As of today, March 5, 3:30p the hospital still has not received it, putting my procedure at risk of being cancelled. I have left multiple messages on their portal and called several departments, but no one answers the phones and there has been no response. When a patient in severe pain cannot obtain a simple clearance form after repeated requests with a direct promise from the physician, it raises serious concerns about communication and patient care in this office.
