prior at UCD and she was normal. But now she was dazed and staring at the walls and ceiling; her speech was mumbled and she wasn't making sense. I had never seen her like that before! I did not feel ok leaving her there for a second night either. We have her transported back to UCD ER. Apparently this doesn't look good on the facility but my only concern is that for my mom's health and well being. When it became time for her to be discharged to another SNF, we did not choose her to return here. read more

DO NOT send your loved ones here! A lot of of the people there are students they are overworked and most likely short staffed. No one knows why you're there when Nurse A leaves she won't relate any information to Nurse B. You will use your call button several times before you will get assistance. The food is terrible. They told me they had great security and that the door is locked from 8 PM to 8 AM. My mom was having an anxiety attack one night and I walked right in at 4 AM. One night they didn't even give my mom dinner. They thought her son took her dinner, but it was her roommate son who took dinner. Most nurses have no bedside manner, there were only a few good ones. Stay as far away from this place as you can! When you tour the place, they'll say all the right things but once you admit your loved ones good luck! read more

My brother went there after having a heart attack. In 2 weeks they helped him get his strength and energy back so he could return home. I want to thank Julie the Intake Coordinator because of the way she greeted and reassured me my brother would get very good care. Also, Molly the Admissions Coordinator for giving a good tour of the facility. From that tour I could see the care and help the staff gives to their patients. All the nurses and physical therapee's staff were wonderful. Also, Jeff the Administrator was friendly and helpful. I would recommend this facility and give them a 5 star rating. read more

Sac Post Acute will tell you they are a 5-star facility. What they won't tell you is that they re-use trach suctioning equipment for up to a week. Trach suctioning is supposed to be a sterile procedure, and new equipment should be opened every time for open-line suctioning. This is why your family member gets pneumonia every couple of weeks, which winds up becoming septic and life threatening. read more

If I could give this place a lower rating, I would. My mom had a massive stroke in May, they rated it a 2 (a rating of 1 is death) so it was severe. After she got out of ICU, we then had to choose a nursing facility, as she was left completely paralyzed on her left side, and she needed help learning how to do things again. We chose this facility because it was a 5 star rated facility based off the Medicare website. Unfortunately they have been able to keep that rating, probably because people don't take time to file a grievance with Medicare, sadly. There were very few people at this SNF who actually showed concern and care when they worked with my mom. I was there from open to close, daily. I would continually find her laying in her wet bed or without sheets because they "ran out". She was forced to feed herself breakfast without help, but they did watch her during lunch and dinner? She needed guidance because she didn't recognize her left side existed after her stroke, and missed her mouth almost every time. During her time there, there were a couple of nurses who were rude when I'd ask for updates on her meds or blood work. She went with specific instructions on how to continue her care, and that she had severe left side neglect. They stopped getting her blood work done until I asked them to resume her blood work, which I shouldn't have had to. The CNA's wouldn't reposition her every 2 hours, like they were supposed to. Thankfully I was there day and night to make sure she got better care than other patients, but I regularly witnessed other patients suffering due to the lack of nurses and CNAs. We were told they were short staffed, and that was why the patients were left in their dirty diapers for so long... they also ran out of diapers, bed pans and other items they should've had on hand being a 5 star rated SNF. There came a time where I couldn't be there with my mom, due to my own illness. During that time she fell off her bed and hurt herself. However, the nurses asked her if her left side hurt anywhere, and of course she said "no". She didn't even recognize she had a left side due to her massive stroke. She wasn't competent enough to tell them, which is why her care was supposed to go through me, I was her advocate. So even though I asked them multiple times to get X-rays, they never did. She was released home, and it later took me 3 days of planning to get her transported to the hospital for X-rays only to find out that she had in fact torn her rotator cuff and possibly her meniscus in her knee, both on the left side. She didn't feel it unless she moved, because again, she didn't even recognize she had a left side of her body. Once we found out, my mom was weeks away from being placed on hospice, so there was nothing they could do, except give her pain medicine. We couldn't get her in and out of bed because it hurt her so bad. I think this was where she gave up. She passed away on 09/12/2023. read more

Worst most inconsistent "medical"facility I've had the misfortune to experience! They lied to me on more than one occasion. They lost my father's hearing aides and clothing and put him in a room that had raving people in it yelling most of the day. The facility is not clean and smells terrible. My husband has been in and out of the hospital and in nursing homes for rehab more this year than he has been at home, due to falls and Parkinson's Disease. He has been to several nursing homes. Sacramento Post-Acute is by far the best in the region. Stellar care, attentive professional staff at every level, sparkling clean facility, cheerful, polite, engaging team members. And, I would receive calls regularly with updates on his condition, questions and with any information I needed to know. Hats office to Admissions Director Molly and Intake Coordinator Julie for their professionalism, engagement and attentiveness. I could write a horror story about one of the well-known nursing homes in the region where my husband stayed, while another nursing home called to mind the movie "One Flew Over the Cuckoo's Nest." Both of these nursing homes are in need of State reviews for substandard care and practices that are quite questionable. Please, please, if you have a loved one who is in need of nursing home care, ask the tough questions. Pay attention to how your questions are responded to and look around. Be mindful of how the staff are working with patients and interacting with one another and visitors. Should my husband be in need of nursing care for rehab again, I will not hesitate to have him at Sacramento Post-Acute for another stay. My mom was sent to this facility for skilled nursing and rehabilitation post breaking her knee. The staff were callous, vengeful and vindictive. They showed no regard whatsoever for family in planning her treatment, doctor visits or her discharge. She was keep in a wheel chair for 5 hrs because they couldn't arrange proper transportation for her to get back home. She lost more than 20% of her body weight because the food was not suitable for her and no adjustments were made. Covid was used as an excuse not to allow family take care of her feeding. I can go on and on. But this I would say don't send your family here, heck, don't send your enemy here. Anywhere else is better insurance or not. A complaint to the appropriate authority is the next step. read more

My body had taken control OVER "STUPID BRAIN". Places like UC Davis Medical Center are good for it's doctors, Surgeons and Staff. Sacramento Post-Acute! Employees "Angels" who wear NURSES UNIFORMS! They are BEYOND THE MOST CARING NURSING STAFFS I've ever seen. I couldn't believe my "Favorite Nurse 'Rachael"! Has to deal some 26 patients and "Residents" like myself. The food is ALWAYS NEW and IMAGINABLE! There COOK REALLY IS! "Such a good guy". I WAS A "Broken Mess". When I arrived here. If What you need is a "TOP NOTCH" The Therapist and Occupational group such as Sam, Ryan, and the "INCOMPURABLE Kaitlin" are DEFENITELY what you need. I think GOD KNEW I NEEDED! To get home! That's why he Personally sent me his Best Angel "Kaitlyn". I also had another Therapist that week! OH WELL! I could keep talking; I'm TERRIBLE REMEMBERING names; but, ALL I can say in closing! This facility treated my dad, although i have read other reviews that are negative,, but this facility has been the best out of 3 we have tried so far. Even though it does look old, this facility people are preety nice, just address your concerns to director or manager, it will be taken care of. My very close friend was checked into this facility a week ago after being in a debilitating car accident. The care was neglectful and staff were cold and unpersonable. When he checked into the facility he had a gold labrodite ring on. We did not take the ring off because it had not been off in years and would not easily pass his knuckle. When he was transferred out of the facility the ring was GONE before he left. None of us removed it and he did not have the strength to do this on his own. When confronted the management denied he had any jewelry on, showing his paperwork made no notes of jewelry. We were witness that IT WAS on him when he arrived. Worst facility. Sent my Dad here. He acquired an infection. He's 83 years old. He was left laying in the bed in a filthy adult brief on numerous occasions. The infection was the enterococcus bacteria found in feces. He wound up in the emergency room at Mercy San Juan on intravenous antibiotics for 12 days. They failed to change himin a timely manner and tried to claim he had the infection when he arrived. I guess that's why they decided to let him lay there without changing him. When I asked the cna's to change him, they said the reason they took so long was because they were "too busy." Never take a loved one here... read more

Extremely upset with the facilities lack of compassion for family members during these Covid times. There are a handful of nurses and staff members who actually care about the residents and treat them as if they were their own family members. The upper management and director of nursing do not care about the residents and have zero empathy. Multiple request have gone unfulfilled. There have been several occasions we're I have complained about my father's basic hygiene and concern about his aspirating.. and they did nothing about it. Don't even waste your time filing a complaint with the obudsman. He pretends to be on your side and really doesn't get anything accomplished. Since the families are not able to visit. I'm just terrified of what is going on in there now. My father fought for this country and lost his leg he does not deserve this kind of treatment. If you can stay anywhere else then do it!!! This was the only place with an available bed that my insurance covered for my back surgery recovery. Some of the staff are very kind and good at what they do but the majority are not. I was moved to a room with 3 other women who needed more help than I did and the nurses didnt answer their call button notifications for help. The woman right next to me had to get her own bed pan because the night before she waited, no one came and she had no other choice than to have a b.m. in her bed. Then she sat in her own feces and waited for help. I finally got up and notified one of the guys in charge. He barely looked up from the computer and said he would get someone to help her. I found a nurse in the hall before he even got up. I could go on and on but i must get back to my very short-staffed and neglectful accommodations at Sacramento Post Acute. Btw the food is disgusting and i wouldn't feed some things even to my lab Teddy (and he eats just about anything)! read more

Sacramento Post Acute is 2.5 stars as an overral facility. But the standout feature of this place is the wonderful nurses that were assigned to my father. My father was there for physical therapy short term rehab and in a lot of pain. His AM and PM nurses Yee and Alyzha were amazing at making sure he was comfortable and taken care of. The night shift nurses never checked on my father and he'd lay in pain all night. Thank you to those two for being great at their job. Only reason I'd recommend this place. read more

I was a patient at this facility twice in the last year. I had deadly pneumonia and was admitted to UC Davis Hospital at the end of June 2016. I fell into a coma and died around July 9,2016. The Drs zapped me and performed a treacheotomy and brought me back. I was at UC Davis Hospital for a month then spent another month at Vibra Hospital to get off the treach \ventilator. Then I had to go to Sac Post Acute to learn how to walk again since my leg muscles had atrofied since I had layed up in bed for 2 months at that point. Thanks to Kyle and Michael in physical and occupational therapy for helping me to gain strength and soon I was walking with the assistance of a walker. And I was wheeling myself around in a wheelchair. I have read several of the other reviews and am shocked at their comments. While I was a patient there the place never stank of feces or urine. The janitorial staff were excellent at keeping the facility clean. They used a separate pad to mop each room. Most of the staff at this facility is good. The new chick Kaitlin is a rude cow and a liar.The first day that she came to get me for o\t she threw a wash cloth and a towel on my bed and tells me to was my face. I am a grownass man. I had already washed my face and brushed my teeth. Then she goes to the head of p\t and o\t and tells her i refused to do o\t. I didn't refuse i told her that I would be available after 11am, what I didn't tell her was that I waiting on the wound care Dr. So the other lady comes storming in my room yelling at me and talking through clenched teeth talking about i was refusing treatment and then she decides to discharge me from occupational therapy. I was like fine whatever. Because I didn't want to work with pig faced Kaitlin anyway. So some time goes by and I am working hard with Kyle and the other guy in p\t. Then someone writes a report to Medicare telling them that I could walk 150ft unassisted and traverse 8 steps all on my own. Which was 100% a complete fabrication and Medicare told them to discharge me from the facility. I fought it and eventually they had to switch me to medical. So those are the staff to watch out for Kaitlin and her boss two rude liars. I found the nursing staff to be competent and caring. One night I fell ill from a salad i was served for dinner but the nurse on duty called the doctor right away and got me something for the nausea and she took my vitals and kept checking on me until I went to bed for the night. Another night there was a power outage and the staff went around and checked on everyone. The emergency generator didn't kick on right away, but the staff took care of it and got it going in a relatively short amount of time. I was really impressed with the way they checked on their patients who were on ventilators and oxygen. I sleep with a cpap machine so, as soon as the power went out i woke up. And I heard the staff barking orders to each other. Excellent training and execution of what could have been a disaster. They even got me an extension cord to plug into an outlet in the hall to run my cpap machine so that I could go back to sleep. Overall I liked it there at Sac Post Acute, I just got mad when I found out about the lies that were sent to medicare. Another thing, the CNAs work really hard and sure some times they are slow to respond to call lights but thats ' because they have more than one patient. I don't understand why most people think they are the only patient and have the balls to get mad and complain that the staff don't respond quick enough. What used to piss me off was this one male nurse,Eric who used to leave me sitting and waiting on him to take my blood sugar before dinner. I finally got tired of his game, and it was a game to him. And I would eat my dinner when it came nice and hot. Speaking of the food, I thought it was pretty good. Hot food was hot and cold food was usually cold. Other than that salad that made me sick, I had no other problems with the food.I I am only giving this place 2stars because of the lies that were told to Medicare and because they refused to let me go back for a third stay in July 2017. Took my uncle there after a stroke and heart-attack, during one visit he was struggling to breathe due to his trach being clogged, we had to go find respiratory to replace it. Then he got an infection and was moved to another room, when we arrived for a visit, his air hose was on the floor, along with his call button, the nurse attempted to place it back over his trach straight off the floor without replacing it with a sterile air line that covers his trach. He got a more serious infection and was moved to room 13. Each time we visited, we pushed the call button for nurses when he needed help, we would wait for hours until we had to personally go find a nurse and would still wait until one would come to the room. When my uncle would talk through the speak valve, he complained about the young boy on night shift, he stated to us in the presence of day shift nurses that he strangles at night and that no one comes when he pushes the call button and that night shift does not suction his trach. We complained many times and was assured the problems would be corrected until one night we were called and informed that they went in his room and found he had stopped breathing. The young man on night shift claims that he suctioned him prior to his death, but while waiting for the hurst to arrive, a nurse removed his trach and threw it in the trash, I put on gloves and retrieved it, and seen that it was clogged. For the lazy night workers, it may be an inconvenience to attend to patients, even when that care is needed to keep them alive, and when a patient is lost, it is just another day at work for them, but I wonder if they had to attend the funerals and watch the agony of the patients families mourning their loss, if maybe that wouldn't get them off their lazy butts to do their job? If you have a relative who needs trach care, sending them to this hospital could be a death sentence, they have no equipment to set off an alarm when the blood oxygen level is low, not that they would respond too quickly if they ever did. read more

One star is too many. The very epitome of elder neglect.Borderline criminal, IMO. Visited a friend here. She was in bad shape - health had declined significantly since transferring from another facility, where she had been recovering from a surgery. When I got there, just for a regular visit, she was complaining of CHEST PAIN and DIFFICULTY BREATHING. She had a history of congestive heart failure. I called for a nurse, who eventually came in and took vitals. Her numbers were shockingly low (don't remember them, this was 7 months ago, but remember systolic was in the 60s). The nurse blew it off, said she'd inquire about administering some pain killers, and left. If I wasn't already in a "hospital" environment, surrounded by nurses, I would have called 911. Shocked at the nonchalant response from the nurse, I called a personal nurse friend on the phone, who relayed vitals and complaints her primary care doctor. He asked to have the charge nurse call him immediately. After a brief conversation, the charge nurse called for an ambulance. 20 minutes later, they arrived, no lights or sirens. They'd just been requested for transport. After arriving, within 30 seconds, they assessed the urgency of the situation, and immediately stepped up their response. She was brought immediately to Mercy San Juan ER. It was there that the doctor showed me a large, festering hole in her abdomen, that had previously been covered with a bandage. She had developed sepsis from her infected wound. Within a few hours of my arrival to visit her at Sac Post-Acute, she passed away at MSJ. I was by her side the whole time and saw the entire situation unfold. Sac Post-Acute totally neglected the urgency of the situation, under-responded, and I am disgusted by their lack of care. I am just a "regular Joe", yet I knew something was wrong. These are supposed to be trained professionals, yet their lack of care clearly contributed to the loss of my friend. read more
