Some people at work recommended this office when I told them that I needed to get some dental work. I'm the type of person that equates the sounds of the dentist office to that of nails scratching on a chalk board. However, i'm happy to report that the staff was great. They were very upfront about why I needed the work down and took the time to explain my x-rays in detail. The procedure that I had been "dreading" for weeks turned out to be painless. I'm amazed at how far dental surgery has come since I was a little kid. I still have more work to get done , but at this point I would recommend this office to any of my friends. read more

I had an extremely disappointing experience at Sage Dental and would not recommend this office to anyone with dental insurance. Before my appointment, I verified that my insurance provides 100% coverage for routine preventive services, including exams, checkups, and cleanings. This was my first dental visit in years, so I expected to receive straightforward information about what was covered and any legitimate treatment recommendations. Instead, I felt pressured throughout my visit to approve additional services that were presented as necessary. I was told I could not receive a standard cleaning because my gums were "infected" and that I required a more expensive cleaning that would cost me an additional $168 out of pocket. I was also told that the alleged infection could not be treated for another 30 days, requiring additional appointments. The entire process felt confusing and misleading. I was repeatedly asked to sign documents for services I declined. My understanding was that I was signing acknowledgments related to discussions we had with the dental staff and assistant manager, including statements that certain costs would be adjusted because my insurance would not fully cover them. However, when I went to check out, staff attempted to bill me for services I had already declined, claiming that my signature authorized them. The front desk staff made me feel embarrassed and pressured to pay charges that I did not believe I owed. Instead of receiving clear explanations, I felt as though I was being treated like I was unwilling to pay for necessary care when my concern was simply being billed for services I never intended to authorize. To make matters worse, I am now receiving bills for approximately $400 in additional charges that I do not believe I approved. Patients deserve transparency regarding treatment recommendations, insurance coverage, and financial responsibility. Typical large corporate dental facility. Constant attempt to upsell the patient to major treatment plan that may not be required. This has gone on with me for three years. I still haven't needed major work. The office staff is friendly but I found my most recent hygienist to be rude perhaps because I chose not to purchase a &750.00 tray to kill bacteria in my gums. I ended up there only because they took my Delta Dental which I'm dropping this month. I've found an individual woman who operates a small practice locally but doesn't accept insurance. Speak to friends and find a dentist they are happy with. For 20 years in another state I had a great dentist. I moved to Florida and my former dentist retired. I would give the Ft Myers Office no stars if I could. I received an estimate of no cost to patient, then received a bill for an amount greater than the cost of a cleaning with a description of service for severe inflammation. Interestingly, my last cleaning 6 months prior, I had no inflammation. This was a first for me. I have always been told I had great teeth and excellent oral hygiene habits. Also, my cleaning lasted for 20 minutes, about 25 minutes less than all of the other cleanings I have had in my life. Wonder what was done for my severe inlimation? This is the current trend with dental offices, they get you in, do x-rays, then send in a manager with everything they want to sell you. Insurance covers a cleaning? They say you need a periodontal cleaning, it'll be a $197 copay. they also said I needed a crown removed and built up, $600 after insurance. They completely missed the broken filling I have that I knew about already. i didn't schedule an appointment because I wasn't going to pay $600 for work I don't need. They scheduled it anyway. They are one of those companies that are only into upselling and sending someone into the room with an astronomical treatment plan and cost. Stay clear. It is not even close to what it was when it was Horizon One. I felt like I was being strong armed into having things done. I was there for a six month cleaning. Of course they found something wrong with a tooth and scheduled me for a filling. I go back for the filling and the assistant is numbing the wrong side of my mouth. The doctor got a chuckle but I almost walked out. Next comes the billing. I pay when I leave and once I get to review my EOB's from insurance, I realize I've overpaid. Not to mention they sneak in an Office Visit Copay of $10 which is not covered by insurance. After a phone call, I'm told they will look into it and will give me a credit towards my next visit. I don't know why you have to have a separate appointment for an exam and a cleaning. When they switched over to sage they told me this and said it wouldn't be like that moving forward (because I had to go on separate days). They cancelled my appointment months ago and couldn't get me in until today. I get there today and my insurance changed and the person who does insurance was at lunch for the next 45 minutes and then I would have to wait for her to approve my insurance before I could be seen. Then they told me that I have only an exam anyways today. I told her no- I need a cleaning and she said "oh yeah well that's going to be an hour after your exam". What kind of ridiculous system is this? My dental "exams" only take 5-10 min IF THAT. Why do I have to wait an hour in their office for the cleaning?! I'm so annoyed. I have a job that I have to go back to- I don't have 3 hours to spend getting a basic cleaning. It's been about a year that I've gotten a cleaning because of this office's lack of caring. I was SOOOO wrong in giving this place a 5 star review as they sure don't deserve it anymore. I was disillusioned when I gave them 5 stars. There are not enough stars in the universe to give them that many negative ratings!! I had 8 teeth extracted at their practice and the next day needed help. I called and the idiot receptionist told me she couldn't help me as it was my problem and I should've talked to the doctor when I was there. I was in dire need of something for pain and she totally ignored me and told me to figure it out myself. I then called the Case Manger at my primary care physician's office to try and get some help as the pain was unbearable. She called Sage dental and was told it was none of her business and who was she to even call to help me. The day of the surgery I was given NO post-surgery instructions and when I asked a question the dr walked out of the room, they brought me a wheelchair and took me to the waiting room downstairs. The receptionist did tell my case manager that an antibiotic was prescribed and to use that for pain (FYI - antibiotics are NOT used to treat pain). However the instructions for that was to take 4 capsules 1 hour before my appointment with my regular dentist at a different practice and when I asked why there were 12 capsules ordered and I'm only to take 4 the answer was ask someone else. After a day of extreme pain the top manager at my primary drs office had to get involved and I was finally at 6:08pm ordered 12 tablets of Tylenol with codeine when I started to try and get something at 8:45am that morning. So I was never given any instructions and when I saw my regular dentist his assistant gave me a whole list of things to do and not to do. I was shocked and my dentist said because I didn't do some of the things recommended my healing process is not going as it should. I could write a book on the terrible way my extractions were handled and the follow up care that Sage Dental refused to give me and I can assure you that a LONG detailed complaint will be sent to every agency I can find on where and how to file a complaint against Sage Dental. And because my healing process is not proceeding as it should I have an appointment tomorrow with an attorney to look into suing Sage Dental. Don't ever go to Sage Dental they are totally incompetent and do not help once you leave their office. Really? My problem? My case manager is in no place to get involved? Figure it out on my own? Type of care from front desk to cleaning hygienists to Dr. BAKOS. they are all uncaring. Nobody wants to be held accountable. Crowns made incorrectly not fitting. Crown fell off. Not seated properly. Cut open gums to uncover implant. Hole cut too large. Implant abutement or cover was too small and not the correct abutement for the particular implant. Caused gingivitis in the gapping hole. He stopped all service and I never was treated for gingivitis or small abutement or crowns that I paid $500 down for. My insurance also paid the Sage Dental for crown I never received because Dr. BAKOS stopped service and told me to find another doctor and he had trust issues. 3 months now I still don't have my Crown because my insurance is hung up and never been refunded $890. When I finally did get back to a room I was told that the front desk still hadn't submitted my paperwork.. really it's 10 steps away and then I was further told that my appointment was only for a exam.. really I booked a cleaning as well?! I'm very disappointed in this office and won't be returning. I made an appointment for a cleaning as a new patient. When I arrived I found out this was just a consultation and X-rays would be taken. Had to pay out of pocket $130 because I wasn't due for X-rays. They never requested me to get X-rays from my previous dentist. Went back a week later and was told I would need a "deep clean". No mention of this was made at my consultation. Another $140 out of pocket! I see a dentist every 6 months and have never been told I need a deep clean. I have been a patient at Sage Dental of Fort Myers, and I have to say that my hygienist, Angie, is absolutely fantastic! She is thorough, gentle, and always make my visits comfortable and stress-free. Unfortunately, I have a major issue with the office policy requiring x-rays every two years, regardless of medical necessity. I believe in following recommended dental care, but I also think patients should have a say in their own health decisions--especially when there is no clinical reason to take x-rays that frequently. When I questioned this policy, the office manager made it clear that I had no choice: either comply or find a new dentist. That level of inflexibility and disregard for patient concerns is disappointing. While I can't say enough great things about my hygienist, the office's rigid policies and dismissive approach to patient choice is quite concerning and I will be looking else where for a new practice. read more

HORRIBLE CUSTOMER SERVICE. You're never taken back on time, when you ask after 35-40 minutes when you'll be taken back they say another 10 minutes. They take you back and you sit in another room for 30 minutes. I had to leave because I was there for so long, no one ever called to reschedule and had to loose out on $5,000 of clear corrects because the manager Brenda would not budge on getting a new scan because they lack in time management with appointments in the past. Please do not go here! Everything that they did to my teeth has to be redone. I called to speak to a manager a week ago and they will not return my call... probably Because I want my money back. I have to replace three of the crowns that they did. Possibly now I have to get a root canal on one of the teeth under the crown because of a ledge and improper fit. My bite is completely off so I also could be looking at Invisalign. I've gotten consultations from three other dentist after going to this practice. I also received Novacaine And my face swelled up due to the Dentist injecting into a blood vessel. I have photographs of everything that was done and documentation from all other consultations. I don't know if I should try and sue them in small claims court for my money. But if I can help somebody else, I need to let whoever's reading this know, do not go here. Please save your money. You come for cleaning and they charge "for evaluation" only first. Then they want you to come again and charge you again for cleaning. Sage Dental suppose to bill insurance first according to their 2025 contract, because they are in their net work. I went here on 3/27/2024 for a dental checkup. It was the Fort Myers, Florida location. I was seen by Dr. Richard Delboccio. He advised me that I would need $10,612.00 in dental services immediately, not to be broken up in covered Dental plan amount, as an "Investment in my Future" (which he said multiple times). I had them send the xrays and forwarded to another dentist. He advised that I need a cleaning and a few other things, that would be covered in my yearly plan amount. The rest could be broken up yearly. I was told my insurance does not cover me. I was not concerned, I knew I could submit the invoice for reimbursement. When I called my insurance company, I was told I could not be reimbursed by them, they had paid the same day as service, an amount lower than I was charged. When I called Sage Dental the office manager was away for the week. Following call back by me and my insurance company were met with the same response over 4 1/2 weeks. During that time, I was told that even though I had paid them, I would have to wait 7 to 10 weeks for my reimbursement. So they double billed the same day! (If that happens often enough that's a nice amount for a company to play with). If you look at Trust Pilot or Yelp on the internet, the other reviews will verify - I will be doing so with anyone else I do any billable service with. I was like pulling teeth. My family had been going to Horizon One for almost ten years with Dr. Kuhn and Dr. Almasi. Since they sold out to Sage Dental, the quality of care dropped significantly. Significant turnover with all new staff in front office and new hygienists (?diversity/equity hires). The hygienist, Angie Johnson, was extremely rude and argumentative during a routine exam. She had difficulty operating the cleaning equipment and stated I had poor care from previous hygienists at Horizon One -- she could not specify exactly. She also performed poorly at explaining how gum disease had apparently developed (never been told this before) and that I wasn't using dental floss properly. Dr. Boogaard came in near the end and offered a quick exam with "looks good" as I expected. Luckily there are competent alternatives in Lee County. What a poor legacy for Dr. Kuhn and Horizon One - hopefully the buyout was worth the price. I used to recommend this clinic to friends. Sadly they have shared similar concerns. My husband and I had a very disappointing experience at Sage. In the past, we loved coming to Horizon Dental and seeing Dr. Kuhn. We both felt the care was excellent and charges were reasonable. The past 2 visits, for just a cleaning were more expensive than we have ever paid. I will say Jennifer was a good hygienist but we haven't seen Dr. Kuhn for the past 2 visits. In the past, he would always come in and do an overall exam. We do not have dental insurance but a plan with Medicare that will help with dental expenses and several of the charges were denied because they were not recognized by Medicare. When I called Sage , I was told one charge was a "corporate charge" and the $40.00 charge was for a mouthwash rinse. I asked if I could get more info to resubmit and was told the " no insurance ever pays for it so there is nothing I could give you." in 2022 i had 2 crowns that needed replacement due to cracks which were replaced by dr kuhn the service at horizon switched to sage and i noticed it was extremely difficult to get appointments- my hygenist left. so i switched to Dr Dunphy in april 2023 and on his initial exam he reported that those 2 new crowns had to be replaced because they were pushing on my gums. my dental insurance denied payment for replacement and I wanted my compensation from Sage which was denied. Needless to say i wanted future customers there to know about this. I am out a lot of money because crowns can only be replaced every 5 years. read more

I, by default, became a patient of Sage Dental by being a former Horizon One patient. Since this transition, I've had two 6 mth cleaning appointments, and after todays appt, I will not be coming back. During my first appt with Sage, I found myself with a new hygientist (I loved my former hygientist when it was under Horizon One). The new Sage hygientist lacked bedside manner considerably and it was a rough cleaning experience, but one that I endured. From prior cleaning visits, I had noted on my chart that my preference was hand scaling however that was overlooked and she began cleaning with the new ultrasonic tool, that I had a previous bad experience with thus noted on my chart my preference. She swapped tools and went through the cleaning without further incidence until I got to the checkout desk and realized she gave me a fluoride treatment (I'm 60 years old, brush my teeth twice a day with fluoride and drink fluoridated water). My insurance did not cover this ,so I had to pay out of pocket. I was not informed of this before they applied the treatment so felt a bit rooked. Today I had my 2nd appt for 8:00 AM to ensure I was first one in. At 8:15 I went to the front desk to ask when my appt was going to begin as I had to get to work. The hygientist came out at that time and brought me back to the room. He then apologized he was late but he was tinkering with a piece of equipment in his room to fix it. When he began the cleaning process, he brought out the ultrasonic tool and I shared with him that I don't prefer that method of scaling/cleaning as I've had a bad experience with it and my chart clearly notes that I want hand scaling. Throughout the rest of the procedure he went on and on and on about how I needed to use the ultrasonic tool and that the last hygientist likely did it wrong. I was kind and declined over and over and thanked him but politely reiterted I wanted the hand scaling as my chart notes. Until I was over it. His unrelenting badgering was harassment and I had enough and got out of the chair and left. Never in all my years have I walked out of a dentists office. I spoke to the office manager and frankly, she listened however the last thing she said was how the hygientist was usually a good person. That did not make me feel heard at all. Oh and by the way, every time the hygientist went on and on about the ultrasonic, he kept calling me honey. I am not your honey! How insulting. Anyway, I am moving on. I've written one review on Yelp in my entire life and that was to compliment great service I received at a nice restaurant. I had to wait 2 hours to see the doctor when my appointment was at 1pm. didn't get seen til 3. they also don't put you under and only give numbing shots and some sort of Xanax in your iv. So I was awake while they pulled the teeth out and I was so scared. Have been speaking with office for needed info for submitting our own claim, (as the office refuses to submit on our behalf) since January. Our out of state insurance needs some numbers to verify dental practice and this office is unwilling to submit by fax the data to our insurance carrier. We were told different things at different times, as was our insurance rep. The office verbally gave me the NPI number but refused to amend the bill to include it or to give it in writing to the insurance company. We, the consumer, are stuck in limbo. Everyone was very professional, review what they were going to do today. No surprises. Being in our network it cost about 1/4 of what it cost with our previous dentist. You will be amazed at the high level of customer service, all of the staff are extremely kind and alway explain what they are doing and why. The wait time is very short & they are always accommodating in meeting my work schedule. I have been going there for over a year and am so glad I switched.
