She lies about when she will visit and barely checks the patient. I am a retired oncology nurse and am very disappointed in this hospice. The director also only gives lip service and not very good at that. I am not in the state so I only can give moral support to my family who are extremely upset at the lack of professionalism and help they have received. No one has given the family instruction in body mechanics , proper information on her meds, and no apology for the lack of any of the above. I was a home care nurse for years and never told my families I did not have time for them. Proper education is very important to aid families feel they are doing their best to take care of their loved one. debbie d RN ONC

Our experience with Haven Hospice Center-Orange Park was deeply disappointing, especially due to…read morethe behavior of the night shift nurse. From the moment she arrived, it was clear she lacked compassion and professionalism. She would administer medications with no effort to speak to the patient or family, often moving through the process as if it were a chore. When we had questions, she acted visibly annoyed, making us feel as though we were a burden. Her demeanor was cold, and she walked around with a sour expression, never offering a reassuring word or a simple smile during such a challenging time for our family. The lack of empathy was disheartening and unacceptable, especially in a hospice setting where care and kindness should be a priority. My hope is that the hospice center takes this feedback seriously and addresses the behavior of their staff to ensure no other family has to endure such treatment during an already difficult time. Families in these situations deserve support, compassion, and understanding--not rudeness and indifference. Havrn responded to my Google review calling the patient their "customer." just like the night nurse, showing zero empathy.

I would strongly caution anyone considering Orange Park Rehabilitation & Nursing Center based on…read moreour family's recent experience in April 2026. My brother was admitted for care and, within hours, was left in respiratory distress without timely suctioning, and his feeding tube was not even connected, leaving him without nutrition for an extended period. Despite repeated requests, staff response was delayed and inconsistent. Throughout his stay, we experienced ongoing issues with call lights going unanswered, lack of basic hygiene care, and unprofessional, dismissive communication from staff. On one occasion, a staff member spoke to him in a demeaning manner rather than addressing his concerns. Most concerning was the lack of urgency during a medical emergency. When he experienced severe respiratory distress, staff response was delayed, and emergency services were only called after persistent family advocacy. He was later hospitalized, where the findings indicated a severe case of pneumonia. No family should have to repeatedly advocate just to ensure basic, timely care for their loved one. This experience raised serious concerns about patient safety, responsiveness, and overall quality of care. I hope the facility takes meaningful corrective action to prevent this from happening to others.
